  • One of the primary objectives of Korean family planning program is to assist parents in having the number of children they want, both by providing fertility-en-hancing services to infecund and subfecund women and by providing fertility-limitation services to fecund women. As far as fertility-enhancing services to infecund and subfecund women is concerned, there has been little effort from the national family planning program, So far, there is no any basic data on infertility prevalence rate among the Korean women. Therefore, an attempt has been made to review data of 1976 and 1978 fertility and family planning surveys in order to estimate the level of infertility rate among the Korean women. In the 1978 survey 3. 7 percent of the current married women responded that they were physically infertile while in the 1976 survey physically infer-tile women was calculated as 3 percent of the total ever married women which is similar level of 1978 data of 3.7 percent. Mean age at first marriage of Korean women is 23 and most of the women are married in the high fecund age. Only 0.3 percent of the respondents married age 30. in addition, the rate of women with no children among the ever married women whose married duration is more than 5 year is very low;2. 5 percent among 5-9 years, 1. 4 percent among 10- 14 years; 1.8 precent among 15-19 years; 1.0 percent among 20-24 years; and 0. 7 percent among 25 or more years of marriage duration. If we consider those data shown above, it is manifest that infertility rate of Korean women is less than 5 percent level which is much lower than the 10 percent level of infertility rate in the United States of America. However, this kind of estimation is still not able to show definite data on fertility rate. Therefore, a nationwide planned survey should be carried out as early as possible to figure out the real situation of infertility rate in Korea.

  • The purpose of the current study was to select vulnerable regions with insufficient child care services based on an in-depth investigation of diverse regional characteristics and to establish a practical goal for the expansion of public child care centers through identifying priority regions in foremost need of public child care centers out of the vulnerable regions. The vulnerable regions were selected from both urban and rural areas with particular criterions to determine their critical situation. First of all, the number of children aged zero to five, private and public child care centers, and capacity and enrolment of the regional child care center were selected from towns (Eup) and townships (Myeon) within rural areas and neighborhoods (Dong) within urban areas to calculate the capacity fulfillment. Specifically, a dense population of low-income families inhabited within urban areas defined the region's vulnerable status for the analysis, whereas poor accessibility of a child care center within rural areas correspond to the lack of child care services. The analytical result displayed that a total of 456 towns and townships did not acquire child care centers. Out of 456 towns and townships, 162 were selected as priority regions. On the other hand, 546 neighborhoods corresponding to upper 30 percentiles of urban area where children receive basic subsidy for low income families were selected as vulnerable regions. Out of 546 neighborhoods, 243 were selected as priority regions according to the analytical result.

  • Purpose: Functional constipation (FC) is a common gastrointestinal (GI) problem affecting children's well-being and quality of life. Although polyethylene glycol (PEG) is recommended as the first line therapy, it is not always applicable in lower socioeconomic populations. Hence, this study aimed to compare clinical courses of FC in children treated with different medications in order to identify prognostic factors related to treatment outcomes. Methods: We reviewed the medical records of patients aged ${\leq}15$ years diagnosed with FC according to the Rome IV criteria from 2007 to 2015 at the GI clinic, Songklanagarind Hospital. Baseline characteristic, medical history, and treatment outcomes were collected at first and subsequent visits. Results: Exactly104 patients (median age at diagnosis, 2.8 years) were diagnosed with FC. The number of follow-up visits per patient ranged from 1 to 35. The median duration of follow-up was 18.0 months (range, 6.0-84.2 months). PEG was given to 21% of patients. During the follow up period, 76% of patients experienced first recovery with a median time to recovery of 9.8 months. There were no significant differences in time until first recovery and relapse between patients who received and those who did not receive PEG (p=0.99 and 0.06, respectively). Age >6 years, normal defecation frequency, no history of cow's milk protein allergy, and use of laxatives were associated with successful outcomes. Conclusion: Treatment outcomes between patients who had and never had PEG demonstrated no significant difference in our study. Hence, current practices in laxative prescriptive patterns may be effective.

  • After the death of Min-sik Kim in Asan, South Chungcheong Province in September 2019, awareness of the school zone has been increasing, and the traffic law is being revised continuously. However, despite these efforts, the number of traffic accidents among children has not decreased significantly. This indicates the need for a new direction considering the behavioral characteristics of children and the traffic environment. Therefore, this study identified the current status and problems of school zones through case analysis of school zones in Korea. In addition, we analyzed the Vision Zero policy, the concept of Home Zone, and engineering-oriented road design cases and operation methods in Sweden, an advanced country in traffic safety. In addition, the cases of the two countries were compared and organized focusing on the basic principles of disaster prevention design. The purpose of this study is to suggest the direction of design and policy to improve school zone problems in Korea. Through this, it is expected that the results of this study will be used as data for future research development for the reduction of school zone fatalities in Korea.

