한방병원 건강검진 수진자의 대사증후군 발생과 관련요인 연구 (Incidence and Related Factors of the Metabolic Syndrome in a Korean Medicine Hospital)

  • The purpose of this study was to investigate incidence and related factors of the metabolic syndrome in a Korean medicine hospital. The 716 subjects were analyzed using biochemical data and survey who took medical examination in Daejeon Korean Medicine Hospital for general health check-up. This investigation was conducted from February in 2008 to July in 2010. The metabolic syndrome was diagnosed according to the definition by the NCEP ATP III. The abdominal obesity guidelines for waist circumference applied by the WHO Western Pacific Region, IASO and IOTF: The Asia-Pacific Perspective in 2000. Incidence of metabolic syndrome was 12% (14.6% in men, 8.2% in women). The groups that have two metabolic risk factors were 21.9% in men and 7.5% in women. The incidence increased with ageing. The mean of metabolic syndrome`s triglyceride was in hypertriglyceridemia, and that of their BMI in men was in primary obese and that of their AST, ALT, ${\gamma}$-GTP means were in abnormal liver function. Smokers in men have metabolic syndrome 10 times more than non-smokers in men. Exercisers that do the exercise once or twice a week in women have metabolic syndrome 0.2 times more than non-exerciser in women. Women that have family history of stroke, were associated with metabolic syndrome by $x^2$-test. Men that have family history of hypertension, have metabolic syndrome 4 times more than otherwise men. Men that have family history of diabetes mellitus, have metabolic syndrome 3 times more than otherwise men.

피티드 원형 개발을 위한 20대 남성의 체형 특성 및 유형화 (Body Type Characteristics and Classification of Men in Their 20s for Development of Fitted Sloper)

  • This study is a preliminary study to suggest a distinguished fitted sloper according to body types. To this end, this study classified body types of men in their 20s in an attempt to examine the characteristics of each type. A total of 842 adult men of age 20 to 29 were studied by using the direct measurement data taken in the 6th Size Korea. The subjects' body types were classified based on 48 measurements. We extracted a total of six factors that characterized the body types of adult men in their 20s: horizontal size, vertical length, shoulder development, trunk area length, hip area length and shoulder slope. Result of the cluster analysis, the body shapes of men in their 20s were classified into four types. We developed the overlapped silhouettes for each of the four body types and compared the front and side shapes to clearly identify the differences of each body types. The findings of this study indicated that Korean adult men in their 20s can be classified into the big inverted triangular type that represents healthy and muscular men, the small inverted triangular type that represents short and skinny men, the triangular type that represents men with abdominal obesity, and the rectangular type that represents men that are fat overall, with a fairly even distribution of each body type.

  • Objectives: The prevalence of metabolic syndrome has recently increased, Payments from the Korea Labor Welfare Corporation for compensation for mortality in workers caused by cardiovascular and cerebrovascular diseases have also increased in Korea in recent years. The association of metabolic syndrome and cardiocerebrovascular disease has been investigated by several researchers in recent studies, This study was conducted in an attempt to characterize the relationship between metabolic syndrome and Korean cardiocerebrovascular disease risk assessment, and to provide basic data to group health practices for the prevention of cardiocerebrovascular disease. Methods: Health examinations were previously conducted for 1526 male researchers at a private laboratory. The prevalence by age and the odds ratio of metabolic syndrome scores into the "cardiocerebrovascular risk group" (sum of low, intermediate, and high risk groups) of the Korean cardiocerebrovascular disease risk assessment were assessed, in an effort to elucidate the associations between metabolic syndrome and cardiocere brovascular disease risk assessment. Results: The prevalence of metabolic syndrome and inclusion in the cardiocerebrovascular risk group was 11,7% and 22.1% respectively. The severity of metabolic syndrome and cardiocerebrovascular risk assessment showed that individuals in their 40's and 50's were at higher risk than those in their 30's (p<0,001). The age-adjusted odds ratio of metabolic syndrome to cardiocere brovascular risk group inclusion was 5.6. Conclusions: An active prevention program for cardiocerebrovascular disease needs to begin in the 40's, as the prevalence of metabolic syndrome and the risk group of cardiocerebrovascular risk assessment peak in the 40's age group. The odds ratio between metabolic syndrome and the cardiocerebrovascular risk group was high, which indicates that metabolic syndrome scores should be utilized as guidelines during the consultation and behavioral modification program for the workplace prevention of cardiocerebrovascular diseases in group health practices.

