Clinical evaluation of sinus bone graft in patients with mucous retention cyst

  • Background: Mucous retention cyst refers to a cyst made by expansion due to the blockage of the salivary gland near the maxillary sinus, and it is surrounded by epithelial cells. Most of them are small; therefore, they cannot be found well and are frequently with antral polyp. The aim of this study was to evaluate the clinical prognosis of sinus bone graft in patients with mucous retention cyst. Methods: This study was performed retrospectively on 23 patients who had sinus bone graft. Group 1 was 8 patients (10 sinuses) who had a mucous retention cyst, and group 2 was 15 patients (17 sinuses) who had no pathologic history about the maxillary sinus. For these patients, sinus bone graft was performed using the lateral approach technique. The total 51 implants were placed 6.22 weeks on the average after sinus bone graft. Sinus membrane perforation during operation, postoperative complications, marginal bone loss after restorative function, implant success rate, and survival rate were analyzed. Results: There was no complication in group 1, and there were three complications in group 2. In group 2, two cases of implants failed. The types of postoperative complications consisted of two minor infections and one wound dehiscence. Two implants of total 51 implants were removed, and the survival rate of implants was 96.08 % (group 1 100 %, group 2 93.5 %). The total success rate of implants was 92.2 % (group 1 95 %, group 2 90.3 %). Conclusions: The clinical prognosis was not affected by the presence of mucous retention cyst.

Incidentally found cystic lymphangioma of the adrenal gland in an elderly male cadaver

  • Adrenal cystic lymphangiomas are extremely rare entities that are often identified incidentally, with less than 60 cases reported to date. We found a protruding ovoid mass consisting of a multiloculated cystic lesion within right adrenal gland in the cadaver of a 75-year-old Korean man. The epithelial cells lining the adrenal cyst were diffusely positive for cluster of differentiation 31 and podoplanin, and negative for pan-cytokeratin. The histopathological diagnosis confirmed a cystic lymphangioma arising from the adrenal gland. Post-mortem findings of the present case are discussed based on the clinicopathological features of adrenal cystic lymphangiomas.

잔존 봉합사에 의해 발생한 점액종 (MUCOCELE CAUSED BY UNREMORED SUTURE SILK : A CASE REPORT)

  • 점액종이란 소타액선 분비관의 폐쇄 또는 파열로 인한 타액의 저류로 나타나는 종창에 대한 임상적인 용어이다. 점액종은 조직학적으로 삼출형과 잔류낭종형으로 분류되며, 삼출형은 외상등으로 소타액선 분비관이 파열된 후 점액이 조직내로 유출되어 조직간격에 고여있는 상태이기 때문에 상피 피복을 볼 수 없고 낭종벽은 육아조직으로 둘러싸여있다. 그리고 잔류낭종형은 소타액선 분비관이 작은 타석에 의해 폐쇄되거나 분비관 주위 반흔조직의 협착으로 발생하며 낭종강은 원주상피 또는 위중층 편평상피로 피개되어 있다. 본 증례는 3년전 하순의 열창 치료를 위해 봉합된 후 발사되지 않는 silk 봉합사에 의해 발생한 점액종의 치험례로, 발사되지 않은 봉합사가 하순 소타액선의 분비관을 관통하여 파열시켜서 점액이 결체조직으로 유출되어 점액종이 발생하였다.

Effect of 2, 6-Dichlorobenzonitrile on Amoebicidal Activity of Multipurpose Contact Lens Disinfecting Solutions

