The Effect of a Workshop on a Urinary Incontinence Self-Management Teaching Program for Community Health Nurses

  • Purpose: This study aimed to examine the effectiveness of the workshop on the nurses' knowledge about urinary incontinence (UI) self-management, attitudes toward UI, and self-efficacy to plan and implement a UI self-help group program for their clients. Methods: A one-group pretest and posttest design was used to examine changes in knowledge, attitudes, and self-efficacy following a one-day training workshop. Twenty-seven community health nurses completed a questionnaire before and after the workshop. Before participating in the workshop, the participants were required to take a UI online continuing education program developed by the researchers. During the workshop, the participants took four sessions which consisted of an introduction of a self-help group program, demonstration of a 5-week UI self-management program contents, pelvic floor muscle training and biofeedback practice, and group discussions to plan the implementation in their workplaces. Results: A significant improvement in knowledge of and attitudes toward UI were found (t=3.53, p=.002; t=2.83, p=.009, respectively) after the workshop. Participants also demonstrated improvement in their self-efficacy to plan and operate a UI self-help group program (Z=-2.64, p=.008). Conclusion: The one-day workshop for community health nurses is a feasible strategy to increase their abilities and confidence in operating a UI self-help group program.

A Study on Community Health Practitioners' Knowledge of, Usee Patterns of, and Attitudes toward Complementary and Alternative Medicine(CAM) (보건진료원의 보완대체요법에 대한 지식정도, 사용경험, 태도에 관한 연구)

  • Purpose: This study was conducted to investigate community health practitioners' knowledge of, use patterns of, and attitudes toward CAM. Method: The subjects of this study were 817 community health practitioners (CHPs) from 9 Provinces in Korea selected through convenient sampling. They completed and submitted self-report questionnaires at the annual meeting of CHPs or sent completed questionnaires to us by mail. Result: Of the CHPs, 8.1% had a certificate of CAM-related education program, and 39.9% worried about the efficacy of CAM. They knew about massage (74.0%), finger pressure (68.1%), hand acupuncture (67.8%), diet (65.5%) and herb (64.7%). They had used herb (43.3%), massage (36.8%), hand acupuncture (32.7%) and vacuum (31.2%) with positive effects and recommended residents to use them. Attitude toward CAM tended to be positive in those with certificates of CAM, those with high academic qualification, those aged 40 or below, and those working in Chungcheong-do. Conclusion: This study showed that CHPs were interested in CAM while they worried about the therapeutic effects of it. Further studies to draw cross-sectional and national data on the use patterns of CAM from the various population and standardized instruments to investigate the types of CAM were suggested.

A Study on the Cognition of Rehabilitation Practitioners Working at Community Rehabilitation Centers Toward Sensory Integration Therapy (장애인복지관에 근무하는 재활전문가들의 감각통합치료에 대한 인식조사)

  • Objective : The purpose of this study is to investigate awareness of rehabilitation practitioners working at community wellness centers regarding the sensory integration (SI) therapy. Methods : 121 rehabilitation practitioners working at community wellness centers, located in Seoul Gyeonggi, Daegu Gyeongbuk and Busan Ulsan, participated in a questionnaire survey to examine their awareness of SI therapy. The questionnaire constructed based on four elements of the information about SI therapy; SI therapy's purpose, target population and technical process, and the qualification of SI therapist. Correlations between general characteristics and the four awareness elements of SI information were explored. The level of recognition on SI therapy were examined in various perspective such as 'have heard', 'means of the recognition', 'easiness of getting information', 'recognition of necessity of SI therapy', 'willing to participate in professional education course' and 'opinions for improving recognition'. Result : There is significant difference in awareness of the all four elements of SI informations depends on several general characteristics; practitioner's clinical field, location of the center, whether SI service has been provided or not at the center, whether the practitioner carry out SI therapy, and the practitioner's clinical experience in SI therapy. Specially, the recognition of occupational therapists is relatively high compared other rehabilitation practitioners. In terms of level of recognition, most rehabilitation practitioners(96.7%) are well-aware of necessity of sensory integration therapy. 79.4% of the practitioners manifest somewhat of difficulty in getting information related SI therapy. 93.4% of the practitioners are willing to participate in professional education course for SI therapy. Opinion for improving recognition with the most number of people(21) chosen is 'educational revitalization'. Conclusion : Many rehabilitation practitioners working at community wellness centers recognize the necessity of understand SI therapy, but there is uneasiness to get relative and valid information. There are difference awareness of the SI therapy Revitalization of education for SI therapy may provide an opportunity to improve level recognition of rehabilitation practitioners. It is suggested that organ related rehabilitation, there is a need for public relations.

