의학전문대학원생의 보완대체의학과 한의학에 대한 인식 (Undergraduate Medical Students' Perception of Complementary and Alternative Medicine and Korean Medicine)

  • 본 연구의 목적은 의학전문대학원생들이 보완대체의학과 한의학에 대하여 어떠한 인식을 가지고 있는지를 분석하는 것이다. 본 연구 대상은 의학전문대학원에 재학 중인 학생 409명이다. 연구목적을 검증하기 위하여 기술통계 분석, MANOVA, 로지스틱 회귀분석을 실시하였다. 보완대체의학과 한의학에 대하여 1학년은 다른 학년 보다 긍정적으로 인식하는 것으로 나타났다. 보완대체의학의 변인(정서적 태도, 적합성, 효과성, 수용의도)들은 학년별, 성별에 따른 유의미한 차이가 나타나지 않았고, 한의학은 학년별로만 모든 변인들이 유의미한 차이가 나타났다. 보완대체의학과 한의학을 의학으로 인식하는데 영향을 미치는 요인은 하위 요인 중 '효과성'으로 나타났다. 의학전문대학원생들이 보완대체의학과 한의학과 관련된 올바른 인식을 갖고 환자 요구에 합리적으로 조언을 할 수 있도록 저학년부터 비판적 수용능력과 의사결정능력을 함양할 수 있는 교육과정을 개발하여야 한다.

구선왕도고가 전지방세포(前脂肪細胞) 3T3-L1의 증식(增殖), 항보체활성(抗補體活性) 및 세포독성(細胞毒性)에 미치는 영향 (Effects of Kuseonwangdogo on the Proliferation of Preadipocyte 3T3-L1 Cells, the Anti-Complementary and the Cytotoxic Effects)

  • To investigate the anti-complementary and cytotoxic effects of oriental prescription, Kuseonwangdogo, on the proliferation of preadipocyte 3T3- L1 cells, we examined biological effects of Kuseonwangdogo. The results obtained were as follows. 1. After 14 days, the body weight of rats treated with Kuseonwangdogo decreased more than that in the control group (p<0.05). However, the weights of liver, spleen and kidney were unchanged. In serum biochemical test, we examined the level of glucose (GLU) and glutamic pyruvic transaminase (GPT). The levels of GOT and CHOL in serum were decreased remarkably by the administration of Kuseonwangdogo (p<0.05). The haematological examination of the tested group showed significant increment of white blood cells (WBC), hemoglobin concentration (HGB), mean corpuscular hemoglobin (MCH) and monocyte (MO). 2. The effect of Kuseonwangdogo on the proliferation of 3T3-L1 cells was tested by the sulforhodamin B(SRB) assay. The high concentration ($100{\mu}l\;and\;200{\mu}l$) of extracts inhibited the proliferation of 3T3- L1 cells. The p-value was <0.01, respectively. 3. The extract of Kuseonwangdogo showed a potent anti -complementary activity. It was suggested that the active principle may be a kind of polysaccharide molecule. 4. The cytotoxic effects of Kuseonwang dogo and its composing herbs in human liver cells (WRL68) and monkey kidney cells (Vero) were examined by the SRB and 3- (4,5- Dimethylthiazol-2-yl) -2,5 diphenyl-2H- tetrazolium bromide (MTT) assay. Cytotoxic effects were not observed.

A Bibliometric Analysis of Acupuncture Treatment of Osteoarthritis over the past 20 Years: 2003-2022

  • This study uses bibliometric methods to analyze publications regarding the use of acupuncture in osteoarthritis over the past 20 years and presents an overview of global research trends. Publications related to acupuncture in osteoarthritis from 2003 to 2022 were retrieved from the Web of Science Core Collection Database. An analysis of the extracted records was conducted according to their publication year, research area, journal title, country, organization, author, and keywords. The VOSviewer program was used to visualize the research trends on acupuncture in osteoarthritis. An analysis of 380 articles indicated a consistent increase in the use of acupuncture for osteoarthritis treatment over the past 20 years. Many articles have been published in research areas such as "integrative complementary medicine" and "general internal medicine." The most prolific journal was Evidence-Based Complementary and Alternative Medicine. In terms of article publication, the most productive country and research organization were China and the Beijing University of Chinese Medicine, respectively. The most frequently occurring keywords were "acupuncture," "knee osteoarthritis," and "pain." This study used a bibliometric analysis to provide an overview of global research trends on acupuncture in osteoarthritis. These findings may suggest the future direction of research on the treatment of acupuncture in osteoarthritis.

