An Analysis of Clerkship Satisfaction in College of Korean Medicine : Focusing on Doctor-patient Role-play and mock CPX (한의과대학 실습 만족도 조사 - 의사-환자 역할극, 모의 CPX를 중심으로 -)

  • Objectives : The purpose of this study is to find an effective clerkship process for the implementation of participatory clinical practice that is practically applicable to most Colleges of Korean Medicine. Methods : Taking the 4th-grade students of the College of Korean Medicine, Kyung Hee University in 2019 as a subject, we conducted Role-playing and simulated CPX in the department of Ophthalmology, Otolaryngology and Dermatology and conducted a satisfaction survey through a questionnaire. Results : 1. The preliminary expectations for overall practice and Ophthalmology, Otolaryngology and Dermatology practice did not differ significantly. 2. The satisfaction, data adequacy, feedback adequacy, and usefulness of Ophthalmology, Otolaryngology and Dermatology course were 79.4%, 80.8%, 87.6%, and 86.5%, respectively. 3. When comparing the Role-playing of Ophthalmology, Otolaryngology with mock CPX of Dermatology, data relevance level was similar, feedback adequacy level was higher in mock CPX, satisfaction and usefulness level were higher in Role-playing. Conclusion : Comparing Role-playing with the simulated CPX method, Role-playing is a relatively realistic form than the CPX method, and moreover students showed higher satisfaction and usefulness in Role-playing.

  • Three patients suffering from widespread erythrodermic cutaneous T-cell lymphoma were treated with low-dose UVAI phototherapy with a single dose of 20 - 30J/$cm^2$, resulting in cumulative doses of 520 - 1890 J/$cm^2$. Two patients showed complete responses, while other had a partial improvement.

  • Hydroa vacciniforme (HV) is a disease of Epstein-Barr virus (EBV)-associated NK/T cell lymphoproliferative disorder, and patients with severe HV-like eruptions have a high risk to progress to various EBV-related hematological complications. Photosensitivity in HV might be a 'photo-Koebner' phenomenon mediated by infiltration of EBV-infected NK/T cells.

  • A noncoherent, broadband, intense pulsed light source has been used for the symptoms of photoaging skin as a nonablative method. The purpose of this study is to investigate the efficacy and tolerability of intense pulsed light in solar lentigines and ephelides on the face. An open study was performed in patients with solar lentigines and ephelides who received three to five treatments of intense pulsed light. Forty-eight percent of patients had more than 50% improvement and 20% had more than 75% improvement. In the group of solar lentigines, 40% of patients showed more than 50% improvement and 16% did more than 75% improvement. Patients with solar 1entigines+ephelides and ephelides responded remarkably with 75% and 71 % of patients having more than 50% improvement, respectively. Intense pulsed light was well tolerated and may be a new modality for the therapy of solar lentigines and ephelides.

  • This study shows that sphingosine-1-phosphate (SPP) significantly inhibits melanin synthesis in a concentration-dependent manner, and that the activity of tyrosinase was also reduced in SPP-treated cells. In contrast. a specific extracellular signal-regulated protein kinase (ERK) pathway inhibitor, PD98059 increased tyrosinase activity and melanin production, and PD98059 restored the reduced tyrosinase activity and pigmentation induced by SPP.   • Human epidermal keratinocytes consist of stem cells. transit amplifying cells. and postmitotic differentiating cells. Among them, stem cells playa critical role in cell renewal. wound healing. and neoplasia. However. till now, specific markers of human epidermal keratinocytes are not clearly defined.   • Treatments for alopecia areata include topical corticosteroid treatment, corticosteroid intralesional injection, systemic corticosteroid treatment, PUVA(psoralen-UVA) and topical immunotherapy. The therapeutic effects are variable. Alopecia totalis is hard to treat completely. Topical immunotherapy with dinitrochlorobenzene (DNCB), squaric acid dibutyl ester (SADBE) or diphenylcyclopropenone (diphencyprone, DPCP) represents the most accepted therapeutic modality for the treatment of extensive alopecia areata. We report two cases of alopecia totalis treated with DPCP. After DPCP treatment, total scalp hair was completely recovered.

  • Clear cell acanthoma (CCA) is an asymptomatic benign lesion of unknown origin that typically appears as a brownish, dome-shaped papule on the leg. It has an unusual clinical feature in that it appears as chronic eczema, Bowen disease, or Paget disease on the areola. Its histopathologic findings are well-demarcated psoriasiform acanthosis with pale keratinocytes (clear cells) that are rich in intracellular glycogen, which stain positively with Periodic acid-Shiff. We report herein on a young female patient with CCA on the areolar areas.

  • Objectives : This study was carried out to investigate the effect of Hwanggigyejiomul-tang on eczema with oozing. Methods : We treated 23 eczema patients with oozing. We used the Visual Analogue Scale(VAS), and recorded the condition of patients as severe, moderate, mild, none. Results : After the treatments, all patients VAS of oozing was remarkably decreased. Conclusions : This study suggests that Hwanggigyejiomul-tang is effective on eczema with oozing.

  • Objectives : This study was carried out to investigate the effect of Gyulpidaehwangbakcho-tang on rhinitis with nasal obstruction. Methods : We treated 18 rhinitis patients with nasal obstruction. We used the Visual Analogue Scale(VAS), and recorded the condition of patients as +++, ++, +, -. Results : After the treatments, all patients VAS of nasal obstruction was remarkably decreased. Conclusions : This study suggests that Gyulpidaehwangbakcho-tang is effective on rhinitis with nasal obstruction.
