500명 여성을 대상으로 한 팔강변증에 따른 맥파 특성 연구 (A Study on the Characteristics of Pulse Waves according to Eight Principle Pattern Identification in 500 Women)

  • This study was conducted to find objective diagnostic indicators for the Eight Principle Pattern Identification using a pulse wave analyzer. Typology Complexion Pulse and Symptom data from 500 women over the age of 18 were used. Five experts made a diagnosis of Eight Principle Pattern Identification and divided the subjects into 'Yin-Yang', 'Exterior-Interior', 'Cold-Heat', 'Deficiency-Excess' groups. Their pulse waves were measured in the left and right radial arteries, and it was investigated whether there was a significant difference between groups in the pulse wave parameter values. 'Yin' group showed a significantly lower value in the left radial artery for Ener, Emin, EIX, T4T, T4TT parameters and in the Right for T2, T2T, T5. The Vmag, As and Ad parameters were significantly different between the 'Exterior' and 'Interior' groups. 'Heat' group showed a significantly higher value in the right radial artery for RAI/t parameter. 'Deficiency' group showed a significantly higher value in the right radial artery for W, Angl parameters. Through this study, significant pulse wave parameters were found, and they can be used as objective diagnostic indicators for Eight Principle Pattern Identification.

기능성 소화불량에서 초음파 위배출능 측정간격에 대한 임상연구 (Clinical Study on Measurement Intervals in the Ultrasonographic Gastric Emptying Test for Functional Dyspepsia)

  • Objective: This study investigated the measurement interval in the ultrasonographic gastric emptying test for patients with functional dyspepsia (FD) and the correlation between gastric emptying and the findings of various questionnaires. Methods: In total, 119 patients (59 patients with FD and 60 healthy controls) were recruited from July 2018 to June 2020. Gastric volume (GV) and gastric emptying half-time (T1/2) were measured by ultrasonography at fasting and again at 0, 5, 10, 15, 30, 45, 60, 90, and 120 min after meals (apple juice, 500 mL, 224 kcal), and the average half-time (average T1/2) was calculated. Questionnaires on food retention (FRQ), phlegm pattern e (PPQ), cold and heat (CHQ), deficiency and excess (DEQ), and spleen-qi deficiency (SQDQ), stomach qi deficiency pattern (SSDQ), visual analogue scale (VAS), and Nepean dyspepsia index-Korean version (NDI-K) were completed by all participants. The differences in GV and T1/2 were analyzed in participants whose maximal GV occurred at 0 min versus after 0 min. The correlation of the average T1/2 with the questionnaire scores was also analyzed after excluding erroneous data. Results: Patients with FD who took a certain amount of time to reach maximal GV after meals had a greater gastric volume up to about 30 minutes after meals, and the PPQ, DEQ, and NDI-K scores, especially for upper gastrointestinal symptoms and general weakness-related symptoms, showed statistically significant correlations with average T1/2. Conclusions: Ultrasonography can be a quantitative evaluation tool for FD. However, further studies on measurement methods based on FD physiopathology are required.

  • "Byunjeung-okham(辨證玉函)" is composed of four volumes, Eumjeung-yangjeungbyun(陰症陽症辨, Vol.1), Heojeungsiljeungbyeon(虛症實症辨, Vol.2), Sangjeunghajeungbyun(上症下症辨, Vol.3) and Jinjeunggajeungbyun(眞症假症辨, Vol.4) (卷之一 "陰症陽症辨", 卷之二 "虛症實症辨", 卷之三 "上症下症辨", 卷之四 "眞症假症辨"). It is based on the 74 types of disease symptoms, and compared to "Byunjeungrok(辨證錄)", it is rather brief and concise. Thus, its name, "Byunjeung-okham". This study researched vol.2, "Heojeungsiljeungbyun(虛症實症辨)", which is organized according to the deficiency and excess pattern of 20 disease patterns, such as cough. The disease pattern system mentioned in this book is unique compared with other disease pattern systems. The role of the kidney is especially emphasized, and the application of the Five-phase[五行] theory is very original. Consequently, it contributed greatly in the process of broadening the disease pattern system of Traditional Korean Medicine.

