갑상샘 암종을 가진 개에서 조절 T 세포의 증가 (Increased Regulatory T cells in Thyroid Adenocarcinoma in a Mixed Breed Dog)

  • 6년령의 개가 오른쪽 턱 밑의 종괴와 고체온증으로 전북대학교 수의과대학 동물병원에 내원하였다. 정확한 위치와 크기를 측정하기 위해 CT 촬영을 진행하였으며 3D 재구성을 실시하였다. CT 촬영 후 수술적으로 제거하였으며 제거 후 조직검사를 실시한 결과 거대세포 갑상샘 암종으로 확인되었다. 또한 본 환자의 혈액 중 조절 T 세포의 비율을 보기 위해 흐름세포측정기를 이용하여 실험한 결과 38.28%로 정상인 개(8마리 정상 비글견에서 조사한 결과 $7.66{\pm}1.65%$ (P<0.01))에서 보다 높게 나왔다. 보호자가 항암치료는 원하지 않아서 실시하지 못하였으며 수술 7일 후 다시 재발하였으며 혈액검사 결과 심한 백혈구 증가증 및 비재생성 빈혈이 나타났다. 또한 폐전이 부위로 의심되는 부위가 악화되었으며 결국 심한 빈혈과 호흡곤란으로 폐사하였다.

습윤/건조 반복 작용이 고흡수율 폴리머를 함유한 시멘트계 재료의 자기치유에 미치는 영향 (Effect of Cyclic Wetting-drying on Self-healing of Cementitious Materials Containing Superabsorbent Polymers)

  • 이 연구에서는 고흡수율 폴리머(SAPs)를 포함하는 시멘트계 재료의 자기치유에 대한 습윤/건조 반복 작용의 영향을 실험적으로 평가하였다. 매 주기마다 SAP 함량을 변수로 하는 시멘트 페이스트 시편을 균열 이후 1시간 동안 습윤 조건에 노출시키고(흡수율시험 및 투수시험 진행), 47시간 동안 건조 조건에 노출시켰다. 흡수율시험 결과 SAP 미혼입 시편, SAP 0.5%, SAP 1.0%, SAP 1.5% 혼입 시편의 균열을 통한 흡수율이 각각 1주기 대비 8주기 이후 22.9%, 36.8%, 42.8%, 46.3% 감소한 것으로 나타났다. 또한, 투수시험 결과 모든 시편의 균열을 통한 물 유출량은 습윤/건조 주기에 따라 점차 감소하였으며, 특히 SAP 함량이 증가할수록 유량 감소율은 증가하였다. 또한, X-ray CT 분석을 통해 균열 내 SAPs의 유입수에 의한 팽윤 거동이 확인되었다. 실험 결과는 SAPs가 유입수를 흡수하여 팽윤함에 따라 유효 균열폭이 감소될 수 있고, SAPs는 흡수한 물을 건조 조건에서 주변 균열부에 공급함에 따라 치유 생성물의 형성을 촉진시킬 수 있음을 나타낸다. 이 연구의 결과는 SAPs의 혼입이 균열의 수밀성을 증가시킴에 따라 시멘트계 재료의 자기치유 성능을 향상시킬 수 있음을 나타낸다.

Regenerative capacity of augmented bone in rat calvarial guided bone augmentation model

