일부지역 치과의 기관, 감염관리자, 환자의 감염관리에 관한 연구 (The Study on Organization, Infection Controller, Patient Infection Control of Dental Clinic in Certain Areas)

  • 치과의료기관의 감염관리 수행 조사를 위해 2014년 6~7월 기간 동안 일부 지역사회 치과위생사 158명을 대상으로 기관, 감염관리자, 환자의 감염관리 수행을 자기기입식 설문조사하였다. 치과병원은 감염관리지침서 구비, 감염관리 담당자 지정, 정기감염관리 교육지원, 1인 멸균핸드피스 사용 비율은 57~74%였으나 치과의원은 13~20% 수준이었다(p<0.05). 감염관리자의 감염관리지침 인지는 치과병원 17.4%, 치과의원 5.9%였고 정기예방접종 시행은 치과병원 60.9%, 치과의원 34.1%였다(p<0.05). 환자의 손 위생 수행, 진료 전 칫솔질, 교차 감염 교육경험은 20% 미만이었고, 병력조사 협조는 90% 이상이었다(p<0.05). 감염관리 실천 상관분석 결과 기관 지원은 치과위생사 감염관리자실천, 환자감염관리협조, 개인방호실천, 진료실감염관리실천, 폐기물관리실천과 양의 상관관계를 나타내었으며 통계적으로 유의하게 분석되었다(p<0.05). 감염관리 실천을 위해 의료진, 환자, 기관의 지원과 협조가 요구되며 감염관리 관리자는 지속적인 교육을 통해 감염관리지침을 인지하고 실천할 것이 요구된다.

치과위생사의 근무환경이 감염관리활동에 미치는 영향 (Influence of working environment on infection control activities in dental hygienists)

  • Objectives: The objective of the study was to infection control by analyzing the influence of working environment on infection control activities in the dental hygienists. Methods: The cross-sectional study was based on a survey on a total of 377 dental hygienists working in dental settings. Multiple linear regression analysis was performed to examine the relationship of general characteristics and infection control activities. All statistical analyses were performed using the SPSS for Windows version 20.0, and p<0.05 was considered to be significant. Results: Predictive powers(=Adjusted $R^2$ of the investigated factors such as operation room, sterilization disinfection laundry, dental unit waterline, staff individual, infection control system, personal protective equipment, medical waste, hand hygiene, oral surgical procedures, clinical contact surfaces were adjusted $R^2=0.394$, 0.306, 0.277, 0.244, 0.241, 0.177, 0.165,, 0.154, 0.134, 0.124 respectively. Conclusions: In order to enhance infection control activities, the program development and implementation for the aseptic procedure is very important. The program should include periodic reinforcement of infection control education and regular monitoring of infection control activities.

치과에서의 감염방지를 위한 개인보호용구착용에 관한 연구 (A study on personal protection equipment for Infection control at dental offices)

  • Objectives : The purpose of this study was to minimize cross infection that can take place within dental office, to make dental workers recognize importance of infection management, and to improve dental-care environment that is exposed to infection. Methods : It conducted the self-reported questionnaire survey on the practice of wearing PPE(personal protective equipment) from April 1, 2008 to May 30 targeting 180 dental workers who are working for dental offices where are located in Jeonnam region. Results : 1. As for research subjects' general characteristics, age was the largest in under 29 years old with 75.0%. It was surveyed to be large in over 5 years(56.1%) for working career, in a city(83.3%) for working region, and in having licence of dental hygienist(75.0%). 2. As a result of surveying the practice of wearing PPE, wearing mask given dental care was surveyed to have the highest practice level with 75.6%. Wearing protective glasses showed the lowest practice ratio with 23.3%. 3. As a result of surveying the difference in the practice of wearing PPE according to general characteristics, the difference according to the working region was surveyed to be the greatest. Conclusions : As the above result, the education of infection management for dental workers needs to be performed continuously even after curriculum of school. Even the in-service education on infection management needs to be vitalized. Also, the necessity was examined for always recognizing and practicing importance of infection by using a method of publishing newsletter via post to dental clinics where are located in rural area, with considering regional deviation.

