Comparative Analysis between Zirconia Implant and Titanium Implant

  • Various ceramic implant systems made of yttria-stabilized tetragonal zirconia polycystal (Y-TZP) have become commercially available in recent years. A search of the literature was performed to assess the clinical success of dental Y-TZP implants and whether the osseointegration of Y-TZP is comparable to that of titanium, the standard implant material. No controlled clinical studies in humans regarding clinical outcomes or osseointegration could be identified. Clinical data were restricted to case studies and case series. Only 7 animal studies were found. Osseointegration was evaluated at 4 weeks to 24 months after placement in different animal models, sites and under different loading conditions. The mean bone-implant contact percentage was above 60% in almost all experimental groups. In studies that used titanium implants as a control, Y-TZP implants were comparable to or even better than titanium implants. Surface modifications may further improve initial bone healing and resistance to removal torque. Y-TZP implants may have the potential to become an alternative to titanium implants but cannot currently be recommended for routine clinical use, as no long-term clinical data are available.

Influence of porcelain veneering on the marginal fit of Digident and Lava CAD/CAM zirconia ceramic crowns

  • PURPOSE. Marginal fit is a very important factor considering the restoration's long-term success. However, adding porcelain to copings can cause distortion and lead to an inadequate fit which exposes more luting material to the oral environment and causes secondary caries. The purpose of this study was to compare the marginal fit of 2 different all-ceramic crown systems before and after porcelain veneering. This study was also intended to verify the marginal fit of crowns originated from green machining of partially sintered blocks of zirconia (Lava CAD/CAM system) and that of crowns obtained through machining of fully sintered blocks of zirconia (Digident CAD/CAM system). MATERIALS AND METHODS. 20 crowns were made per each system and the marginal fit was evaluated through a light microscope with image processing (Accura 2000) at 50 points that were randomly selected. Each crown was measured twice: the first measurement was done after obtaining a 0.5 mm coping and the second measurement was done after porcelain veneering. The means and standard deviations were calculated and statistical inferences among the 2 groups were made using independent t-test and within the same group through paired t-test. RESULTS. The means and standard deviations of the marginal fit were $61.52{\pm}2.88{\mu}m$ for the Digident CAD/CAM zirconia ceramic crowns before porcelain veneering and $83.15{\pm}3.51{\mu}m$ after porcelain veneering. Lava CAD/CAM zirconia ceramic crowns showed means and standard deviations of $62.22{\pm}1.78{\mu}m$ before porcelain veneering and $82.03{\pm}1.85{\mu}m$ after porcelain veneering. Both groups showed significant differences when analyzing the marginal gaps before and after porcelain veneering within each group. However, no significant differences were found when comparing the marginal gaps of each group before porcelain veneering and after porcelain veneering as well. CONCLUSION. The 2 all-ceramic crown systems showed marginal gaps that were within a reported clinically acceptable range of marginal discrepancy.

  • 구강암에 대한 외과적 처치로 구강 내 광범위한 경조직, 연조직을 제거한 환자는 저작, 연하, 발음 등의 기능적 문제와 치열, 안모의 심미적 문제를 갖게 된다. 악골 재건 후에도 치조골 등 합병증으로 인해 가철성 보철물의 유지 및 지지를 받는데 제한적이다. 임플란트 지지 고정성 보철물은 이러한 환자들에게 적절한 보철수복 방법이 되어 왔다. 본 증례는 하악 재건 후 computer-aided design과 computer-aided manufacturing을 이용하여 지르코니아 프레임워크 상부에 단일 구조 지르코니아 크라운을 제작하여 현재 치열의 기능적, 심미적 회복을 하여 이에 보고하고자 한다.

Impact of surface roughness of gypsum materials on adaptation of zirconia cores

  • PURPOSE. The present study investigated the influences of various gypsum materials on the precision of fit of CAD/CAM-fabricated prostheses and analyzed their correlation with surface roughness. MATERIALS AND METHODS. The master model of the mandibular right first molar was replicated, and four experimental groups based on two types of Type IV stone (GC Fujirock EP, Die keen) and two types of scannable stone (Aesthetic-Basegold, Everest Rock) were created to include a total of 40 specimens, 10 in each group. The surface roughness of the working models for the respective experimental groups was measured. Once the zirconia cores had been fabricated, the marginal and internal fits were measured with a digital microscope using the silicone replica technique. The mean and standard deviation of the respective points of measurement were computed and analyzed through the one-way ANOVA and Tukey's HSD test. The correlation between surface roughness and the precision of fit of the zirconia core was analyzed using the Pearson correlation analysis (${\alpha}$=.05). RESULTS. The zirconia cores fabricated from the scannable stone working models exhibited a superior precision of fit as compared to those fabricated from the Type IV stone working models. The correlation analysis results showed a clear positive correlation between surface roughness and the precision of fit of zirconia cores in all of the experimental groups (P<.05). CONCLUSION. The results confirmed that the surface roughness of dental working models has a decisive influence on the precision of fit of zirconia cores.

