치과의원의 감염방지 실태 및 치위생과 학생의 B형 간염 예방현황 (Investigation of infection control in the private dental clinics and prevention of hepatitis B virus infection among the dental hygiene students)

  • Objectives: Dental personnels have high chances of exposure to various infections during many dental procedures. This study was performed to investigate the state of infection control in the private dental clinics and prevention state of hepatitis B virus infection among the dental hygiene students in Kwanju city, Korea. Methods: Questionnaires were obtained from 94 dental hygiene students who participated in dental practice in private dental clinics for more than five weeks. Results: 83.9% of dentists and 17.2% of dental hygienists routinely used the mask for treating all patients, 32.3% of dentists and 6.5% of dental hygienists routinely used the rubber gloves. The use of protective eyeware was much lower in each group. Disinfectant was used in 52.7% for sanitization of dental instruments before cleansing. The prevalence of HBsAg and anti-HBs were 3.8% and 67.1% respectively, 52.7% of dental hygiene students had history of accidental needle stick. Conclusion: Routine use of personal barrier techniques by dental personnels should be emphasized. Dental hygiene students were not properly immunized against hepatitis B virus and had high incidence of accidental needle stick. It is necessary to establish specific regulations or recommendations for cross infection control in dental practice and to performed scheduled vaccination program for hepatitis B virus for dental hygiene students.

  • The dental care delivery system and the dental specialty system have a very close relationship each other. Compared to Korea. Western European countries with predominant public sector in dental service have recognized merely 2 or 3 dental specialties, while North American countries with predominant private sector, 8 or 9 ones. It is desirable to adopt the dental specialty system as soon as possible in Korea to encourage scientific development in various dental specialties and qualitative advance in dental service. We, however, have to establish equitable dental care delivery system which can use limited dental resources efficiently as follows. 1. clarifying the different roles in assignments between general dentists and specialists by the amendment of the related laws such as the Medical Act and establishing the organic patient-referral system. 2. adopting the dental specialty system and expanding personnel and equipments so that the dental college hospitals, especially dental divisions of general hospitals, night function as secondary care facilities with specialties. 3. determining the size of dental specialists according to the national needs for dental specialized service's, whose number is to be not more than 10% of the total dentists. 4. transferring the function of accredating dental specialists to the efficient, self-controlled professional organization such as the Korean Dental Association rather than putting it under the governmental control. 5. conducting a comprehensive review of specialty education and practice for re-recognition, and maintaining competence of specialists by re-accredating them periodically. I expect this article to contribute to further discussion about the dental specialty system in Korea in productive and practical way. I am sure that we can Establish this system in the near future when people in every walks of life-the academic circle, the press, the authority concerned, consumer groups and the Korean Dental Association-take part in the discussion with special concern.

치위생학과와 치의학과의 교육과정 비교 (Comparison of Curriculum between Dental Hygienists and Dentists in Korea)

  • Introduction: All work of dental hygienists in Korea is to be carried out under the supervision of a dentist, but the main tasks of dental hygienists are systematic and sufficiently trained within the college curriculum. The purpose of this study was to compare the curriculum between the Department of Dental Hygiene and the Department of Dentistry to provide a basis for legal revision of the dental hygienist's unique work. Methods: The curriculum was acquired from the homepage of the Department of Dental Hygiene and Dentistry. The curriculum in 23 of 28 dental hygiene schools and 6 of 11 dental schools was identified on the homepage. The swere classified based on the scope of dental hygienist work. Results: The Department of Dentistry has more than twice the number of credits and subjects compared to the Department of Dental Hygiene, and the top subjects belong to the treatment and rehabilitation of dental diseases. However, in the field of preventive dentistry, which is a unique work of dental hygienists, the Department of Dental Hygiene has a score of 9.3 times higher than that of dentistry. In the public oral health field, the oral health education field, and the dental management field, dental hygienists scored twice as high as dentists. Conclusion: Since dental hygienists are receiving more education than dentists in the areas of preventive dentistry, public oral health, oral health education, and dental management, which are unique tasks, it is suggested that the work can be performed independently without supervision of the dentist.

전문치과위생사제도에 대한 견해 (Opinions of dental hygienists on the advanced dental hygienist system)

  • Objectives: This study aimed to provide basic data to lay the groundwork for the introduction of an advanced dental hygienist system by sampling dental hygienists' views about the system. Methods: A nationwide questionnaire survey was conducted targeting 857 dental hygienists working at dental hospitals and clinics, local health institutions, and educational institutions. The collected data were analyzed using frequency analysis, t-test, one-way analysis of variance (ANOVA) (Duncan as post-analysis), and crossover analysis. Results: The average interest level in the advanced dental hygienist system was 3.83±0.95 points. The necessity by field was confirmed to be the highest during dental hygiene for the elderly and persons with disabilities. The working experience necessary for becoming a specialized dental hygienist is 5.56±2.99 (years). The education period necessary for becoming a specialized dental hygienist is 77.30±77.61 (hours). The work authority level for an advanced dental hygienist was indicated to be 50 respondents (5.8%), who said they required direct guidance from a dentist, 313 respondents (48.2%) who said they needed indirect guidance from a dentist, 200 respondents (23.3%) who said OK when given an advanced dental hygienist's separate judgment, 194 respondents (22.6%), who said that the authority must be varied depending on the work. Conclusions: The interest and need of the advanced dental hygienist system were proven to be high and are expected to be applied to basic data for the introduction and settlement of the system.