치과위생사의 유니폼 관리 실태조사 (Survey on the uniform management of dental hygienist)

  • Objectives: The purpose of the study is to investigate the uniform management of the dental hygienists in Korea. Methods: The study was carried out by direct telephone survey to dental gygienists in Gangneung, Wonju, and Seongnam from March 15 to April 10, 2015. The subjects were seventy dental clinics by random sampling. The questionnaire consisted of general characteristics of the subjects(4 items), infection control education(3 items), and infection control activity(9 items). Cronbach's alpha was 0.781 in the study. After informed consent from the dental hygienists, the survey was done by telephone. Data were analyzed by SPSS 21.0 program. Results: The dental hygienists taking infection control education tended to wash the uniforms twice a week (${\chi}^2$=15.95, p<001). The majority of the dental hygienists thought that the best washing place of uniform would be dental clinics than any other place (${\chi}^2$=38.76, p<001). Conclusions: It is very important to implement the infection control education for the dental hygienists. The establishment of standard guideline for infection control of the uniform washing will enhance the dental service quality and satisfaction of the dental service consumers.

치위생과 학생의 감염관리에 대한 인지도와 실천도 조사 (A study on recognition and practice of dental hygiene students for Infection control dental hygiene major courses)

  • The purpose of the study was to provide the basic for the prevention of infection control by analyzing the level of recognition and practice of dental hygiene students. It is important for dental clinic staffs to know and practice infection control measures in favor of effective infection control. Thus, this study conducted an one-month questionnaire survey (during October 2008) for dental hygiene students who ever experienced in 16-week clinical practice, and analyzed total 206 questionnaire forms as collected from them, so that it could determine potential associations between their recognition and practice of infection control and thereby suggest an efficient solution for infection control in dental hygiene. 1. It was found that our students' recognition about infection control averaged $4.49{\pm}.58$ points on the whole, and they were best aware of instrumental disinfection and sterilization among other measures, which was followed by hand washing, extirpation control, post-sterilization management, personal protective outfit and instrumental surface maintenance respectively. 2. It was found that our students' practice of infection control averaged $3.85{\pm}.57$ points on the whole, and they practiced hand washing most actively among other measures, which was followed by extirpation control, post-sterilization management, instrumental disinfection and sterilization, personal protective outfit and instrumental surface maintenance respectively. 3. It was found that our students scored higher mean points in recognition about every measure of infection control than those in practice (t=15.676, p=0.000). Particularly, it was notable that there were significant differences between their mean points in recognition and practice of some infection control measures, such as instrumental surface maintenance (t=15.361, p=0.000), personal protective outfit (t=15.245, p=0.000) and instrumental disinfection and sterilization (t=11.169, p=0.000). But there was least significant difference between mean points in recognition and practice of hand washing (t=5.460, p=0.000). 4. For potential associations between recognition and practice of infection control, it was found that higher recognition was in significantly positive associations with higher practice in every measure of infection control (r=.478, p=.000), such as extirpation control (r=.630, p=.000), instrumental disinfection and sterilization (r=.477, p=.000) and post-sterilization management (r=.433, p=.000). 5. It was found that there were differences in our students' recognition depending upon availability of infection control guideline (t=4.587, p=.011), and there were significant differences in practice depending on necessity of infection control education on a statistical basis (t=2.229, p=.027). Overall, it is found that our dental hygiene students are very likely to practice hand washing and extirpation control, because both of these measures are relatively easy to practice or are considered legally binding.

치과의원의 감염방지 실태 및 치위생과 학생의 B형 간염 예방현황 (Investigation of infection control in the private dental clinics and prevention of hepatitis B virus infection among the dental hygiene students)

  • Objectives: Dental personnels have high chances of exposure to various infections during many dental procedures. This study was performed to investigate the state of infection control in the private dental clinics and prevention state of hepatitis B virus infection among the dental hygiene students in Kwanju city, Korea. Methods: Questionnaires were obtained from 94 dental hygiene students who participated in dental practice in private dental clinics for more than five weeks. Results: 83.9% of dentists and 17.2% of dental hygienists routinely used the mask for treating all patients, 32.3% of dentists and 6.5% of dental hygienists routinely used the rubber gloves. The use of protective eyeware was much lower in each group. Disinfectant was used in 52.7% for sanitization of dental instruments before cleansing. The prevalence of HBsAg and anti-HBs were 3.8% and 67.1% respectively, 52.7% of dental hygiene students had history of accidental needle stick. Conclusion: Routine use of personal barrier techniques by dental personnels should be emphasized. Dental hygiene students were not properly immunized against hepatitis B virus and had high incidence of accidental needle stick. It is necessary to establish specific regulations or recommendations for cross infection control in dental practice and to performed scheduled vaccination program for hepatitis B virus for dental hygiene students.