  • This study is peformed to examine the relationship between air pollution exposure and mortality in Incheon for the years of 1998 - 2001. Daily counts of death were analyzed by general additive Poisson model, with adjustment for effects of seasonal trend, air temperature, humidity, and day of the week as confounders in a nonparametric approach. Daily death counts were associated with CO(1 day before), O$_3$(2 day before), PM$_{10}$(1 day before), NO$_2$(1day before), SO$_2$(1 day before). Increase of 32.21 ${\mu}$g/m$^3$(interquartile range) in PM$_{10}$ was associated with 1.9 % (95% CI = 0.8 % - 2.9 %) increase in the daily number of death. This effect was greater in children(less than 15 aged) and elderly(more than 65 aged). We concluded that Incheon had 2 - 4 % increase in mortality in association with IQR in air pollutants. Daily variations in air pollution within the range currently occurring in Incheon might have an adverse effect on daily mortality. These findings also support the hypothesis that air pollution, at levels below the current ambient air quality standards of Korea, is harmful to sensitive subjects, such as children or elderly.

  • This study was conducted to determine the degree of satisfaction and the demand for the current education service of language therapy. We surveyed a total of fifty two nurturers of children who had been diagnosed as autistic. A three-part questionnaire that covered the environment of the therapy institution (7 questions), the programs provided by the Speech-Language Pathologist (7 questions) and the degree of satisfaction and the demand for therapy education (7 questions) was used. The study results showed, in the area of the institution environment for therapy education, that the degree of satisfaction was highest for Social Welfare Center users due to the therapy costs. The level of satisfaction then decreased in the order of private clinic users and general hospital users. However, in the area of education programs provided by Speech-Language Pathologists, it was highest for the users of general hospitals due to the proper contents of the programs and consulting service. The satisfaction level then decreased in the order of private clinic users and Social Welfare Center users. Regarding the level of satisfaction with therapy education and related demand, the degree of satisfaction with language therapy and all therapy education services was generally satisfactory. After the language therapy, the children showed considerable improvement in their living skills (60% of survey respondents) and psychological stability (stress reduction, 30% of the total survey respondents). Regarding the future demand for therapy education, the degree of demand was high for the individual and group language therapy education service. In summary, the results of this study uncovered a number of problems related to services provided by language therapy education institutions and suggested various implications that Speech-Language Pathologists and relevant professionals should endeavor to use so as to enhance the satisfaction level of therapy education for autistic children.

  • The number of multi-cultural members, such as immigrant labors, international marriage women are gradually increasing in Japan society. Now it might be said that Japan has become a multi-cultural society. Recently in Japanese kindergartens and nursery schools the numbers of foreign and bi-cultural children are increasing. The aim of this paper is to review the researches on the cultural diversity in the Japan, especially in childcare. Methodology proceeds from a review of related literature to internet-based data and to the statistics issued by the government. Through reviewing them, the problems of teachers' assumptions and the importance of discussions about teachers' competencies to educate minority children and to communicate with their parents were suggested. As a switch over to multi-cultural society is occurring in Japan, the attitude of assimilationism is weakening, and the local community is now beginning to tolerate the multi-cultural childcare. Consequently, It was over to age that is distinguish identity from only nationality.

  • This paper studies regarding the problems of transmitting disease through food and its prevention in terms of current problematic food poisoning and the transfer of bacterial disease while providing lunch for school children. As the problems of school lunch program, the number of food poisoning and of the transfer of bacterial disease is highly increasing because of the shortcomings of systematic safety management toward food and of the lack of meal supplier's awareness about sanitation. For the prevention the following suggestions are provided:1. It is necessary to shift from the system of meal supply for nutrition to that of meal supply for health. 2. A system that offers a responsibility for nutrition to that of meal supply to a person in charge of hygienist, or a cook, including the a nutritionist, is needed. 3. It is necessary to gain the help from the department of sanitation in city or country for the sanitation guidance and inspection of school meal supply, not to totally rely on the local ministry of education. 4. A manager or employees related to school/group meal supply need to get education constantly on sanitation and public information. 5. For the long term, a systematic sanitary management needs to be accomplished by introducing HACCP system.

  • Steroid 21-hydroxylase deficiency is the most prevalent form of congenital adrenal hyperplasia (CAH), accounting for approximately 95% of cases. With the advent of newborn screening and hormone replacement therapy, most children with CAH survive into adulthood. Adolescents and adults with CAH experience a number of complications, including short stature, obesity, infertility, tumor, osteoporosis, and reduced quality of life. Transition from pediatric to adult care and management of long-term complications are challenging for both patients and health-care providers. Psychosocial issues frequently affect adherence to glucocorticoid treatment. Therefore, the safe transition of adolescents to adult care requires regular follow-up of patients by a multidisciplinary team including pediatric and adult endocrinologists. The major goals for management of adults with 21-hydroxylase deficiency are to minimize the long-term complications of glucocorticoid therapy, reduce hyperandrogenism, prevent adrenal or testicular adrenal rest tumors, maintain fertility, and improve quality of life. Optimized medical or surgical treatment strategies should be developed through coordinated care, both during transition periods and throughout patients' lifetimes. This review will summarize current knowledge on the management of adults with CAH, and suggested appropriate approaches to the transition from pediatric to adult care.