  • Multipurpose contact lens disinfecting solutions (MPDS) are widely used to cleanse and disinfect microorganisms. However, disinfection efficacy of these MPDS against Acanthamoeba cyst remain insufficient. 2, 6-dichlorobenzonitrile (DCB), a cellulose synthesis inhibitor, is capable of increasing the amoebical effect against Acanthamoeba by inhibiting its encystation. In this study, we investigated the possibility of DCB as a disinfecting agent to improve the amoebicidal activity of MPDS against Acanthamoeba cyst. Eight commercial MPDS (from a to h) were assessed, all of which displayed insufficient amoebicidal activity against the mature cysts. Solution e, f, and h showed strong amoebicidal effect on the immature cysts. Amoebicidal efficacy against mature cysts remained inadequate even when the 8 MPDS were combined with $100{\mu}M$ DCB. However, 4 kinds of MPDS (solution d, e, f, and h) including $100{\mu}M$ DCB demonstrated strong amoebicidal activity against the immature cysts. The amoebicidal activity of solution d was increased by addition of DCB. Cytotoxicity was absent in human corneal epithelial cells treated with either DCB or mixture of DCB with MPDS. These results suggested that DCB can enhance the amoebicical activity of MPDS against Acanthamoeba immature cyst in vitro.

양식 조피볼락(Sebastes schlegeli)의 Epitheliocystis, Microcotyle sebastes 및 미동정 생물체의 혼합감염 (A mixed infection of epitheliocystis, Microcotyle sebstes and unknown aetiology in cultured rockfish (Sebastes schlegeli) from farms in Huksan Island)

  • 흑산도 근해의 가두리 양식장에서 아가미에 백색 결절을 주 증상으로 하는 조피볼락이 발견 되었다. 간과 신장에서는 병원균의 검출과 병리 조직학적인 변화가 전혀 발견되지 않았다. 아가미의 경우, epitheliocystis, Microcotyle sebastes 및 아가미 새변의 연골내에 미동정 생물(unknown aetiology)의 cyst가 확인되었다. Epitheliocystis의 cyst크기는 약 44.1-68.9 $\mu{m}$이었고 주로 새변의 끝과 중간부위에 기생하고 있었다. 그러나 새변내의 cyst형성에도 불구하고 숙주의 반응은 미약했다. M. sebastes는 2차새변에 고착기를 이용하여 달라붙어있었으며, 이로 인하여 상피세포의 증생이 발생하였다. 미동정 생물체의 경우, cyst는 호산성의 물질로 충만되어 있었고 cyst전체가 연골에 의해 싸여 있는 형태를 보였다. 하지만 이러한 cyst의 형성에도 불구하고 새변의 크기가 증대된 것 이외에는 별다른 병리학적인 증상은 보이지 않았다. 이들 기생충들의 의한 숙주세포의 조직반응은 미약하여 폐사는 유발되지 않았지만, 호흡과 삼투압 조절에는 장애가 있을 것으로 추정된다.

Mouse models of polycystic kidney disease induced by defects of ciliary proteins

  • Polycystic kidney disease (PKD) is a common hereditary disorder which is characterized by fluid-filled cysts in the kidney. Mutation in either PKD1, encoding polycystin-1 (PC1), or PKD2, encoding polycystin-2 (PC2), are causative genes of PKD. Recent studies indicate that renal cilia, known as mechanosensors, detecting flow stimulation through renal tubules, have a critical function in maintaining homeostasis of renal epithelial cells. Because most proteins related to PKD are localized to renal cilia or have a function in ciliogenesis. PC1/PC2 heterodimer is localized to the cilia, playing a role in calcium channels. Also, disruptions of ciliary proteins, except for PC1 and PC2, could be involved in the induction of polycystic kidney disease. Based on these findings, various PKD mice models were produced to understand the roles of primary cilia defects in renal cyst formation. In this review, we will describe the general role of cilia in renal epithelial cells, and the relationship between ciliary defects and PKD. We also discuss mouse models of PKD related to ciliary defects based on recent studies.

