Halitosis and Related Factors among Rural Residents (농촌지역 주민들의 구취실태와 유발요인)

  • This study was conducted through an interview process in which questionnaires were administered to 293 people. The questionnaires related to the behaviors of oral hygiene care, and disease history related to halitosis, and status of halitosis, halitosis measurement, oral examination, and caries activity tests such as the snyder test, Salivary flow rate test, and Salivary buffering capacity test. Our sample was taken from 293 rural residents within the period from 4th to 21st of January 2006. This was done in order to provide basic data to prepare both policies of halitosis prevention and a device to efficiently measure halitosis status and investigate the factors related therein. The major findings of this study results are as follows: 1. As for frequency of tooth brushing, twice a day occupied the greatest portion at 46.1 % Women exceeded men in frequency of tooth brushing. Tongue brushing everyday produced a 25.6 % result among subjects and The use of auxiliary oral hygiene devices occupied 9.2 %. 2. As for degree of usual self-awareness of halitosis: 62.5 %. This result also demonstrate that the severest time of self-awareness in regards to halitosis is wake up time in the morning. The time period produced the highest portion of 72.7 % in times of self-awareness. In terms of the area in which halitosis was observed, gum resulted in 23.0 %. As for types of halitosis, fetid smell was the most frequent at 37.2 %. 3. As for the result of halitosis measurement, values of OG less than 50 ppm occupied 54.3 % and $50{\sim}100ppm$ occupied 41.6 %. As for $NH_3$ values, $20{\sim}60ppm$ showed the highest value range of 52.6 %. 4. As for OG per disease history related to halitosis, values of OG were significantly high in the ranges of $50{\sim}100ppm$ within family history groups of food impaction by dental caries, diabetes mellitus and halitosis. As for values of $NH_3$, there showed a significant difference in respiratory system disease groups. 5 Value range of OG per ordinary halitosis self-awareness degree: values ranging less than 50 ppm were recorded at 55.9 % from the group realizing not aware of smell. 57.5 % from groups only realizing sometimes, while values range of $50{\sim}100ppm$ were recorded at 52.0 % from groups always aware of smell. 63.6 % from groups always strongly aware of smell. Meanwhile as for the values ranges of $NH_3$, $20{\sim}60ppm$. they occupied high portions for all groups of exams. 6. Values of OG per oral examination: the more pulp-exposed teeth and food impaction and the higher the tongue plaque index, values of OG increased within the range of $50{\sim}100ppm$. As for values of $NH_3$, the more prosthetic teeth and the higher the tongue plaque index, this value increased significantly, and the values increased up to no less than 60 ppm for groups of mandibular partial denture. 7. Within the realm of caries activity test: as for the Snyder test, high activity was highest by 43.0 % wherewith the higher the activity of acidogenic bacteria the higher the OG values. As for the salivary flow rate test, the number of cases below 8.0 ml showed the highest tendency by 62.5 %. The larger the salivary flow rate the more decreased OG values distribution. As for the salivary buffering capacity test, $6{\sim}10$ drops of 0.1N lactic acid showed the overwhelming trend by 58.7 % whereby the higher the salivary buffering capacity the greater distribution occupancy ratio of OG values below 50 ppm which is scentless to on ordinary person. 8. As for the correlation between oral environment and halitosis, OG showed the positive correlation with pulp exposed teeth, filled teeth, present teeth, tongue plaque index, and food impaction, while the negative correlation with salivary flow rate and prosthetic teeth. $NH_3$ showed a positive correlation with prosthetic teeth and frequency of tooth brushing, while decayed teeth was negative correlation. 9. As for the multiple regression analysis result, there have been selected female, pulp exposed teeth, prosthetic teeth, food impaction, salivary flow rate, tongue plaque index and severe activities in the Snyder test as factors affecting OG wherein explanatory power on it was 45.1 %. There have been selected females, pulp exposed teeth, tongue plaque index, and prosthetic teeth as factors affecting on $NH_3$ wherein explanatory power on it was 6.6 %. With the aforementioned results in mind, the status of halitosis among rural residents is considered to bare a close relation with oral environments and other factors related to halitosis such as the Snyder test from caries activity test, and salivary flow rate test. For the prevention of halitosis of residents in rural areas, we have to focus on correct tooth brushing methods and tongue brushing, with using auxiliary oral hygiene devices to remove fur of tongue plaque and food impaction. Also, when the cause and ingredients of halitosis are diverse and complex, in order to analyze exactly the factors of individual halitosis development, we need continuous and systematic study in order to provide rural residents with programs of oral hygiene education and encourage the use of dental hygienists in public health centers.

