항암단을 투여 받은 림프절 외 변연부 B세포 림프종 환자에 대한 증례보고 (A Case Report on Extranodal Marginal Zone B Cell of Mucosa-associated Lymphoid Tissue (MALT) Type Lymphoma Treated with Hangam-dan)

  • Objective : The aim of this case report is to drive further studies evaluating the effectiveness of Korean oriental medicine on extranodal marginal zone B cells of MALT (mucosa-associated lymphoid tissue) type lymphoma. Methods : This case report is about a patient who was diagnosed with extranodal marginal zone B cell of MALT type lymphoma, who refused chemotherapy and preferred to be treated with Korean oriental medicine. Neck, chest abdomen & pelvis with enhanced computed tomography (CT) and positron emission tomography computed tomography (PET-CT) were performed to evaluate the anticancer effect of Hangam-dan (HAD). Results : The patient diagnosed with MALT showed partial response after receiving 17 months of Korean oriental medical treatment. Neck, chest, abdomen & pelvic CT and PET-CT show decrease in size. Conclusion : This case report shows a possibility that Korean oriental treatment could offer potential benefits for patients with extranodal marginal zone B cell of MALT type lymphoma.

  • Yolk sac tumors are rare malignant germ cell neoplasms that usually arise from the gonads. Extragonadal yolk sac tumors (EGYSTs) frequently occur in the mediastinum in post-pubertal females. EGYSTs in the pelvis are extremely rare, and to date, only thirteen cases have been reported in the English literature. Among them, the primary EGYST of the pelvic peritoneum in post-pubertal females has only been reported in ten cases. The present case describes a 26-year-old female diagnosed with primary peritoneal yolk sac tumor located in the rectouterine pouch. We report clinical and tumor imaging features, including ultrasound, computed tomography (CT), magnetic resonance images (MRI), positron emission tomography-computed tomography (PET-CT), and present a review of the literature.

봉독약침을 시행한 우측상악동의 악성 흑색종 환자에 대한 증례보고 (A Case Report on the Patient of Malignant Melanoma at Right Maxilla with the Treatment of Bee Venom Phamacopuncture)

  • Objectives : It is the aim of this study to derive lurker studies evaluating the effectiveness of bee-venom phamacopuncture on malignant melanoma patients. We present a patient of malignant melanoma at right maxilla who survives over one year with stable disease (SD) by the treatment of Bee Venom Phamacopuncture (BVP). Methods : We followed the treatment and examination. We prescribed to the patient what to be taken 1.5cc BVP once a day. Picture series, Head series were followed-up and Neck computed tomography (CT) and positron emission tomography computed tomography (PET CT) were performed to evaluate the therapeutic efficacy. Results : The patient survives over one year and continued stable disease over 6 months. Picture series, Head series X-ray, neck CT and PET CT were shown no interval change. Conclusion : This case may give us the possibility that BVP offers potential benefits for patients with malignant melanoma.

Breast PET CT 영상 재구성 변화에 따른 대조도 대 잡음비와 신호 대 잡음비의 비교평가 (Evaluation and Comparison of Contrast to Noise Ratio and Signal to Noise Ratio According to Change of Reconstruction on Breast PET/CT)

  • 본 연구는 양전자 컴퓨터단층촬영(Positron Emission Computed Tomography; PET-CT) 유방암 영상에서 질환관심영역(Region of Interest; ROI)의 대조도 대 잡음비(Contrast to Noise Ratio; CNR)와 신호 대 잡음비(Signal to Noise Ratio; SNR)를 재구성 기법별로 측정 평가하여 통계적 분석을 하고자 하였다. 연구대상은 서울소재 대학병원에서 유방 양전자 컴퓨터단층촬영(Breast PET-CT)을 검사한 환자 100명이다. 측정방법은 ImageJ 프로그램을 사용하여 표본의 인구사회학적 특성, 영상 재구성 기법별 유방암의 SNR과 CNR의 평균값, 95% 신뢰구간 값, SNR과 CNR의 평균차이 값 등을 분석하였다. SPSS Statistics21 통계프로그램으로 ANOVA 분석을 하였으며 p<0.05에서 유의한 것으로 판단하였다. 영상 재구성 기법별 분석결과는 CNR과 SNR 모두 TrueX_TOF, Iterative, Iterative-TOF, FBP-TOF 순으로 높은 값을 나타냈다(p<0.05). CNR과 SNR수치로 영상 재구성 기법의 특징을 조합하여 PET/CT 검사의 정확성과 편의성을 제공할 것이라 사료된다.

