Total fertility rate (TFR) increased from the lowest 1.08 in 2005 to 1.13 in 2006, and a debate is made whether the increase is temporary or continuous as a result of various pro-natal policies. This study intends to explore policy implications revealed in recent fertility change using vital statistics. For this purpose, tempo-adjusted fertility rate by birth order, fertility rate by age of mother and birth order, age-specific fertility for married, and age-specific divorce rate for married are analyzed. The increase of TFR and births for 2006 is largely due to increase of first births at early thirties with slowdown of delayed first marriage and first child birth. The increase of female population (the third wave effect of baby boom) and first marriages of late twenties in 2006 and 2007 would lead to increase of fertility during 2007-2008. But further increase is uncertain because of the decrease trend of marital fertility and increase trend of never-married for twenties. TFRs for first and second births reduced rapidly, while TFRs for third and above births showed no changes, and second births were largely affected by tempo adjustment of fertility. Thus, constructing social environment for first and second births is more effective and necessary than encouraging third births. In addition, social responsibility of child care, child-women health issues due to delayed births, and the need for multi-cultural family support system are discussed.

Background: Vascular endothelial growth factor and matrix metalloproteinases are two important factors for angiogenesis associated with breast cancer growth and progression. The present study was aimed to examine the effects of tamoxifen and tranilast drugs singly or in combination on proliferation of breast cancer cells and also to evaluate VEGF and MMP-9 expression and VEGF secretion levels. Materials and Methods: Human breast cancer cell lines, MCF-7 and MDA-MB-231, were treated with tamoxifen and/or tranilast alone or in combination and percentage cell survival and proliferative activity were evaluated using LDH leakage and MTT assays. mRNA expression and protein levels were examined by real-time RT-PCR and ELISA assay, respectively. Results: LDH and MTT assays showed that the combined treatment of tamoxifen and tranilast resulted in a significant decrease in cell viability and cell proliferation compared with tamoxifen or tranilast treatment alone, with significant decrease in VEGF mRNA and protein levels. We also found that tamoxifen as a single agent rarely increased MMP-9 expression. A decrease in MMP-9 expression was seen after treatment with tranilast alone and in the combined treatment MMP-9 mRNA level was decreased. Conclusions: This combination treatment can able to inhibit growth, proliferation and angiogenesis of breast cancer cells.

Objective: As paternal age increases, the quality of sperm decreases due to increased DNA fragmentation and aneuploidy. Higher levels of structural chromosomal aberrations in the gametes ultimately decrease both the morphologic quality of embryos and the pregnancy rate. In this study, we investigated whether paternal age affected the euploidy rate. Methods: This study was performed using the medical records of patients who underwent in vitro fertilization (IVF) procedures with preimplantation genetic screening (PGS) from January 2016 to August 2017 at a single center. Based on their morphological grade, embryos were categorized as good- or poor-quality blastocysts. The effects of paternal age were elucidated by adjusting for maternal age. Results: Among the 571 total blastocysts, 219 euploid blastocysts were analyzed by PGS (38.4%). When the study population was divided into four groups according to both maternal and paternal age, significant differences were only noted between groups that differed by maternal age (group 1 vs. 3, p= 0.031; group 2 vs. 4, p= 0.027). Further analysis revealed no significant differences in the euploidy rate among the groups according to the morphological grade of the embryos. Conclusion: Paternal age did not have a significant impact on euploidy rates when PGS was performed. An additional study with a larger sample size is needed to clarify the effects of advanced paternal age on IVF outcomes.

Purpose: This research aimed to study the awareness of marriage, childbirth, fertility and high-risk pregnancy among university students. Methods: 250 university students from Seoul and Gyeonggi-do completed questionnaires regarding their awareness on marriage, childbirth, fertility, and high-risk pregnancy. Results: The average age of participants was 22.5. The ideal marriage age was 29.8. 201 (80.4%) participants had an intention of getting married, and 160 (60.4%) answered they should have children. The ideal age of first childbirth was 30.4 and last childbirth was 35.1. There were 126 (50.4%) with prior education regarding high-risk pregnancy, subfertility or infertility. The score of needs for education associated with pregnancy was 7.6 out of 10. The average correct answer rate of awareness of fertility issues was 30.7%. There were significant differences between men and women, 25% of men and 38% of women overestimated the age at women are most fertile. Only 23.7% of men and 25.9% of women knew the correct age when there was marked decrease in women's fertility. Conclusion: Awareness of a lack of marriage, childbirth, fertility, and high-risk pregnancy may influence future family planning and health. These results can be used to plan programs or education for marriage, pregnancy, and childbirth.