A Study on the Relationship of Sociological Characteristics to Oral Health Status in Population International Marriage Migrant Women (국제결혼 이주여성의 인구사회학적 특성과 구강건강상태와의 관계 연구)

  • The purpose of this study is to figure out and establish the basic data that can help improve management of oral healthcare for the international marriage migrantwomen. In this sense, we surveyed questions on 237 women out of 1,300 immigrant women, who have participated the program in operation by multi-cultural household supporting center, in a fashion of face-to-face investigation and on-the-spot direct cavity inspection simultaneously from May 1th to October 31th, 2010. Collected data were electro-statistically computerized under SPSS 17.0 program and analyzed with frequency analysis, recurrence analysis and logistic regression analysis respectively. Foundings were revealed as follows; On the nationality base, show the data in the ratio of 38.8% for Vietnamese, 29.1% for Philippine, 12.2% for Chinese and 6.8% for Japanese respectively. Current oral cavity status shows in the ratio of 60% with carious tooth symptoms, 40% without carious tooth symptom. Philippine women have irregular sets of tooth in many a case. (i.e., in short of numbers of teeth, due to removing individual tooth.) Japanese ladies are indicated to have less irregular sets of tooth, in comparison with that of the other immigrants from overseas. And lastly, high-income immigrant women in general were found having less numbers of unhealthy sets of tooth. Getting older, it appears that decayed tooth symptom is on the tendency of getting less in the field of dentistry circles. Immigrant ladies living with husbands under higher education background usually are found living a life in the less ratio of having toothache. A full-time immigrant housewives, however, are living everyday life in more times of suffering with tooth disease.

A Study on the Verification of the Profile of Seo구s Elderly Stress Scale (SESS) (노인 스트레스 측정 도구(SESS)의 신뢰도 및 타당도 검증 연구)

  • The purpose of this study was to verify the use of Seo's Elderly Stress Scale (SESS), which was developed in 1996. Through the modified tool, it is possible to examine the stress of Korean elders and to contribute to the welfare of them. The subjects were 350 elders over 65 years old who live in Seoul, Kwang-Ju, Yang-Ju Gun Kyung-ki Do, Ui-Jong Bu, and Young-Am Kun, Jeun-Ra Nam Do. the data of 331 elders (94%) were analyzed. Data were collected between January and March in 1996 and analyzed using the SPSS Win 8.0. The result are as follows: 1. Items with low correlation with the total items were removed. So 27 items were removed and 37 items remained. This 37 items were death in the family and/or close friends, family member's behavior not meeting expectations, marriage of daughter, marriage of son, friction with daughter- in-law, argument among children, children refuse to live with parent, children leaving home, sex injury or accident, in frequest visits from children and grandchildren, providing care for your daughter or daughter-in-law post-partum, decrease in decision making and authority in home, Lunar new year and the harvest featival, house sitting, working in the house, performing a sacrificial rite, missed birthday, not living with the eldest son, decreased eyesight, decreased strength, decreased memory, sleep pattern changes, thoughts about death, loneliness, decreased hearing, change of dental condition, change in your diet or eating style, difficulty in self care, moving because of disease or aging, argument with friend or neighbour, travel, dealing with the procedure of heritage, loss of money or property, not enough pocket money, hearing on elderly neglect in television or radio, hope of going home and ignorant from others. 2. Overlapped items were discussed by colleagues and were modified. 'marriage of daughter' and 'marriage of son' were modified in 'marriage of children'. 'self injury or accidents' and 'family accidents' were modified in to self or family accidents. 3. Factor analysis was done in order to identify validity and three factors were obtained from the result. The first factor familial relation area, included 17 items. The second factor, physical area, included 9 items. The third factor, psycho-socio-economic area, included 9 items. Cronbach coefficient alpha for the 35 items was .923. 4. Pearson's correlation was .704 between SESS and SOS (Symptoms of Stress) in order to confirm construct validity. Based on the result, the following is suggested; 1. The modified SESS needs to be reverified with elder. 2. Korean elder's health promotion can be made by development of stress intervention which was accurately measured with SESS.

