Association between physical activity and periodontitis as stratified by obesity in Koreans (한국인에서 비만에 따른 신체활동와 치주질환과의 연관성)

  • Background: The aim of this study was to examine the association between physical activity and periodontitis in the Korean population. Methods: This study utilized data from 9,191 participants of the Korea National Health and Nutrition Examination Survey. Periodontitis was defined as a CPI score of 3 or 4. Physical activity was assessed by the trained interviewer using the International Physical Activity Questionnaire-Short Form (IPAQ-SF). IPAQ-SF is composed of vigorous physical activity, moderate physical activity, and walking activity. Multivariable logistic regression analyses and stratified analyses by obesity were performed. All analyses were conducted separately for males and females. Results: Females who engaged in vigorous and moderate physical activity had a 28% (OR = 0.72, 95% CI = 0.54 - 0.95) and 34% (OR = 0.66, 95% CI = 0.44 - 0.98) lower risk of periodontitis, respectively. In obese females, moderate physical activity in obese had a 65% lower risk of periodontitis (OR = 0.35, 95% CI = 0.18 - 0.67). Conclusions: Our findings suggest that moderate-intensity physical activity was inversely associated with a lower risk of periodontitis. In obese females, moderate physical activity had an independent inverse association with periodontitis.

The Beneficial Effects of Pectin on Obesity In vitro and In vivo (In vitro 및 In vivo에서 펙틴의 비만 억제 효과)

  • The effects of pectin on obesity was studied using 3T3-L1 pre-adipocytes and rats fed 20% high fat diets. The concentration of leptin released from 3T3-L1 adipocytes in the presence of pectin was significantly decreased by 85% compared to that of the control (p<0.05), however, glycerol concentration was not changed. These data indicate that pectin seems to inhibit lipids accumulation in the adipocytes rather than enhance the lipolytic activity. Forty Sprague Dawley rats were fed 20% high fat diet for 8 weeks to induce obesity and then divided equally into four groups. Experimental groups were normal diet group (ND), high fat diet group (HFD), HDF with 10% pectin group (HFP10), and HDF with 20% pectin group (HFP20). Diet for the each group was prepared to be iso-caloric following AIN-76 guideline. After obesity was induced, rats were placed on an restricted diet for 9 weeks. The body weight of HFD increased 50% (p<0.05) compared to the ND, while it was decreased by 12% and 16% for HFP10 and HFP20, respectively (p<0.05). The relative amount of visceral fats for HFDl0 and HFD20 were decreased by 45% and 59% compared to that of HDF (130%), respectively (p<0.05). Pectin seems to have a greater effect on reducing visceral fats accumulation than weight reduction. Significantly increased level of triglyceride, total cholesterol or LDL-cholesterol in the plasma of HFD was returned to the normal or even below the normal by pectin diet, while the level of HDL-cholesterol increased. Lipid lowering effect was also observed in the liver and heart. These effects of pectin were dosedependent. In conclusion, the beneficial effect of pectin on the obesity was observed from cell culture experiment and animal study in terms of inhibiting the accumulation of lipids in the adipocytes.

  • Purpose: Leptin is an adipocyte-derived blood-borne satiety factor that acts on its cognate leptin receptor in the hypothalamus, thereby regulating food intake and energy expenditure. We measured the leptin concentrations in serum of normal and obese children with human leptin ELISA kit, unlike previous study with leptin RIA kit and investigated the relationship between leptin concentrations and body mass index, gender, and age. Methods: We measured serum concentrations of leptin in 67 children who were visited to the Department of Pediatrics, Chungnam National University Hospital during the period of 5 months from February, 1999 to June, 1999. Height, weight, obesity index, and body mass index were measured in 67 subjects. Leptin values in serum were measured by sandwich ELISA method. Data analysis was done according to the obesity, body mass index, gender and age. Results: The mean concentration of leptin was $7.69{\pm}8.83\;ng/ml$ in normal children group and $36.34{\pm}18.57\;ng/ml$ in obese group. Serum leptin concentrations were significant correlation with the body mass index (p<0.01). Serum leptin concentration was significant higher in the group of over 10 years of age (p<0.01). Leptin levels showed no significant difference by gender. Conclusion: Serum leptin levels were significantly higher in obesity group than in control one, and they were correlated with body mass index and age. Measurements of leptin value by sandwich ELISA method are very useful and easily applicable to determine obesity.

