일부지역 치위생과 학생의 감염관리 인식 및 실천도에 관한 조사 (A Study on Awareness and Degree of Practice about Infection Control by Dental Hygienics Student's in Some Ares)

  • 본 연구는 치위생과 학생들의 감염관리 인식 및 실천도를 분석하고자 2013년 3월 25일부터 6월 14일까지 광주 및 전남지역의 치위생과에 재학중인 학생 324명을 대상으로 설문조사 실시하였으며 다음과 같은 결론을 얻었다. 1. 학년별에 따른 감염성 질환에 대한 인식도를 조사한 결과 2학년이 가장 높게 나타났으며, 환자대상 스켈링 경험이 있는 경우가 감염성 질환에 대한 인식도가 높게 나타났다(p<0.05). 2. 예방접종여부와 감염방지 교육경험에 따른 감염성 질환 인식도를 조사한 결과 예방접종 여부 및 감염방지 교육 경험 따른 감염성 질환 발생위험 수위의 경우 'B형 간염'이 가장 높은 비율을 보였고, '결핵', 'AIDS'가 가장 낮게 나타났다(p<0.05). 3. 연구 대상자의 특성에 따른 손씻기 실천도는 학년이 올라갈수록 중요시 생각하고 실천하는 것으로 나타났다(p<0.05). 4. 예방접종여부에 따른 손씻기 실천도는 예방접종, 감염 방지 교육경험, 환자대상 스켈링 경험이 있는 군이 높게 나타났다(p<0.05). 이상의 결과로 볼 때 감염관리교육을 경험한 학생들의 감염관리 인식 및 실천도가 높게 나타났으므로 정기적인 감염 관리 교육을 통하여 중요성을 인식시키고 실천도를 높일 수 있는 효과적인 감염관리에 크게 기여할 것으로 생각된다.

Fungal Osteomyelitis of Temporomandibular Joint and Skull Base Caused by Chronic Otitis Media

  • Chronic otitis media (COM) is a chronic inflammatory disease which affects the middle ear, mastoid cavity. It presents hearing loss, ear pain, dizziness, headache, temporomandibular joint (TMJ) inflammation and intracranial complication. Intracranial complications such as skull base osteomyelitis (SBO) may occur secondary to COM due to transmission of infection by a number of possible routes. SBO is an uncommon condition with a significant morbidity and mortality if not treated in the early stages. We report a-67-year-old male patient with diabetes and untreated COM who presented atypical severe TMJ, periorbital and postmandibular pain. By computerized tomography (CT), magnetic resonance imaging (MRI) and whole body bone scan (WBBS), he was diagnosed with SBO spreading from untreated COM via infective arthritis of TMJ. Through this case, we suggest proper utilization of diagnostic imaging, especially CT or MRI for the early detection of SBO in the case of COM accompanying with the greater risk of infection developments such as diabetes.

항응고제 투여중인 다발성 전신질환자에서 과도한 발치창 출혈부의 진정요법과 국소마취 시행하에 창상주위 봉합과 배농술 통한 출혈과 감염조절 (Bleeding & Infection Control by the Circumferential Suture & Drainage on Active Bleeding Extraction Socket under Sedation And Local Anesthesia in a Multiple Medically Compromised Patient with Anticoagulation Drug)

  • There are five principal causes for excessive bleeding in the immediate postextraction phase ; (1) Vascular wall alteration (wound infection, scurvy, chemicals, allergy) (2) Disorders of platelet function (genetic defect, drug-aspirin, autoimmune disease) (3) Thrombocytopenic purpuras (radiation, leukemia), (4) Inherited disorders of coagulation (hemophilia, Christmas disease, vitamin deficiency, anticoagulation drug-heparin, coumarin, aspirin, plavix). If the hemorrhage from postextraction wound is unusually aggressive, and then dehydration and airway problem are occurred, the socket must be packed with gelatine sponge(Gelfoam) that was moistened with thrombin and wound closure & pressure dressing are applied. The thrombin clots fibrinogen to produce rapid hemostasis. Gelatine sponges moistened with thrombin provide effective coagulation of hemorrhage from small veins and capillaries. But, in dental alveoli, gelatine sponges may absorb oral microorganisms and cause alveolar osteitis (infection). This is a case report of bleeding and infection control by the circumferential suture and iodoform gauze drainage on infected active bleeding extraction socket under sedation and local anesthesia in a 71-years-old male patient with anticoagulation drug.