  • When an implant-supported maxillary full-arch fixed prosthesis is planned for patients with the horizontal and vertical bone resorption induced by periodontal disease, it is necessary to consider the masticatory function, esthetics and phonetics when placing implants. For this reason, thorough clinical and radiological diagnosis is necessary. Extensive bone and soft tissue grafting may be required as well. Since there is no clear guideline for proper number of implants, segment or splinting of substructure and method of retaining prosthesis, these should be considered during diagnostic process. This clinical report describes a patient who has experienced several tooth extractions and periodontal treatment due to severe periodontitis on maxilla and mandible. With bone and soft tissue graft before dental implant placement, the patient have satisfactory result in esthetic and functional aspect with the implant-supported maxillary full-arch fixed prosthesis opposing mandibular natural dentition.

  • To examine whether HSV, VZV, H. pylori and Candida that are known to be microorganisms causing ulcerative disease in oral cavity and have the relatively high contigiousness are detected in saliva of patients with RAU and related to the development with RAU, PCR and culture were performed on the saliva of 29 patients with RAU and 29 control subjects who visited the Department of Oral Medicine, Dental Hospital, Chosun University. The results were obtained as follows; 1. HSV DNA was detected in 41.4% patients with RAU, and 55.2% control subjects, however, a significant difference between the two groups was not detected, (P>0.05), and VZV DNA was not detected in both groups. 2. H. pylori DNA was detected in 27.6% patients with RAU, and 48.3% control subjects, however, a significant difference between the two groups was not detected (P>0.05). 3. Candida was cultured in 13.8% patients with RAU, and 6.9% control subjects, however, a significant difference between the two groups was not detected (P>0.05). This results suggest that HSV, VZV, H. pylori and Candida can not be regarded to play a direct role in the development of RAU. Thus it is considered that in future, on a larger sample, also, it has to be examined whether other microorganisms acts as a trigger factor of the development of RAU.

Detection of Collagenase in Inflammatory Gingiva using Root planning and Argon Laser (치근면 활택술과 아르곤 레이저 사용에 따른 염증성 치은의 교원질 분해효소 검출 비교)

  • The major cause of periodontal disease is microorganism in the dental plaque. Gingival sulcular fluid, which is exudate released from the tissue near crevicular epithelium is related with inflammation. The purpose of this study was to evaluate the argon laser efficiency between the clinical index and onset of collagenase of gingival sulcular fluid. Material divided 16 patients into 4 groups. The first control was without treatmemt. The second was with just treatment of argon laser, The third was treated by scaling and root planning and the fourth was treated with both scailing and root planning and argon laser. The level of periocheck test, the index of bleeding, and the depth of periodontal pocket were evaluated from for 128 teeth of 64 anterior teeth and 64 posterior teeth. The results were as follows ; 1. In the score of periocheck test, root planing group(group 3) was significantly reduced more than the group without treatment(group 1) and the argon laser treatment(group 2) for results of 3 days and 7 days. But root planing plus argon laser treatment(group 4) in the 7days after experiment, was significantly reduced than no treatment(group 1) and root planing treatment(group 3)(P<0.05), in the 3 days after experiment, was significantly reduced than root planing(group3)(P<0.05). The score of periocheck test to the root planning group(group 3) were significantly reduced between days1, day3 and day7(P<0.05). Root planning plus argon laser group(group 4) were significantly reduced to 1 or 7days and 3 or 7days(P<0.05). The argon laser group(group 2) didn't show any changes. 2. In the case of sulcus bleeding index, the root planning group(group 3) and root planning plus argon laser group(group 4) were reduced more than without treatment group(group 1)(P<0.05) and sulcus bleeding index in the root planning group(group 3) were reduced more than the argon laser group(group 2)(P<0.05). 3. There wasn't any changes of pocket depth between the control and the experiment group as with experiment periods also.

