In Vitro and In Vivo Physiological Characteristics of Dietary Fiber from By-product of Aloe vera Gel Processing (알로에 베라 유래 식이섬유의 In Vitro 및 In Vivo 생리기능 특성)

  • A fiber fraction (Aloe cellulose), the by-product obtained from Aloe vera gel processing was freeze dried and investigated for in vitro glucose/ bile acid retarding effects of powdered sample (100 mesh) comparing with commercial $\alpha$-cellulose as a reference sample. We also examined the effectiveness of physiological functionality such as the antiobesity and anti-constipation on Sprague-Dawley (SD) rat. The Aloe cellulose powders during in vitro dialysis experiment for 2 hours exhibited the glucose and bile acid retarding index of 20.32-35.2% and 53.13-28.30%, respectively. Especially, freeze dried aloe cellulose showed the 2.5 and 1.2-6 times higher effect on in vitro glucose and bile acid retardation than those of $\alpha$-cellulose. These relatively good retarding effects on glucose and bile acid diffusion suggest a potential of preventing from diabetes and arteriosclerosis of some extent. Also, the results from animal experiments on SD rats fed a high-fat diet for 4 weeks suggested that Aloe cellulose might be used as a novel dietary fiber showing an effective anti-obesity and anti-constipation effect.

A Case Report on Right Hemiparesis after Left Thalamus Infarction Treated with Boyanghwanoh-tang-gagam and Complex Korean Medicine Treatment (보양환오탕가감방 및 복합한의치료로 호전된 시상 부위 뇌경색 후 우측 편마비 환자 치험 1례)

  • Objectives: This study reports the case of a patient with right hemiparesis after a left thalamic infarction, in which the patient experienced improved clinical symptoms (hemiparesis, constipation) after treatment with a complex Korean medicine treatment. Methods: A 81-year-old male patient diagnosed with acute left thalamic infarction was treated with daily acupuncture and herbal medicine (Boyanghwanoh-tang-gagam) three times a day. Jowiseunggi-tang extract granules were administered to treat constipation by adjusting the dosage according to the reported symptoms. The patient's clinical course was assessed using changes in Modified Barthel Index (MBI) score, Manual Muscle Testing (MMT) grade, and defecation frequency . Results: The patient's MBI score improved from 35 to 69 and was discharged with the ability to walk under supervision. The MMT grade also improved from 3/4 to 5/5 in the upper/lower extremities. Other clinical symptoms, including constipation, also improved, leading to the discontinuation of magnesium medication. Conclusion: These findings suggest that complex Korean medicine treatments, including herbal medicine and acupuncture, may be an effective treatment for post-stroke hemiplegic patients.

Effect of Resistant Starch on the Large Bowel Environment and Plasma Lipid in Rats with Loperamide-Induced Constipation (저항전분 투여가 Loperamide 유도 변비 쥐의 대장환경과 혈청지질에 미치는 효과)

  • The present study was conducted to evaluate the effect of resistant starch (RS) on the large bowel function and plasma lipids in rats with constipation induced by Loperamide. Animals were divided into six groups: normal control-5% cellulose, constipation-5% cellulose, constipation-5% pectin, constipation-5% RS-type 2 (RS2), constipation-8% RS2 and constipation-5% RS type 3 (RS3) groups, and fed experimental diets for five weeks. The results from RS groups were compared with those from other dietary fiber groups. The groups supplemented with RS3 or high level of RS2 showed significantly increased counts of bifidobacteria in the cecum than the other groups. The production of total short chain fatty acids in the cecal contents was significantly high in pectin, RS3 and high RS2 groups. The pH in the cecal contents of the RS supplemented groups was significantly decreased compared with the cellulose supplemented groups. The production of prostaglandin E2 in the colon mucus of the RS groups was higher than the normal group; however, it was significantly decreased compared to the cellulose or pectin supplemented constipated groups. The thickness of the mucus layer and the production of mucus from epithelial cells were significantly increased in RS3 group compared to the constipated cellulose group. Supplementation of resistant starch significantly elevated the ratio of HDL-cholesterol to total cholesterol and significantly lowered plasma atherogenic index compared with cellulose or pectin supplementation in constipated rats. The results of the present study demonstrated that resistant starch supplementation may help in improving the large bowel environment by stimulation of bifidobacterial proliferation, reduction of pH and inflammation factor and by increased production of mucus. It has also been found that an additional health benefit is improvement in lipid levels of serum.

