Weighting of Stroke Pattern Identification Using an AHP (AHP 기법을 이용한 중풍 변증지표의 가중치 설정)

  • In this study, we structuralized the diagnostic indices used for pattern identification (PI) of stroke, and suggested an AHP method to obtain the weights of PI indices. AHP of the subjects under consistency ratio 0.1 showed that the critical indices for stroke PI consists of 9 for Qi-deficiency, 13 for Phlegm/dampness, 7 for blood stagnation, 12 for Yin-deficiency and 16 for Fire/heat. Furthermore, AHP analysis rendered the weights of indices of each PI that will be useful for oriental medical experts to perform objective PI.

  • Cerebral creatine deficiency syndrome (CCDS) is a disorder where a defect is present in transport of creatine in the brain. Creatine plays an essential role in the energy metabolism of the brain. This is a genetic disorder, autosomal recessive or X linked, characterized by intellectual disability, speech and language delay, epilepsy, hypotonia, etc. Until recently very few number of cases have been reported. Here we report a case of 1.5-year-old boy who had epilepsy (epileptic spasm and generalized tonic clonic seizure), intellectual disability, microcephaly, hypotonia and speech delay. His magnetic resonance imaging of brain showed cortical atrophy and electroencephalography showed burst suppression pattern. The diagnosis was confirmed by clinical exome sequencing which showed novel mutation of SLC6A8+ in exon 9, suggestive of X linked recessive CCDS. The patient was then treated with glycine, L-arginine and creatine monohydrate with multiple antiepileptic drugs.

  • The purpose of this study is to report the effectiveness of oriental medical treatment including Hubaksenggangbanhagamchoinsam-tang(厚朴生薑半夏甘草人蔘湯) and moxibustion (and acupuncture) on abdominal distension. Two patients complainted of abdominal distension, vomiting, dyschezia and so on. According to the pattern identification (辯證論治), the distension of both patients were diagnosed as deficiency distension(虛脹). So we decided to treat the patients by means of tonification the deficiency(補虛) including Hubaksengganggamchoinsam-tang and moxibustion. And the symptoms improved after treatment. So we report these cases.

Preliminary Study for Development of Pattern Identification Tool for Radiation Pneumonitis (방사선폐렴 변증(辨證)도구 개발을 위한 기초연구)

  • Objective: The purpose of this study was to develop a standard tool for pattern identification of radiation pneumonitis. Methods: Textbooks, published studies, and references with comments about patterns were reviewed. Through the Delphi method, we determined pattern identifications based on advice from a committee of experts composed of 13 Korean respiratory internal medicine professors. Results: Using the Delphi method, four pattern identifications were chosen: Qi Deficiency (氣虛), Yin Deficiency (陰虛), Heat Toxin (熱毒), and Phlegm Dampness (痰濕). The tool was developed in a question-and-answer format with 35 questions. Conclusions: A pattern identification tool that can discriminate the patterns of radiation pneumonitis for standardized diagnosis was developed through expert consultation. Further study of its validity and reliability is necessary.

A Correlation Study of Pattern Identification Instrument and Biomarkers for Polycystic Ovary Syndrome (다낭성난소증후군 변증도구와 생체지표 간의 상관성 연구)

  • Objectives: The purpose of this study was to investigate the correlation between Pattern identification instrument and biomarkers in polycystic ovary syndrome patients. Methods: Pattern identification instrument questionnaire, Sasang constitutional test, body composition test, pulse analyzing test, laboratory test were performed and the results were analyzed. Correlation analysis was performed between pattern identification and laboratory test results. Results: Testosterone, SHBG, $17{\alpha}$-OH-progesterone, AMH were highest in the Kidney-Yang Deficiency (腎陽虛) group. LH, FSH, LH/FSH ratio were the highest in the Endogenous Heat due to Yin Deficiency (陰虛內熱) group. DHEA-S was the highest in the Dampness-Heat in Liver Meridian (肝經濕熱) group. E2, Prolactin, Cholesterol, Triglyceride, HDL-Cholesterol, LDL-Cholesterol, Glucose (FBS), Insulin, HOMA-IR ratio, HbA1c were the highest in the Phlegm-Dampness (痰濕) group. Conclusions: In this study, we obtained basic data analyzing the correlation between pattern identification instrument and biomarkers in polycystic ovary syndrome patients. If further studies are performed, we expect to be able to obtain clues to study the mechanism of polycystic ovary syndrome.