Expression and secretion of CXCL12 are enhanced in autosomal dominant polycystic kidney disease

  • Autosomal dominant polycystic kidney disease (ADPKD), one of the most common human monogenic diseases (frequency of 1/1000-1/400), is characterized by numerous fluid-filled renal cysts (RCs). Inactivation of the PKD1 or PKD2 gene by germline and somatic mutations is necessary for cyst formation in ADPKD. To mechanistically understand cyst formation and growth, we isolated RCs from Korean patients with ADPKD and immortalized them with human telomerase reverse transcriptase (hTERT). Three hTERT-immortalized RC cell lines were characterized as proximal epithelial cells with germline and somatic PKD1 mutations. Thus, we first established hTERT-immortalized proximal cyst cells with somatic PKD1 mutations. Through transcriptome sequencing and Gene Ontology (GO) analysis, we found that upregulated genes were related to cell division and that downregulated genes were related to cell differentiation. We wondered whether the upregulated gene for the chemokine CXCL12 is related to the mTOR signaling pathway in cyst growth in ADPKD. CXCL12 mRNA expression and secretion were increased in RC cell lines. We then examined CXCL12 levels in RC fluids from patients with ADPKD and found increased CXCL12 levels. The CXCL12 receptor CXC chemokine receptor 4 (CXCR4) was upregulated, and the mTOR signaling pathway, which is downstream of the CXCL12/CXCR4 axis, was activated in ADPKD kidney tissue. To confirm activation of the mTOR signaling pathway by CXCL12 via CXCR4, we treated the RC cell lines with recombinant CXCL12 and the CXCR4 antagonist AMD3100; CXCL12 induced the mTOR signaling pathway, but the CXCR4 antagonist AMD3100 blocked the mTOR signaling pathway. Taken together, these results suggest that enhanced CXCL12 in RC fluids activates the mTOR signaling pathway via CXCR4 in ADPKD cyst growth.

하악 소구치 부위에 발생한 석회화상피성치성종양이 혼재된 선양치성종양: 증례보고 (Combined Adenomatoid Odontogenic Tumor and Calcifying Epithelial Odontogenic Tumor in the Mandible: Case Report)

  • Adenomatoid odontogenic tumors represent 3 to 7 percent of all odontogenic tumors. These tumors are more common in the maxilla than the mandible and usually include the anterior region. Clinically, the most common symptom is painless swelling and the tumor is associated with an unerupted tooth, typically a maxillary or mandibular cuspid. The adenomatoid odontogenic tumor appears radiographically as a unilocular radiolucency around the crown of an impacted tooth, resembling a dentigerous cyst. More often, it contains fine calcifications. Histopathologically, there is a thick wall cystic structure with a prominent intraluminal proliferation of the odontogenic epithelium. The most striking pattern is varying-sized solid nodules of spindle-shaped or cuboidal epithelial cells forming nests or rosette-like structures with minimal stromal connective tissues. Conspicuous within the cellular areas are structures of tubular or duct-like appearance. The duct-like spaces are lined with a single row of cuboidal or low columnar epithelial cells, of which the ovoid nuclei are polarized away from the luminal surface. Small foci of calcification may also be scattered throughout the tumor. These have been interpreted as abortive enamel formations. In some adenomatoid odontogenic tumors, the material has been interpreted as dentoid or cementum.

Cytopathic Change and Inflammatory Response of Human Corneal Epithelial Cells Induced by Acanthamoeba castellanii Trophozoites and Cysts

  • Acanthamoeba castellanii has ubiquitous distribution and causes primary acanthamoebic keratitis (AK). AK is a common disease in contact lens wearers and results in permanent visual impairment or blindness. In this study, we observed the cytopathic effect, in vitro cytotoxicity, and secretion pattern of cytokines in human corneal epithelial cells (HCECs) induced by A. castellanii trophozoites and/or cysts. Morphological observation revealed that panked dendritic HCECs co-cultured with amoeba cysts had changed into round shape and gradually died. Such changes were more severe in co-culture with cyst than those of co-cultivation with trophozoites. In vitro cytotoxicity assay revealed the highest cytotoxicity to HCECs in the co-culture system with amoeba cysts. A. castellanii induced the expression of $IL-1{\alpha}$, IL-6, IL-8, and CXCL1 in HCECs. Secreted levels of $IL-1{\alpha}$, IL-6, and IL-8 in HCECs co-cultured with both trophozoites and cysts were increased at an early incubation time (3 and 6 hr). These results suggested that cytopathic changes and pro-inflammatory cytokines release of HCECs in response to A. castellanii, especially amoebic cysts, are an important mechanism for AK development.