  • Objectives: The purpose is to advertise the protocol of Best Case Series Programme of National Cancer Institute of USA to korean oriental medical oncologists and researchers Method: We reviewed the submission packet of NCI and related Website and articles. Result and Conclusion: BCS programme is the first step to evaluate the efficacy of complementary and alternative treatment, and to launch the clinical research funded by NCI.

33개국 대한민국 재외공관을 통한 전통의학 및 보완대체의학 관련 제도 조사 (Survey on Regulatory Status of Traditional and Complementary Medicine through Korean Embassies in 33 Countries)

  • Objective : This study aims to investigate legal and regulatory status of traditional and complementary medicine (T&CM) focusing on regulation on health practitioners and health practice in 33 countries. Method : 33 countries were selected based on several factors such as interest of Korean medical doctors, strategic importance, and distribution over the world. The questionnaire was distributed to Korean embassies in 33 countries in March 2014 through Ministry of Foreign Affairs, and the answers from those countries were collected from April to September. 24 countries that provided sufficient information were included in the analysis. Results : 18 countries have law or regulation on T&CM. Only five countries regulate T&CM practitioners as medical personnel or health practitioner by law, and 12 countries have regulation on license or certificate. Half of 24 countries recognize license of T&CM practitioners issued abroad. There are nine countries that recognize T&CM practice as medical practice, and four of them regulate acupuncture as medical practice by western medical doctors or a few health practitioners recognized by the government. There are six countries that do not recognize T&CM practice as medical practice by law, but regulate it as practice that affect public health, and these countries have law or regulation on T&CM. Conclusion : As T&CM have great impact on public health, many countries have recently legislated law or regulation on T&CM. Rapid change in regulatory status of T&CM affects globalization of Korean medicine. Thus, development of timely strategies will be essential for it.

일개 시지역 저소득 골관절염 환자의 보완대체요법 이용실태 및 비용 -의료급여 및 건강보험하위 20% 대상자를 중심으로- (Utilization and Out-of-pocket Expenditure of Complementary and Alternative Medicine in Low-income Patients with Osteoarthritis in a City)

  • Objective: The use of complementary and alternative medicine (CAM) is common especially among patients with osteoarthritis The aim of this study was to investigate the utilization rate and expenditures of patients who use CAM. Method: Two hundred seventy four patients with osteoarthritis were interviewed by a telephone survey. A structured questionnaire about sociodemographic features and type, cost, satisfaction and reason of CAM utilization was used Results: Among 274 patients with osteoarthritis, 251 patients(91.6%) had used at least one type of CAM during six months. There was a significant difference in sex (female), age (70 years), medical security (insurance), educational level between the user and non-user of CAM. Hyperthermia was the most use. The average cost for CAM utilization was 120 thousands won/person during six months and there was no difference in sociodemographic features among the out-of-pocket cost of users. The scores of satisfaction for CAM use were ranged between 60-70. Conclusions: CAM became a popular source of health care because of elderly and lay referral system. And Korean spent a substantial amount of out-of-pocket money on CAM without benefit. Health care system and professionals should pay more attention to CAM, make a evidence for CAM.

Complementary and Alternative Medicine in Clinical Practice Guideline for Insomnia

  • Objectives: The aim of this review was to investigate whether evidence of complementary and alternative medicine (CAM) was reflected in clinical practice guidelines (CPGs) for insomnia based on relevant clinical trials. Methods: We conducted a systematic search on domestic and international CPG databases and medical databases. In addition, we conducted manual searches of relevant articles. Three authors independently searched and selected relevant studies; any disagreement was resolved by discussion. We extracted and analyzed the following data: published language, country, development group, participants, interventions, presence or absence of recommendations for CAM, level of evidence, grade of recommendation for CAM, and methods of development. Results: We identified 8,241 records from domestic and international databases, and 22 CPGs were included. Eleven of the 22 CPGs mentioned CAM interventions including herbal medicine, relaxation, acupuncture moxibustion, Tai Chi, meditation, hypnosis, biofeedback, Tuina, and external herbal medicine. However, most of the CPGs indicated 'no recommendation' or 'weak recommendation' for CAM interventions. Only Valeriana dageletiana Nakai and relaxation were considered to have experimental evidence. Valeriana dageletiana Nakai was recommended for improvement of sleep latency, sleep maintenance, total sleeping time, and sleep cycle. Relaxation was recommended as effective intervention for relieving physical and psychological arousal. Conclusions: Despite systematic reviews and randomized controlled trials on CAM for insomnia, most of the CPGs for insomnia did not reflect the evidence obtained. Further CPGs for insomnia should be developed by considering the current advanced studies in the field of CAM.

