The management effect of TDC(Total Dental Care) internship program (일개 대학 치위생과 대학생들의 TDC(Total Dental Care) 인턴십 프로그램의 운영효과)

  • Objectives : The purpose of the study is to identify the efficiency of total dental care(TDC) internship program. Methods : Dental hygiene students participated in the total dental care(TDC) internship program for 4 weeks in dental clinics practice. A self-reported questionnaire was filled out from June to July in 2012. The questionnaire consisted of oral health condition and dental care services by the students, dentists, and dental hygienists after TDC internship program. Results : Satisfaction degree of the students was $3.92{\pm}0.68$ points. The patients satisfied with TDC internship program and it was helpful to their dental health care(4.0 points). There were significant changes in tooth brushing method, frequency and O'Leary plaque index. Satisfaction degree of the dentists was 4.09 points and that of the dental hygienists was 3.80 points. Conclusions : TDC internship program is very helpful for the students not only to play an outstanding role in their dental care activities after employment but also to establish the real identity of the dental hygienist.

Oral health care of the elderly in long-term care facilities in Korea (요양보호사가 입소노인에게 제공하는 구강관리실태)

  • Objectives : The aim of this study was to assess the oral health care of the elderly in long-term care facility on caregivers' behaviors. Methods : Cross-sectional study in a cluster sample of 171 caregivers recruited from 17 facilities located in the Province of Gangwon. The Questionnaire was consisted of 16 items contained oral hygiene care, denture care, oral health education and general characteristics(Chronba's ${\alpha}$=0.87). Using SPSS WIN 12.0, descriptive statistics and chi-square test were conducted to examine the subjects general characteristics, the status of oral health education and denture care. Results : 1. The mean age of care givers was 42 years and 88.9% education experience rate was found. 2. Above 70% of them was found in daily oral hygiene care after every meal. They used toothbrush and toothpaste, they keep the toothbrush properly. 3. The denture care was conducted by most of care givers, containing proper storage. But three of ten care givers was cleaned denture by toothpaste or used water only. It was severe at the care givers didn't received oral health education(p<0.05). Conclusions : The contents of oral health education for care givers should contain the denture care(time and the reason shouldn't use toothpaste). Further large-scale longitudinal studies are needed to determine professional oral health care and to develop evidence of the dental hygiene practice for the elderly in long-term care facilities.

The relationship between precarious work and unmet dental care needs in South Korea: focus on job and income insecurity (한국 노동시장 불안정성과 미충족 치과의료의 관련성: 고용과 소득 불안정성을 중심으로)

  • Objectives: The aim of this study was to analyze the accessibility of dental care services among individuals with precarious employment in South Korea. Methods: We used the $9^{th}$ wave of the Korean Health Panel data (2015) and included 7,736 wage and non-wage earners in our study. We determined precariousness in the labor market as a combination of employment relationship and job income, and categorized individuals based on this into the following four groups: Group A comprising those who report job and income security, Group B comprising those who experience job insecurity alone, Group C comprising those who report a stable job but low income, and Group D comprising those who experience both job and income insecurity. Accessibility to dental care services was determined by experience of unmet dental care needs and unmet dental care needs caused primarily by financial burden. Logistic regression analyses were used to assess the effect of precarious work on access to dental care services. Results: Individuals with job insecurity (Group B; OR=1.445; 95% CI=1.22-1.70) and both job and income insecurity (Group D; OR=1.899; 95% CI=1.61-2.24) were more likely to have unmet needs than the comparison group. Both groups B and D were also 2.048 (95% CI=1.57-2.66) times and 4.435 (95% CI =3.46-5.68) times more likely, respectively, to have unmet dental care needs caused by financial burden. Education status, health insurance, and health status were all also effective factors influencing unmet dental care needs. Conclusions: Unstable employment and low income resulted in diminished access to dental care services. Therefore, governments should consider health policy solutions to reduce barriers preventing individuals with employment and income instability from accessing adequate dental care.