A Study of the Health Interests of Women's College Students in Inchun and Ansan (여성 건강교육을 위한 건강흥미 조사연구 - 일부 여자 대학생을 중심으로 -)

  • The purpose of this study was to make a comparative analysis of nursing students interest in health matters concerning themselves and compare it to non-nursing students. The results were to be categorized and used as health content recommendation when organizing health curriculum and planning health education programmes. The instruments used this study were 'The Health Inventory and The Health Need Inventory introduced to 80 items from 125 items by Lee in Korea(1980). There was a very high correlation between the 125 items and condensed 80 items (r=0.9508, t=11.0643. p<.001). The convenience sample was chosen from 226 nursing students and 204 non-nursing students in the departments of humanities of college in Inchun and Ansan from March 4 to April 5. 1996. Data were analyzed by the SAS computer Program. The results are as follow: When comparing health interest of nursing student group with non-nursing student group, total group mean of nursing students was significantly higher than that of non-nursing students (t=2.977 p<.01). But the rank order of the nursing student group was similar to that of the non-nursing group. and what nursing student felt interested was coincident with what non-nursing students felt as being necessary. The most interested health area between nursing student and non-nursing students were ${\ulcorner$personal hygiene & grooming${\lrcorner}$ ${\ulcorner$weight control${\lrcorner}$, ${\ulcorner$sex education${\lrcorner}$ ${\ulcorner$mental health${\lrcorner}$. On the other hand the least interested health areas were. ${\ulcorner$smoking and health${\lrcorner}$ ${\ulcorner$alcohol and health${\lrcorner}$, ${\ulcorner$drug and narcotics${\lrcorner}$ In non -nursing students but in nursing students ${\ulcorner$structure and function of body${\lrcorner}$, ${\ulcorner$dental health${\lrcorner}$ ${\ulcorner$safety${\lrcorner}$. Comparing students health interest by major In college, total group mean of nursing students is significantly higher than that of non-nursing students in 14 health areas including ${\ulcorner$community health${\lrcorner}$, ${\ulcorner$control and prevention disease${\lrcorner}$ ${\ulcorner$consumer health${\lrcorner}$ ${\ulcorner$family health${\lrcorner}$ ${\ulcorner$mental health${\lrcorner}$ ${\ulcorner$personal hygiene and grooming${\lrcorner}$ ${\ulcorner$safety${\lrcorner}$ ${\ulcorner$structure and function of body${\lrcorner}$, ${\ulcorner$sex education${\lrcorner}$ ${\ulcorner$alcohol and health ${\lrcorner}$ ${\ulcorner$ drug and narcotics${\lrcorner}$ and ${\ulcorner$smoking and health${\lrcorner}$ The following recommendations could be made considering the above mentioned factors: 1. The priority of health content should reflect the items to the highest interest area of students and highest need area of adults. 2. The health items which were significantly different in the area of health interest, by major, should be used in planning health education programmes and organizing health curricula.

  • Neurofibromatosis is known as an autosomal dominant disorder caused by a mutation of a tumor suppressor gene on the long arm of chromosome 17 that affects the bone, nervous system, soft tissue, skin, and endocrine system. The most characteristic finding, which is helpful for clinical diagnosis as well, is the neurofibroma. Likewise brown macules called Cafe-au-lait spots with color of caffee latte, and Lisch nodules found around iris are useful to diagnose the disease. As known, the possibility of the neurofibromatosis occurred in oral cavity is relatively rare, and in most of cases it is related to soft tissue changes with single or multiple neurofibromatosis. The purpose of this report is to present characteristic dental findings which were found in a 4-year-old male and his father, both diagnosed as neurofibromatosis at Chonbuk National University Hospital before visiting our department, pediatric dentistry.