Relationship between convergence awareness for healthy weight management and eating behavior, creativity and convergence competency of adolescents (청소년의 건강 체중관리를 위한 융합 인식 및 식행동과 창의·융합역량과의 관련성)

  • Purpose: Obesity can lead to several chronic diseases. With a continuous increase in the obesity rates, sustainable healthy weight management among growing adolescents has become essential. This study was conducted to examine the relationship between obesity-related factors and creative convergence ability. This could form the basis for convergence education for the sustainable healthy weight management of adolescents. Methods: Data were collected from 430 adolescents in Daegu, Gyeongbuk, and Gyeonggi regions using a self-administered 5-Likert questionnaire from November to December 2020. Results: The average score of the male students was higher than that of the female students in terms of healthy weight management convergence awareness, creativity, convergence thinking ability, self-efficacy, exercise, and dietary diversity. On the other hand, the dietary restraint score and the number of weight loss attempts were higher in the female students. Dietary restraint, disinhibition scores and weight loss attempts were higher in obese adolescents compared to underweight adolescents. It was found that dietary restraint, convergence thinking ability, problem-solving ability, exercise, and dietary diversity factors had a positive effect on healthy weight management convergence awareness. Healthy weight management convergence awareness, exercise, meal amount management, junk food management, weight loss belief, and weight loss trial experience had a positive effect on dietary restraint. Convergence thinking ability, weight loss experience, and obesity factors had a positive effect on dietary disinhibition. Conclusion: The sustainable healthy weight management education of adolescents is based on weight management knowledge and awareness of convergence, and must include gender differences. Thus, the problem-solving ability for healthy weight management can be creatively cultivated to enhance self-efficacy.

Evaluation of nutrient and food intake status, and dietary quality in Korean female adults according to obesity : Based on 2007-2009 Korean National Health and Nutrition Examination Survey (성인 여성에서 비만 여부에 따른 식사섭취상태 및 식사의 질 평가 : 2007-2009 국민건강영양조사 자료를 이용하여)

  • The purpose of this study was to compare nutrient intake and diet quality of obese women to those of non-obese women, and to investigate the relationship between diet quality and obesity index in females. We analyzed data from the combined 2007-2009 Korean National Health and Nutrition Examination Survey (KNHANES). In this study, according to their BMI (above $25kg/m^2$), we classified the subjects into the obese group (n = 724) and non-obese control group (n = 2,841). Nutrient adequacy ratio (NAR), the number of foods (Dietary Variety Score, DVS), and food groups consumed (Dietary Diversity Score, DDS) were analyzed by using the data from the 24-recall method. The average energy intake of both the obese and control group were 1634.94 kcal and 1,707.81 kcal, respectively (p = 0.0199). The obese group consumed significantly lower quantities of calcium per 1,000 kcal compared to the control group (p = 0.0261). With regards to diet quality, calcium NAR in the obese group was significantly lower than that of the control group (p = 0.0158). MAR values for the obese and the control group was 0.79 and 0.80, respectively and revealed no statistical difference. The obese group showed a significantly lower consumption of milk compared to the control group (p = 0.0162). The average DVS of the obese and control group was 28.63 and 30.41, respectively and revealed a statistical significance (p = 0.0003). DDS in the obese group (3.686) was significantly lower than that of the control group (3.769)(p = 0.0253). DVS showed considerable negative relationships with BMI or waist circumference after it was adjusted for potential confounding factors. In conclusion, obese women had lower calcium intake quality, milk consumption, DVS and DDS than those of the control group. In Korean females, food intake variety did adversely affect obesity index. Further studies are needed to confirm this finding.