치과 감염관리에 대한 의료소비자의 인지도에 관한 사례 조사 (A survey of cognition on infection control of the clients in dental hospital)

  • Objectives : To figure out the status of infection control (IC) in dental healthcare field and to propose the way for improvements through assessment about cognition of the clients. Methods : Study subjects were 151 patients who visited H dental health organization (DHO) of the four DHO in Busan city. The questionnaire survey performed from November 10 to December 7, 2010. Results : 1. 43.0% of consumers were interested in perception and experience in the infection control, and 38.4% were related behavioral changes in the infection control. 2. In the clients survey, the major considerations about the performance for IC were hand washing(73.5%). 3. There was significantly higher in middle-aged to think for ages infection prevention behaviors recognized as important to wear safety glasses. 4. In the confidence for dental care organization according to gender and age, appearance and outfits of staffs was the highest major consideration items. Conclusions : Infection control closely related to the clients' satisfaction and credits, therefor changing of interest, cognition and attitude for the client's IC have to be emphasized.

치과의료기관의 특성에 따른 감염관리실태 -대구, 경북지역을 중심으로- (Infection Control practices in accordance characteristic of Dental institutions -Daegu and North Gyeongsang Province around-)

  • 본 연구는 치과의료기관의 감염관리실태의 규명과 감염예방을 위해 대구, 경북지역 2012년, 2017년 2개 년도 동일한 114개의 치과의료기관의 특성에 따른 감염관리실태와 오폐수처리 및 기구소독을 설문조사하였다. 감염관리실태 8문항 평균 3.37이며 '환자의 질병병력 기록'은 95.5%로 높고 '오폐수시설이 있는 경우' 1.8%로 낮았다. 오폐수처리 및 기구소독의 9항목 평균 4.87이며 '메탈트레이 소독' 94.7%로 높고, '타구통 폐기물을 폐기물통에 보관 후 위탁처리' 17.5%, '로 낮았다. 일반적인 특징과 치과의료기관 특성에 따른 감염관리실태와 오폐수처리 및 기구소독은 2012년보다 2017년이 높았으며, 치과위생사, 대학졸업이상, 환자상담 및 관리, 병원급 이상, 치과의사 수(치과위생사 수, 체어 수, 환자 수)는 많은 경우 높았다. 총괄적으로 치과 특성을 고려하여 감염관리를 하여야 할 것이며 병원규모와 관계없이 지속적인 교육과 홍보를 필요할 것으로 사료된다.

치과감염관리 영향 요인에 대한 연구 -치과근무자의 손세척 및 장갑사용 실태를 중심으로- (A Study on Affect Factors in Dental Infection Control: Focus on Wash Hand and Put Gloves on of a Dental Step)

  • 치과위생사의 감염관리 교육, 감염관리에 필요한 보호장비의 구비 및 착용 등이 모두 치과병원장과 관련된다. 따라서 본 연구는 치과 감염관리에서 치과병원장의 중요성에 대하여 살펴보고자 하였다. 1. 치과감염관리에 대한 추가교육이 치과위생사의 94.2%가 필요하다고 응답하였으며, 교육의 주체는 치과위생사협회가 되어야 한다고 47.1%가 응답하였다. 2. 치과의사의 관심도가 높을수록(높은편이다 이상 36.7%, 낮은편이다 이하 16.0%) 치과위생사가 교육경험이 높고, 치과의사의 관심도가 낮을수록(높은편이다 이상 18.8%, 낮은편이다 이하 31.3%) 치과위생사의 교육경험이 낮은 것으로 나타났다. 따라서 치과의사의 관심은 곧 치과위생사의 감염관리 교육과 관련있음을 알 수 있었다. 그러나 통계적으로 유의하지는 않았다. 3. 치과위생사의 의료용 장갑 사용은 매환자 사용이 42.4%, 필요시 51.8%로 통계적으로 유의한(<.001)것으로 조사되었다. 치과의사는 진료전 70.7%만이 항상 손세척을 하며 필요시 22.0%, 안하는 경우도 7.3%로 조사되었다. 이 결과는 통계처리 결과 유의한 것으로 조사되었다(<.001). 4. 감염관리에서 가장 의식개혁이 필요한 사람은 치과병원장이 66.5%, 치과위생사 31.9%로 조사되었다. 5. 감염관리가 잘 되기 위해서는 병원장의 의지가 37.2%로 가장 중요한 것으로 조사되었다. 다음으로 치과위생사의 26.2%, 법적규제와 의료보험 수가에 반영이 각각 20.4%, 15.2%로 조사되었다.

  • Infection control is critical to good dental practice. To this end, it is essential that the practitioner not only understand the concepts involved in the development and applications of antimicrobial(physical and chemical) agents but also know how to use them properly in the dental operatory. Practical applications of infection control are more fully described in several of the suggested references.