The practices of dental implant maintenance care in dental service consumers according to their knowledge and attitude (지식, 태도에 따른 치과의료소비자의 임플란트 유지관리 실태)

  • Objectives: The purpose of this study is to understand the practices of the dental implant maintenance care according to knowledge and attitude toward dental implant in the dental service consumers. Methods: T-test, ANOVA, and cross tabulation were carried out to understand the knowledge and attitude toward the dental implant, the experiences of the treatments, and the practices of their maintenance care depending on the general characteristics and the oral-health education experience. The collected data were analyzed using by SPSS Windows Program 23.0. Results: The actual state of the implant maintenance care was revealed to be high in the use of oral care products with 83.9% and in the professional maintenance care with 86.0%. In terms of the implant-related experiences, the participation and the participation frequency in the professional maintenance care were resulted to be higher especially in those with more cases of surgical procedures and in those with more failure experiences. Examining the practices of management according to knowledge and attitude toward dental implant, the higher in knowledge and attitude led to the higher uses of oral care products. The periodically professional maintenance care was indicated to be received even if being taken high management cost. Conclusions: Effective education methods and programs are necessary to be developed and executed so that information and knowledge can lead the correct practices in the dental service consumers.

Study on unmet dental care needs in postmenopausal women: the 7th Korea National Health and Nutrition Examination Survey (폐경여성의 미충족 치과의료에 관한 연구: 제7기 국민건강영양조사를 바탕으로)

  • Objectives: In this study, raw data from the 7th (2016-2018) of the Korea National Health and Nutrition Examination Survey were used, and a total of 2,430 people were selected as participants to analyze the factors related to unmet dental care needs in postmenopausal women. Methods: Frequency analysis, cross analysis, and logistic regression analysis were performed for general characteristics, oral health characteristics, and unmet dental care needs related factors using IBM SPSS Statistics 21.0 program. Results: The unmet dental care related factors were 1.527 times higher in the case of not having oral examinations than in the case of having oral examinations. For those who did not make use of the dental clinic, 8.667 times, 2.913 times for bad oral health, and 1.912 times for usually showed that unmet dental care was higher. Inconvenience with speaking was 1.578 times higher, and in the absence of implants, unmet dental care needs was 1.510 times higher. In the case of no chewing difficulty, was 0.380 times lower. Conclusions: Based on the above results, in order to achieve the policy goal to reduce unmet dental care needs, policy support and interest are needed above all to accurately identify and solve specific problems.

Effect of Mothers' Oral Health Knowledge and Behaviour on Dental Caries in Their Preschool Children (데이터마이닝을 이용한 유치치아우식증 관련요인 분석)

  • In order to investigate correlation between mother's dental ca re for her children and their dental caries, this study was conducted wi th the dental examination record of 365 children who showed the same number of questionnaires with those examined for dental conditions and questionnaires written by mothers among children between three and six years of age and their mothers in Yeoncheon, Gyeonggi province in June 2004 to estimate frequency and percentage of general properties of subjects and mother's oral health care behaviors for her children by research items, to carry out cross-tabulation analysis and correlation analysis following Chi-square distribution for the presence of dental caries in deciduous teeth and oral health care behaviors, and to use decision tree analysis among data mining techniques for those factors associated with the presence of dental caries in deciduous teeth, and drew the following conclusions. 1. For mother's oral health care behaviors and attitudes for her children, 225 mothers(61.6%) confirmed their children's teeth-brushing; 278(76.2%) used no fluorine; and 286(78.6%) observed their children's teeth, 322 mothers(88.2%) instructed their children in teeth-brushing while 268 (73.4%) provided dental care, 232 mothers(63.7%) treated their children's cavity; 290(79.4%) believed that their children had good dental conditions; and 294(80.5%) answered that they began to provide their children with dental care in deciduous teeth. 2. As for the presence of dental caries in deciduous teeth and dental health care behaviors, there were statistically significant differences in employment, confirmation after teeth-brushing, teeth observation, instruction in time for teeth-brushing, use of fluorine, cavity treatment, time for dental care, and perception of dental conditions(p<0.05). 3. As for correlation between dental caries in deciduous teeth and oral health care behaviors, mothers who worked, who believed that their children didn't have good dental condition, and who thought that it was necessary to begin to provide dental care in permanent teeth were found to get their children to suffer from dental caries in deciduous teeth. Besides, those who failed to confirm teeth-brushing, who used no fluorine, and who failed to observe teeth and gave no instruction in time for teeth-brushing were shown to get their children to suffer from dental caries in deciduous teeth. 4. Variables to determine the presence of dental caries in deciduous teeth were classified by cavity treatment, mother's employment, time for dental care, and observation of children's teeth. The first node to determine the presence of dental caries in deciduous teeth was found to be cavity treatment; the next criteria for classification after cavity treatment were shown to be mother's employment and time for dental care. In case of children with no cavity, they were found to be mother's employment and teeth observation.