  • Purpose: Guided bone regeneration (GBR) is the most widely used technique to regenerate and augment bones. Even though augmented bones (ABs) have been examined histologically in many studies, few studies have been conducted to examine the biological potential of these bones and the healing dynamics following their use. Moreover, whether the bone obtained from the GBR procedure possesses the same functions as the existing autogenous bone is uncertain. In particular, little attention has been paid to the regenerative ability of GBR bone. Therefore, the present study histologically evaluated the regenerative capacity of AB in the occlusive space of a rat guided bone augmentation (GBA) model. Methods: The calvaria of 30 rats were exposed, and plastic caps were placed on the right of the calvaria in 10 of the 30 rats. After a 12-week healing phase, critical-sized calvarial bone defects (diameter: 5.0 mm) were trephined into the dorsal parietal bone on the left of the calvaria. Bone particles were harvested from the AB or the cortical bone (CB) using a bone scraper and transplanted into the critical defects. Results: The newly generated bone at the defects' edge was evaluated using micro-computed tomography (micro-CT) and histological sections. In the micro-CT analysis, the radiopacity in both the augmented and the CB groups remained high throughout the observational period. In the histological analysis, the closure rate of the CB was significantly higher than in the AB group. The numbers of cells positive for runt-related transcription factor 2 (Runx2) and tartrate-resistant acid phosphatase (TRAP) in the AB group were larger than in the CB group. Conclusions: The regenerative capacity of AB in the occlusive space of the rat GBA model was confirmed. Within the limitations of this study, the regenerative ability of the AB particulate transplant was inferior to that of the CB particulate transplant.

Alternative Method of Retrocrural Approach during Celiac Plexus Block Using a Bent Tip Needle

  • Background: This study sought to determine safe ranges of oblique angle, skin entry point and needle length by reviewing computed tomography (CT) scans and to evaluate the usefulness of a bent tip needle during celiac plexus block (CPB). Methods: CT scans of 60 CPB patients were reviewed. Image of the uppermost margin of L2 vertebral body was used to measure the minimal and maximal oblique angles and the distances from the midline to skin puncture point. The imaginary needle trajectory distance was calculated by three-dimensional measurement. When the procedure was performed by using a $10^{\circ}$ bent tip needle under a $20^{\circ}$ oblique X-ray fluoroscopic view, the distance (GF/G'F) from the midline to the actual puncture site was measured. Results: The imaginary safe oblique angle range was $26.4-34.2^{\circ}$ and $27.7-36.0^{\circ}$ on the right and left, respectively. The distance from the midline to skin puncture point was 6.1-7.6 cm on the right and 6.3-7.6 cm on the left. The needle trajectory distance at minimal angle was 9.6-11.6 cm on the right and 9.5-11.5 cm on the left. The distance of GF/G'F was 5.1-6.5 cm and 5.0-6.4 cm on the right and left, respectively. All imaginary parameters were correlated with BMI except for GF/G'F. All complications were mild and transient. Conclusions: We identified safe values of angles and distances using a straight needle. Furthermore, using a bent tip needle under a $20^{\circ}$ oblique fluoroscopic view, we could safely perform CPB with smaller parameter values.

안심연료단지 주변지역 주민의 호흡기계질환에 대한 설문조사 및 건강검진 결과 분석 (A Study on the Results of Questionnaire Survey and Health Examination for Respiratory Disease among Residents in Briquette Fuel Complex in Ansim, Daegu)

  • Objectives: This study was conducted to better understand the relationship between health effects and exposure to dusts from a briquette fuel complex in Ansim, Daegu, Korea. Methods: The subjects of this study consisted of 2,980 persons over 40 years old who had lived 20 years or more around a briquette fuel complex in Daegu. We conducted a questionnaire survey, and chest radiography was performed. In addition, chest computed tomography(CT) (335 cases) and pulmonary function test(PFT) (658 cases) were done. Pneumoconiosis was diagnosed if one of three radiologists determined(or suspected) pneumoconiosis. We also conducted in-depth interviews for pneumoconiosis cases. We defined the exposed group as subjects residing within a 500 meter radius from the walls of the briquette fuel complex, and the others were defined as the control group. Results: Subjects in the exposed and control groups are respectively 715(24%) and 2,265 cases(76%). Major respiratory symptoms in the exposed group such as sputum, dyspnea, chest tightness and wheezing were significantly higher than in the control group. By chest radiography, 173 cases of pneumoconiosis or suspicious pneumoconiosis were detected. By PFT, 62 cases(29.5%) of chronic obstructive pulmonary disease(COPD) among 210 asymptomatic subjects were detected. Finally, by chest CT we concluded 28 cases to be pneumoconiosis, and eight cases among them proved to be pneumoconiosis by environmental exposure. Conclusions: Through this study, we concluded that health outcomes such as respiratory symptoms, pneumoconiosis, and COPD were caused by continuous exposure to dusts from the briquette fuel complex. Policies to reduce environmental exposure are needed, and cases of environmental disease should be intensively followed up by the government.

