환자특성에 따른 의료이용에 대한 환자만족도 비교 (The Relationship between Patient Characteristics and Satisfaction with Hospital Care)

  • Purpose: The purpose of this study was to identify the relationship between patient characteristics and patient satisfaction. Methods: A cross-sectional questionnaire survey was conducted in an acute care hospital. The subjects were 317 patients discharged from general medical and surgical nursing care units during September, 2005. Patient satisfaction was measured using the short-form satisfaction scale of Hwang and Park(2001). Additional information about patient characteristics, including general demographics and health care utilization variables, was collected from the hospital information systems. Multiple regression analysis was performed to determine patient characteristics influencing patient satisfaction. Results: Patients were satisfied with hospital care with an average of 4.10 on a five-point Likert scale. Patient characteristics explained 13.5% of the variance of patient satisfaction. The significant factors influencing patient satisfaction were patients' age and perceived health status. There was no significant relationship between structural variables and patient satisfaction. Conclusion: This study showed that patients' characteristics were significant factors explaining patient satisfaction. Therefore, these characteristics should be adjusted in reporting patient satisfaction as an indicator for hospital-level or department-level rating.

여성 차별적 다중 전환기동안의 폐경 경험: 저소득 한인 이민 여성 (Neglecting and Ignoring Menopause Within A Gendered Multiple Transitional Context: Low Income Korean Immigrant Women)

  • Researchers have rarely explored menopausal experience in the context of the totality of women's lives, subsequently making the picture of menopause incomplete, discrete and fragmented. Respecting the totality of women's lives, this study addressed how a vulnerable group of women-low income Korean immigrant women-experience menopause within a context of multiple transitions. This is a cross-sectional study using methodological triangulation. A sample of 119 first-generation Korean immigrant women aged 40 to 60 years, who were in low-income jobs, was recruited using convenience sampling methods. From the total sample, 21 peri-or post-menopausal women were recruited for in-depth interviews following the collection of the survey data. Questionnaires, short interviews, and in- depth interviews were used to collect data. The quantitative data were analyzed using descriptive and inferential statistics. Thematic analysis was used to interpret interview data. The findings indicate that menopause was given the lowest priority amidst women's multiple and demanding roles within a gendered multiple transitional (immigration, work and menopause) context. The lack of language clarity to describe women's experience, cultural background, inadequate knowledge, and lack of social supports made menopause hidden, invisible, and inaudible. Conclusions and implications for nursing practice are guided by the goal of understanding women's experiences and meanings of menopause and supporting women through reflecting these experiences into their health care.

국내여성과 미국이주 한국여성의 유방암에 대한 건강신념과 유방자가검진 비교 (A Comparative Study of Korean and Korean-American Women in Their Health Beliefs related to Breast Cancer and the Performance of Breast Self-Examination)

  • Purpose: This cross-sectional survey was undertaken to examine the differences of BSE (breast-self examination) performance and health beliefs between Korean and Korean-American women and to identify which factors influence the BSE based on the HBM variables. Method: The study subjects were recruited from both Korea(189 women) and Cleveland in Ohio, USA(I46 women). The HBM variables were measured using a reliable and valid Health Belief Model Scale. The subjects were also asked whether or not they did a BSE in the last year. Result: The Korean-American women who performed the BSE was statistically higher than that of Korean women. Regarding to the BSE-related health belief, the scores of benefits, confidence, and health motivation was significantly higher in Korean-American. After controlling for living places, age, education, and job, barriers and confidence variables significantly explained the BSE performance of Korean and Korean-American women. Conclusion: There was a differences in BSE-related health belief and performance between Korean and Korean-American women. Among health belief variables, barriers and confidence were core variables predicting the BSE performance of Korean and Korean-American women together.