지방흡입술 후 초기 대사성 변화 및 고찰 (Early Metabolic Changes and Its Considerations after Liposuction)

  • Purpose: Advanced techniques now make it possible to remove considerable amounts of subcutaneous adipose tissue more safely with minimum blood loss. However, few have analyzed the metabolic consequences of liposuction. The purpose of this study was to identify the early effects of the surgical removal of subcutaneous fat on metabolic changes in patients who have undergone liposuction. Methods: Nineteen patients were evaluated from June 2005 to December 2005. Preoperative body weight, serums levels of lipids, apolipoprotein A1, dehydroepiandrosterone(DHEA), uric acid, insulin, and glucose were evaluated. Insulin resistance was determined using the homeostasis model assessment of insulin resistance (HOMA-IR), which is based on fasting glucose and insulin concentrations. All of these data were remeasured in 1 and 4 weeks postoperatively. Tumescent fluid was infiltrated using the superwet technique. The liposuction device used was a $Liposlim^{(R)}$ power-assisted unit. Results: Average volumes of infiltrate and aspirate were 3,268mL and 2,892mL, respectively. Results in 1 week postoperatively demonstrated a significant difference in high-density lipoprotein(HDL) cholesterol, apolipoprotein A1, insulin, and HOMA-IR levels. However, all values were within normal limits and returned to baseline in 4 weeks postoperatively.Conclusion: This study provides little to support the presumed therapeutic effect of liposuction. And, it is unclear whether liposuction can prevent or be used to treat the metabolic complications of obesity. However, the results of the present study lead us to believe that liposuction is a metabolically safe procedure.

한국인의 계란 섭취와 대사증후군과의 관계: 2007-2008 국민건강영양조사 자료를 이용하여 (Egg Consumption and Prevalence of Metabolic Syndrome in Korean Adults: Based on 2007-2008 Korean National Health and Nutrition Examination Survey)

  • There is a great public concern that higher consumption of eggs may increase the risk of chronic disease due to high levels of cholesterol in eggs. In contrast to this concern, limited studies have examined this relationship, and the study results have been inconsistent. This study was to evaluate the cross-sectional associations between egg consumption, blood markers and metabolic syndrome in Korean adults. We analyzed data from the combined 2007-2008 Korean National Health and Nutrition Examination Survey (KNHANES). Egg intakes were assessed using a food frequency questionnaire. Metabolic syndrome was ascertained using criteria of the NCEP ATP III (2001) and abdominal obesity criteria of the WHO. In 5,548 subjects, 937 subjects were defined as having metabolic syndrome. In linear regression models, subjects reporting one or more egg consumption per day had significantly lower levels of triglyceride, fasting blood glucose and blood pressure, and a higher level of HDL cholesterol, compared to those who rarely consumed eggs (P-trends < 0.05). After adjustments for potential confounding factors, the associations were no longer significant except for total cholesterol, showing higher levels with frequent egg consumption. More frequent intakes of egg (1 egg or more/day) were associated with lower risk of metabolic syndrome in a logistic regression model (OR: 0.69, 95% CI: 0.50-0.96), but this association also became insignificant after adjustments for multiple confounding factors (OR: 1.05, 95% CI: 0.71-1.57). In healthy Korean adults, frequent intakes of eggs did not adversely affect the risk of metabolic syndrome. Further studies are needed to confirm this finding.