저농도 조영제를 사용한 CT검사에서 저관전압 기법에 따른 유용성 평가 (Evaluation of the Low Tube Voltage in the Computed Tomography Scan Technique using a Low Concentration Contrast Agent)

  • 본 연구는 전산화단층검사(computed tomography; CT)에서 저농도 조영제와 저관전압 기법이 선량과 영상에 미치는 유용성을 팬텀으로 평가하였다. 조영제 농도별로 희석정도를 달리하여 실험팬텀에 삽입 후 관전압과 관전류를 단계적으로 변화하여 촬영하였고, 획득된 팬텀영상을 통해 선량과 HU를 평가하였다. 그 결과 표준조건(350 mgI/ml, 120 kV)보다 저농도 조영제와 저관전압 조건(300 mgI/ml, 100 kV)설정이 체적 선량평가(CTDIvol; computed tomography dose $index_{vol}$)에서 평균 21% 감소하였다. SNR은 저농도 조영제와 저관전압 조건이 팬텀에서 측정한 모든 깊이(center, 4.5cm, 2.25cm)의 조영제와 생리식염수를 1:10, 1:20으로 희석한 실험에서 각각 12.2(26%), 6.2(17%) 증가하였다. CNR은 각각 11.5(32%), 6.3(26%) 증가하였다. CT 검사에서 조영제 부작용 감소를 위한 저농도 조영제의 사용과 피폭선량 감소를 위한 저관전압 사용으로 영상에서 높은 감약계수를 통한 다양한 연구가 필요할 것으로 사료된다.

컴퓨터 단층촬영을 이용한 요로결석 검사에서 저선량 CT의 적용에 대한 유용성 평가 (Usability Evaluation of Applied Low-dose CT When Examining Urinary Calculus Using Computed Tomography)

  • 본 연구는 컴퓨터단층촬영을 이용한 요로결석 검사에서 저선량 CT(Low dose Computed Tomography) 프로토콜의 적용에 따른 유용성을 평가하였다. 연구대상은 2016년 6월-12월까지 부산지역 일개 의료기관을 내원한 비뇨기계 환자이며 연구에 이용된 프로토콜은 통계적 반복 재구성법(Adaptive Statistical Iterative Reconstruction, ASIR)을 50% 적용한 저선량 CT 프로토콜이다. 정량적 분석 결과 횡단면상(Axial image)의 신장의 관심 영역(region of image. ROI) 내 평균 화소 값과 표준편차는 복부촬영 프로토콜 $26.21{\pm}7.08$, 저선량 CT 프로토콜 $20.03{\pm}8.16$이며 관상면(Coronal imalge) 영상에서 신장의 관심영역 내 평균 화소 값과 표준편차는 복부촬영 프로토콜 $22.07{\pm}7.35$, 저선량 CT 프로토콜 $21.67{\pm}6.11$이었다. 정성적 분석결과 인공물에 대한 4명의 관찰자 평균값은 복부촬영 프로토콜 $19.14{\pm}0.36$, 저선량 CT 프로토콜 $19.17{\pm}0.43$이며 해상도 및 대조도의 평균값은 복부촬영 프로토콜 $19.35{\pm}0.70$, 저선량 CT 프로토콜 $19.29{\pm}0.58$이었다. 피폭선량 분석 결과 복부촬영 프로토콜의 CTDIvol 평균값은 18.02 mGy, DLP 평균값은 $887.51mGy{\cdot}cm$이며 저선량 CT 프로토콜 CTDIvol 평균값은 7.412 mGy, DLP 평균값은 $361.22mGy{\cdot}cm$이었다. 이로 인한 선량의 감소율은 각 58.82%, 59.29% 이었다.

Incidental Abnormal FDG Uptake in the Prostate on 18-fluoro-2-Deoxyglucose Positron Emission Tomography-Computed Tomography Scans

  • 18-fluoro-2-deoxyglucose positron emission tomography-computed tomography ($^{18}F$-FDG PET/CT) scans are commonly used for the staging and restaging of various malignancies, such as head and neck, breast, colorectal and gynecological cancers. However, the value of FDG PET/CT for detecting prostate cancer is unknown. The aim of this study was to evaluate the clinical value of incidental prostate $^{18}F$-FDG uptake on PET/CT scans. We reviewed $^{18}F$-FDG PET/CT scan reports from September 2009 to September 2013, and selected cases that reported focal/diffuse FDG uptake in the prostate. We analyzed the correlation between $^{18}F$-FDG PET/CT scan findings and data collected during evaluations such as serum prostate-specific antigen (PSA) levels, digital rectal examination (DRE), transrectal ultrasound (TRUS), and/or biopsy to confirm prostate cancer. Of a total of 18,393 cases, 106 (0.6%) exhibited abnormal hypermetabolism in the prostate. Additional evaluations were performed in 66 patients. Serum PSA levels were not significantly correlated with maximum standardized uptake values (SUVmax) in all patients (rho 0.483, p=0.132). Prostate biopsies were performed in 15 patients, and prostate cancer was confirmed in 11. The median serum PSA level was 4.8 (0.55-7.06) ng/mL and 127.4 (1.06-495) ng/mL in the benign and prostate cancer groups, respectively. The median SUVmax was higher in the prostate cancer group (mean 10.1, range 3.8-24.5) than in the benign group (mean 4.3, range 3.1-8.8), but the difference was not statistically significant (p=0.078). There was no significant correlation between SUVmax and serum PSA, prostatic volume, or Gleason score. $^{18}F$-FDG PET/CT scans did not reliably differentiate malignant or benign from abnormal uptake lesions in the prostate, and routine prostate biopsy was not usually recommended in patients with abnormal FDG uptake. Nevertheless, patients with incidental prostate uptake on $^{18}F$-FDG PET/CT scans should not be ignored and should be undergo further clinical evaluations, such as PSA and DRE.