This study analyzes the effects of women's labour force participation and work-family reconciliation support on life-cycle fertility in Korea. The analysis is based on the longitudinal data from Korean Labour and Income Panel Study (KLIPS), which include the available information on life-cycle fertility and employment history. Employing a dynamic model of fertility, we estimated the life-cycle fertility of all the 15-49 years old women considered in this study by using a duration model. The major results of this study were as follows: First, women's labour force participation had a negative effect on the first birth, second birth, and all births (transition to births starting at different parities). Women's employment tended to lengthen the interval between births. Second, the availability of maternal leave had a positive effect on the first birth and all births for working women. Providing maternal leave to working women decreased the opportunity cost of childbearing and in turn, reduced the interbirth interval of women. However, the availability of parental leave had no significant effect on the births of working women. Third, the financial support for childcare had a positive effect on the first birth and all births. The economic support for childcare led to the reduction in the interbirth interval of women by increasing the probability of births. The use of a childcare center for the first child, which substitutes for the time that women needed to take care of their children, classified as time-intensive consumption goods, did not have any effect on the second birth. Fourth, the part-time employment of women had a positive effect on the second birth. A flexible working time schedule tended to decrease the interval between the first and the second births.
This study was conducted to investigate for its effects on reproductive and developmental toxicity of recombinant human epidermal growth factor (rhEGF) in Sprague-Dawley rats. Male rats were administered rhEGF at doses of 1, 10, 100, and 1000$\mu$g/kg/day, respective1y, by subcutaneous injection from 63 days before and throughout to mating period until the day before sacrifice. Female rats were administered rhEGF at the same doses from 14 days before mating to day 20 of gestation or to day 21 of lactation. We examined the male and female fertility indices and maternal toxicity of F0 parental animals. Also, we examined the external, visceral, or skeletal malformation of fetuses, growth and development, behavior, and/or reproductive performance of F1 animals. At the highest dose (1,000 $\mu$g/kg), the mean body weights of F0 animals were significantly increased in males and females at 3 or 2 weeks after treatment, respective1y. No clinical signs and food intakes were observed at any time during the experimental period by rhEGF treatment. In autopsy examination, the relative and absolute liver weights significantly increased in both sexes of 1,000 $\mu$g/kg. At the highest dose (1,000 $\mu$g/kg), there was a statistically significant increase of pregnancy period and the number of dead fetuses. Moreover, significant increase of mean fetal body weight and decrease of number of live fetuses, which related to the difficult dilivery were observed in highest dose group. In Fl examination, no adverse effects on external, visceral, and skeletal malformation, physical and functional development, behavior or reproductive ability of Fl animals were observed in any group. Also, there was no significant difference between control and treated groups in copulation or fertility indices of Fl animals. These results indicate that rhEGF had no adverse effect on fertility and reproductive ability of Sprague-Dawley rats.

In the present study, an attempt has been made to assess whether the effect of benzene extract of Ocimum sanctum leaves on the ultrastructural changes in the epithelial cells of the cauda epididymis, its subsequent recovery in the seminiferous epithelium and fertility of male albino rats. Wistar strain male albino rats were orally administered benzene extract of 250 mg/kg body weight of O. sanctum leaves followed by subsequent recovery maintaining suitable controls for 48 days. Results indicate decrease in the weights of testis, epididymis and seminal vesicles. Other accessory organs were not affected. Total count, cell and nuclei diameters of germ cells and Leydig cells were reduced. Cauda epididymis exhibited significant reduction in epithelial height and nuclei diameter of epithelial cells. Cells showed vacuolization with exhibit of signs of degeneration. Ultra study revealed that, in general, the cauda epididymis was affected and in particular, the principal, clear and basal cells were highly disturbed. Further, there was decrease in the size of lipid droplets, mitochondria, Golgi complex, endoplasmic reticulum and accumulation of lysosomal bodies. Fertility performance test showed no implantation in female rats mated with O. sanctum treated rats. Moreover, their recovery after withdrawal of treatment was observed suggesting that the effect of the treatment is transient and reversible. A recovery period resulted in normal spermatogenesis and fertility, suggesting reversible antispermatogenic and antifertility effects of the plant.