The Latest Trend of Dynamic MR Defecography for the Chronic Constipation Patient (만성 기능성 변비 환자에서 동적 MR Defecography의 최신동향)

  • With advancement of the medical imaging technology, the dynamic pelvic MRI (magnetic resonance imaging) has been introduced and used for dynamic MR defecography to improved diagnosis of the patients. At the early stage of its use, it was mostly used to diagnose enterocele or cystocele, then its use was extended to diagnose the organ prolapse and other dysfunctional pelvis organs. There now have been many reports of other diseases such as the functional constipation and others. This paper introduces the pelvis MRI and the dynamic MR defecography and reports the future trend in their clinical applications. Until recently, the studies with pelvic MRI were mostly focused on observing the movement of the pelvis in the supine position. Yang and 26 others reported good result in observing the patients with the pelvic organ prolapse by using the pubococcygeal line as the anatomical index. Using the fast gradient recalled acquisition (fast GRASS), they compared cystocoele, genitourinary prolapse, enterocoele and rectocoele with the control group. Kruyt et al. observed the posterior compartment and reported that MRI was more helpful than the fluoroscopy. Healy et al. applied the dynamic MRI test on the patients with constipation or incontinence as well as the control group without those symptoms. Since then, MRI technology has further advance by Lienemann, who was able to attain the more detailed images using the fast T2 weighted turbo spin echo technology, and others. If its limitation in diagnosing intussusception and the like, since the observation can be made only from the supine position, can be overcome with open MR or others, it is envisages that the method can eventually replace the radiological defecography.

The Effects of Moxibustion on Cornell Medical Index in the Elderly (쑥뜸요법이 노인의 CMI에 미치는 효과)

  • Purpose: To improve health care of the elderly, we have used Moxibustion to examine the effectiveness of this product from 1st Feb. 2007 to 31 st May for 78 people who are older than 65 years old twice a week. Method: The tool of research was modified Cornell Medical Index(CMI) which Nam Ho Chang(1965) modified for the Korean and CMI which consists of 26 questions. Data were analyzed in frequency, paired t-test by SPSS. Result: Hypothesis: 'It was supported that CMI increases for those who have treated Moxinustion'. There were significant differences between before and after using Moxibustion when the symptom is one of those which are sore throat, tonsillar, edema, tightness of nose, common cold, cough, tightness of chest, palpitation, dyspnea, edema of leg, difficulty of urination, anorexia, heavy on stomach, nausea, vomiting, dyspepsia, fullness of abdomen, stomachache, diarrhea, constipation, incontinency of stool, arthralgia, shoulder or backache, lumbago, headache, burning or chillness, vertigo and dizziness. Conclusion: As a result, Moxibustion is effective for such symptoms so that it is considered to be using in everyday life this product for better health care.

Effects of Medicinal Enzyme Powder on Intestinal Mobility, Lipid Level, and Blood Parameters of Loperamide-Induced Constipation in Rats (약선효소 분말식이가 Loperamide의 투여로 유발된 변비 쥐의 장 운동과 지질 함량 및 혈액학적 성분 변화에 미치는 영향)

  • This study was aimed at investigating whether dietary therapy using medicinal enzyme powder is effective in reducing constipation caused by loperamide in rats. Nine-week-old male Sprague Dawley were subdivided into 4 groups: normal diet group (C), loperamide treatment and normal diet (CL), medicinal enzyme powder diet (E), and loperamide treatment and medicinal enzyme powder diet (EL). Constipation was induced by subcutaneous injection of loperamide (1.5 mg/kg) 3 days prior to sacrifice. The treatment with loperamide led to an increase in weight gain, a decrease in the number and wet weight of fecal pellets, and a decrease in intestinal motility. The administration of the medicinal enzyme powder significantly reduced weight gain but increased intestinal mobility compared with the loperamide-treated group. The treatment with loperamide in the normal diet group reduced the activities of both suggesting that constipation may be involved in the low level of glutamic oxaloacetic transaminase (GOT) and glutamic pyruvic transaminase (GPT). Additionally, the loperamide treatment in the medicinal enzyme powder diet group increased the level of GOT, but reduced the level of GPT. Loperamide treatment also reduced cholesterol and increased the atherogenic index (AI) and cardiac risk factors (CRFs). Interestingly, the treatment with the medicinal enzyme powder effectively attenuated both the increase in AI and the reduction in high density lipopretein (HDL)-cholesterol, caused by the treatment with loperamide. Although there were no significant differences in the blood protein level, including hemoglobin and hematocrit, between the normal diet group and the loperamide-treated group, the administration of the medicinal enzyme powder to the loperamide-treated group effectively increased the levels of both hemoglobin and hematocrit. Collectively, the results demonstrate that the medicinal enzyme powder can help to combat the negative events caused by constipation.