Epidemiology of Esophageal Cancer in Ardabil Province During 2003-2011

  • Background: The aim of this research was to perform an epidemiologic survey of esophageal cancer in Ardabil province. Materials and Methods: In this cross-sectional descriptive study, 661 patients diagnosed with EC were studied from March 2002 to May 2011 e. The necessary data were collected with a checklist from the documents in Ardabil Cancer Registry (ACR) and analyzed by statistical methods with SPSS.18 software. Results: Of the total new cases of EC registered in ARC during study period, 430 (65.1%) of patients were male with the male to female standard ratio was 1.18, with a statistically significant gender bias. The most common morphology of EC was squamous cell carcinoma (SCC, 68.8%) followed by adenocarcinoma (28.5%). It was observed that in most of patients, EC lesions were in the middle third of esophagus. In addition, most patients were rural and about 40% had smoking habits. The age-standardized incidence rate of cancers was 48.4 per 100,000 among females and males. The annual incidence rates in males and females was 7.1 and 6.7 per 100,000; respectively. Conclusions: Results showed that the prevalence and annual incidence rate of cancer in Ardabil province is lower than other areas of the country with a male predominance and a relatively high proprortion of adenocarcinomas.

Duties of Nurse Practitioners in the Community and Management of Primary Health Care Posts (보건진료원의 업무 및 보건진료소 운영에 관한 고찰)

  • By the rural area health care special law in 1980, Primary health care posts were established in rural areas as fundamental elements of the national health system. Nurses have been deployed to the posts after taking an education course mandated by the special law. However, health care posts have confronted environmental changes over the past 30 years such as an aging and decreasing rural population and advanced traffic systems, which make it necessary to reshape their form and role. Therefore, some guidelines are suggested for future role enlargement of health care posts by analyzing their current management and duties. The guidelines are as follows: 1) enlarging the portion of prevention and management of chronic degenerative diseases, 2) development and practice of diverse health promotion programs, 3) extension of primary health care for the increasing older population, 4) development of health programs for married immigrants, 5) practice of timely maternal child health programs, 6) development of adequate health care posts for low-income people in rapidly urbanizing rural areas and in poor areas in big cities, and 7) revision of laws and institutional arrangements for the role enlargement of health care posts to match social changes and customer needs.

Effects of an Infection Control Program on Clonorchis Sinensis in People Living Near Rivers (강변 유역 주민을 위한 간흡충 감염관리 프로그램의 효과)

  • Purpose: The purpose of this study was to estimate the effects of an infection control program on the Clonorchis sinensis (CS) infection ratio, CS-related knowledge and health behavior of people living near the Geum River. Method: A one-group pretest-posttest design was used with 102 participants. For 12 months the participants were provided with promotion announcements, health education, counseling, and medication. Outcome variables measured were the CS infection ratio by stool examination, CS-related knowledge and health behavior from self-report questionnaires. The pre intervention data were collected from January to February 2008 and the post intervention data during the same period in 2009. Collected data were analyzed using descriptive statistics, generalized estimating equation analysis, and paired t-test with SPSS for Windows version 15.0. Results: In the pre test 21 of the 102 participants showed infection with CS for the first time. In the post test 9 were newly infected with CS, and one was re-infected. The CS-related knowledge was significantly improved after the infection control program (p<0.05). The CS-related health behaviors did not improve. Conclusion: These results showed that an infection control program is effective in decreasing CS infection ratio and improving CS-related knowledge of people living near the river.

Factors Influencing Cognitive Function According to Degree of Cognition in Community Dwelling Elders (재가노인의 인지 정도와 인지 영향요인)

  • Purpose: This study was done to identify differences in factors influencing cognitive function according to the cognition of community dwelling elders. Methods: A convenience sample of 565 community dwelling elders participated in this study and from May to June, 2010 trained research assistants collected data using structured questionnaires. Collected data were analyzed using descriptive statistics, t-test, one-way ANOVA, correlation, and multiple regression with the SPSS Win 15.0 version. Results: The elders had a slightly low degree of cognitive function. Of the 565 elders, 41.1% were in the normal group for cognitive function and 58.9% in the mild impairment group. Although there were no significant differences in general characteristics between the groups, factors influencing cognitive function were different. Factors influencing elders in the normal group were leisure activity and moderately good health status. These variables accounted for 5.7% of the variance in cognitive function. Factors influencing elders in the mild impairment group were leisure activity, depression, and education level limited to elementary school graduation. These variables accounted for 19.9% of the variance cognitive function. Conclusion: This study provides comprehensive understanding of factors influencing cognitive function in elders. Differentiated nursing interventions according to degree of cognitive function are suggested.