A Review of Etiology, Pattern Identification, Treatment of Traditional Chinese Medicine for Childhood Anorexia (소아 식욕부진의 병인, 변증, 치료에 대한 고찰 -중의학 논문을 중심으로-)

  • Objectives This study aimed to provide a basis for applying Korean medical treatment for childhood anorexia in clinical practice by examining Korean medical etiology, pattern differentiation, and treatment, and focusing on research articles on Chinese medicine. Methods Articles on Chinese medicine related to childhood anorexia published before November 4, 2021, in the China National Knowledge Infrastructure (CNKI), were analyzed. The etiology, pattern differentiation, and Chinese medical treatment were summarized. Results Of a total of 73 studies, 13 were randomized controlled trials (RCT), 32 were case studies, and 28 were review papers. The most common Chinese medical etiology of childhood anorexia was emotional instability, and the western medical etiology was problems with diet and lifestyle. The most frequently reported pattern differentiations were spleen-stomach-qi deficiency (脾胃氣虛), stomach-yin deficiency (胃陰不足), and spleen failing in transportation syndrome (脾失健運). The most frequent prescriptions were modified Yangwijeungaektang (养胃增液湯加減), Samryongbakchulsan (蔘苓白术散加减), and Ekongsan (異功散加減). As frequntly used tuina acupoints, Naepalgwae (内八卦), Joksamli (足三里), and Bigyeong (脾經) were mentioned. Conclusions This study analyzed the etiology, pattern differentiation, and Korean medical treatment of anorexia in children. Based on this study, standardization and well-designed clinical studies on Korean medical treatments for childhood anorexia can be expected in the future.

Lymphopenia by Pure Zinc Deficiency : Role of Corticosterone

  • The effect of zinc deficiency on certain immunological parameters was investigated using intragastric tube feeding to obviate decreased food intake and altered eating pattern. Male, Fischer 344 rats were bilaterally adrenalectomized(ADX) or sham operated(SHAM). ADX rats received 0.9% NaCl in their drinking water and corticosterone injections at the dose of 1mg/kg of body weight three times per day. After recovery, one half of ADX and SHAM animals were tube-fed a purified, liquid diet containing either two ppm of zinc(zinc-deficient, force-fed ; ZDF) or 50ppm(zinc-replete, force-fed ; ZRF) for 19 days. They received identical amounts of diet based on the intake of ad libitum-fed, zinc-replete rats. Although they received identical amounts of food, ZDF rats grew at a slower rate compared to ZRF rats in both SHAM and ADX rats. Regardless of surgery, force-feeding rats the zinc-deficient diet resulted in a substantial decrease in serum zinc levels. The weights of the thymus, lymph node, and spleen were lower in SHAM-ZRF rats compared to SHAM-ARF rats. Marginal zinc deficiency caused lymphopenia in SHAM animals. However, these differences in lymphoid tissues and cells between SHAM-ZDF and SHAM-ZRF rats disappeared in ADX rats. These results indicate that the impaired growth of lymphoid tissues observed in zinc-deficient, sham-operated animals can be attributed to elevated serum corticosterone levels under the conditions of our experiments.

