다낭성난소증후군 변증도구와 생체지표 간의 상관성 연구 (A Correlation Study of Pattern Identification Instrument and Biomarkers for Polycystic Ovary Syndrome)

  • Objectives: The purpose of this study was to investigate the correlation between Pattern identification instrument and biomarkers in polycystic ovary syndrome patients. Methods: Pattern identification instrument questionnaire, Sasang constitutional test, body composition test, pulse analyzing test, laboratory test were performed and the results were analyzed. Correlation analysis was performed between pattern identification and laboratory test results. Results: Testosterone, SHBG, $17{\alpha}$-OH-progesterone, AMH were highest in the Kidney-Yang Deficiency (腎陽虛) group. LH, FSH, LH/FSH ratio were the highest in the Endogenous Heat due to Yin Deficiency (陰虛內熱) group. DHEA-S was the highest in the Dampness-Heat in Liver Meridian (肝經濕熱) group. E2, Prolactin, Cholesterol, Triglyceride, HDL-Cholesterol, LDL-Cholesterol, Glucose (FBS), Insulin, HOMA-IR ratio, HbA1c were the highest in the Phlegm-Dampness (痰濕) group. Conclusions: In this study, we obtained basic data analyzing the correlation between pattern identification instrument and biomarkers in polycystic ovary syndrome patients. If further studies are performed, we expect to be able to obtain clues to study the mechanism of polycystic ovary syndrome.

고혈압 단계별 변증특성 분석 (Pattern Analysis in Patients with Hypertension grades)

  • To investigate and characterize basic patterns of prehypertension. Participants were divided into three groups; ideal blood pressure(n=40), prehypertension(n=62), and stage 1 hypertension groups(n=68) according to the JNC 7 standards. They answered questionnaire asking various symptoms and received clinical examination. Data were collected and analyzed in the focus of three Zheng patterns, cold-heat, deficiency-excess and four major hypertension types. Analysis of variance was used to find differences among groups. In addition, relationship between the cold-heat trends and risk factors of hypertension were analyzed using Pearson's correlation analysis. Three risk factors; age, body mass index (BMI), and fasting blood sugar (FBS) showed intergroup differences. Statistical significances were revealed in the cold-heat pattern and two hypertension subtypes. The cold scores decreased from $4.8{\pm}1.84$ to $3.9{\pm}1.88$ and $3.7{\pm}1.27$ (p=0.022), while the heat increased from $1.9{\pm}1.32$ to $2.8{\pm}1.72$ and $2.8{\pm}1.48$ (p=0.009). Additionally two hypertension subtypes, the excessive liver fire, and the yin-yang deficiency showed significant differences. Cold had negative correlations with blood pressure (both systolic and diastolic), BMI, triglyceride, and FBS. Heat had positive correlations with systolic blood pressure, BMI, triglyceride, and FBS. Prehypertension could be characterized by using the cold-heat patterns. The cold-heat are correlated with Blood pressure, BMI, blood lipids and sugar.

『상한론(傷寒論)·변맥법(辨脈法)』에 관한 연구(2) (The study on ShangHanLun BianMaiFa (2))

  • The BianMaiFa chapter, which is the first chapter of SongBon ShangHanLun, is the scholarly description(專論) of the pulse and pathological mechanism but has not been studied and published yet. This study is about article 11-23 which is the second part of The BianMaiFa chapter. We compared the original texts within the editions, comparing and analyzing the annotations of successive dynastic medical group. The articles of The SongBon ShangHanLun BianMaiFa chapter 11-23 is consisted as is shown: article 11 12 13 state about a dignosis of yin-yang, deficiency-excess, fluid-humor from difference of a pulse shape and a sign of recovery from a disease, article 14 states about a prognosis of a disease according to a pulse, article 15 states about a pulse from which we predict a recovery, article 16 states about normal pulse according to season, article 17 states about ups and downs of a disease according to yin-yang of day and night, article 18 states about a location of a disease in connection with a pulse, article 19 states about a pathological mechanism of a diarrhea through a anterior tibial pulse(趺陽脈), a lesser yin pulse(少陰脈), article 20 states about a cause of disease, a symptom, a therapy about a floating and tight pulse(浮緊脈), article 21 states about a pathological mechanism of symptoms through a anterior tibial pulse(趺陽脈), article 22 states about a pulse, a pathological mechanism, symptoms caused by erroneous treatment, article 23 states about a pulse, a symptom, a therapy with internal heat. The BianMaiFa chapter 11-23 discussed a diagnosis, a prognosis of a disease, understanding of pathological mechanism through pulse. And The BianMaiFa suggests a various use of pulse.