A Survey on the Consumer Attitude Toward Health Food in Korea (I) -Consumer Perception on Health and Food Habit- (건강식품에 대한 소비자 인식 연구 (I) -건강과 식습관에 관한 소비자 의식구조-)

  • The consumer perception on health and food habit, the experience of health food use and the discrimination between health food and drug of Korean consumer were surveyed by using a questionnaire containing 15 items in order to obtain the basic data for the assessment of the benefit and risk of health foods in Korea. A total of 1,000 people over 20 years of age living in Seoul and the vicinities were interviewed and asked to fill out the questionnaire during the period from the October 1995 to the February 1996. Among the 882 answers collected, 23 was incomplete data, and 859 answers were used for the statistical analysis by using SAS program. The perception of Korean consumer on health and food habit indicated that food habit was considered the most important factor for the maintenance of health, as appeared in 39.8% of the subjects, among which 93.9 % believed that food habit could cause disease, and 97.1% believed that disease could be cured by changing food habit. The most worried disease was cancer (30.6%), degenerative diseases (14.1%), diseases by accident (12.6%) and obesity (10.0%). The disease which likely to be caused by food habit was diabetes (35.6%), obesity (22.4%), high blood pressure (12.8%), constipation (12.7%) and cancer (7.9%). The disease which was believed to be cured by changing food habit was diabetes (40.1%), obesity (25.9%), constipation (16.5%), high blood pressure (7.4%) and cancer (3.3%). It appeared that the people had a perception that food habit was highly related with diabetes and obesity, but less with cancer which was mostly worried.

Associations of the eating alone behavior with nutrient intake, obesity and metabolic syndrome in middle-aged adults based on the 2013~2017 Korean National Health and Nutrition Examination Survey (중년 성인에서 혼자 식사하는 행동과 영양소 섭취, 비만 및 대사증후군 연관성 연구 : 2013~2017년 국민건강영양조사 자료를 이용하여)

  • Purpose: This study examined the changes in the nutrient intake and the prevalences of obesity and metabolic syndrome according to the eating alone behavior in middle-aged men and women. Methods: The data from the 2013 ~ 2017 Korean National Health and Nutrition Examination Survey were analyzed. A total of 7,728 adults (3,404 male, 4,324 female) aged 40 ~ 64 years old were included. The subjects were classified into three groups according to the number of times eating alone per day (0, 1 ~ 2, and 3 times/day). Dietary data were collected by a 1-day 24-h recall. The nutrient intakes were compared among the three groups. Survey logistic regression analyses were conducted to examine the association of the eating alone behavior with obesity and metabolic syndrome, adjusting for the related confounding variables. Results: In men, eating alone was associated significantly with a lower intake of potassium (p-for-trend = 0.048) and lower intake of calories from protein (p-for-trend = 0.04). In women, the proportion of subjects consuming energy less than 75% of the estimated energy requirement (p = 0.001) and less than the estimated adequate requirement of riboflavin (p < 0.001) differed significantly according to the eating alone behavior. The eating alone behavior was positively associated with an increased risk of developing metabolic syndrome (p-for-trend = 0.033), increased blood pressure (p-for-trend < 0.001), and increased waist circumference (p-for-trend = 0.004). On the other hand, in women, however, the eating alone behavior was associated with a decreased risks of developing obesity (p-for-trend = 0.02). No association was found between the eating alone behavior and the risk of metabolic syndrome in women. Conclusion: These results suggest that the eating alone behavior is a risk factor for the development of metabolic syndrome in middle-aged Korean men.