고혈압 단계별 변증특성 분석 (Pattern Analysis in Patients with Hypertension grades)

  • To investigate and characterize basic patterns of prehypertension. Participants were divided into three groups; ideal blood pressure(n=40), prehypertension(n=62), and stage 1 hypertension groups(n=68) according to the JNC 7 standards. They answered questionnaire asking various symptoms and received clinical examination. Data were collected and analyzed in the focus of three Zheng patterns, cold-heat, deficiency-excess and four major hypertension types. Analysis of variance was used to find differences among groups. In addition, relationship between the cold-heat trends and risk factors of hypertension were analyzed using Pearson's correlation analysis. Three risk factors; age, body mass index (BMI), and fasting blood sugar (FBS) showed intergroup differences. Statistical significances were revealed in the cold-heat pattern and two hypertension subtypes. The cold scores decreased from $4.8{\pm}1.84$ to $3.9{\pm}1.88$ and $3.7{\pm}1.27$ (p=0.022), while the heat increased from $1.9{\pm}1.32$ to $2.8{\pm}1.72$ and $2.8{\pm}1.48$ (p=0.009). Additionally two hypertension subtypes, the excessive liver fire, and the yin-yang deficiency showed significant differences. Cold had negative correlations with blood pressure (both systolic and diastolic), BMI, triglyceride, and FBS. Heat had positive correlations with systolic blood pressure, BMI, triglyceride, and FBS. Prehypertension could be characterized by using the cold-heat patterns. The cold-heat are correlated with Blood pressure, BMI, blood lipids and sugar.

식사가 정상인의 맥에 미치는 영향 분석 (Effect of Taking Meal on Pulse Diagnosis in Healthy Subjects)

  • The pulse diagnosis studies reported to date has mainly been performed to clinically reveal the pulse wave characteristics according to the specific diseases, whereas no attempts have been made to study the effects on the pulse wave characteristics of the daily activities such as taking meals, exercise, and sleep, etc. This work reports the effect of feeding stimulus on the healthy subjects on the pulse wave pattern which has quantitatively been analyzed using the objective model for the pulse diagnosis in oriental medicine. The pulse waves right before/after the meal and 30 minutes after the meal were measured using the pulse analyzing equipment (3D-Mac, Daeyo Medi, Korea) and at the same time oriental medicine doctors' diagnoses were given. The pulse parameters obtained from the equipment and clinical records on the subjects were statistically processed and the variables showing statistically significant differences were analyzed. The results indicate that the pulse pressure, the pulse rate, and the respiratory rate increase while the blood pressure decreases after the meal. For the floating/sinking and the deficient/excess coefficients characterizing the pulse states described in the oriental medicine, the floating/sinking coefficients were observed to decrease whereas the deficiency/excess coefficients increase after the meal. The results indicates that besides the standard bio-indicators like blood pressure and respiratory rate, etc., the pulse wave characterization in terms of the pulse classifications in the oriental medicine using the floating/sinking, deficient/excess pulse states provide an important piece of biomedical information.

음양변증(陰陽辨證)의 정의와 기능 및 판별방법 연구 (Definition, Role and Method of Yinyang Pattern Differentiation)

  • In order to ensure the fact that eight principle pattern differentiation is used clinically as a basic guideline for Korean medicine practice, the definition, role and method of yin-yang pattern differentiation with its case report were explored at first. Yinyang Pattern Differentiation is a method of discriminating human tendencies or morbidity based on the yin and yang characteristics expressed in living bodies. And yin and yang are the two contrasting characteristics and aspects of the interaction when certain physical conditions that have a lasting effect on the human physiological metabolic function are correlated with the morbidity. Specific methods of yinyang pattern differentiation can be divided into several types of yin and yang indicators. First, time and space factors like day and night, hot and cold seasons, above and below, topographical districts. Second, colors and pulse and their/or relative clearness and muddiness, hardness and softness, moving and resting. Third, diagnose yin and yang patterns through distinguishing the true and false of a fever and cold in an emergency phase such as increase of brain pressure and shock state. Fourth, general characteristics of the propensity and constitution of a subject such as body type, speech, behavior, and physiological metabolism. And for clinical use, these were summarized again as a symptom indicators of physical signs and color, pulse, tongue and questionnaire indicators of propensity, body type, and space-time characteristics. Conclusively, it was confirmed that yinyang pattern differentiation has its own diagnostic significance which is distinct from exterior-interior, cold-heat and deficiency-excess pattern differentiation.

2012년~2013년 국민건강영양조사 자료를 이용한 중년 남성의 단백질섭취비와 섭취패턴에 따른 대사증후군 위험도 비교 (Metabolic Syndrome Risk by Intake Ratio and Intake Pattern of Proteins in Middle-aged Men Based on the 2012-2013 Korean National Health and Nutrition Examination Survey Data)

