X-ray PIV Measurements of Velocity Field of Blood Flows

  • The x-ray PIV method was improved for measuring quantitative velocity fields of real blood flows using a coherent synchrotron x-ray source. Without using any contrast media or seeding particles, this method can visualize flow pattern of blood by enhancing the phase-contrast and interference characteristics of blood cells based on a synchrotron x-ray imaging mechanism. The enhanced x-ray images were achieved by optimizing the sample-to-scintillator distance, the sample thickness, and hematocrit. The quantitative velocity fields of blood flows inside opaque tubes were obtained by applying a 2-frame PIV algorithm to the x-ray images of the blood flows. The measured velocity field data show typical features of blood flows such as the yield stress effect. The non-Newtonian flow characteristics of blood flows were analyzed using the x-ray PIV method and the experimental results were compared with hemodynamic models.

초음파 조영제를 애용한 Echo PIV 기법의 개발 (Development of Echo PIV Using Ultrasound Contrast Agent)

  • The combination of ultrasound echo images with digital particle image velocimetry (DPIV) methods has resulted in a two-dimensional, two-component velocity field measurement technique appropriate for opaque flow conditions including blood flow in clinical applications. Advanced PIV processing algorithms including an iterative scheme and window offsetting were used to increase spatial resolution. The optimum concentration of the ultrasound contrast agent used for seeding was explored. Velocity validation tests in fully developed laminar pipe flow result of echo PIV showed good agreement with both optical PIV measurements and the known analytic solution based on a volume flow measurement.

Enhancement of Speckle Contrast in vivo by Combining Linearly Polarized Laser Light and an Analyzer

  • Speckle imaging is capable of dynamic data acquisition at high spatiotemporal resolution, and has played a vital role in the functional study of biological specimens. The presence of various optical scatterers within the tissue causes alteration of speckle contrast. Thus structures like blood vessels can be delineated and quantified. Although laser speckle imaging is frequently used, an optimization process to ensure the maximum speckle contrast has not been available. In this respect, we here report an experimental procedure to optimize speckle contrast via applying different combinations of varying polarization of the illuminating laser light and multiple analyzer angles. Specifically, samples were illuminated by the p-polarization, 45°-polarization, and s-polarization of the incident laser, and speckle images were recorded without and with the analyzer rotated from 0° to 180° (Δ = 30°). Following the baseline imaging of a solid diffuser and a fixed brain sample, laser speckle contrast imaging (LSCI) was successfully performed to visualize in vivo mouse-brain blood flow. For oblique laser illumination, the maximum contrast achieved with p-polarized and s-polarized light was perpendicular to the analyzer's axis. This study demonstrates the optimization process for maximizing the speckle contrast, which can improve blood-flow estimation in vivo.

Contrast Enhanced Ultrasonography and CT Features of Gastrointestinal Stromal Tumor in a Dog

  • A large abdominal mass was incidentally found in a 13-year-old mixed-breed dog and was confirmed to be a cecal gastrointestinal stromal tumor (GIST). Contrast-enhanced ultrasound and post-contrast computed tomography (CT) showed mild contrast enhancement of the mass, indicating low blood flow. The tumor origin was determined to be the cecum by identifying the vessels supplying the mass on post-contrast CT. The exophytic growth of the tumor left the cecal lumen intact without obstruction. This report described the CEUS and CT perfusion of the cecal GIST and perfusion evaluation can help diagnose and characterize GISTs in dogs.

요추 경추간공 경막외강 약물주입 시 조영상의 분석 (An Analysis of the Contrast Patterns of Lumbar Transforaminal Epidural Injection)

  • Background: Lumbar transforaminal epidural injections (LTEIs) have been utilized in the treatment of radicular pain, and LTEIs have the advantage of target-specificity. However, there have not been enough studies on the contrast patterns in LTEIs with using fluoroscopy. The purpose of this study was to evaluate the spreading epidural contrast patterns that are seen during real-time fluoroscopic guided LTEIs. Methods: A total of 131 patients who underwent fluoroscopic guided LTEIs were studied. The inclusion criteria were those patients with low back pain and/or lower extremity pain that was caused by a herniated nucleus pulposus, lumbar spinal stenosis, failed back surgery syndrome, and herpes zoster-associated pain. We classified the contrast patterns in regard to the contrast flow spreading to the nerve root and/or the unilateral, bilateral or cylinderic type of epidural spreading on the AP view of the fluoroscopy and the ventral or dorsal epidural filling on the lateral view. In addition to the pattern analysis, we evaluated the range of contrast spreading from the cranial to the caudal epidural filling and the incidence of an intravascular flow pattern. Results: Epidural spreading was seen in 126 cases (96.2%) of the total patients through the nerve root. Ventral spreading occurred in 120 cases (95.2%). On the AP view, a nerve root with unilateral, bilateral and cylinderic epidural filling was noted for 108 (85.7%), 9 (7.1%) and 9 (7.1%) cases, respectively. The contrast spreading to vertebral segments was smaller for the patients with lumbar spinal stenosis and failed back surgery syndrome than for the other groups (P < 0.0083). The incidence of intravascular injection was 11.1% (14/126). Conclusions: LTEIs using fluoroscopic visualization provided excellent assessment of the ventral epidural filling as well as nerve root filling. However, unilateral epidural spreading was prominent for the LTEIs.

