A study on the state of infection control in dental clinic (치과진료실에서의 감염관리 실태 조사)

  • The purpose of this study was to examine the state of infection control provided to members of Korean Dental Hygienists Association. The subjects in this study were dental hygienists who attended a symposium on July 1. 2006. after a survey was conducted, the answer sheets from 489 participants were analyzed, and the findings of the study were as follows: 1. Possession of disinfection room was being(72.7%), and person of infection control was zero(52.9%). Number of sterilizer was one(62.2%). 2. As a repetition choice, type of sterilizer was autoclave(97.9%), UV sterilizer(67.4%) and EO gas sterilizer(21.4%). As a repetition choice, infection materials was ethanol(84.1%). 3. Water tube of unit and chair was using of sterilized water(42.9%). Sterilizing of compressed air was no(69.0%). 4. Re-using of disposal was not using(62.5%), re-using disposal was suction tip(28.2%)(repetition choice) 5. In sterilization of instruments, hand-piece was every using time(28.4%), and reamer-file, bur, mirror, pincette, explorer, hand scaler and ultrasonic scaler were high in every using time. 6. Individual protection was high of using, cleaning of hands before treatment was every treatment(87.0%). Type of soap was liquid type in dental clinic(48.2%), infection soap in dental hospital(41.2%) and solid soap in public health center(50.6%). Answered that they need regular oral health education, and 82.9% respondents answered that they need oral health technicians in school. And 87.8% respondents needed individual oral health education for the benefit of better oral health.

  • The concept of dental clinic have been changed along the advanced dental science to meet the need of setting up high-technology-digital appliances. As the general idea about the dental clinic has been changed from systematic connection, the expansion of zoning needs to become more accurate and mote detailed. In the spacious waiting room. the interment cafe has been established to supply the information services, and the various multi-imaged channels, which were built in, will relieve the boredom of the waiting time. The formation of water space leads to the effect of humidification and makes parents share visual and auditory sympathy All of the unit-chairs attached cameras in the consultation room enables people to diagnose more quickly and precisely through the computerized system. Partitions have been set of to put it in and to help diagnosis process to be done smoothly in the formational space. The whole atmosphere is intended to be harmony with the natural surroundings in Je-Ju. It will inspire people visiting this clinic to image the deep blue sea in Je-Ju. By using projective non-material a lot, especially material wooden windows and doors, it is believed to create the bright and stable atmosphere. It will lead patients feel comfortable.

Interhemispheric Modulation on Afferent Sensory Transmission to the Ventral Posterior Medial Thalamus by Contralateral Primary Somatosensory Cortex

  • Single unit responses of the ventral posterior medial (VPM) thalamic neurons to stimulation were monitored in anesthetized rats during activation of contralateral primary somatosensory (SI) cortex by GABA antagonist. The temporal changes of afferent sensory transmission were quantitatively analyzed by poststimulus time histogram (PSTH). Mainly, afferent sensory transmission to VPM thalamus was facilitated (15 neurons of total 23) by GABA antagonist (bicuculline) applied to contralateral cortex, while 7 neurons were suppressed. However, when ipsilateral cortex was inactivated by GABA agonist, musimol, there was significant suppression of afferent sensory transmission of VPM thalamus. This suppressed responsiveness by ipsilateral musimol was not affected by bicuculline applied to contralateral cortex. These results suggest that afferent transmission to VPM thalamus may be subjected to the interhemispheric modulation via ipsilateral cortex during inactivation of GABAergic neurons in contralateral SI cortex.

Improved stress analyses of dental systems implant by homogenization technique (균질화기법을 이용한 치과 임플란트 시스템의 응력해석)

  • Homogenization technique is employed to investigate the series of stress analyses of mandible for three different types of dental implants. This technique helps to make proper material model of bone and analyze such a non homogeneous structure at the level of individual microstructural unit. The stress analyses with homogenization technique show much higher stress level in the sponge bone, compared to those of conventional FEM. It also manifested that even a minor lateral force results in crucial stresses in the dental implant system and that the macroscale model should take the shape and size after real mandible to produce reasonable solution in the analyses of dental implant systems. The shapes of dental implants simulated in this study are rectangular-cross-sectioned type, hemi-sphere rooted type, and wedge type implant. The stress states of mandible with hemisphere rooted type implant and wedge type implant show similar levels, while those with sectioned rectangular implant results in higher stresses. It is suggested that the distance between the implant tip and cortical bone be kept far enough to prevent stress concentrations in the mandible.