  • Herpes virus family is highly infectious to patients, their families and dentists. The diagnosis of herpes infection is based on the characteristic clinical appearance and the location of the lesions. Herpes Simplex Virus(HSV) usually acquired through direct contact with infected lesions or body fluids, and the prevalence of HSV infection increases progressively from childhood. Primary infections provoke herpetic gingivostomatis typically affects the tongue, lips, gingival, buccal mucosa and palate. Recurrent infections give rise to vesiculo-ulcerative lesions at vermilion border of lip(herpes labialis). In the form of chickenpox, Varicella Zoster Virus(VZV) usually is infected in childhood. VZV spreads in the affected primary afferent nerve to the skin and produces a vesicular rash and pain. Epstein-Barr Virus(EBV) infects B cells and cause infectious mononucleosis. Latent EBV infection has also been implicated in Burkitt lymphoma, nasopharyngeal carcinoma. Cytomegalovirus(CMV) is associated with immune-compromised patient such as organ transplantation and AIDS patients.

Oral Hairy Leukoplakia in Patient with Human Immunodeficiency Virus Infection

  • Oral hairy leukoplakia occurs on the lateral surface of the tongue that clinically as an asymptomatic white lesion. It is mainly found in patient with human immunodeficiency virus infection. However, it rarely outbreak immunosuppressed patients after organ transplantation, or the patients taking steroids or immunosuppressants. It is the result of proliferating Epstein-Barr virus in the oral epithelium. Most of human immunodeficiency virus infected patients with oral hairy leukoplakia are highly contagious and possible to progress acquired immunodeficiency syndrome. Therefore, the early diagnosis of oral hairy leukoplakia is very important. Taking a thorough history and human immunodeficiency virus screening test is highly recommended in case oral hairy leukoplakia is detected. In this case, a 29-year-old man presented with whitish lesion on lateral border of tongue is diagnosed as oral hairy leukoplakia and human immunodeficiency virus infection.

Pattern of Methicillin-resistant Staphylococcus aureus in Dental and Medical Environments

  • Methicillin-resistant Staphylococcus aureus (MRSA) is one of the most prevalent pathogens in hospitals. To investigate cross contamination by this bacterium in both dental and medical settings, the pathogens that cause acute pyogenic infection and one of the major microbes responsible for nosocomial infection were isolated from health care providers, nurses and patients. We used VITEK II to measure drug sensitivity, and we further performed biochemical testing, coagulase serotype testing and pulse-field gel electrophoresis (PFGE) for isolated MRSA colonies. The isolation rate of Staphylococcus aureus from nasal swabs was 75.0% from dental health care providers and 18.8% from the medical health care providers. A total of 10 MRSA strains were isolated from 40 health care providers and 2 patients and the prevalent coagulase serotype from patients and health care providers was VII. The antimicrobial drug resistance and partial PFGE types of the isolated MRSA strains showed a similar pattern. These results suggest that MRSA may be one of the principal causes of nosocomial infection in dental and medical hospitals.

스케일링 시 에어로졸에 의한 술자의 의복 오염도 (Contamination of operator's clothing by aerosols during scaling)