Development of School Health Nursing Phenomena in Korea by Retrospective Method of ICNP (ICNP의 후향적 개발방법에 의한 한국의 학교간호현상)

  • The objectives of this study were to identify the phenomena of school health nursing at schools in Korea and to contribute to building a school health domain of International Classification for Nursing Practice. A retrospective method was used in this study to develop ICNP during the period from July to October 200l. The procedure of the study involved choosing nursing phenomena using preliminary terms from the reports on the field of school health nursing practice documented by nursing students in 10 different nursing colleges. The detail procedures of the study were as follows. 1) Choosing nursing phenomena by using preliminary terms 2) Choosing the characteristics of school health nursing practice from the selected nursing phenomena 3) In order to make a consensus regarding the appropriate characteristics of phenomena. 15 study group members re-categorized the nursing phenomena through 5 times of cyber meetings and 3 times of formal meetings. 4) To verify each characteristic, 5 community nursing faculties and 25 school health nurses participated in the procedure to give scores on nursing characteristics. 5) Classification of the definite nursing phenomena and characteristics. Following the 5 step procedures, school health nursing phenomena were categorized into human and environmental domains. Human domains were classified into human behavioral and functional domains. Environmental domains were classified into physical and psychosocial domains. The essential characteristics of each phenomena were selected when it obtains the mean score of 3.0 or over at the related characteristics. The human behavioral domain consisted of 7 phenomena including risk for spinal disorder, inadequate dietary habit, inadequate weight control, smoking and substance abuse, inadequate stress management, inadequate sex related coping strategies and inadequate accident management. The human functional domain consisted of 6 phenomena including inadequate eye care and visual management, risk for respiratory disorder, inadequate dental health care, inappropriate infectious disease control, risk for gastrointestinal disorder, and lack of sexual identity. The physical environmental domain consisted of 6 phenomena including risk for incident at inside classroom, risk for incident at outside classroom, risk for incident around school, risk for exposure to hazardous facilities around school. inadequate garbage and disposal management, and inadequate physical environment for learning. The psychosocial domain included impaired social interaction at school. Each phenomenon was composed of 2 to 8 characteristics and all phenomena will include a total number of 85 characteristics. The phenomena of school health nursing in Korea partially confirmed school health architecture of ICNP. Further study on verification of school health nursing phenomena in Korea needs to be done to support the findings of this study through review of literature on nursing classifications or field studies.

A retrospective analysis of risk factors of oromaxillofacial infection in patients presenting to a hospital emergency ward

  • Background: The purpose of this study was to review the clinical features of oromaxillofacial infections in patients presenting to a hospital emergency ward, to identify the key factors affecting the requirement for hospitalization, and the potential risk factors predisposing to a prolonged length of hospital stay. Methods: A retrospective medical record review of the 598 patients treated for oromaxillofacial infection from 2013 to 2017 at the oral and maxillofacial surgery department, Yangsan Pusan National University Hospital, was conducted. The following information was collected from each patient: sex, age, past medical history, site of infection, etiology, admission or outpatient care, level of C-reactive protein (mg/dL), fascial spaces involved, treatment method, and duration of hospitalization. Chi-squared tests were used to identify risk factors, which were further analyzed using multivariable logistic regression. Results: A total of 606 patients were eligible for inclusion in the study, of which eight were excluded due to having incomplete charts; thus, 598 patients were included: 55% were male, mean patient age was 47.1 ± 19.9 years, and 12.9% of patients were diabetic. Furthermore, 71.2% of patients had infection originating in the mandible; the most common tooth of origin was lower posterior, and 29.8% of patients were hospitalized. Risk factors for hospital admission were elderly patients with concurrent disease, elevated C-reactive protein level, and multiple-space infection in the oromaxillofacial area. The duration of hospitalization was correlated with both diabetes and age. Conclusions: The requirement for hospital admission is determined by the severity of the infection; even severe infections, once treated with appropriate surgery, have no relation to the length of hospital stay. The important risk factors for increased duration of hospitalization are diabetes mellitus and older age. The understanding of risk factors associated with a prolonged hospital stay during the treatment of oromaxillofacial infection will aid in treatment planning as well as highlight the importance of adequate diabetes control in patients at risk of such infection.