저강도 초음파 조사 부위의 골 소실 억제 효과 -골 밀도 및 형태학적 특성 평가 (Suppression of Osteoporotic bone loss on the site to which low Intensity Ultrasound is Irradiated - In vivo test on BMD and Morphological Characteristics)

  • Purpose : The aim of this study is to evaluate a possibility of clinical application for the effects of low intensity ultrasound stimulation (LIUS) in morphological characteristics (i.e., structure, bone mineral density) of bone on osteoporotic fracturesprevention. Materials and Methods : Eight virgin 14-week-old ICR mice (approximate weight 25g) were used and ovariectomized (OVX) to induce osteoporosis. Right tibia (US) for each mouse served as the LIUS (1.5MHz frequency, 1.0 kHz pulse repetition on frequency, $30mW/cm^2$ intensity, $200{\mu}s$ pulse length, and stimulation for 20 minutes a day and 5 days a week over a 6-week period). Left tibia (CON) for each mouse served as the non-stimulated controls. Structural parameters and bone mineral density ($g/cm^3$) on trabecular bone of tibiae were calculated and measured from images derived in-vivo micro computed tomography (micro-CT) at 0 week and after 6weeks. Results : The BV/TV and Tb.N in US group were significantly bigger than those in CON group. The Tb.Pf in US group, moreover, was significantly smaller than that in CON group (p<0.05).For the others structural parameters and BMD, however, there were no significant difference between US group and CON group (p>0.05). Conclusion : The LIUS might prevent bone loss and keep bone connectivity in osteoporotic bones. Therefore, the LIUS might prevent effectively the osteoporotic fractures in clinics.

Bone regeneration effects of human allogenous bone substitutes: a preliminary study

  • Purpose: The purpose of this study was to compare the bone regeneration effects of cortical, cancellous, and cortico-cancellous human bone substitutes on calvarial defects of rabbits. Methods: Four 8-mm diameter calvarial defects were created in each of nine New Zealand white rabbits. Freeze-dried cortical bone, freeze-dried cortico-cancellous bone, and demineralized bone matrix with freeze-dried cancellous bone were inserted into the defects, while the non-grafted defect was regarded as the control. After 4, 8, and 12 weeks of healing, the experimental animals were euthanized for specimen preparation. Micro-computed tomography (micro-CT) was performed to calculate the percent bone volume. After histological evaluation, histomorphometric analysis was performed to quantify new bone formation. Results: In micro-CT evaluation, freeze-dried cortico-cancellous human bone showed the highest percent bone volume value among the experimental groups at week 4. At week 8 and week 12, freeze-dried cortical human bone showed the highest percent bone volume value among the experimental groups. In histologic evaluation, at week 4, freeze-dried cortico-cancellous human bone showed more prominent osteoid tissue than any other group. New bone formation was increased in all of the experimental groups at week 8 and 12. Histomorphometric data showed that freeze-dried cortico-cancellous human bone showed a significantly higher new bone formation percentile value than any other experimental group at week 4. At week 8, freeze-dried cortical human bone showed the highest value, of which a significant difference existed between freeze-dried cortical human bone and demineralized bone matrix with freeze-dried cancellous human bone. At week 12, there were no significant differences among the experimental groups. Conclusions: Freeze-dried cortico-cancellous human bone showed swift new bone formation at the 4-week healing phase, whereas there was less difference in new bone formation among the experimental groups in the following healing phases.