Cerebral Blood Flow Monitoring by Diffuse Speckle Contrast Analysis during MCAO Surgery in the Rat

  • The rodent model has been used frequently to understand stroke pathophysiology, due to its low cost and the large spectrum of genetic strains available. Here, we present a diffuse speckle contrast analysis system (DSCA) with a $1{\times}2$ optical switch that was used to non-invasively assess cerebral blood flow (CBF) changes in the rat during intraluminal suturing for middle cerebral artery occlusion (MCAO) surgery. The blood flow index (BFI) in the left hemisphere was lower than that in the right hemisphere because the left middle cerebral artery was occluded. Furthermore, the performance of the DSCA system was compared with that of commercial laser Doppler flowmetry. The changes in the BFI measured by the two systems were correlated strongly. The DSCA system was less sensitive to motion artifacts and able to measure relatively deep tissue flow in the rat's brain. In conclusion, the DSCA system secured CBF monitoring during surgery in a rodent model without craniotomy.

혈관조영검사에서 매개변수 변화에 따른 Roadmap 영상의 화질평가 (Evaluation of Roadmap Image Quality by Parameter Change in Angiography)

  • 이 연구의 목적은 Roadmap 영상에서 화질에 영향을 미치는 인자들을 알아보기 위한 것으로, 조영제의 희석률, Collimation Field, Flow Rate를 변화하여 연구를 하였다. 화질의 정량적인 평가를 위해, 아크릴를 이용하여 3mm 혈관모형의 Water Phantom을 자체 제작하였고, 자체 제작한 혈관모형의 Water Phantom으로 Roadmap 영상을 획득하고, SNR(Signal to Noise Ratio)과 CNR(Contrast to Noise Ratio)을 분석하였다. CM : N/S 희석률 변화에 대한 연구에서 CM : N/S 희석률을 (100%~10% : 100%)로 변화를 주었으며, 혈관모형 Water Phantom을 이용하여 촬영한 Roadmap 영상의 SNR과 CNR의 측정 결과 CM에 N/S 희석률이 높아질수록 SNR의 측정값이 점차적으로 낮아짐을 나타났고, CNR의 측정값도 점차적으로 낮아짐을 나타났다. 결론적으로 CM : N/S의 희석률이 높아질수록 SNR과 CNR 낮아짐을 확인하였고, CM : N/S의 희석률(100%~70 : 30%)에서 유의한 이미지를 얻을 수 있음을 확인하였다. Collimation Field 변화에 대한 연구에서 혈관모형 Water Phantom을 이용하여 Colimation Field를 혈관모형 중심으로 좌, 우 2 cm 간격으로 좁히면서 0 cm, 2 cm, 4 cm, 6 cm, 8 cm 10 cm, 12 cm으로 각각 변화를 주었으며, Roadmap을 촬영한 영상의 SNR과 CNR의 측정 결과는 Collimation Field를 혈관모형 중심으로 좁힐수록 SNR과 CNR의 측정값이 증가하는 것을 확인할 수 있었다. Flow rate 변화에 대한 연구에서 Autoinjector의 Volume을 15로 일정하게 하고, Flow Rate를 1, 2, 3, 4, 5, 6, 7, 8, 9, 10 으로 각각 변화를 주었다. 혈관모형 Water Phantom을 이용하여 Roadmap 영상을 촬영한 이미지의 SNR과 CNR의 측정 결과 Flow Rate를 증가했을 때, SNR의 측정값이 점차적으로 감소하다가 Flow Rate 9~10에서 SNR의 측정값이 점차적 증가를 보였고, CNR의 측정값도 점차적으로 감소하다가 Flow Rate 9~10에서 CNR의 측정값이 점차적으로 증가를 보였다. 그러나 ROI Mean 값과 Background Mean 값으로 SNR과 CNR의 상관관계를 확인할 수 없었다. 상관관계를 확인하기 위해 Flow Rate 변화에 따른 Roadmap 연구는 향후 더 많은 연구로 확인해야 할 것으로 사료된다. 결론적으로 Roadmap 영상의 화질에 영향을 미치는 인자들을 알아보기 위해 조영제의 희석률, Collimation Field, Flow Rate 변화에 대한 연구에서 조영제에 N/S의 희석률이 증가할수록 SNR과 CNR이 낮아져 화질과 대조도가 낮아지는 것을 확인하였으며, Collimation Field를 좁힐수록 SNR과 CNR이 증가하여 화질과 대조도가 높아지는 것을 확인하였다. 그러나 Flow Rate 변화에 대한 연구에서는 상관관계를 확인할 수 없었다. 검사 및 시술을 할 때 신장의 영향을 최소화하기 위해 적절한 조영제 농도 선택과 대조도 향상 및 피폭 감소를 위한 적절한 Collimation Field를 사용하는 것이 유용할 것으로 판단된다.