부산지역 주민의 연령별 패스트푸드 이용실태 (A Comparative Study on Fast Food Consumption Patterns Classified by Age in Busan)

  • This study was carried out to investigate the fast food consumption patterns classified by age in Busan. The survey was conducted from October 15 to November 15, 2006 by questionnaires and data analyzed by SPSS program. The results are summarized as follows: Forty one point five percent of the elementary group, 40.5% of the high school group, 24.1% of the 30's, 35.0% of the 40's and 18.1% of the 50's took fast foods over once a week. Seventy two point five percent of the elementary group, 61.5% of the high school group, 16.8% of the 30's group, 10.0% of the 40's, and 14.6% of the 50's preferred fast foods. There was a significant difference in the basis for selecting menus among the groups. The most important basis for selecting menus was 'price' in the elementary group and the high school group, but was 'preference' in the adult groups. As their age increased, they spend more money for fast food. Forty six percent of the elementary group and 49.5% of the high school group, 32.1% of the 30's, 36.5% of the 40's, 34.7% of the 50's thought that fast food can substitute for a meal. The age affected significantly the substitutability for the meal of the fast food (p < 0.001). In the high school group, the ratio of skipping breakfast is higher than in the other age groups. Dietary attitude has direct effects on the preference of the fast foods. Nutrition knowledge, degree of unbalanced diet and obesity rate have direct and indirect effects which were mediated by dietary attitude. Nutrition knowledge showed the greatest total causal effect in relation to the preference of the fast foods. Therefore, nutrition education for the subjects is needed to encourage them to choose more nutritious food and have healthier dietary pattern.

비만성인의 영양소 섭취량 및 식사 다양성 평가 (Evaluation of the Dietary Diversity and Nutrient Intakes in Obese Adults)

  • The purpose of this study was to assess the diet diversity, food habit and nutrient intake of obese adults who were visiting the health promotion center. This study was accomplished with the 138 obese adults (men = 103, women= 35) aged over 20 years old whose BMI were above $25 kg/m^2$. Nutrient adequacy ratio (NAR), the number of foods (Dietary Variety Score DVS), and food group consumed (Dietary Diversity Score, DDS) by using the data from the three days record were analyzed and the food habit and lifestyle were assessed by self reporting questionnaire. The average enemy intake of men was 2150.2 kcal which was significantly higher than that of women (p < 0.05). The intake ratio of carbohydrate, protein and fat over total energy was 54.8% : 19.3% : 25.8% in men, 59.5% : 17.8% : 22.6% in women respectively. Frequency of the breakfast in a week above 4, $2{\sim}3$ time and under one time was 75.7% 10.7% and 9.7% in men 77.1%, 5.7% and 14.3% in women respectively. frequency of eating between meals in a day under one time was 73.8% in men, 57.1% in women (p < 0.05). The average DDS and DVS was $3.63{\pm}0.07$ and $14.10 {\pm}3.45$, respectively which was significantly correlated with MAR (r=0.40 in DDS, r=0.64 in DVS, p < 0.01). The most frequent style of food pattern was DMGFV = 01101 in 35% of men, and DMGFV= 01111 in 37.1% of women. Our results show that dietary diversity and variety are useful parameters far evaluating nutrient intakes in obese adults. These findings suggest that nutritional education based on obese persons' eating behavior and eating diversity may be required to increase educational efficiency of weight control programs.

Trends in energy intake among Korean adults, 1998-2015: Results from the Korea National Health and Nutrition Examination Survey