Marginal and internal fitness of three-unit zirconia cores fabricated using several CAD/CAM systems (다양한 CAD/CAM 시스템으로 제작된 3 본 고정성 가공의치 지르코니아 코어의 변연 및 내면 적합도 평가)

  • Purpose: This study was aimed to compare the margin and internal fitness of 3-unit zirconia bridge cores fabricated by several CAD/CAM systems using replica technique. Materials and methods: Three unit-bridge models in which upper canine and upper second premolar were used as abutments and upper first premolar was missed, were fabricated. Fourty models were classified into 4 groups (Cerasys$^{(R)}$ (Group C), Dentaim$^{(R)}$ (Group D), KaVo Everest$^{(R)}$ (Group K), $Lava^{TM}$ (Group L)), and zirconia cores were fabricated by each company. Sixteen points were measured on each abutment by replica technique. Statistical analysis was accomplished with two way ANOVA and Dunnett T3 (${\alpha}$=.05). Results: In most systems, there was a larger gap on inter margin than outer margin. In the Group K, overall fitness was excellent, but the incisal gap was very large. In the Group C, marginal gap was significantly larger than Group K, but overall internal gap was uniform (P<.05). The axial gap was under $100\;{\mu}m$ in all system. The difference between internal and external gap was small on Group L and C. However, internal gap was significantly larger than external gap in Group D (P<.05). The fitness of canine was better than second premolar among abutments (P<.05). Conclusion: The marginal and internal gap was within the clinically allowed range in all of the three systems. There was a larger gap on second premolar than canine on internal and marginal surface. In most systems, there was a larger gap on occlusal surface than axial surface.

Risk factors for postoperative nausea and vomiting in patients of orthognathic surgery according to the initial onset time: a cross-sectional study

  • Background: A high incidence (40-73%) of postoperative nausea and vomiting (PONV) has been reported following orthognathic surgery, and various risk factors have been associated with it. Identifying PONV risk factors based on initial onset time will help establish preventive measures. This study aimed to identify factors that are significantly related to PONV based on the initial onset time after orthognathic surgery. Methods: This study included 590 patients who underwent orthognathic surgery. Multivariate logistic regression analysis was performed to identify the risk factors that are significantly related to PONV. The objective variables were classified into three categories: no PONV, early PONV (initial onset time: 0-2 h after anesthesia), and late PONV (initial onset time: 2-24 h after anesthesia). The explanatory variables included relevant risk factors for PONV, as considered in previous studies. Results: Total intravenous anesthesia with propofol was a significant depressant factor for early PONV (adjusted odds ratio [aOR] = 0.340, 95% confidence interval [CI] = 0.209-0.555) and late PONV (aOR = 0.535, 95% CI = 0.352-0.814). The administration of a combination of intraoperative antiemetics (vs. no administration) significantly reduced the risk of early PONV (aOR = 0.464, 95% CI = 0.230-0.961). Female sex and young age were significant risk factors for late PONV (aOR = 1.492, 95% CI = 1.170-1.925 and unit aOR = 1.033, 95% CI = 1.010-1.057, respectively). Conclusion: We identified factors that are significantly related to PONV based on the initial onset time after orthognathic surgery. Total intravenous anesthesia with propofol significantly reduced the risk of PONV not only in the early period (0-2 h after anesthesia) but also in the late period (2-24 h after anesthesia).

