임플란트 시술에서 CAD/CAM을 포함한 컴퓨터 기술의 발달은 단순히 임플란트를 보다 정확하고, 예측 가능하게 식립하는 것뿐 아니라, 무피판(flapless) 수술과 즉시하중을 가능하게 하였다. 그러므로 술후 출혈과 불편감의 감소 및 시술시간과 치유기간을 단축시키는 장점이 있으며, 시술 후 즉시 보철물을 장착해 줌으로써 환자의 구강기능을 단기간에 회복시키고 경조직과 연조직의 형태를 보존하여 최종보철 시 유리한 환경을 만들어 줄 수 있게 되었다. 본 증례는 상 하악 무치악 상태로 내원한 40세 남성 환자로 하악 총의치의 불안정성 및 불편감 등을 호소하였으며, 하악에 임플란트 지지 고정성 보철물을 통한 수복을 원하였다. 충분한 골량, 환자의 참여도, 경제적인 여건, 전신적인 건강 등을 고려하여 하악에는 CAD/CAM 기반의 수술용 스텐트를 이용한 수술 및 미리 제작된 고정성 임시보철물을 이용하여 즉시하중을 부여하는NobelGuide 보철을 계획하였으며, 상악에는 총의치를 계획하였다. 환자의 안모를 평가하여 임시의치를 제작하였으며, 복제한 임시의치를 바탕으로 스텐트를 제작하고, 컴퓨터단층촬영을 시행하였다. 3차원으로 변환된 영상을 기반으로 하악에 7개의 고정체를 생역학적 조건과 치조골의 상태에 맞게 분산 배치하였다. 제작된 수술용 스텐트에 맞게 주모형을 제작하고, 임시고정성 보철물을 제작하여 장착함으로써 즉시하중을 부여하였다. 식립 3개월 후 골유착 정도를 평가하고 최종보철물을 제작하였다. 추후 발생할 수 있는 상악 골흡수를 방지하기 위해 주기적인 내원 및 검사를 통해 성공적인 치료가 될 수 있도록 노력하였다.
The osseous or osteocutaneous free fibula flap has become the gold standard for most mandibular reconstructions because of its favorable osseous characteristics. However, disadvantages, such as the time-consuming reconstructive step, difficulty in performing the osteotomies to precisely recreate the shape of the missing segment of mandible and poor bone-to-bone contact play a role in making the surgeons look for alternative flaps. With the advent of computerized design software, which accurately plans complex 3-dimensional reconstructions, has become a process that is more efficient and precise. However, the ability to transfer the computerized plan into the surgical field with stereolithographic models and guides has been a significant development in advancing reconstruction in the maxillofacial regions. The ability to "pre-plan" the case, mirror and superimpose natural structures into diseased and deformed areas, as well as the ability to reproduce these plans with good surgical precision has decreased overall operative time, and has helped facilitate functional and esthetic reconstruction. We describe a complex case treated with this technique, showing the power and elegance of computer assisted maxillofacial reconstruction from the University of Michigan, Oral and Maxillofacial Surgery.
디지털 방식을 이용한 치과 임플란트 진료에서 가장 주목받는 분야 중 하나가 디지털 소프트웨어와 컴퓨터 단층촬영을 이용하여 수술용 스텐트를 제작, 임플란트를 식립하는 분야이다. 컴퓨터를 이용한 디지털 임플란트 치료계획 시스템과 이를 이용한 수술용 가이드는 예지성과 심미성, 기능성을 갖춘 임플란트의 식립과 수복을 가능하게 한다. 특히 이러한 가이드 수술은 임플란트 수복물의 형태와 기능을 고려한 임플란트의 식립 시 더욱 유리하다. 최근 여러 제조사에서 보철물 제작을 위해 치과용 스캔부터 제작방법까지 다양한 디지털 방식의 제작과정을 제시하고 있다. 이러한 방법들은 정확하고 예지성 있는 예후를 위해 여러 연구자들에 의해 연구되어지고 있으며, 점차 발전하고 있다. 이번 증례는 완전무치악의 여성 환자에서 임플란트 고정성 보철물을 이용한 수복증례이다. 하악은 all-on-four 방식을 적용하여 4개의 임플란트를 식립하였으며, 상악은 이에 기초하여 6개의 임플란트를 식립하였다. 최종보철물에 기초한 임플란트의 식립을 위해 임플란트 수술용 스텐트를 이용하였으며, 환자는 치료 결과에 만족하였으며, 5년 추적관찰 결과, 임플란트와 보철물 모두 안정적으로 유지되고 있다.