PBL기반의 치과의료커뮤니케이션 교육과정을 통한 일부 치위생학과 학생들의 치위생 의사소통 역량수준 평가에 대한 사례연구 (A Case Study on the Level of Communication of Dental Hygiene Students after Completing a PBL-Based Dental Communication Curriculum)

  • Background: The purpose of the present study was to evaluate changes in the communication capabilities of dental hygiene students after completing a problem based learning (PBL) dental communication curriculum. With this information, we intend to gather the basic data necessary to stress the need for and value of an education in communication regarding dental hygiene. Methods: PBL-based education was provided to a total of 49 third-year dental hygiene students who took the dental communication class taught by the Department of Dental Hygiene at S University during the 2nd semester of 2020. The relevant self-evaluation was modified, based on the aim of the present study, to refer to three basic key competencies related to the communication capabilities of dental hygienists. An assessment of dental communication competency was conducted by analyzing the changes in self-evaluations before and after completing the course, for each question, using a paired t-test. The statistical significance level was set at p < 0.05. Results: Analysis of core competencies before and after PBL-based dental communication education indicated that all competencies were significantly improved after education (p < 0.01). Conclusion: Dental hygiene students' dental communication skills increased significantly after completing PBL-based dental communication education. Therefore, a PBL-based dental communication curriculum is effective in improving dental communication skills for dental hygiene students.

Radiography Work Performed by Dental Hygienists according to the Workplace Type

  • Background: Dental hygienists study dental radiology through the dental hygiene department and curriculum, and most of the radiography work in dental clinics is performed by dental hygienists; however, the legal work regulations for dental hygienists place restrictions on the type of radiography performed. This study aimed to identify the actual conditions of the radiographic work performed by dental hygienists and to determine the difference according to the type of hospital. Methods: This study included 195 dental hygienists working at dental medical institutions in the metropolitan area. A survey was conducted on regarding the radiographic work performed and the clinical career of the main performers. The radiography work was divided into periapical radiography, bite-wing radiography, occlusal radiography, panoramic radiography, computed tomography (CT), and cephalometric radiography. Results: The frequency of performing intraoral radiography was as follows: periapical radiography, 94.9%; bite-wing radiography, 93.8%; and occlusal radiography, 77.9%. The frequency of performing extraoral radiography was 94.4% for panoramic radiography, 89.7% for CT, and 73.3% for cephalometric radiography. The frequency of internal and external radiography performance was higher among hygienists in dental clinics than among those in dental hospitals and university hospitals. The analysis of the dental hygienists' clinical experience in the areas of intraoral and extraoral radiography showed that those working at university hospitals, dental hospitals, and dental clinics had over 5 years, 2~4 years, and 1 year of clinical experience, respectively. The hygienists with less than 1 year of clinical experience showed high performance frequency (p<0.05). Conclusion: For the dental hygienists to perform radiography safely, a discussion regarding the revision of related laws and regulations is warranted.

치과의사 및 치과위생사의 치과팀 구성 및 업무 분장에 대한 견해 (A Survey of Dentists and Dental Hygienists on Dental Team Composition and Work Assignments)

  • The purpose of this study was to investigate the status and views on the dental team composition and works, the experience of a dental manpower shortage and opinions on the solution. The subjects were 313 dentists and dental hygienists. The mobile questionnaires were conducted on the status of dental team composition and works, the experience of a dental manpower shortage and opinions on the solution. There was no big difference between the current dental team composition and ideal composition ratio, and dentists answered that it would be more ideal to reduce dental hygiene and increase nurse assistant when the working dentist was 3~5. 85.6% of the dentists and 56.4% of the dental hygienists had difficulty in employing the dental auxiliary personnel. The difficulty level was 8 out of 10 points among dentists and 5.7 points among dental hygienists. 91.1% of the dentists and 95.5% of the dental hygienists agreed on the necessity of expanding the dental hygienist's work. The dental works were temporary crown making and setting, surgical operation assistance, and overall dental assistance. Based on the results of this study, it is necessary to continuously make efforts to communicate with the dental personnel in Korea to solve a lot of problems such as the job assignment, the dental personnel policy and system, and the work environment.

