LABIAL APPROACH OF PULP TREATMENT AND RESIN RESTORATION ON DISCOLORED NECROTIC PRIMARY ANTERIOR TOOTH (변색된 유전치의 순측접근에 의한 치수치료 및 레진수복)

  • Traditional method of pulpectomy for a necrotic primary anterior tooth was done on lingual side. But it could not recover the discoloration of crown effectively. For the purpose of treating the discoloration of crown after lingual pulpectomy, additional methods of crown restoration were needed like : celluloid crown, open-faced crown, rasin-faced crown. Neverthless, these kinds of complete coverage methods had some disadvantages such as possibility of tooth fracture by increased tooth preparation. In order to overcome the shortcomings of lingual pulpectomy, labial treatment could be considered as an alternative. It is a method that treats necrotic pulp through the labial access opening. After finishing the pulp treatment, discolored labial tooth structure was removed extending from access opening. Discoloration of deep area could be masked effectively using opaque sealant. Cavity on labial side was restored with composite resin. This labial approach method has several advantages. First, it gives a direct vision for effective pulp treatment which is also very useful for children with poor behavior. Second, most of lingual tooth structure could be saved and occlusal contact of lingual surface remains undisrupted. Only nonfunctional discolored labial surface may removed. Third, complete removal of discolored part of a labial tooth and immediate resin restoration could be done effectively after pulp treatment. Moreover, it also could be used for pulp treatment having serious dental caries on labial surface with sound lingual tooth structure. This report presents cases with discolored upper anteior primary tooth, approaching labial side with successful restoration.

  • Cerebral palsy is an umbrella term encompassing a group of non-progressive non-contagious motor conditions that cause physical disability in human development. Motor disorder of cerebral palsy is often accompanied by disturbances of sensation, perception, cognition, communication, behavior and seizure disorder. Disharmony of motor function leads to frequent falling down. Moreover patients have high prevalence of class II malocclusion. Compared to normal patients, the patients with cerebral palsy tends to have high prevalence of recurrent trauma and bruxism which make restoration of the anterior tooth more difficult. This case report is consisted of three cases of cerebral palsy patients who have challenging problems with restoration of anterior teeth.

Analysis of Mission, Vision and Core values in Korean Tertiary General Hospitals Through Text Mining (텍스트 마이닝을 통한 상급종합병원의 미션, 비전, 핵심가치 분석 연구)

  • Purposes: This research is conducted to identify main features and trends of mission, vision and core values in Korean tertiary general hospitals by using text-mining. Methodology: For the study, 45 mission, 112 vision and 190 core values are collected from 45 tertiary general hospitals' homepages in 2022 and use word frequency analysis and Leyword co-occurrence analysis. Findings: In the tertiary general hospitals' mission, there are high frequency words such as 'health', 'humanity', 'medical treatment', 'education', 'research', 'happiness', 'love', 'best', 'spirit', and mission mainly includes the content of contributing humanity's health and happiness with these words. In case of vision, high frequency words are 'hospital', 'medical treatment', 'research', 'lead', 'trust', 'centered', 'patient', 'best', 'future'. By using these words in vision, it represents the definition and characteristics of vision such as ideal organizations in the future, goals and targets. As a result of the Leyword co-occurrence analysis, vision includes the content of 'high-tech medical treatment', 'special care for patients', 'leading education and research', 'the highest trust with customer', 'creative talents training'. -astly, the high frequency word-pairs in core values are 'social distribution', 'innovation pursuit', 'cooperation and harmony', and it defines standards of behavior for organizations. Practical Implication: To correct the problems of vision, mission and core values from findings, firstly, it needs for Korean tertiary general hospitals to use the words that can explain organization's identity and differentiate others in their mission. Secondly, considering strengthening the role of hospitals in their community and the importance of members in organizations, it is necessary to establish vision with considering community and members to activate vision effectively. Thirdly, because there are no specific guidelines of establishing mission, vision and core values for healthcare organizations, this research concepts and results could be utilized when other organizations establish mission, vision and core values.