임플란트를 이용한 고정성 보철 치료를 위해서는 보철 치료에 적합한 위치에 임플란트가 식립되어야 하며 이를 위해 guided implant surgery와 CAD-CAM을 활용한 보철물 제작이 최근 널리 사용되고 있다. 수술 시에는 surgical guide를 이용해 flapless 방식으로 최소침습 보철수복이 가능 하다. 본 증례의 환자는 86세 남성 환자로 기존 의치 사용 시 불편함으로 치료를 원하였다. 환자는 전신질환으로 인해 가철성 국소의치 사용이 어려워 상악은 총의치, 하악의 경우 임플란트 고정성 보철물 수복하였다. 전신질환으로 인해 높은 출혈 위험성을 가지므로 surgical guide를 사용해 flapless 방식으로 임플란트를 식립하였다. 최종적으로 상악 총의치 및 하악 나사 유지형 지르코니아를 보철물을 제작하였으며, 만족스러운 기능 및 심미 회복을 보였기에 보고하는 바이다.
In image-guided surgery, automatic bone segmentation of Computed Tomography (CT) images is an important but challenging step. Previous attempts include intensity-, edge-, region-, and deformable curve-based approaches [1], but none claims fully satisfactory performance. Although active contour (AC) techniques possess many excellent characteristics, their applications in CT image segmentation have not worthily exploited yet. In this study, we have evaluated the automaticity and performance of the model of Chan-Vese Multiphase AC Without Edges towards knee bone segmentation from CT images. This model is suitable because it is initialization-insensitive and topology-adaptive. Its segmentation results have been qualitatively compared with those from four other widely used AC models: namely Gradient Vector Flow (GVF) AC, Geometric AC, Geodesic AC, and GVF Fast Geometric AC. To quantitatively evaluate its performance, the results from a commercial software and a medical expert have been used. The evaluation results show that the Chan-Vese model provides superior performance with least user interaction, proving its suitability for automatic bone segmentation from CT images.
Accuracy and reproducibility of coordinates, angles/areas and volume measurements are the mai goal of imaging-guided stereotactic systems. Errors in measurements are due to pitfalls in a present systems. Factors responsible for inaccuracy and variability on measurements are inappropriate display window settings, unequal spatial resolution, display/film distortion, inappropriate slice width, lack of isocentricity between gantry and frame, and nonparallelism between frame and scanning plan. The most important factor responsible for errors when using stereotactic frames is the nonparallel relationship to the plane of scanning. For the solution of above problem, author developed a computer program for the measurement of the coordinates of intracerebral target, which is operated using the personal computer. This program can calculate the actual spatial coordinates regardless of the inappropriate parallelism between frame and scanning plane and decrease the range of errors of measurements.
With the advancement of information technology, the application of augmented reality (AR) in dentistry is an emerging research field of image-guided surgery and dental education. In addition, the digital approach to incorporating AR in dental practice management is considered to be feasible. A prototype is developed to apply AR to dental daily clinical practice in order to help dentists to access electronic dental records. This prototype delivers patients' information and related clinical data to dental clinicians directly without the need to search for the appropriate patients. Wearable AR devices are considered to be a convenient tool for practicing dentists because dental practitioners are not always able to use a computer during active clinical sessions, such as implant placement, root canal treatment, and patient-doctor communication. The use of AR to visualize passive transferred patient data would be valuable for practicing dentists.