Effects of school incremental dental care program (학교구강보건실 운영 효과에 관한 조사)

  • Objectives : This study was conducted to purpose the fundamental data for further developments of the school dental clinic program(SDCP) through comparing results of oral health promotion program for 4 years relating to compare oral health status of children who had experienced and had not experienced the program. Methods : The subjects were 66 children of elementary school in Busan Metropolitan City who had benefited school dental clinic program(SDCP) from 2004 to 2007 and control group was 98 children of elementary school who had not benefited school dental clinic program(SDCP). Data analysis were done with SPSS program (ver 13.0) through basic statistical data, t-test. Results : The DMF rate of elementary school students who had benefited program was 41.5% and it was lower than that of control group(52.5%), however it was not significant statistically. The DMT index of elementary school students who had benefited program was 1.81 and it was lower than that of control group(2.44)(p<0.05). The DMFT rate of elementary school students who had benefited program was 4.44% and it was lower than that of control group(7.45%)(p<0.05). Conclusions : In oder to promote oral health of children, school dental clinic in elementary school should be extended to more elementary schools as possible and the operating system should be changed to improve the efficiency. Oral health educational materials should be developed and utilized to improve educational effect.

Anticaries Effect of Ethanol Extract of Terminalia chebula

  • Background: Dental caries is mainly composed of various cellular components and is deposited around the tooth surface and gums, causing a number of periodontal diseases. Streptococcus mutans is commonly found in the human oral cavity and is a significant contributor to tooth decay. The use of antibacterial ingredients in oral hygiene products has demonstrated usefulness in the management of dental caries. This study investigated the anticaries effect of the ethanol extract of Terminalia chebula (EETC) against S. mutans and their cytotoxicity to gingival epithelial cells. Methods: The EETC was prepared from T. chebula fruit using ethanol extraction. Disk diffusion, minimum inhibitory concentration (MIC), minimum bactericidal concentration (MBC), and colony forming unit (CFU) were analyzed to investigate the antimicrobial activity of the EETC. Glucan formation was measured using the filtrate of the bacterial culture medium and sucrose. Gene expression was analyzed using reverse transcription-polymerase chain reaction (RT-PCR). Cytotoxicity was analyzed via the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide assay. Results: The antibacterial activity of the EETC was explored using disc diffusion and CFU measurements. The MIC and MBC of the EETC were 10 and 20 ㎍/ml, respectively. EETC treatment decreased insoluble glucan formation by S. mutans enzymes and also resulted in reduced glycosyltransferase B (gtf B), gtf C, gtf D, and fructosyltransferase (ftf), expressions on RT-PCR. In addition, at effective antibacterial concentrations, EETC treatment was not cytotoxic to gingival epithelial cells. Conclusion: These results demonstrate that the EETC is an effective anticaries ingredient with low cytotoxicity to gingival epithelial cells. The EETC may be useful in antibacterial oral hygiene products for the management of dental caries.

Analysis of the surface sterilization effect of disinfectants (소독제 별 표면소독 효과 분석)

  • The purpose of this study was to compare the number of bacteria before and after the use of surface disinfectants, demonstrate the bactericidal effect of surface disinfectants, and emphasize on the importance of surface disinfectants by recognizing the importance of infection control in dentistry. Chlorhexidine, hydrogen peroxide, ethanol, and chemical disinfectants are commonly used in dentistry. NaOCl was selected as the experimental group, and the bacterium test results obtained by comparing the table without surface sterilization as a control group showed that all disinfectants had an effective bactericidal effect (p<0.05). In the growth inhibition test comparing the experimental and control groups, all results were 100%, proving the effectiveness of surface disinfectants. The results showed that all surface disinfectants preferred and used by medical institutions were effective. Therefore, all surface disinfectants used in the experimental group were effective for surface disinfection for infection control. Dental clinicians should be aware of the necessity of disinfection of surfaces, such as table, chairs, and unit chairs, and make an active effort to ensure that both clinicians and patients are safe from infection.

  • The treatment of esthetic areas with single-tooth implants represents a new challenge for the clinician. In 1993, a modification of the forced eruption technique, called "orthodontic extrusive remodelling," was proposed as a way to augment both soft- and hard-tissue profiles at potential implant sites. This case report describes augmentation of the coronal soft and hard tissues around a fractured maxillary lateral incisor associated with alveolar bone loss, which was achieved by forced orthodontic extrusion before implant placement. Through these procedures we could reconstruct esthetics and function in a hopeless tooth diagnosed with subgingival root fracture by trauma.