A Basic Survey for Management of Elderly Day Care Centers (노인주간보호시설의 운영을 위한 기초조사)

  • The purpose of this study was to survey basic data for management of elderly day care centers. In this study, occupation, leisure life, and food, clothing and habituation, as well as social relationship, health state and behavior, and general characteristics of subjects were examined to identify the factors affecting their cognition, need and preference of the day care centers. The subjects of this study were 392 elderly, aged 60 years or over, living in Gangnung and neighboring districts. Data were collected by educated interviewers from November 4 through November 22, 2002. The subjects were interviewed face to face, one for one after the interviewee's agreements on the survey. The collected data were analysed with logistic regression analysis by SAS (statistical analysis system). Logistic regression analysis was done to identify affecting factors for cognition, need and preference of the elderly day care centers. The major findings are as follows: 1. The factors affecting cognition for the day care centers were analysed. The subjects with an occupation and a lower satisfaction level of living environment and friendship, who were economically secure enough to manage a sudden accident, showed a higher level of cognition of the day care centers. The subjects with a higher level of mental health state and a lower level of IADL also showed a higher level of cognition. On the general characteristics younger female subjects showed a higher level of cognition. 2. The subjects with a lower level of perceived economic condition who did not own their housing and were not economically safe enough to manage a sudden accident, had showed a higher level of need for the day care centers. It showed that the subjects with a high level of mental health state, a bad eye sight and dental condition, a good perceived health condition, and a lower level of IADL, needed the centers. 3. The subjects who had an occupation, however, not capable of making their own daily expenses, and a low occupation satisfaction level, and who did not own their housing, and were economically poor not enough to manage a sudden accident, showed a higher preference for the day care centers. The subjects with higher levels of friendship satisfaction and perceived health condition, not living with their spouse, and a higher education level, showed a higher preference for the centers. In conclusion, the common factors affecting their cognition, need and preference of the day care centers were occupation, economic security enough to manage accidents, and friendship satisfaction level. Especially, the subjects who had an occupation, however, not economically secure enough to manage accidents, and who did not live in their own housing with a good perceived health condition, showed high levels of need and preference for the day care centers. These results can be used as basic data to develop the efficient elderly day care centers, thus contribute to the elderly welfare in a local community.

Analgesic Effects of Triptolide and N-nitro-L-arginine Methyl Ester in Rat's Temporomandibular Joint Pain Model (흰 쥐의 측두하악관절 통증모델에서 Triptolide와 N-nitro-L-arginine Methyl Ester의 통증조절효과)

  • The aim of this study was to investigate whether intracisternal administrations of triptolide and N-nitro-L-arginine Methyl Ester (L-NAME) are involved in the regulation of temporomandibular joint (TMJ) pain. The TMJ pain was induced by the injection of 5% formalin ($30{\mu}l$) into TMJ capsule of rats. The pain behavioral responses was recorded the number of grooming or scratching on the left TMJ area for 9 successive 5 minutes intervals. Triptolide and L-NAME were administrated intracisternally 10 minutes before formalin injection. The intra-articular injection of formalin produced a biphasic pattern of pain response (first phase: 0~10 minutes and second phase: 11~45 minutes). The intracisternal administration of triptolide ($1{\mu}g/10{\mu}l$) and L-NAME ($0.1{\mu}g/10{\mu}l$) suppressed the TMJ pain behavior in each experiment. Co-administration of two drugs was shown the enhanced effect than the analgesic effect by single-administration of triptolide ($1{\mu}g/10{\mu}l$). The triptolide could be a useful analgesic agent for the treatment of TMJ pain, and it is expected to reduce the substantial amount of it via co-administration of synthetic chemical compound and natural products.