중소기업의 외부지식 탐색·활용 정도가 신규시장 확대에 미치는 영향 (Effects of the External Knowledge Search and Utilization Activities of SMEs on Market Expansion)

  • To increase their market shares and grow continuously, it is very important for small and medium-sized enterprises (SMEs) to expand their markets. Although various factors may influence an SME's effort to cultivate a new market, this research focused on activities related to the search and utilization of external knowledge. After conducting Tobit analysis based on the dataset of 959 Korean SMEs included in the 2010 Korean Innovation Survey, we found that external knowledge search and utilization activities positively affect the market expansion of SMEs. This result has two implications: (1) SMEs should actively search for appropriate external knowledge sources with which they can expand their markets and reduce their dependence on internal R&D activities; and (2) they should implement an efficient corporate system to effectively absorb and utilize external knowledge inside the firms. Despite these contributions, this research has its shortcoming in that it utilized a cross-sectional dataset, which can be further analyzed by incorporating the dataset from previous and future periods.

노인성 만성 질환자의 특수영양 및 건강보조 식품의 섭취실태 (Supplement Use of Age-Related Chronic Disease Outpatients in Korea)

  • In order to investigate supplement use in middle-aged and elderly outpatients suffering from age-related diseases over the last year, a cross-sectional survey was conducted from July to December 2001. The subjects were 1702 (male 731, female 971) age-related disease outpatients over age 50. Data was collected using a standardized questionnaire and in-person interviews. The prevalence of supplement use and the different categories of supplements taken by the subjects were examined using SPSS statistical package. Supplements were used by 48.9% of the subjects. The mean number of kinds of dietary supplements taken by subjects was 1.7. Chinese medicine was the most commonly used supplements in both the male and female subjects. Among the reasons for using supplements, health promotion ranked as the most common. Most of the supplement users, however, did not know or only knew roughly about the health claims of their supplements. Subjects reported the information source on supplements as family, friends or relatives followed by doctors or pharmacists. These results may provide basic information for proper supplement use among Korean middle-aged and elderly outpatients with age-related diseases.

Association between Omega Fatty Acid Intake and Suicidality : Sex Differences in the General Korean Population

  • Objectives : Epidemiological studies in other countries show that a low intake of omega-3 fatty acids (FAs) increases the risk of depression or suicidality. However, no studies have investigated the associations of suicidality with omega-3 FAs in Korea. Hence, this cross-sectional study aimed to investigate the effects of omega FAs on suicidality in the general South Korean population. Methods : The data in this study were sourced from adults (n=215,860) who completed the Sixth Korea National Health and Nutrition Examination Survey (KNHANES VI), and the associations between omega FAs and suicidality were analyzed using multivariate logistic regressions. Results : Our results demonstrated that high omega-3 FA intake was associated with a decreased risk of suicide (OR=0.83, 95% CI : 0.71-0.98) and the high omega-6 to omega-3 FA ratio was associated with an increased risk of suicide (OR=1.25, 95% CI : 1.02-1.54). Additionally, a high intake of omega-3 FAs was associated with a decreased risk of suicide in men, but not in women (OR=0.72, 95% CI : 0.59-0.88). Conclusions : Overall, our findings suggest that a lower intake of omega-3 FA is associated with the increased risk of suicidality in the general Korean population, especially in men.

Psychometric Properties of Korean Version of Modified Leeds Sleep Evaluation Questionnaire (KMLSEQ)

  • Purpose: The Leeds Sleep Evaluation Questionnaire (LSEQ) translated into Korean was modified to easily apply and reduce respondents' confusion and was evaluated for psychometric properties and discriminant ability. Methods: A total of 960 Korean adults aged 45 years and older participated in this cross-sectional survey. To test reliability, validity and discriminant ability, Cronbach's alpha, correlation analysis, confirmatory factor analysis, simple regression analysis and receiver operating characteristics (ROC) curve analysis were used. Results: Item-total correlations ranged between 62~.85 and Cronbach's alpha was .95. Area under ROC was .86 (95% CI: .83~.90) and the optimal cutoff score was identified as ${\leq}$ 66 (sensitivity, .77; specificity, .84; positive/negative predictive values, .49/.95). Using this cutoff score, the prevalence of insomnia in the study sample was 25.8% and tended to be more common in female and older groups. Conclusion: The data supported the psychometric properties of Korean Modified Leeds Sleep Evaluation Questionnaire (KMLSEQ) as an acceptable sleep measurement. In addition, KMLSEQ is likely to be a useful screening tool for insomnia.