Relationship between Halitosis and Gastric Fever Analysed by Questionnaire Cold-Heat Patternization (구취의 한열변증 분석: 구취는 위열증으로 발생하는가?)

  • Objectives: We performed this study to check the effect of Cold-Heat attribute analyzed quantitatively by questionnaire on halitosis patient. Methods: We made a questionnaire composed of 15 items(6 categories) about the contents of Cold-Heat and asked 105 subjects to answer in the form Likert-like seven-points score. And then, we extracted Cold-Heat attribute from 15 items, 6 categories, 6 weighted categories, heat score and cold score. On the other hand we measured the halimeter measurements. VAS, winkle tongue coating index. Afterward, the data were analyzed by statistical methods. Results:1) In the 15 items, question number 6, 7, 8 is negative correlated with halimeter measurements. Question number10 is positive correlated with halimeter measurements. And question number 11, 12, 14 is positive correlated with VAS. 2) In the 6 categories, constipation is negative correlated with halimeter measurements. Chill is positive correlated with VAS. And Cold-limbs is positive correlated with halimeter measurements. 3) In the 6 weighted score categories, constipation is negative correlated with halimeter measurements. Chill is positive correlated with VAS. And Cold-limbs is positive correlated with halimeter measurements. Conclusions: Further clinical research is necessary in the development of the questionnaire's items. And to get the better of this study, we should research more analytic method made casual relationship clear between Cold-Heat attributes and the index of halitosis.

A Comparative Study between First Attack and Reattack Groups in C.V.A (중풍환자의 초발군과 재발군에 관한 임상적 고찰)

  • Objectives : This study was designed to investigate significant differences between the first attack and reattack groups to form fundamental data for decreasing recurrence and secondary prevention of stroke. Methods : I studied 204 patients admitted within 7 days of onset, after the diagnosis of stroke, in the Oriental Medical Hospital of Dongeui University from February to July in 2001.compared the reattack group with the first attack group for risk factors, clinical symptoms and progress, average mark and degree of improvement in Activity Index. Results : 1. Meaningful risk factors associated with the reattack group were male sex, a past history of hypertension and transient ischemic attack, and a family history of stroke. 2. The reattack group had conscious or cognitive disorder in early stage of onset, dysphagia, constipation, urinary incontinence, visual field defect in acute stage, high blood pressure and tachycardia in abnormalvital sign in acute stage, neuropsychosis, shoulder pain and rigidity, and pneumonia in complications more than the first attack group. 3. In Activity Index, the average mark of reattack group was lower than that of the first stroke group and improvement of the reattack group was delayed compared with the first attack group. Conclusions : The reattack group had more severe symptoms and clinical progress than the first attack group.

  • The Purpose of this study· was to investigate the dietary intake and health for male and female old persons living in Young Dong by mean of questionnaire and interview. The results from the above survey are summerized as follows ; 1. The aged average height, weight, and physical index were 166cm, 60kg, 19.31 in male, 156cm, 50kg, 20.27 in female which were almost the same as the Korean average standard (male; 167. 0cm, 61. 0kg, 21.8, female; 156cm, 53.0kg, 21.8). Among the condition of diseases, neuralgia was 45.3%, hypertension was 12.2%, constipation was 10.4%. 2. Correlation coefficients between systolic blood pressure, diastolic blood pressure and age were significant(P<0.05, P<0.01), and also those between systolic blood pressure, diastolic blood pressure and Body Mass Index were significant(P<0.01, P<0.005). 3. Correlation of food intake frequency of protein group, calcium group, vitamin and mineral group was divided three levels-Good, Fair, Poor. Food intake frequency as the factors(three groups) which might influence the condition of health was partly significant (P<0.05, P<0.1).

  • Introduction : Atopic dermatitis is very epidermic disease in these days. Atopic dermatitis is difficult for patient to be treated because there is no specific treatment. The oriental method of atopic dermatitis is varied as patient situation and doctor's view point. The Treatise on Febrile Diseases(傷寒論) may be a hint for atopy disease. Object: Although we use various method such as acupucture, aroma therapy, a medicine for external use, we use the two decoctions in the Treatise on Febrile Diseases according to differentiation of syndromes and abdominal diagnosis. Result : After inpatient treatment in 21 days, the itching, redness, exudation, crust, lichen get better and insomnia and constipation was removed quickly. The SCORAD index is decreased from 84.2 to 25.9. Conclusion : These decoction can treat the atopic dermatitis although there is no symptom in atopy because we cure the all situation of the patient. We can cure the atopic dermatitis not only by routine remedy for various symptoms but also by diagnosis and treatment based on overall analysis of symptoms and signs.