Application of Machine Learning to Predict Weight Loss in Overweight, and Obese Patients on Korean Medicine Weight Management Program (한의 체중 조절 프로그램에 참여한 과체중, 비만 환자에서의 머신러닝 기법을 적용한 체중 감량 예측 연구)

  • Objectives: The purpose of this study is to predict the weight loss by applying machine learning using real-world clinical data from overweight and obese adults on weight loss program in 4 Korean Medicine obesity clinics. Methods: From January, 2017 to May, 2019, we collected data from overweight and obese adults (BMI≥23 kg/m2) who registered for a 3-month Gamitaeeumjowi-tang prescription program. Predictive analysis was conducted at the time of three prescriptions, and the expected reduced rate and reduced weight at the next order of prescription were predicted as binary classification (classification benchmark: highest quartile, median, lowest quartile). For the median, further analysis was conducted after using the variable selection method. The data set for each analysis was 25,988 in the first, 6,304 in the second, and 833 in the third. 5-fold cross validation was used to prevent overfitting. Results: Prediction accuracy was increased from 1st to 2nd and 3rd analysis. After selecting the variables based on the median, artificial neural network showed the highest accuracy in 1st (54.69%), 2nd (73.52%), and 3rd (81.88%) prediction analysis based on reduced rate. The prediction performance was additionally confirmed through AUC, Random Forest showed the highest in 1st (0.640), 2nd (0.816), and 3rd (0.939) prediction analysis based on reduced weight. Conclusions: The prediction of weight loss by applying machine learning showed that the accuracy was improved by using the initial weight loss information. There is a possibility that it can be used to screen patients who need intensive intervention when expected weight loss is low.

Effect of Health status and Health Behavior on the Diabetes Mellitus Prevalence (성인의 건강상태, 건강행위가 당뇨병 유병률에 미치는 영향)

  • Diabetes Mellitus (DM) is well known for increasing morbidity and mortality, especially related to their complications. The purpose of this study was to investigate the factors affecting the prevalence rate of DM and provide a fundamental material to develop an intervention strategy to reduce the prevalence rate of DM. The study subjects were adults aged over 19 on the basis of the primitive data of "The Fifth Korean National Health and Nutrition Examination Survey, 2012". Therefore, the data of 5995 participants were analyzed. For data process, the complex sample analysis module of SPSS 18.0 program was employed to add weighting before analysis. According to the analysis, the prevalence rate of DM of the study subjects was 10.5%. Regarding the odds ratio of DM prevalence, the subjects who graduated from middle school had the odds ratio 2.51 times higher than those who graduated from college and more; those in subjective bad health condition 4.77 times higher than those in subjective good health condition; those in obesity 1.44 times higher; those with high blood pressure 2.57 times higher; those with hyperlipidemia 2.63 times higher; those who fail to control their weight 1.31 times higher; those going on a diet 2.75 times higher. This study revealed that a level of education, perceived health status, obesity, high blood pressure, hyperlipidemia, weight control, and dietary therapy were the predictable variables of the prevalence rate of DM, and thereby suggested the nursing direction and research direction to reduce the prevalence rate of DM.

Analysis of the Mediating and Moderating Effects of Body Mass Index and Subjective Body Shape Perception on the Relationship Between Physical Activity and Self-Rated Health in Adolescents (청소년의 신체활동과 주관적 건강 인식 관계에서 체질량지수와 주관적 체형 인식의 매개효과와 조절효과 분석)

  • The study aim was to investigate the relationship between adolescents' physical activity and subjective health perceptions and determine whether body mass index (BMI), an obesity-related indicator, had a mediating or moderating effect on this relationship. To achieve the purpose of this study, raw data from the 18th Youth Health Behavior Online Survey (2022) were used, and data from 23,612 people were ultimately selected for the research analysis. A confirmatory factor analysis and structural equation modeling were applied to the data, and bootstrapping techniques were used to verify the mediating effect. To verify the moderating effect, a multigroup analysis of the structural equation model was applied to calculate pairwise parameter comparison values. All statistical significance levels were set at .05. The results of this study follow. First, the greater the amount of physical activity among adolescents, the more positive was the effect on subjective health perception. Second, subjective body type perception had a partial mediating effect on the relationship between physical activity and subjective health perception. Third, subjective body type had a moderating effect on physical activity and subjective health perception. Specifically, people who perceived their subjective body types as average viewed their subjective health more positively when they engaged in more physical activity than those who perceived their body types as slightly or very overweight. In conclusion, preventing obesity and positively changing the subjective health status through sufficient physical activity during adolescence are important.