치과 의료기관 종사자의 감정노동과 직무스트레스가 직무만족에 미치는 영향 (The effects of emotional labor and stress on job satisfaction in oral health professional)

  • Objectives : This study was to investigate the level of emotional labor and stress on job satisfaction and the significant factors influencing job satisfaction in oral health professional. Methods : Data were obtained from a cross-sectional survey of 297 oral health professional(mean age=$29.95{\pm}8.10$). Participants in the study were recruited when they attended the dental health form in Gwangju. Emotional labor was classified into two factors: external, inner. Stress consisted of four factors: sleep disorder&anxiety, self-confidence, health&vital, social role work. job satisfaction included growth development, stability chase. The data was analysed using t-test, one-way ANOVA, Pearson correlation and multiple regression. Results : The mean score of emotional labor and score was 3.13 out of a maximum 7 points. job satisfaction was negatively correlated with emotional labor and stress. job satisfaction explained 32.0%. Conclusions : Based on the findings, The emotional labor and stress was correlated with job satisfaction. These results suggest that further development should be continued to develop the effective emotional labor and stress to improve the job satisfaction of oral healthcare professional.

보건치과위생사의 자율(독립)적 직무수행 요구도 조사 (A study on the needs of independent dental hygiene practices in the public dental hygienists)

  • Objectives: The purpose of this study was to identify the needs of independent dental hygiene practices in the Korean public dental hygienists. Methods: This research was a cross-sectional survey study. 159 Korean public dental hygienists were selected by simple random sampling on March 10, 2017. 133 public dental hygienists were participated, after excluding 26 hygienists who showed insufficient responses. The respondents were asked to select one of the following options to reflect their needs: direct supervision, indirect supervision, and independent dental hygiene practice. Results: The results showed that the public dental hygienists wanted both public oral health practices (62.5%) and assistance in preventive dental treatments (63.4%) to be performed independently. Conclusions: The public dental hygienists were required to independently perform oral health education and fluoride mouth rinsing projects. Dental hygienists should improve their capability of independently practicing dental hygiene first, and then indicators. It should be established and measured to evaluate their competency in this respect. Legal protection should be considered for independent dental hygiene practices.

The Effect of Cigarette Smoking on Physical Fitness and Depression of Patients with Chronic Low Back Pain

  • Purpose: This study was conducted to investigate the effect of cigarette smoking on physical fitness (dominant hand grip power, dominant isokinetic leg muscle strength, abdominal muscle endurance, flexibility, cardiopulmonary endurance) and depression in patients with chronic low back pain. Methods: This study was a cross sectional study and subjects consisted of 60 young males with chronic low back pain. The subjects were allocated to two groups following a self-report survey: cigarette smoking group (n=25) or non-smoking group (n=35). Physical fitness (dominant hand grip power, dominant isokinetic leg muscle strength, abdominal muscle endurance, flexibility, cardiopulmonary endurance) were measured using objective methods and depression in patients with chronic low back pain was measured using the Korean version of center for epidemiologic studies depression (CES-D) scale. Results: The results of this study were as follows: In physical fitness, the cigarette smoking group showed a significant decrease in abdominal muscle endurance, flexibility, and cardiopulmonary endurance compared with the non-smoking group. Depression index (CES-D scale score) was significantly higher in the cigarette smoking group than in the non-smoking group. Conclusion: These results suggest that cigarette smoking had a negative effect on abdominal muscle endurance, flexibility, and cardiopulmonary endurance in patients with chronic low back pain. In addition, depression of patients with chronic low back pain was affected by cigarette smoking. Thus, we suggested that cigarette smoking may play a significant role in the deterioration of physical fitness and depression of chronic low back pain patients.