Nanoparticle Contrast in Magneto-Motive Optical Doppler Tomography

  • We introduce a novel contrast mechanism for imaging superparamagnetic iron oxide (SPIO) nanoparticles (average diameter ${\sim}100nm$) using magneto-motive optical Doppler tomography (MM-ODT), which combines an externally applied temporally oscillating high-strength magnetic field with ODT to detect the nanoparticles flowing through a glass capillary tube. A solenoid cone-shaped ferrite core extensively increased the magnetic field strength ($B_{max}=1\;T,\;{\Delta}|B|^2=220T^2/m$) at the tip of the core and also focused the magnetic force on targeted samples. Nanoparticle contrast was demonstrated in a capillary tube filled with the SPIO solution by imaging the Doppler frequency shift which was observed independent of the flow rate and direction. Results suggest that MM-ODT may be a promising technique to enhance SPIO nanoparticle contrast for imaging fluid flow.

유동모형을 이용한 저속유동의 자기공명영상 (MR Imaging of Slow-flow Using a Flow Phantom)

  • 목적 : 고식적 스핀에코, 위상 대조 경사에코, 고속 GRASS, 중T-2강조 고속스핀에코 연쇄를 각각 유동모형에 적용시켜, 저속유동에 대한 자기공명영상의 민감도를 찾아내고자 한다. 대상 및 방법 뇌실-복강 단락 도관과 GE 모형으로 이루어진 유동모형으로 싸이폰 효과에 의한 지속적인 가변 유속의 흐름을 내보내고, 각각의 유속에 위의 네 가지 자기공명영상연쇄를 적용시켰다. 0.08 ml/min 에서 1.7 ml/min 범위의 유량의 흐르는 액체와 정지된 액체에서 획득된 자기공명영상의 신호강도를 비교한 결과를 요약하면 다음과 같다. 결과 0.17 ml/min 이상의 느린 흐름에서는 고식적 스핀에코영상이 가장 우수한 정지-유동 액체사이의 겉보기대조를 보이나, 그 이하의 매우 느린 흐름에서는 GRASS 영상이 더 높은 민감도를 보였다. 결론 : 4가지 자기공명펄스연쇄는 0.08 ml/min에서 1.7 ml/min 범위의 유량의 흐름에서 서로 다른 민감도를 보였다. 이 결과는 임상적으로, 단락수술후의 뇌척수액의 흐름이나 혈관내의 혈전에 의한 흐름의 변이 등의 인체내의 저속유동을 검출하는 비침습적방법으로 유용하게 적용될 수 있을 것이다.

관류자기공명영상의 이해 (Understanding of Perfusion MR Imaging)

  • Perfusion MR imaging is how to use exogenous and endogenous contrast agent. Exogenous perfusion MRI methods which are dynamic susceptibility contrast using $T2^*$ effect and dynamic contrast-enhanced using T1 weighted image after injection contrast media. An endogenous perfusion MRI method which is arterial spin labeling using arterial blood flow in body. In order to exam perfusion MRI in human, technical access are very important according to disease conditions. For instance, dynamic susceptibility contrast is used in patients with acute stroke because of short exam time, while dynamic susceptibility contrast or dynamic contrast enhancement provides the various perfusion information for patients with tumor, vascular stenosis. Arterial spin labeling is useful for children, women who are expected to be pregnant. In this regard, perfusion MR imaging is required to understanding, and the author would like to share information with clinical users