  • 지르코니아 강화 리튬 실리케이트 세라믹 재료는, 현재 널리 사용되고 있는 e.max(리튬디실리케이트 세라믹) 재료에 비하여 더 개선된 강도를 지니고 있다. 단일 크라운의 수복에 사용될 수 있으며, 1.5mm 의 두께를 확보하는 것이 예지성 있는 치료를 위해 매우 중요하다. Celtra Duo의 경우 열처리를 수행하는 것이 강도나 마모 저항성 측면에서 도움이 될 것이다. 접착을 위해서는 불산의 처리가 도움이 되며, 너무 짧은 시간의 불산은 접착 강도의 개선에 도움을 주지 못할 수 있으므로 충분한 시간의 불산 처리가 필요하다. 지르코니아 강화 리튬 실리케이트 세라믹 재료는 실험실 연구가 지속적으로 수행되고 출판되고 있지만, 아직 신뢰할만한 임상연구는 매우 부족한 실정이다. 추가적인 임상연구를 통해 과학적인 근거를 마련하는 것이 매우 중요한 부분이 될 것이다.

Analysis of osteogenic potential on 3mol% yttria-stabilized tetragonal zirconia polycrystals and two different niobium oxide containing zirconia ceramics

  • PURPOSE. This study was performed to evaluate the osteogenic potential of 3mol% yttria-stabilized tetragonal zirconia polycrystals (3Y-TZP) and niobium oxide containing Y-TZPs with specific ratios, new (Y,Nb)-TZPs, namely YN4533 and YN4533/Al20 discs. MATERIALS AND METHODS. 3Y-TZP, YN4533 and YN4533/Al20 discs (15 mm diameter and 1 mm thickness) were prepared and their average surface roughness ($R_a$) and surface topography were analyzed using 3-D confocal laser microscope (CLSM) and scanning electron microscope (SEM). Mouse pre-osteoblast MC3T3-E1 cells were seeded onto all zirconia discs and evaluated with regard to cell attachment and morphology by (CLSM), cell proliferation by PicoGreen assay, and cell differentiation by Reverse-Transcription PCR and Quantitative Real-Time PCR, and alkaline phosphatase (Alp) staining. RESULTS. The cellular morphology of MC3T3-E1 pre-osteoblasts was more stretched on a smooth surface than on a rough surface, regardless of the material. Cellular proliferation was higher on smooth surfaces, but there were no significant differences between 3Y-TZP, YN4533, and YN4533/Al20. Osteoblast differentiation patterns on YN4533 and YN4533/Al20 were similar to or slightly higher than seen in 3Y-TZP. Although there were no significant differences in bone marker gene expression (alkaline phosphatase and osteocalcin), Alp staining indicated better osteoblast differentiation on YN4533 and YN4533/Al20 compared to 3Y-TZP. CONCLUSION. Based on these results, niobium oxide containing Y-TZPs have comparable osteogenic potential to 3Y-TZP and are expected to be suitable alternative ceramics dental implant materials to titanium for aesthetically important areas.

In-vitro evaluation of marginal and internal fit of 3-unit monolithic zirconia restorations fabricated using digital scanning technologies

  • PURPOSE. This study aimed to compare the marginal and internal fit of 3-unit monolithic zirconia restorations that were designed by using the data obtained with the aid of intraoral and laboratory scanners. MATERIALS AND METHODS. For the fabrication of 3-unit monolithic zirconia restorations using impressions taken from the maxillary master cast, plaster cast was created and scanned in laboratory scanners (InEos X5 and D900L). The main cast was also scanned with different intraoral scanners (Omnicam [OMNI], Primescan [PS], Trios 3 [T3], Trios 4 [T4]) (n = 12 per group). Zirconia fixed partial dentures were virtually designed, produced from presintered block, and subsequently sintered. Marginal and internal discrepancy values (in ㎛) were measured by using silicone replica method under stereomicroscope. Data were statistically analyzed by using 1-way ANOVA and Kruskal Wallis tests (P<.05). RESULTS. In terms of marginal adaptation, the measurements on the canine tooth indicated better performance with intraoral scanners than those in laboratory scanners, but there was no difference among intraoral scanners (P<.05). In the premolar tooth, PS had the lowest marginal (86.9 ± 19.2 ㎛) and axial (92.4 ± 14.8 ㎛), and T4 had the lowest axio-occlusal (89.4 ± 15.6 ㎛) and occlusal (89.1 ± 13.9 ㎛) discrepancy value. In both canine and premolar teeth, the D900L was found to be the most marginally and internally inconsistent scanner. CONCLUSION. Within the limits of the study, marginal and internal discrepancy values were generally lower in intraoral scanners than in laboratory scanners. Marginal discrepancy values of scanners were clinically acceptable (< 120 ㎛), except D900L.