A Study of Current Status and Activities of the Community Health Practitioners in Rural Areas in Korea (한국의 농촌지역에서의 보건진료원 현황 및 활동분석연구)

  • 한국의 농촌 보건의료 문제를 해결 하기위하여 일차보건의료가 도입되었으며 일차보건의료에서 핵심적인 역할을 수행하고 있는 보건진료원의 활동을 분석하였다. 분석 목적은 보건진료원의 일반현황과 보건진료원 활동에 대한 구체적인 분석을 통하여 활동개선 방안을 제시하는데있다. 분석방법은 사회조사 방법을 적용하였으며 조사는 1998년 7월과 11월기간에 구조화된 설문지를 보건진료원 전원에게 송부하여 스스로 작성하게 하였다. 1880명의 보건진료원을 대상으로 설문지를 배포하였으며, 1663명의 응답자가 작성한 설문지가 최종분석되었으므로 분석율은 88.5%였다. 분석결과 보건진료원의 일반현황으로 광역사의 보건진료원 평균 연령은 39.7세이며 시지역 보건진료원의 연령이 군지역보다 높았으며 통계적으로도 유의한 차이가 있었다. 총근무기간도 시지역의 보건진료원의 12.4년-13.6년으로 농촌지역보다 걸었으며 통계적으로도 유의한 차이가 있었다. 보건진료원의 평균 관할 인구수는 901명이었다. 보건진료소 관할지역의 노인인구 및 만성질환자의 분포가 매우 높았다. 보건진료소로부터 민간의료기관이 위치한 거리는 시지역 (7.1-11.3km) 보다 농촌지역이 12.1km로서 지리적 접근성이 시지역보다 낫은 것으로 나타났다. 활동건수분석결과 월평균 가정방문수는 평균 47.8건, 일평균 내소자수는 14.1건 및 일평균 전화상담은 5.2건으로 나타났다. 연령별 활동수준은 40세이상이 4세 이하보다 월평균 가정방문수와 일평균 내소자수가 많았으며 통계적요로도 유의한 차이가 있었다. 경력이 많을수록 일평균가정 방문수 및 전화상담 건수가 많았으며 통계적으로 유의한 차이가 있었다. 근무조건에 대한 만족도는 80%정도로 높았다. 연구결과 보건진료원의 활동지역내 노인인구 및 만성질환자의 비율이 높고 민간의료기관의 지리적 접근성도 낮고, 관할인구규모가 적어서 인구규모를 늘리고 노인 및 만성질환자 건강관리 프로그램의 개발이 더욱 요구되며 농촌지역주민의 건강관리자로서 보건진료원의 활동을 강화할 필요가 있다.

The Health Promoting Activity of Old Aged in a Rural Area (일부 농촌지역 노인들의 건강증진활동 수행정도 및 관련요인)

  • This study was to evaluate the performance status and associated factors with the health promoting activities. Two hundred three old aged over 65 years old sampled by quota sampling among 660 old aged in a rural area, Dae-Ma Myun, Young-Kwang Gun, Chollanam Do. The questionnaire survey was performed from June 1st to August 31st in 1997. The major findings were as follows ; 1. The performance rate of health promoting activities were higher in female, dwellers at community health practitioner post area, and old aged believes in a religion and high educated old aged. 2. A multiple regression analysis showed that physical health promoting activity was related to psychiatric wellbeing status, the disability of daily living, residental area and family numbers(P<0.05). 3. The disability of daily living and psychiatric wellbeing status were related to mental health promoting activities by a multiple regression analysis(P<0.05). In conclusion, psychiatric wellbeing status, the disability of daily living, the disability of daily living and residental area were associated the performance of health promoting activities for old aged in a rural area.

Analysis of the Results of Health Service Programs in the Primary Health Care Posts (보건진료소 보건사업 현황 분석)

  • Purpose: To analyze health programs of the PHCP (Primary Health Care Posts) Method: From August 2006 to July 2007, data on the general quality and health program of the PHCP was requested by official letter and replies were received via E-mail. From December 8 to December 30, 2007, data from 1,268 (66.8%) PHCP out of 1,897 PHCP were analyzed using SPSS 12.0 Win program. Results: The average population covered by each PHCP is 878.3 people. For the health and special programs, Community Health Practitioners report high motivation for programs on health promotion, management of chronic illness, social welfare (40-50%). Demand by the residents was reported at 10% and increases in the health of the residents were attributed to high interest and demand. Volunteer work was 83.3% for bathing, 54.5% for equipment support and 46% for exercise programs. As elders make up 30% of the population in rural areas, there is an increasing demand for volunteer work in bathing programs. Conclusions: As the number of elders in the population increases and there is an increased need for more medical treatment for older people who are sick, the role of PHCP must be strengthened to include visits to homes of community residents. Where financial support for the PHCP is difficult, it is necessary to develop sound data on demographic characteristics of the population in order to develop efficient and effective health promotion programs. The finding that 54.7% of the population need management of chronic illness has difficulty in seeing a physician indicates a need to enhance the health care delivery system by strengthening the role of the Community Health Practitioners and including them in the civil service system to ensure stability of the PHCP.