  • BACKGROUND/OBJECTIVES: Assessing changes in energy intake and dietary sources is important to understand trends in the prevalence of obesity. Thus, we examined trends in energy intake and its nutrient and food sources in Korean adults from 1998 through 2015. SUBJECTS/METHODS: This study included 70,769 subjects aged ${\geq}19$ years who completed a nutrition survey. Subject data were obtained from the 1998, 2001, 2005, 2007-2009, 2010-2012, and 2013-2015 Korea National Health and Nutrition Examination Surveys. Dietary intake was assessed by a 1-day 24-hour recall method. RESULTS: In men, the daily energy intake significantly increased from 2,196 kcal in 1998 to 2,489 kcal in 2013-2015 (P for trend < 0.0001). However, the daily energy intake among women did not change significantly over the same period (P for trend = 0.5772). The percentages of energy intake from animal foods (e.g., meat and milk) and beverages increased during the study period in both men and women. However, the percentage of energy intake from plant foods decreased due to a marked decrease in the intake of white rice. Changes in food sources of energy intake led to changes in the nutrient sources of energy intake; for example, the increase of energy intake from fat and decrease of energy intake from carbohydrate. CONCLUSIONS: This study suggests that since 1998, energy intake has increased among Korean adult men, but not among women. However, the composition of food and nutrient sources of energy intake has changed in both men and women. Energy intake and its nutrient and food sources should continue to be monitored regularly in the Korean adult population.

2015 한국 성인의 수분 섭취기준 설정 (Establishment of reference intake of water for Korean adults in 2015)

  • Purpose: This review was performed to establish a reference intake of water for Korean (KDRI-water) adults in 2015 by examining current knowledge of the relationship between water intake and health and the general status of water intake and loss. We expect that this study will be utilized for further refinement of KDRI-water. Methods: Documents were searched using RISS, NDSL, DBPIA, CINAHL, and Pubmed with the keywords 'water intake, water supply, water ingestion, hydration, dehydration, water balance, and fluid balance'. Results: Water balance is essential for the maintenance of health. Based on this assumption, numerous studies have been performed to investigate the association of water intake with several diseases such as urolithasis, obesity, diabetes, and cancer as well as other health problems, including constipation, cognition, and fetal weight. Effects of water intake for prevention or relief of these health problems vary. Water is supplied to the body by eating foods and drinking liquids such as plain water and beverages. Metabolic water is another source of water input. Water is lost through urine, skin, respiration, and feces. KDRI-water 2015 was set by adequate intake (AI) based on water intake volume, which was the sum of water intakes from foods and fluids reported by the Korea National Health and Nutrition Examination Survey, with extra milk intake of 200 mL. AIs in some age groups were modified considering their estimated energy requirements. Conclusion: Accurate data of water intake is critically important for the establishment of KDRI-water. Therefore, improvement of systems investigating water intake is required, and more studies on the status of water intake and loss in Korean people are needed for definite KDRI-water establishment.

지수치를 이용한 노년 여성의 상반신 체형 분류와 판별에 관한 연구 (Upper Body Somatotype Classification and Discrimination of Elderly Women according to Index)

  • The aim of this study is to provide fundamental data on the development of ready-to-wear clothes appropriate for the body types of elderly women. The study was conducted targeting 318 elderly women over 60 years of age whose fields of action were colleges for the elderly, sports centers, or business sites in Seoul and the neighboring districts. A total of 44 features in the upper body were used for the anthropometric measurement and analysis using anthropometry and photometry. The results of the study are as follows: 1. Somatotypes were classified into three types according to a cluster analysis using height and weight indices. Type 1 is the group with long and undersized upper body and straight body type since the face of the upper body is long relative to height and width, girth and depth are the smallest relative to weight, the breasts are somewhat fat, with a small extent of drooping and a straight back. Type 2 is the group that is considered fat relative to the body, has broad shoulders, drooping breasts with a wide space between them, and a back-bent upper body. Type 3 is the group that has a bent shape, the shortest upper body relative to height, and showing average obesity factors. 2. Indices of height and weight were used for factor analysis, cluster analysis, and discriminant analysis in order to classify upper body somatotype according to shape while excluding size factors of elderly women's upper body somatotype. The same method was used to compare and verify the result according to the absolute measurement and height index. Classification based on height and weight indices demonstrate that such somatotype classification minimizes the personal equation of body shape and it induces better classification based on shape as the results showed the highest cumulative sum of square(CUSUM) at 78.38% while six factors showed the smallest result and the hit rate for the classified three groups showed the highest result at 95.30%.