한양방협진으로 호전된 당뇨병 환자 1례 (A Case of Diabetes Mellitus Treated with Oriental and Western Medical Combination Theraphy)

  • Diabetes mellitus is a metabolic disorder characterized by hyperglycemia which results from one or both of decreased insulin secretion and increased insulin resistance. Chronic hyperglycemia causes damage to the eyes, kidneys, nerves, heart and blood vessels. The major goal in treating diabetes mellitus is controlling elevated blood sugars without causing abnormally low levels of blood sugar. A 67-year-old woman was admitted with hyperglycemia, thirst, Lt. leg numbness and both leg weakness. The patient was diagnosed as So-gal(消渴) due to dry ness-heat based on yin-deficiency(陰虛燥熱) through oriental medicine and NIDDM through western medicine. The patient was treated with oriental and western medicines. Specifically herbal medicine(Gamijihwangtang), acupuncture, moxa theraphy were used as well as oral medications. Clinical symptoms improved and hyperglycemia dramatically stablized.

  • Ikwi-tang (IW) is an oriental medicine that has been used for the treatment of general symptoms due to a stomach yin deficiency. The aim of this study is to investigate precisely the effect of IW on allergic rhinitis (AR). We well show the effects and the mechanism of IW in the ovalbumin-sensitized AR model. IW significantly decreased the number of nasal/ear rubs and the increment of IgE levels in the AR mice. The levels of interferon-${\gamma}$ were enhanced while the levels of interleukin (IL)-4 were reduced in the spleen tissue of the IW-administered AR mice. Expressions of IL-$1{\beta}$ and cyclooxygenase-2 were inhibited by IW-administration in the nasal mucosa tissues. Infiltration of eosinophils and mast cells was decreased in the IW-administered AR mice. Our results indicate that IW may attenuate the development of AR by the inhibition of caspase-1 activity.

한약에 대한 N-of-1 임상시험의 현황 - 문헌 고찰 (Current Status of N-of-1 Trials for Herbal Medicine -Literature Review)

  • Objectives: To overcome the limitations of randomized controlled trials, many other trials design is tested. The n-of-1 trial is a promising research method in the field of Korean medicine because of this methodology can examine the optimal treatment for each patient strictly. Therefore, we reviewed the status of N-of-1 studies on herbal medicine. Methods: A systematic literature review was conducted based on the pubmed database. The search term were 'N-of-1 Trial', 'Chinese Medicine', 'Herbal Medicine', 'Kampo'. There was no restriction in year. Results: Four clinical trials have been identified to demonstrate the effectiveness of herbal medicines for Kidney-yin-deficiency syndrome, bronchiectasis and gastric cancer. These studies suggest that the N-of-1 design is a study that encourages patient involvement, demonstrates the effectiveness of herbal medicines and helps reduce unnecessary medication. Conclusion: The N-of-1 clinical trial may be a rigorous methodology suitable for the clinical setting and may help the development of evidence-based Korean medicine. Attention is also needed in this research method in Korea.