TREATMENT OF ZINC DEFICIENCY IN SHEEP BY ZINC CONTAINING BOLUSES

  • A study was conducted to investigate the release pattern of zinc form the zinc containing boluses and to see whether the released zinc can cure a zinc deficiency in sheep. Three sheep were used in this experiment and were fed a low zinc semi-synthetic diet throughout the experimental period. Each sheep was given a single pre-weighed zinc containing bolus when blood variables showed continuous zinc deficiency. The zinc containing boluses when placed within the reticulo-rumen of zinc deficient sheep, release zinc at the rate of 106.6 mg zinc/day for 39 days. At the end of depletion period there was a reduced feed consumption, plasma zinc concentration, plasma alkaline phosphatase activity and increased plasma zinc binding capacity which were 409 g, 0.18 mg/l, 87 U/l and 88.7% respectively and 521 g, 0.18 mg/l, 142 U/l, and 89.5% respectively before first and second blousing. After the administration of the first and second boluses, the feed consumption, plasma zinc levels and plasma alkaline phosphatase activities rose rapidly and far exceeded the starting values. The zinc binding capacity was reduced to 21.9% due to the administration of the first and second boluses. It is concluded that zinc boluses can be used for curing a zinc deficiency in sheep.

A Clinical Case Report of Severe Hematuria Patient after Ureterolith Lithotripsy (요관결석 쇄석술 후 심한 혈뇨 소견을 보인 환자 임상증례)

  • Objectives : The purpose of this study is to report the improvement after the Korean medical treatment about a severe hematuria patient after ureterolith lithotripsy. Methods : This study was carried out on 60 year-old female patient who suffered from severe hematuria. We diagnosed a severe hematuria patient after ureterolith lithotripsy as kidney deficiency and blood deficiency pattern in the symptom-differentiation system of Korean medicine and applied herbal medicine, acupuncture and moxibustion to the patient. Results : After Korean medical treatment, we observed improvement of the symptom process by Numeric Rating Scale (NRS), hematologic findings and urinalysis. Blood count of blood and urine had been in the normal range, and NRS of three symptoms had dropped below 0.5. Conclusions : We concluded that Korean medical treatment was an effective treatment for a hematuria patient after ureterolith lithotripsy.

Correlation between Instrument on Pattern Identification for Depression and Psychological Tests by Statistical Analysis (통계적 분석을 통한 우울증 변증도구와 심리검사의 상관성 연구)

  • Objectives: This study was performed to determine the correlation between Instrument on Pattern Identification for Depression and Psychological tests by Pearson Correlation Coefficient and Regression analysis.Methods: Two assessors carried out the evaluation using the instrument on pattern identification for depression. They also performed the following psychological tests: Beck Depression Inventory (BDI), State-Trait Anxiety Inventory (STAI), State-Trait Anger Expression Inventory (STAXI), the Temperament and Character Inventory (TCI), Insomnia Severity Index (ISI_Total), Self-disclosure Inventory, subjective well-being Inventory, Health perception Inventory, and Personality Assessment Inventory (PAI). A total of 167 participants who got HAM-D score over 12 were targeted for the evaluation. Our research team carried out Pearson correlation coefficient analyses and regression analysis between pattern identification for depression and Psychological tests. We listed the results by descending order and interpreted the results.Results: Pearson correlation analysis revealed the following results: 1) Stagnation of Liver Gi was associated with BDI (0.60) and STAI (0.55); 2) Dual Deficiency of the Heart and Spleen was associated with BDI (0.60), ISI_Total (0.52), and STAI (0.42); 3) Relieving stagnation of Phlegm-Gi was associated with BDI (0.65), STAI (0.54), and Subjective well-being inventory (−0.52); 4) Gi-deficiency Mingled with sputum was associated with BDI (0.50), ISI_Total (0.40), and STAI (0.395); 5) Stagnant Gi transforming into fire was associated with BDI (0.56), STAI_TR (0.51), and Health perception Inventory (−0.458); 6) Yin-Deficiency with Effulgent Fire was associated with BDI (0.55), ISI_total (0.54), and STAI (0.41).Conclusions: Through correlation analysis between Instrument on Pattern Identification for Depression and Psychological tests, we could suggest a System for Oriental Medical Diagnosis for Depression.