연세대학교 치위생학과 구강위생교육실을 방문한 대상자의 구강위생관리 서비스체계에 대한 만족도 조사 (Clients Satisfaction with Oral Hygiene Care Services System Provided by the Dental Hygiene Clinics in the Department of Dental Hygiene, Yonsei University)

  • The setting of dental hygiene clinics is very important to dental hygiene education, which is the place not only to educate students but also to care clients. The purpose of this study is to provide basic research material for improvement of dental hygiene care system in dental hygiene clinics by analyzing the client satisfaction. A questionnaire survey by means of self-entry method was conducted to find out satisfaction of the client, who was visited to the dental hygiene clinics in the department of dental hygiene, Yonsei University. An analysis of frequency, one way ANOVA and T-Test were performed through SPSS 12.0K program. 1. Most clients were mainly composed of students in Wonju College of Medicine. 2. The clients visited for scaling(85.8%) and oral examination(9.73%) were much than treatment(4.42%). 3. Clients aged 21 to 25 were relatively lower in satisfaction with the facilities, system, attitude than any other ages. 4. The dental hygiene students are the lowest group in satisfaction with the facility, system, attitude than medical and nursing students. 5. The clients satisfaction with dental hygiene clinics was decreased in reverse proportion to visiting frequency. 6. Most of the clients pointed out the problems of appointment system(54.0%) and fee(23.0%), which should be improved than any other operation conditions. 7. Most of the clients were not satisfied with chair time and pain during care. 8. Most clients recognized to receive the better care service than other dental offices(81.3%). Especially, they paid attention to oral health education using phase-contrast microscope. 9. Many clients were dissatisfied with facilities of the dental hygiene clinics(71.7%). The problems of appointment system(54.0%) and chair time of dental hygiene care services(63.6%) had also inconvenienced to clients. The dental hygiene clinics in school play a crucial role in dental hygiene education to foster the student to be competent as a professional dental hygienist in the future. Therefore, well-organized dental hygiene care program based on dental hygiene process is essential. It is also required to improve the environment of dental hygiene clinics including facilities, appointment system and fee etc.

일부 어머니의 구강보건지식 및 행태에 따른 자녀의 구강건강상태에 관한 연구 (Research on the status of children's dental health following mothers' dental health knowledge and behavior)

  • Objectives : This research analyzed the general characteristics that closely affect the children's dental health and the mothers' dental health knowledge and dental health behavior to identify the correlation of the latter with the status of children's dental health in order to provide the basic data for the development of dental health business that would target children. Methods : Research subjects were selected arbitrarily from three nursery schools located in Busan metropolitan city, targeting 186 children between the ages of four to six and their mothers. The children were subjected to dental Inspection, and the status of their dental caries was studied whereas the mothers were subjected to the surveys on the general characteristics, dental health knowledge and dental health behavior. Results : 1. Level of mothers' dental health knowledge is higher when the mothers' educational level is higher(p=0.02) and when the household's monthly income is higher(p=0.009). 2. When the level of mothers' dental health knowledge is higher, children tended to brush their toothbrushing using proper method(p=0.025). Moreover, when the level of mothers' dental health knowledge is higher, they tended to take their children to dental clinic mostly for preventive measures than for treating cavity(p=0.023). 3. When the level of mothers' dental health knowledge is higher, children's dmft index was significantly low(p=0.02). When the mothers use fluoride-containing toothpaste, children's children' dmft index was even lower(p=0.02). 4. As the children tended to brush their teeth more often, dmft index was lower(p=0.003). When the reason that the children visited dental clinic was more to prevent, than to treat cavity, dmft index was even lower(p=0.000) Conclusions : When the above mentioned results are summarized, it is possible to know that the mothers' dental health knowledge and dental health behavior significantly affect children's dental health. Accordingly, it is necessary to develop maternal and child dental health program that factors in both the mothers and children in order to prevent children's dental caries and to increase their dental health, and continued care is required to discover and treat dental caries early on.

치과내원 환자의 치과공포 감소를 위한 중재법 적용 (Intervention for Reducing Dental Fear and Anxiety of Dental Patients)

  • 본 연구는 서울시 일개 치과의원에 내원한 환자를 대상으로 치과공포 수준을 측정하고, 치과공포 수준 감소를 위한 중재법에 대해 환자가 요구하는 방법을 적용한 후 치과공포 중재법의 효과를 측정하였다. 개입한 치과공포 중재법에 대해서는 전체적으로 '도움이 된다(3.57점)'는 의견이었고, 항목별로는 '어떤 진료인지 미리 설명해 준다(4.38점)', '진료 중에 환자가 손을 들면 언제든 치료를 잠시 멈추어 준다(4.35점)', '진료 중 통증이 발생할 것 같으면 미리 말해준다(4.24점)', '의료진이 부드럽고 웃는 표정을 한다(4.06점)' 순으로 도움이 된다고 하였다. 치과공포 중재법 적용 전과 후의 전체적인 만족도는 각각 4.37점과 4.35점으로 높은 수준이었으나, 적용 후 미미하게 감소하였으며 이는 치과치료 항목과 밀접한 연관이 있을 것으로 사료된다. 치과공포 수준을 감소하기 위해서는 환자의 치과공포 수준 뿐 아니라 치과공포 중재를 위한 요구도를 파악하여 이에 맞는 개입방법을 적용하여 주는 것이 효과적이며, 이에 대한 치과의료 인력에 대한 교육과 매뉴얼 개발이 필요하리라 생각된다.