경옥고와 경옥고가연자육의 조골세포 증식과 골흡수 억제효과 (Inhibitory Effects on Bone Resorption and osteoblast proliferation of Kyungok-go)

  • Objectives : Kyungok-go(KOG), the first herbal formulation of donguibogam, has been used for treating of many symptoms of yin deficiency. In this study, we examined the effect of KOG on bone resorption. Methods : We determined the effects of water extract of KOG in RANKL(Receptor Activator for Nuclear Factor ${\kappa}B$ Ligand)-induced osteoclast differentiation culture system and osteoblast proliferation. In addition, we determined the effects of water extract of ABR on LPS-induced bone-loss with mice. Results : Water extract of KOG showed proliferation effect on osteoblast without cytotoxicity and no effect on RANKL-treated osteoclast differentiation. KOG rescued bone erosion by LPS induction in vivo study. Conclusions : These results demonstrated that KOG can be a useful remedy for treating of bone-loss disease such as osteoporosis.

뇌교 경색에 의한 일측성핵간마비로 유발된 외사시 환자에 대한 증례보고 (A Clinical Study on One Case of Exotropia Caused by Unilateral-internuclear- ophthalmoplegia due to Pontine Infarction)

  • Objectives : This study is to report one case of the patient who has exotropia caused by unilateral internuclear ophthalmoplegia due to pontine infarction, which is a rare disease so there has few research about the effect of acupuncture therapy on it, treated with the SAAM(舍巖) Acupuncture Sojangjeonggyeok(小湯正洛) Methods & Results : In the point of differentiation of Syndrome, these subjects were diagnosed as deficiency of Yin and blood(陰血不足). We treated her with SAAM Acupuncture Sojangjeonggyeok(小湯正洛), herb medication(淸肝湯, 六味地黃湯), and the symptoms were resolved clearly by above treatment during about a week. Conclusion : This case shows proper oriental medical treatment based on the exact differentiation of symptoms has good effect on this disease as one of conservative therapies.

肝斑에 關한 文獻的 考察 (Bibliographical Study on Liver Spot)

  • In order to satisfy the demand of good treatment of Oriental medicine, I investigated many books mentioned about Liver spot. The results were as follows: 1. They have called the Liver spot many other terms in the oriental medicine. 2.. The Liver spot is facial disease, so it is deeply related with yang-myong kyong(陽明經) 3. As the external cause of the Liver spot, wind and fire evil have been emphasized. 4. Intoxication of bud of banha(半夏) poison and alcohol absorption can be one of the causes. 5. As the internal cause of the Liver spot, each other vapour asthenia fever from constipation seven modes of emotion, asthenia of the spleen and stomach and insufficiency of kidney-yin(腎陰) 6. Nowadays, the deficiency of kidney-yang(腎陽) is also included as the cause of the Liver spot. 7. Many oriental doctors have talked about the relationship between the Liver spot and women, but I can't find any concrete talking about the relationship between menstration and the Liver spot, pregnancy and the Liver spot. 8. They have administrated many other herb medicines for the treatment the Liver spot.

노화에 의한 목혼(目昏), 이명(耳鳴), 요부리(尿不利), 마비(麻痺)의 침구치료에 관한 문헌 고찰 (A literature study on acupuncture treatment for blurred vision, tinnitus, difficulty in urination and bi-syndrome in the process of aging)

  • Objectives : This study is aimed to investigate acupuncture treatments for blurred vision, tinnitus, difficulty in urination and bi-syndrome caused by aging process through oriental medical documents. methods : We researched 21 oriental medical classics for the acupuncture treatments of blurred vision, tinnitus, difficulty in urination and bi-syndrome caused by aging or deficiency. Results & conclusions : 1. Disharmony between yin and yang, disharmony between organs, disharmony of essence, qi, spirit, blood and body fluid are the main features of aging. 2. ST36, BL18, BL10, CV24, ST1, BL23 have been most frequently used for acupuncture to treat blurred vision caused by aging. For moxibustion, ST36 has been most frequently used. 3. GB2, TE21, TE17, SI19, TE3 are the mainly used points for acupuncture to treat tinnitus and deafness caused by aging. For moxibustion, LI1 and BL15 have been most frequently used. 4. CV3, CV7, CV4, BL67, LR8, LR4 have been most frequently used to treat difficulty in urination caused by aging. 5. LI11, GB34, GB30, LI4, ST36, TE10, TE5, Sama (extra point) have been most frequently used to treat numbness, paralysis and bi syndrome caused by aging.