  • 감염관리는 전반적인 의료행위에서 중요성이 부각되고 있으며 치과 또한 철저한 감염관리가 필요하다. 감염의 매개체에는 여러 가지가 있고, 그 중 의료진 의복으로 인한 감염이 있다. 병원 직원의 오염된 의복은 원내감염의 매개체가 될 수 있다. 이에 스케일링 시 에어로졸에 의한 의복오염도를 실험하였다. 가슴, 배, 허벅지, 소매 끝, 바지 끝 부위를 실험 대상으로 하였다. 실험한 자료는 IBM SPSS (ver20.0) 통계프로그램으로 분석하였다. 스케일링 전(대조군)과 스케일링 후(실험군) colony수의 평균값은 ANOVA (일원배치분산분석)을 사용하였으며, 사후 분석은 Scheffe를 시행하였다. 스케일링 전-후 colony수의 평균값에 대한 유의성 검증은 Paired t-test 비모수 검정방법을 사용하였다. 1. 의복의 가슴 부위에서 스케일링 전 colony수의 평균은 2.13 CFU이고, 스케일링 후 colony 수의 평균은 50.00 CFU로 약 25배 증가한 것으로 나타났다(p=0.012). 2. 의복의 배 부위에서 스케일링 전 colony수의 평균은 4.00 CFU이고, 스케일링 후 colony수의 평균은 16.63 CFU로 약 4배 증가한 것으로 나타났다(p=0.018). 3. 의복의 허벅지 부위에서 스케일링 전 colony수의 평균은 3.63 CFU이고, 스케일링 후 colony수의 평균은 22.88 CFU로 약 7배 증가한 것으로 나타났다(p=0.017). 4. 의복 소매 끝에서 스케일링 전 colony수의 평균은 3.63 CFU이고, 스케일링 후 colony수의 평균은 17.38 CFU로 약 6배 증가한 것으로 나타났다(p=0.028). 5. 의복 바지 끝에서 스케일링 전 colony수의 평균은 2.38 CFU이고, 스케일링 후 colony수의 평균은 33.63 CFU로 약 17배 증가한 것으로 나타났다(p=0.012). 6. 스케일링 후 의복 부위에서 colony수의 평균은 가슴, 바지 끝, 허벅지, 배, 소매 끝 순으로 높게 나타났다. 7. 스케일링 전-후 colony수의 증가율은 가슴, 바지 끝, 허벅지, 소매 끝, 배 순으로 높게 나타났다. 스케일링 시 에어졸로에 의해 의복이 오염되는 것을 확인하였다. 그러므로 우리는 스케일링에 의한 에어로졸로 의복이 오염되는 것을 인지하고 스케일링 후에는 의복을 깨끗이 해야 할 필요성이 있다. 이에 따라 치과기구관리와 함께 의복도 감염방지 대책이 시급한 것으로 보인다. 또한 치과 의료인들에게 자세한 감염교육이 필요할 것이다. 실험 결과로 의복오염의 심각성을 일깨우고, 의복에 대한 감염의식을 높여야 된다.

치위생과 학생들의 B형 간염 바이러스의 표지자와 인지도에 관한 연구 (A Study on occurrence rates from serologic marker of hepatitis B virus and on perception in dental hygiene students)

  • This study is aimed at offering basic data about prevention of Hepatitis B virus and infection control to dental hygiene students who will work mostly at dental offices which are characteristically exposed to a high risk of HBV infection. For this study, surveyed were 188 sophomores and juniors of the dental hygiene department who volunteered to undergo the tests of HBsAg and HBsAb. The examination of the blood collected from the samples and analysis of their perception about hygiene resulted in following conclusion: Positive HBsAg Four was found in 4 testees(2.1%) while 118(62.8%) revealed positive HBsAb. Juniors showed higher positive rate in HBsAg whereas sophomores had higher positive rate in HBsAb. But such difference has statistically no significance. Generally, students seemed to pay little attention to HBV, judging from the survey of their perception about the present state of HBsAg and HBsAb(p>0.05), conduct of infection(p<0.05), completion of 3 requested vaccinations(p>0.05), and formation of antibody(p>0.05). With regard to the infection routes of HBV, most students(92.4%) replied "through blood", which is statistically insignificant, though. Next ratio goes to the reply "through contaminated injectors". This reply came more from sophomores than from juniors, a difference which is statistically significant(p<0.05). The lowest rate of possible infection(29.2%) was thought to be "through breast-feeding of a positive mother"(p<0.05). In general, it turned out that sophomores had more knowledge about the infection routes of HBV than juniors. In terms of clinical history among family members, 6(3.1%) reported that some of their family members are currently suffering from a liver complaint, 3(1.6%) replied their family members were once afflicted, and 4(2.1%) said their members died of hepatitis. Except 10(4.7%), all the surveyees replied that their states of health are better than normal. Generally, sophomores are healthier than juniors except for the very health case, a difference which is statistically of no value.