두경부암 환자의 대체요법 이용 실태 (Complementary and Alternative Medicine in the Patients with Head and Neck Cancer)

  • Background and Objectives: The popularity of complementary and alternative medicine(CAM) has grown at an astonishing rate in recent years. Because patients are interested in CAM and want to use it, physicians also should be concerned about CAM and know about the current situation of its usage. In this study, we tried to determine the prevalence of CAM use in the patients with head and neck cancer and correlate with demographics and tumor characteristics. Materials and Methods: A total of 87 patients with head and neck cancer previously treated with surgery at Seoul National University Hospital from March through June, 2004 were enrolled in this study. The patients were completed, questionnaire about the use of CAM and demographic information. Tumor characteristics were obtained from the patient's medical records. Results: Of the 87 patients, 33 patients(37.9%) use at least one CAM. There was no statistically significant correlation between patient's data and prevalence of CAM use. Of those patients using CAM, 16(48.5%) of 33 were using 2 or more therapies. The majority of agents used by patients in this study are mushrooms such as Phellinus and Reishi, followed by dietary therapy. A majority(60.6%) of patients were introduced to alternative medicine by friends or family members. Conclusion: Considerable cancer patients use CAM. Head and neck oncologists should inform themselves about CAM to counsel and educate the patients. Further scientific investigations on promising CAM agents may help advance cancer treatment.

세계 전통의학 동향과 주요 한의학 R&D 분야에 관한 연구 (A Study on the Trend of World Traditional Medicine and Key Area of Traditional Korean Medicine(TKM) R&D)

  • The medical and healthy paradigm is changing by various reasons such as improving of life quality, the limit of western medicine, demographic changes and so on. And the demand, interest and market size of traditional medicine(TM) and complementary and alternative medicine(CAM) are increasing continuously and there is a good traditional medicine called TKM(Traditional Korean Medicine) in Korea. TKM have differentiated characteristics like nature-friendly, systematic, preventive and personalized medicine from western medicine. TKM with these characteristics have possibility to increase national competitiveness and contribute to growth of economy. At this time, it is necessary to select key area of TKM R&D to increase competitiveness of TKM and to create new growth engine. In this paper, we suggest key area of TKM R&D through SWOT analysis, the analysis of world traditional medicine and expert's counsel. As a result, 3 key area and 10 specific fields of TKM R&D was selected, which they are 'the standardization for TKM technology and technique', 'the development of generic technology based on TKM' and 'the establishment of infrastructure for TKM information and material'. This study may contribute to make plan for TKM research and policy such as TKM middle and long term plan and TKM promotion plan.

Cupping Therapy for the Treatment of Migraine Headache: a systematic review and meta-analysis of clinical trials

  • Objectives: Cupping therapy is a widely used complementary medicine for the treatment of migraine headaches globally. However, conflicting evidence exists on its effectiveness. To evaluate the safety and efficacy of cupping therapy in treating migraine headache disorder. Methods: Seven databases were systematically searched: PubMed/MEDLINE, Clinicaltrials.gov, Cochrane CENTRAL, ScienceDirect, ProQuest, SinoMed, and the National Science and Technology Library. The primary endpoints are the treatment success and the pain intensity reduction. The secondary endpoints were adverse events (AEs) risk and improvement in quality of life (QoL), which was based on the Migraine Disability Scale (MIDAS). Subgroup analyses were performed based on the cupping techniques (wet and dry cupping) and adjunctive complementary treatments (i.e. acupuncture and/or collateral pricking). Results: Eighteen trials out of 348 records were included, pooling 1,446 participants (n = 797 received cupping therapy). Treatment success was significantly higher among those with cupping therapy (risk ratio [RR] [95% CI] = 1.83 [1.52-2.21]); with significant improvement observed only with wet cupping (RR [95% CI] = 1.88 [1.53-2.30]). The adjunctive complementary therapy did not achieve a greater amplitude of treatment success compared to cupping therapy alone. Furthermore, cupping therapy showed significant pain reduction compared to baseline (standardized mean difference [SMD] [95% CI] = 0.55 [0.39-0.70]) and achieved fewer risks of AEs (RR [95% CI] = 1.88 [1.53-2.30]). However, cupping did not improve the overall QoL (MIDAS SMD [95% CI] = -0.79 [-3.55-1.98]). Conclusion: Cupping therapy was an effective complementary modality to treat migraine headaches. However, it did not demonstrate improvement in QoL (PROSPERO: CRD42024514509).