Factors of the Quality of Dental Care Service Upon Intention to Revisit (치과 의료서비스 질이 재이용의사에 미치는 영향)

  • Purpose: Identify the factors that influence following up control and reuse intention of patient who used dental health care institution after dental heath care service and in order to prepare the method which improve the quality of dental health service which dental heath care institution service afford. Methods: Data were collected through random sampling from June 20th to August 20th 2010 (for 60 days). Once we explained the purpose of our survey to people who experienced the dental service within one year, we distributed the questionnaires to someone who volunteered to respond and they answered all questions by themselves based on the actual experience of dental health care organizations. Even if the survey was conducted for 610 people, only 585 properly answered questionnaires were analyzed because responses which had many unanswered questions and had errors in responsive way were excluded. Results: Result of multiple regression analysis, the value of dental clinic service, the following up control after dental treatment, the technique and kindness of dentist, the environment of treatment, type of dental service and the kindness of dental staff is significant main cause to intention of reuse dental clinic. Conclusion: In order to increase the rate of patient reuse, enhance the value of service with following up control after health treatment and the high quality of dental health service.

Determining factors related to unmet dental needs among adolescents using Anderson Model (앤더슨 모형을 이용한 청소년 미충족 치과진료관련 요인)

  • Objectives: The purpose of this study is to identify factors related to unmet dental care needs among Korean youth, enhance the utilization of dental services by adolescents, furnish basic data for youth-oriented dental projects, and improve access to medical care. Methods: The analysis utilized combined data from the basic and oral databases from the 6th (2013-2015), 7th (2016-2018), and 1st year of the 8th (2019) National Health and Nutrition Examination Survey. Results: The factors influencing unmet dental care needs were gender, age, household income level, and health insurance. Statistically significant results were observed in relation to the type of dental clinic, recent visits to a dental hospital, experiences of toothache within the past year, subjective oral health status, occurrences of permanent dental caries, and engagement in orthodontic treatment. Conclusions: When formulating future national policies and projects, it is imperative to consider the factors and underlying reasons for the unmet dental care needs of adolescents. It is also necessary to establish oral health policies and institutional measures tailored to the dental care of adolescents.

The oral health awareness and oral health care provided by workers in long-term elderly care facilities (일부 지역 노인요양기관 종사자의 구강건강 인식정도와 구강건강관리 제공 실태 조사)

  • Objectives: The purpose of this study is to evaluate the oral health awareness and oral health care provided by workers in the long-term elderly care facilities. Methods: A self-reported questionnaire was completed by 213 workers in long-term elderly care facilities. The questionnaire consisted of general characteristics, oral health awareness, oral health behavior, oral health knowledge, oral health care professionals, oral health care, oral health care improvement, and denture care. Except the incomplete answers, 200 data were analyzed by the statistical software of SPSS WIN 18.0. Results: Highly educated people tended to have higher oral health awareness. The workers in the facility maintained the oral health care but they suggested that dental professionals are needed. Conclusions: It is necessary to suggest the oral health care management by dental professionals.

Comparison of oral hygiene status by clinical dental hygiene care performance (임상치위생관리과정 수행 전·후에 따른 구강위생 상태 비교)

  • Objectives : The purpose of this study was to investigate the oral hygiene status before and after the dental hygiene care performance to patients by the dental hygienists. Methods : Subjects were 55 adults visiting to dental clinics in Chungnam. The patients agreed to undergo a dental hygiene care performance. A skilled dental hygienist provided dental hygiene care performance including periodontal status, halitosis and oral hygiene status. Results : The periodontal pocket depth was compared before and after the performance. The depth dropped from 4.02 mm to 2.81 mm, and the value of halitosis dropped from 45.78 bbv to 35.76 bbv. The O'Leary index of the patients dropped form 49.37 to 32.84, and all the differences were statistically significant. Conclusions : Proper use of oral hygiene supplies and regular dental checkup can prevent periodontal diseases. This study will provide the useful information of the effective application of dental hygiene care performance.