삼차원 전산화단층촬영사진과 측모두부 방사선규격사진의 계측자에 따른 계측오차에 대한 비교분석 (Comparison of the observer reliability of cranial anatomic landmarks based on cephalometric radiograph and three-dimensional computed tomography scans)

  • Introduction: Accurate diagnosis and treatment planning are very important for orthognathic surgery. A small error in diagnosis can cause postoperative functional and esthetic problems. Pre-existing 2-dimensional (D) chephalogram analysis has a high likelihood of error due to its intrinsic and extrinsic problems. A cephalogram can also be inaccurate due to the limited anatomic points, superimposition of the image, and the considerable time and effort required. Recently, an improvement in technology and popularization of computed tomography (CT) provides patients with 3-D computer based cephalometric analysis, which complements traditional analysis in many ways. However, the results are affected by the experience and the subject of the investigator. Materials and Methods: The effects of the sources human error in 2-D cephalogram analysis and 3-D computerized tomography cephalometric analysis were compared using Simplant CMF program. From 2008 Jan to 2009 June, patients who had undergone CT, cephalo AP, lat were investigated. Results: 1. In the 3 D and 2 D images, 10 out of 93 variables (10.4%) and 11 out 44 variables (25%), respectively, showed a significant difference. 2. Landmarks that showed a significant difference in the 2 D image were the points frequently superimposed anatomically. 3. Go Po Orb landmarks, which showed a significant difference in the 3 D images, were found to be the artificial points for analysis in the 2 D image, and in the current definition, these points cannot be used for reproducibility in the 3 D image. Conclusion: Generally, 3-D CT images provide more precise identification of the traditional cephalometric landmark. Greater variability of certain landmarks in the mediolateral direction is probably related to the inadequate definition of the landmarks in the third dimension.

  • 국소적인 진행성 위암의 근치적인 치료는 수술이 첫 번째 방법이다. 하지만 위암을 진단받은 환자 중에서 약 30% 만이 수술이 가능한 진행성 위암으로 진단을 받으며 그 중 약 40-60% 만이 R0 절제술이 가능하다. 저자들은 위 주위 림프절과 복강 림프절, 장막을 침범한 진행성 위암을 진단받은 환자를 3주간의 수술 전 항암화학요법으로 치료하였고, 근치적인 목적으로 위 절제술을 시행하였다. 조직병리검사에서 완전히 괴사된 종양 조직을 관찰할 수 있었고, 수술 후 현재까지 3차례의 추가적인 전신 항암화학요법을 시행하고 있으며, 추적 복부 전산화단층촬영에서 재발을 시사하는 소견을 보이지 않았다.

Early CT Findings of Coronavirus Disease 2019 (COVID-19) in Asymptomatic Children: A Single-Center Experience

  • Objective: The current study reported a case series to illustrate the early computed tomography (CT) findings of coronavirus disease 2019 (COVID-19) in pediatric patients. Materials and Methods: All pediatric patients who were diagnosed with COVID-19 and who underwent CT scan in Zhongnan Hospital of Wuhan University from January 20, 2020 to February 28, 2020 were included in the current study. Data on clinical and CT features were collected and analyzed. Results: Four children were included in the current study. All of them were asymptomatic throughout the disease course (ranging from 7 days to 15 days), and none of them showed abnormalities in blood cell counts. Familial cluster was the main transmission pattern. Thin-section CT revealed abnormalities in three patients, and one patient did not present with any abnormal CT findings. Unilateral lung involvement was observed in two patients, and one patient showed bilateral lung involvement. In total, five small lesions were identified, including ground-glass opacity (n = 4) and consolidation (n = 1). All lesions had ill-defined margins with peripheral distribution and predilection of lower lobe. Conclusion: Small patches of ground-glass opacity with subpleural distribution and unilateral lung involvement were common findings on CT scans of pediatric patients in the early stage of the disease.