29세 남자에서 다발성 낭종성 병변으로 진행한 폐선암 1예 (Adenocarcinoma of the Lung Progressing to Multiple Cystic Lesions in a 29-Year-Old Man)

  • Cystic lesions or progressive cystic changes in adenocarcinoma of the lung have rarely been reported. We report a case of lung adenocarcinoma that progressed from ground-glass opacities (GGOs) and consolidations or nodules to extensive cystic lesions during 12 months in a young adult patient. A 29-year-old male was initially diagnosed with primary lung adenocarcinoma by transbronchial lung biopsy of the right lower lobe and lung to lung metastasis in both lungs according to imaging findings. The initial chest computed tomography (CT) scans showed multifocal GGOs, consolidations, and nodules in both lungs. Despite treatment with palliative chemotherapy, the patient's follow-up CT scans showed multiple, cystic changes in both lungs and that the lesions had progressed more extensively. He died of hypoxic respiratory failure one year after his diagnosis.

기관지확장증 환자의 객담 균주 분석: 울산지역의 일개 대학병원 보고 (Bacteriologic Analysis of Expectorated Sputum in Patient with Bronchiectasis)

  • Background: Bronchiectasis (BE) remains a rare respiratory disease in Korea. This retrospective study was done to investigate the potential pathogenic microorganisms (PPMs) that cause in patients with BE, through the use of sputum specimens. Methods: One hundred eleven adult patients, who had undergone chest computed tomography (CT), sputum gram stain/culture, and BE detected by chest CT, were included in this study. Sputum adequacy was determined by using Murray-Washington classification. Results: The mean (${\pm}$SD) age of patients was 60.9 (${\pm}$14.0). The number of PPMs was 167 (67%) in the total 248 isolated organisms. The most frequent PPMs were P. aeruginosa (23.4%), K. pneumoniae (10.5%), and S. aureus (8.4%). The proportion of adequate sputum (AS) was 25.8% in the total sputum specimens. The patients with AS were 41 (37%) and the patients with inadequate sputum (IS) were 70 (63%). The proportion of P. aeruginosa was higher in AS compared to that of IS (44% vs. 19%, p=0.004). The BE score was also higher in P. aeruginosa (+) patients compared to that of P. aeruginosa (-) patients (10.8 vs. 7.6, p=0.001). Conclusion: Although the proportion of AS in the total sputum was low, PPMs were isolated in most patients with BE. It is likely that P. aeruginosa was isolated in AS and AS patients had higher BE scores.

MDCT의 관전압, 관전류, 슬라이스 두께 변화에 따른 팬텀의 선량 분포 측정 (Dose Measurements using Phantoms for Tube Voltage, Tube Current, Slice Thickness in MDCT)

  • 본 연구는 MDCT (multi-detector computed tomography) 파라미터 변화에 따른 환자선량을 측정하고 평가 하고자 하였다. MDCT 파라미터의 다양한 변화에 의한 환자선량은 MDCT (GE light speed plus 4 slice, USA)와 model 2026C electrometer (RADICAL 2026C, USA), 그리고 head와 body의 CT선량 표준팬텀(standard polymethylmethacrylate)을 사용하여 측정 하였다. 그 결과 환자선량 $CTDI_w$ 값은 관전압과 관전류가 증가할수록 선형적으로 증가하였고 beam collimation이 증가 할수록 감소하였다. 따라서 관전압, 관전류는 환자의 피폭선량에 직접적인 영향을 주고 슬라이스 두께는 영향이 적은 것을 알 수 있었다. 임상의 전형적인 MDCT 복부 scan에서 120 kVp, 180 mAs, 20 mm collimation과 0.75 pitch의 조건하에 $CTDI_w$$CTDI_{vol}$의 측정값은 각각 20.2 mGy, 26.9 mGy이었고 스캔 길이가 271.3 mm인 환자의 DLP와 유효선량은 각각 $729.1\;mGy{\cdot}cm$, 10.9 mSv였다.