Although Korean population has been growing steadily during the past four decades, the nation is rapidly becoming an aging society because of its declining birth rate combined with an increasing life expectancy. In addition, Korea has one of the lowest fertility rates in the world due to fewer married couples, advanced maternal age, and falling birth rate. The prevalence of low birth weight infants and multiple births has been increased compared with the decrease in the birth rate. Moreover, the number of congenital anomalies is expected to increase due to the advanced maternal age. In addition, the number of interracial children is expected to increase due to the rise in the number of international marriages. However, the maternal education level is high, single-mother birth rate is low, and the gender imbalance has lessened. The number of overweight babies has been decreased, as more pregnant women are receiving adequate prenatal care. Compared to the Asian average birth weight, the average birth weight is the highest in Asia. Moreover, the rate of low birth weight infants is low, and infant mortality is similarly low across Asia. Using birth data from Statistics Korea and studies of birth outcomes in Korea and abroad, this study aimed to assess the changes in maternal and infant characteristics associated with birth outcomes during the past four decades and identify necessary information infrastructures to study countermeasures the decrease in birth rate and increase in low birth weight infants in Korea.
Korea fertility level has dropped since the 1960's and speed of decline has accelerated in the 1980's. In the resuls, the growth rate reached to less then 1 percent in 1990 and will be $\ulcorner$0 $\lrcorner$ percent growth in 2021. The total population will increase to 50, 586 thousand persons in 2021 then will decrease. With the rapid fertility decline the age structure of Korean population has changed : while the proportion of child population aged 0-14 is decreasing sharply, that of old-age population aged 65 and over is increasing. Because of the recent increase of sex ratio at birth, the sex structure among the young generation has been destorted; the sex ratio at age 0-4 was as high as high as 112.0 in 1990. The effects of these population phenomena on Korean society are the followings: 1) Old age dependency ratio is increasing rapidly and continuously and will be about 40.0 from the 2050's. 2) Because of the rapid decline of the number of births, the absolute number of the major labour force at age 25-34 will decrease after around 2000 and then from the 2010's become less than that in 1990. 3) Since the large fertility defferentials by womens's educational level have continued and the relation between the educational levels of mothers and children are very strong, level has dropped among higher educated women first when the average fertility level has declined in Korea, the average educational level of the children is may be expected to be relatively low and then the future productivity is will be also low, which causes the decline of 'quality of population. 4) When the high sex ratio at birth (over 110 since 1986) continues, number of brides will be short by over 20 percent from the 2010s, which will bring various kinds of social prblems. Therefore, the counter measures in concrete to relieve the heavy problems are recommended as follows: 1) The extention of the birth intervals, which helps to drop the population growth rate at the same level of number of children per women. 2) The positive supports for out-migrants, which helps to reduce the population size without any destortion of age-sex structure. 3) The defferent supports of family planning for the higher and lower classes, which helps to reduce the gaps between different ferility levels of the classes and to keep and improve the quality of population. 4)population education for both students and adults, which helps to minimize the gap between the private and public requirements.
Objectives: This study aims to analyze the results of the 2022 Gyeonggi-do Oriental Medicine Fertility Treatment Support Project, review the outcomes and satisfaction of Oriental Medicine fertility support initiatives, and propose progressive fertility treatment support strategies by comparing them with the results of the past three years. Methods: Total of 242 women and 205 spouses participated in the Korean Medicine fertility treatment support project, which encompassed herbal medicine, acupuncture, and counseling treatments over a 3-month period, followed by a 3-month post-treatment follow-up. Data pertaining to patients' general, demographic, and fertility-related characteristics were collected before treatment initiation. During treatment, information regarding the treatments administered by Korean medical doctors was recorded, along with post-treatment outcomes and satisfaction levels. Safety assessments included pre- and post-treatment blood tests and monitoring for adverse events. Results: Among the 242 female subjects, 209 successfully completed the treatment program. Of these, 35 (16.7%) achieved pregnancy, with 30 (15%) attaining pregnancy through herbal monotherapy. Out of the 35 pregnancies, 17 were maintained, while 10 resulted in miscarriage. Notably, 83.8% of patients expressed satisfaction with the treatment outcomes. An analysis spanning three years revealed a continuous increase in the average age of patients, surpassing 38 years in 2022, a critical point in fertility decline age. Additionally, there was a notable rise in the prevalence of patients with a history of gynecological issues, advanced spouse age, and semen abnormalities, which is assumed to have contributed to the decrease in the pregnancy success rate. Conclusions: This study compares the clinical results of the 2022 Gyeonggi-do Korean Medicine Fertility Treatment Support Project with those of the past three years. Based on these findings, recommendations are made to enhance the project, including stricter age criteria for patient selection, enhanced specialized treatment tools for medical doctors, and combining Korean Medicine treatment and medical assisted reproductive technologies, all aimed at increasing pregnancy success rates. These results provide a foundation for the development of fertility support projects and related policies.