  • Objectives: The purpose of the study was to compare intake of energy nutrients, physical characteristics, and the prevalence of metabolic syndrome according to protein intake group. Methods: Subjects were 827 men aged 40-65 years. The results presented were based on data from the 2012-2013 National Health and Nutrition Examination Survey and analyzed using SPSS. The odds ratio (OR) of metabolic syndrome was assessed according to the protein intake group and intake pattern of protein-rich foods. Results: The mean of protein intake was $73.96{\pm}0.71g$. According to level of protein intake, four groups (deficient, normal, excess 1, excess 2) were created and their percentages were 8.3%, 39.6%, 37.1%, and 15.0% respectively. The mean of daily energy intake was $2,312.33{\pm}24.08kcal$. It was higher in excess group 2 than in the deficiency group (p < 0.001). Moreover, the intake of all energy nutrients increased significantly with protein intake group (p < 0.001). The main contribution to daily protein included mixed grains ($10.96{\pm}0.32g$), milled rice ($7.14{\pm}0.30g$), chicken ($3.50{\pm}0.21g$), and grilled pork belly ($3.04{\pm}0.16g$). With regard to physical characteristics, and blood pressure and blood test results, only body mass index increased significantly according to protein intake groups (p < 0.05). The prevalence of metabolic syndrome in subjects was 38.5%, and there was no significant correlation with protein intake group. The OR of metabolic syndrome increased with protein intake, and was higher 4.452 times in excess group 2 than in the normal group (p < 0.05). Conversely, the OR of metabolic syndrome according to the frequency of protein-rich food intake did not show a significant correlation. Conclusions: The results of this study can be used as significant supporting data to establish guidelines for protein intake in middle-aged men.

  • Objectives: This study examined the correlation between the Scandinavian Stroke Scale (SSS) and the Motricity Index (MI) in acute cerebral infarction patients. Methods: We studied 75 inpatients within a month after the onset of cerebral infarction; patients were recruited from July 2012 to November 2013. We analyzed the correlation between changes in SSS and MI by pattern identification. Results: A significant moderate positive linear correlation was noted between SSS and MI in the total patient cohort and in a Fire-heat group, Dampness-phlegm group, Qi deficiency group, and Deficiency syndrome group. A significant strong positive linear correlation existed between SSS and MI in the Excess syndrome group. The Yin deficiency group showed a weak positive linear correlation, but it was not statistically significant. Conclusions: This study provides evidence that the correlation between SSS and MI in acute cerebral infarction patients could differ depending on pattern identification.

Effects of Insamsansa-eum (Renshenshanzha-yin) on Hypercholesterolemia and Analysis of Its Effects according to the Pattern Identification

  • Backgrounds : Hyperlipidemia is a major cause of cardiovascular disease (CVD). Lowering serum cholesterol levels could reduce the risk of CVD. Insamsansa-eum (Renshenshanzha-yin, ISE), composed of Ginseng Radix and Crataegii Fructus, is a new medicine developed to treat hyperlipidemia and CVD. Objectives : In this study, we intended to explore the clinical effects of ISE on patients with hypercholesterolemia, and moreover we also compared its effects according to the pattern identification. Methods : Subjects were administered ISE with the dose of 600 mg three times a day for 4 weeks. Patterns of subjects were identified with diagnostic scoring system for Yin-Yang and the condition of Excess-Deficiency before treatment. Serum lipids were measured at baseline and after 4 weeks of medication. Results : ISE lowered total cholesterol(TC), triglyceride(TG), total lipid(TL), phospholipid(PL) and low density lipoprotein cholesterol(LDL) significantly. Compared with the data of our previous study, it was less effective than Atorvastatin but showed equal lipids-lowering effect to Chunghyul-dan (Qingxue-dan, CHD). In Yang pattern group, ISE was less effective in lowering TG and LDL than it was in not-Yang-not-Yinpattern group. On safety assessment, there was no adverse effect, hepatic or renal toxicity. Conclusions : We suggest that ISE is a safe and useful herbal medicine for hypercholesterolemia, and moreover it could be more useful when it is used for patients with not Yang pattern.

반려견 인지기능장애증후군에 대한 한의 진단 및 한약치료 적용 가능성 고찰: 치매환자 국내한의치료기술과 비교 분석 (Potential application of herbal medicine treatment based on pattern identification for canine cognitive dysfunctional syndrome: a comparative analysis of Korea medicine therapy for patients with dementia)

  • Canine cognitive dysfunction syndrome (CDS) is a neurodegenerative disease that causes cognitive and behavioral disorders and reduces the quality of life in dogs and their guardians. This study reviewed the complementary and alternative medicine (CAM) for CDS and compared the diagnosis and therapy of CAM between CDS in canines and dementia in humans. The evaluation tools for the diagnosis of CDS and dementia were similar in the neurological and neuropsychiatric examinations, daily life activity, cognitive tests, and neuroimaging, but the evaluation for dementia was further subdivided. In CAM, pattern identification is a diagnostic method for accurate, personalized treatment, such as herbal medicine. For herbal medicine treatment of cognitive impairment in canines and humans, a similar pattern identification classified as deficiency (Qi, blood, and Yin) and Excess (phlegm, Qi stagnation, and blood stasis) is being used. However, the veterinary clinical basis for verifying the efficacy and safety of CAM therapies for CDS is limited. Therefore, based on CAM evidence in dementia, it is necessary to establish CDS-targeted CAM diagnostic methods and therapeutic techniques considering the anatomical, physiological, and pathological characteristics of dogs.