Research on the Smoking Conditions of College Students and the Behavior and Awareness of Oral Hygiene (일부 대학생의 흡연 실태와 구강건강에 관한 인식 및 행태 조사연구)

  • This study was conducted to investigate the differences between smokers and non-smokers' oral hygiene and college students' smoking awareness. The survey was carried out based on the 260 college students enrolled in four-year universities located in Kyungbuk, Daegu, Gangwon-do. Collected date used the SPSS 17.0 to perform the frequency and cross-analysis. 1. The daily smoking amounts of less than half pack and the smoking duration of more than 1 year and less than 3 years were to be the highest. The initiation of smoking was triggered by the curiosity and the proximal social environment. The reason of smoking was to relieve stress and habitual smoking respectively. 2. There was a significant difference between smokers and non-smokers' tooth brushing time, regular dentist visit, recent dental admission, the purpose of dental visits, scaling and oral health education. 3. There was a significant difference between smokers and non-smokers' awareness of oral hygiene in the subjective oral hygiene status. 4. There was a significant difference between smokers and non-smokers' awareness towards the impact of second-hand, after meal and one or two cigarette smoking. Based on the above results, smokers lack the awareness of smoking and oral hygiene compared to non-smokers. Therefore, the activation and the development of programs for the smoking and oral hygiene education targeting college students are considered to be necessary.

Smoking rate of Electronic Cigarettes and its related factors within the last one month (최근 한 달 이내 전자담배 흡연율 및 관련요인)

  • The study was conducted to identify the relationship between the e-cigarette smoking rate and demographic factors, health status and health behavior factors within the last one month of adults over 19 years of age, and to provide basic data for cessation education and health policy establishment to reduce the e-cigarette smoking rate. This is a secondary data analysis study using survey data of 22,908 people over the age of 19 who participated in the 2018 Community Health Survey in Seoul and the SAS program was used for analysis. Smoking rate of e-cigarettes within the last one month was 4.5%, and as a result of multivariate logistic regression analysis, factors related to e-cigarette smoking were sex, age, education level, occupation and general cigarette smoking status. In order to prevent e-cigarette smoking and reduce the smoking rate, education should first be given to professionals with educational backgrounds of college graduates or higher to inform the dangers of e-cigarettes. In addition, public relations and campaigns to inform the general public that e-cigarette smoking is no different from general cigarette smoking are required, and deployment of the same anti-smoking policy that treats e-cigarettes as general cigarettes is required.

Comparative Study on the Actual Conditions about Hypertension and Diabetes Case Management of the Elderly at the Hall for the Aged and the D Senior's College (D 노인대학과 경로당 노인들의 건강행태 및 고혈압당뇨병 관리실태 비교조사)

  • The purpose of this study was to provide the basic data for effective intervention of oral health behaviors strategy and to compare the actual conditions about hypertension and diabetes case management of the elderly at the hall for the aged and the D senior's college. The research method was a questionnaire including hypertension and diabetes case management of the elderly and the subjects were 174 of the elderly(65 age over) at the hall for the aged(100) and the senior's college(74). The results of this study were as follows; 1. Hypertension 1)The incidence of hypertension of elderly at the hall for the aged and the senior's college were 32.2%. 2)83.9% of the hypertension cases were initially diagnosed during hospital examination(p < 0.05). 3)Regular blood pressure checks were performed more than one time monthly on 76.8% of the cases(p < 0.05). 4)Blood pressure control was well controlled on 75%(p < 0.05). 5)85.7% of the elderly at the hall for the aged took hypertension drugs daily and 42.9% of the elderly at the senior's college took no drug alternatively(p < 0.05). 2. Diabetes 1)The incidence of the diabetes of elderly at the hall for the aged and the senior's college were 14.4%. 2)80.0% of the diabetes cases were initially diagnosed during hospital examination(p < 0.05). 3)64.0% of the cases did not have blood sugar measuring instrument(p < 0.05). 4. In the quality of life, the thinking of no difficulty in walking and no anxiety/depression was more presented on the elderly at the senior's college than those at the hall for the aged(p < 0.05). 5. The subjective health condition scores were higher on the elderly at the senior's college than those at the hall for the aged(p < 0.05).