지르코니아 임플란트 지대주의 기계적 강도에 관한 연구 (Mechanical strength of Zirconia Abutment in Implant Restoration)

  • 목적: 임플란트 보철 수복에서 심미적 요구도가 증진되면서 최근에는 고강도, 고인성을 지니면서 생체적합성 우수하고 치아의 색과 조화로운 지르코니아 지대주의 사용이 증가하는 추세이다. 몇몇 임상보고를 통해 지르코니아 지대주의 우수한 주위조직 반응과 단기간의 성공적인 결과가 보고 되었으나, 장기간의 안정성을 평가하는 기계적 강도에 관한 연구는 미흡한 실정이다. 이에 본 연구에서는 직경, 각도, 연결방식이 상이한 임플란트 고정체 - 지르코니아 지대주의 단순 파절강도와 반복하중에 의한 피로한계 측정을 통해 지르코니아 지대주의 기계적 안정성을 평가해 보고자 하였다. 재료와 방법: ISO규정에 따라 지르코니아 지대주-고정체를 변연골이 3mm 흡수된 조건에서 $30^{\circ}$경사하중으로 압축굽힘강도를 측정하고, 그 값의 80%를 최대하중으로 10%를 최소하중으로 하여 10Hz의 sine형 반복하중을 $5{\times}10^6$ 주기를 초과하는 조건의 내구성한계를 측정하였다. 지르코니아 지대주로는 regular diameter를 가지면서 external butt joint의 straight 지대주와 $17^{\circ}$ angled 지대주, 그리고 narrow diameter 이면서 external butt joint의 straight 지대주와 internal conical joint의 straight 지대주를 사용하였다. 압축굽힘강도 값을 일원분산분석과 사후검정으로 검증하였으며, 피로파절면을 전자주사현미경을 통해 관찰했다. 결과: 압축굽힘강도는 최소 927N 이상으로, 직경의 차이에 따른 유의한 차이를 보였으며( P<.05), 내구성 한계는 503N에서 868N까지의 번위를 보였다. 결론: 지르코니아 지대주는 구강내 기능하중을 견딜수 있는 적절한 기계적 강도를 가지는 것으로 판단된다.

Fabrication of a CAD/CAM monolithic zirconia crown to fit an existing partial removable dental prosthesis

  • Fabricating a surveyed prosthesis beneath an existing partial removable dental prosthesis (PRDP) is a challenging and time-consuming procedure. The computer-aided design/computer-assisted manufacturing (CAD/CAM) technology was applied to fabricate a retrofitted, surveyed zirconia prosthesis to an existing PRDP. CAD/CAM technology enabled precise and easy replication of the contour of the planned surveyed crown on the existing abutment tooth. This technology ensured excellent adaptation and fit of newly fabricated crown to the existing PRDP with minimal adjustments. In this case report, a seventy-year-old male patient presented with fractured existing surveyed crown. Because the existing PRDP was serviceable, new crown was fabricated to the existing PRDP.

Comparison of the optical properties of pre-colored dental monolithic zirconia ceramics sintered in a conventional furnace versus a microwave oven

  • PURPOSE. The purpose of this study was to compare the optical properties of pre-colored dental monolithic zirconia ceramics of various thicknesses sintered in a microwave and those in a conventional furnace. MATERIALS AND METHODS. A2-shade of pre-colored monolithic zirconia ceramic specimens ($22.0mm{\times}22.0mm$) in 3 thickness groups of 0.5, 1.0, and 1.5 mm were divided into 2 subgroups according to the sintering methods (n=9): microwave and conventional sintering. A spectrophotometer was used to obtain CIELab color coordinates, and translucency parameters and CIEDE2000 color differences (${\Delta}E_{00}$) were measured. The relative amount of monoclinic phase ($X_m$) was estimated with x-ray diffraction. The surface topography was analyzed by atomic force microscope and scanning electron microscope. Statistical analyses were conducted with two-way ANOVA (${\alpha}=.05$). RESULTS. There were small interaction effects on CIE $L^*$, $a^*$, and TP between sintering method and thickness (P<.001): $L^*$ (partial eta squared ${{\eta}_p}^2=0.115$), $a^*$ (${{\eta}_p}^2=0.136$), and TP (${{\eta}_p}^2=0.206$), although higher $b^*$ values were noted for microwave sintering regardless of thickness. Color differences between two sintering methods ranged from 0.52 to 0.96 ${\Delta}E_{00}$ units. The $X_m$ values ranged from 7.03% to 9.89% for conventional sintering, and from 7.31% to 9.17% for microwave sintering. The microwave-sintered specimen demonstrated a smoother surface and a more uniform grain structure compared to the conventionally-sintered specimen. CONCLUSION. With reduced processing time, microwave-sintered pre-colored dental monolithic zirconia ceramics can exhibit similar color perception and translucency to those by conventional sintering.