Long-term survival and prognosis of narrow-diameter implants have been reported to be adequate to consider them a safe method for treating a deficient alveolar ridge. The objective of this study was to perform case report of narrow-diameter implants with a trapezoid-shape in anterior teeth alveolar bone. A 50-year-old male patient presented with discomfort due to mobility of all of the maxillary teeth and mandibular incisors. Due to destruction of alveolar bone, four anterior mandibular teeth were extracted. Soft tissue healing was allowed for approximately 3 months after the extraction, and a new design of implant placement was planned for the mandibular incisor area, followed by clinical and radiological evaluation. Implant placement was determined using an R2GATE surgical stent. The stability of the implants was assessed by ISQ measurements at the first and second implant surgery and after prosthetic placement. At 1 and 3 months and 1 year after implantation of the prosthesis, clinical and radiological examinations were performed. Another 50-year-old male patient presented with discomfort due to mobility of the mandibular central incisors. For the same reason as in the first patient, implant placement was carried out in the same way after extraction. ISQ measurements and clinical and radiological examinations were performed as in the previous case. In these two clinical cases, 12 months of follow-up revealed that the implant remained stable without inflammation or additional bone loss, and there was no discomfort to the patient. In conclusion, computer-guided implant surgery was used to place an implant in an optimal position considering the upper prosthesis. A new design of a narrow-diameter implant with a trapezoid-shape into anterior mandibular alveolar bone is a less invasive treatment method and is based on the contour of the deficient alveolar ridge. Through all of these procedures, we were able to reduce the number of traumas during surgery, reduce the operation time and total treatment period, and provide patients with more comfortable treatment.
Purpose: This study investigated the accuracy of free-hand implant surgery performed by an experienced operator compared to static guided implant surgery performed by an inexperienced operator on an anterior maxillary dental model arch. Methods: A maxillary dental model with missing teeth (No. 11, 22, and 23) was used for this in vitro study. An intraoral scan was performed on the model, with the resulting digital impression exported as a stereolithography file. Next, a cone-beam computed tomography (CBCT) scan was performed, with the resulting image exported as a Digital Imaging and Communications in Medicine file. Both files were imported into the RealGUIDE 5.0 dental implant planning software. Active Bio implants were selected to place into the model. A single stereolithographic 3-dimensional surgical guide was printed for all cases. Ten clinicians, divided into 2 groups, placed a total of 60 implants in 20 acrylic resin maxillary models. Due to the small sample size, the Mann-Whitney test was used to analyze mean values in the 2 groups. Statistical analyses were performed using SAS version 9.4. Results: The accuracy of implant placement using a surgical guide was significantly higher than that of free-hand implantation. The mean difference between the planned and actual implant positions at the apex was 0.68 mm for the experienced group using the free-hand technique and 0.14 mm for the non-experienced group using the surgical guide technique (P=0.019). At the top of the implant, the mean difference was 1.04 mm for the experienced group using the free-hand technique and 0.52 mm for the non-experienced group using the surgical guide technique (P=0.044). Conclusions: The data from this study will provide valuable insights for future studies, since in vitro studies should be conducted extensively in advance of retrospective or prospective studies to avoid burdening patients unnecessarily.
CAD-CAM 기술의 사용은 다양한 부분에서 치료의 결과를 향상시킨다. 본 증례는 법랑질형 성부전증 환자를 디지털 기술을 이용해 치료한 전악구강회복 증례이다. 임상적 검사 결과 occlusal stop의 부재로 저작기능의 저하와 치아 상실, 왜소치로 인한 치간사이 공간, 불충분한 수직피개 등의 문제점이 파악되었다. 하악의 치아 상실 부위를 수복키 위해 CAD-CAM 수술 스텐트를 이용한 임플란트 식립이 진행되었고 추가적으로 부족한 골량을 보강하기 위해 골유도재생술이 동반되었다. 상하악 자연치는 모두 전장관수복을 통해 새로운 교합평면 설정 및 안정적인 수직교합관계를 형성하였다. ARCUS digma 장비를 통한 잠정수복물 평가 후 디지털 기술을 이용하여 잠정수복물 상태의 수복물을 더블스캔을 통한 복제를 하여 최종수복물을 제작, 환자에게 수복하여 전악수복을 완성하였다. 이를 통해 치료의 효율성과 예지성 있는 결과, 환자의 적응기간 감소와 기능적, 심미적으로 높은 만족도 등을 이루어냈다.