Analgesic Effect of Hippophae rhamnoides Extract in Orofacial Pain in Rats (구강안면통증모델에서 산자나무 추출물의 진통효과)

  • Hippophae rhamnoides L. (sea buckthorn) is a shrub wood that belongs to the bamboo tree family, and is rich in vitamin C, D, and E; it is referred to as a vitamin tree. It is mainly grown in the high mountains of Europe and Central Asia, and has been widely used in China and Russia as natural medicine. Recent studies have shown that it is effective in the treatment of cancer, liver diseases, cardiovascular diseases, and gastrointestinal diseases. However, results of studies on its effect on the regulation of pain are insufficient. In this study, we investigated the effect of sea buckthorn on the development and control of pain in two facial areas. The experimental animals included 7- to 8-week-old Sprague-Dawley rats (240~260 g). Formalin (5%), which is known as an inflammation inducer, was injected into the vibrissa pad or temporomandibular joints to induce orofacial acute pain. Rubbing or scraping of the region injected with formalin was regarded as a pain index, and the behavioral response was observed for 45 minutes after the injection. Sea buckthorn extract diluted to 150, 300 mg/kg (in 1 ml of distilled water) was orally administered 30 minutes prior to the acute pain. The facial pain behavior was effectively reduced in the 300 mg/kg group when compared to the control group (vehicle). Likewise, in an experiment in which formalin was injected into the temporomandibular joints, effective pain alleviation was confirmed at the same drug concentration. These results suggest that sea buckthorn extract may be useful in the development of therapeutic agents for acute inflammatory pain in the orofacial area and for controlling temporomandibular joint pain.

A Study on Subjective Symptoms of a Oral Malodor in Korean Adults (한국 성인의 주관적 구취 자각증상에 대한 연구)

  • The purpose of this study was to investigate the actual conditions of the subjective oral malodor perception of the survey subjects. Through the subjective questionnaire method over about 3 months from June to August in 2007, the data was collected targeting 275 adults. Statistical methods used were Chi-square test, Fisher's exact test and One-way ANOVA. And, the following results were obtained. 1. When having seen the ratio of those who were conscious of oral malodor, the adults, who responded with saying of feeling oral malodor at present and at ordinary times, accounted for 68.4% and 79.3%, respectively. And, the adults, who responded with saying of not feeling, were indicated to be 31.6%(at present) and 20.7%(at ordinary times), respectively. 2. Almost 83.5% of total people answered that they had most severe oral malodor at the time of awakening and 51.9% of subjects answered tongue contributed to the their oral malodor. 3. About the type of oral malodor, although 37.2% of the group thought the type of oral malodor was a food smell and fetid smell was second general burning smell occurred the time of hunger(22.5%). 4. In the answers of hoping for treatment of oral malodor, 66.9% of the people showed positive response to dental treatment for controlling their oral malodor. Especially 7.3% of subjects showed very big desire for treatment. 5. In the level of oral malodor that is felt now according to the time of a meal, the case of within 1 hour after having a meal was responded to be not felt the oral malodor with 44.7%. And, there was response with saying of not feeling oral malodor in 37.6% as for 1-2 hours, 16.3% as for 2-3 hours, 14.8% as for 3-4 hours, and 23.7% as for more than 4 hours. Thus, the level of oral malodor according to the time of a meal showed the statistically significant difference(p<0.01). Through the results of this study, it could be known about whether or not being conscious of oral malodor in general people, the main cause and region for oral malodor, the importance and practicing method of tongue brush through literature. Accordingly, there is necessity of further researching and comparing whether or not being conscious of oral malodor by diversely surveying relevancy with factors of causing oral malodor based on the objective oral malodor measurement value. The future diverse researches are thought to be necessarily performed in order to prevent or reduce oral malodor.
