중풍언어장애에 대한 동서의학적 고찰 (The Oriental and Western Medical Study of Communication Disorder with Post-Stroke)

  • Objectives : The purpose of this study is to search for more effective methods of diagnosis and treatment of Communication Disorder with Post-Stroke. Methods : Literature review on Communication Disorder with Post-Stroke in view of oriental and western medicine. Conclusions : Communication Disorder with Post-Stroke is relative with aphasia in western medicine, Sul-um(舌瘖) oriental medicine Aphasia, apraxia of speech, and dysarthria come after strock Each of them has its own mechanism. Yomchon(CV23), Amun(GV15), Chuldol(CV22), Tongni(HT5), Pungnyung(ST40), Pungbu(GV16), Chigu(TE6), Yongchon(KI1), Hapkok(L14), Peakoe(GV20), Kokchi(LI11), Puryu(KI7), Shinsu(BL23), Kumjin-okaek, and Chohea(KI6) was used on Communication Disorder with Post-Stroke treatment.

사회적 의사소통장애의 임상적 이해 (Clinical Implications of Social Communication Disorder)

  • Social (pragmatic) communication disorder (SCD) is a new diagnosis included under communication disorders in the neurodevelopmental disorders section of Diagnostic and Statistical Manual of Mental Disorders-5. SCD is defined as a primary deficit in the social use of nonverbal and verbal communication. SCD has very much in common with pragmatic language impairment, which is characterized by difficulties in understanding and using language in context and following the social rules of language, despite relative strengths in word knowledge and grammar. SCD and Autism Spectrum Disorder (ASD) are similar in that they both involve deficits in social communication skills, however individuals with SCD do not demonstrate restricted interests, repetitive behaviors, insistence on sameness, or sensory abnormalities. It is essential to rule out a diagnosis of ASD by verifying the lack of these additional symptoms, current or past. The criteria for SCD are qualitatively different from those of ASD and are not equivalent to those of mild ASD. It is clinically important that SCD should be differentiated from high-functioning ASD (such as Asperger syndrome) and nonverbal learning disabilities. The ultimate goals are the refinement of the conceptualization, development and validation of assessment tools and interventions, and obtaining a comprehensive understanding of the shared and unique etiologic factors for SCD in relation to those of other neurodevelopmental disorders.

농촌지역 중학생의 섭식장애와 부모-자녀 의사소통과의 관계 (The Relationship between Eating Disorders and Parent-Adolescent Communication in Middle School Students in Rural Areas)

  • Purpose: The purpose of this study was to examine the relationship between eating disorders and parent-adolescent communication in middle school students. Method: There were 267 adolescents taken as participants from two middle schools in rural areas. Data were collected from June to July 2005, using the Eating Disorder Inventory-2 questionnaire (23 items) and Parent-Adolescent Communication Inventory (10 items for open family communication, 10 items for problem in family communication). The SPSS Win version 12.0 was used for descriptive analysis, t-test, and partial correlation coefficient. Result: The mean score on the eating disorder was 2.74 (range: 1-6). Meanwhile, the mean scores on the parent-adolescent communication was 3.37 (range: 1-5). There were significant differences in eating disorders according to gender, age, negative perception of the participant's body weight, family's perception of obesity, diet experiences, and BMI. Eating disorders showed a significant negative correlation with parent-adolescent communication. Conclusion: In this study, there was a significant negative correlation between eating disorders and parent-adolescent communication. In order to prevent an eating disorder, education and training to enhance communication skills should be provided to adolescents and their parents as well.

  • A Case Report of Communication disorder associated with Post-stroke Objectives : The Objective Is the Report of Improvement in Communication Disorder Associated with Post-Stroke. Methods : Acupuncture And Medicinal Treatments Were Performed from 17th March 2006 until 8th April 2006. The Medicines Bangpungtongsung-san and Chungpesagan-tang Were Used to Improve Patient Bowel Movement. The Sa-Am Acupuncture Method Was Used to Improve Patient Communication Disorders. Every 5 Days Articulation Accuracy, Vowel Accuracy, Alternation and Speed of Reading Sentences Were Evaluated. Results : After 23 Days of Treatment There was Improvement of Articulation Accuracy, Alternation and Speed of Reading Sentences.

  • 치료적 효과에 대한 선행연구의 분석 결과로 게임놀이치료가 ADHD, 틱장애, 의사소통 발달 장애, 자폐성 등과 같은 장애를 가진 아동의 주의집중력, 과잉행동 및 틱증상 감소, 언어적 의사소통 증가, 부적절한 사회적 행동 감소 등과 같은 사회 정서 발달에 효과적인 것으로 보고되었다. 본 연구는 아날로그와 디지털을 병합한 디지로그 방식의 놀이치료 컨텐츠를 개발하고자 한다. 이는 일반인을 비롯하여 치매노인, 지적장애인등의 치료에도 효과적인 방법이라 판단이 된다. 또한 아날로그 놀이와 디지털적 요소들을 융합하여 유아 스마트폰 중독이 초래하는 문제점을 방지하기 위한 디지로그적 관점의 새로운 놀이 시스템을 개발하고자 한다.

복합틱장애 증상이 있는 딸을 둔 부부에 대한 가족치료 사례연구 (A Case Study on Family Therapy for Parents with a Daughter Suffering from Multiple Tic Disorder)

  • This study deals with a counseling case in which a mother requested counseling due to the problems of her daughter, who suffers from multiple tic disorder. The participants of this study included five family members (grandmother, father, mother and two female children) and a total of 23 sessions were held from September 2006 to December 2007. Additional counseling ($24^{th}$ counseling session) was conducted on April 1st, 2009. The data was based on recorded transcripts and notes from 24 family therapy sessions. The study used a constant comparative analysis, which uses matrix and network display as an analysis method suggested by Miles&Huberman(1994). The characteristics of the family of origin and the indifference of the husband had caused the wife stress. The couple had frequent conflicts due to dysfunctional communication methods, a clash of values, sexual dissatisfaction, and a lack of communication. This marital conflict became the primary factor of the daughter's multiple tic disorder. Intervention of the family therapist resulted in the setting of treatment goals based on MRI's communication theory and Bowen's family systems theory in order to solve the problem of the daughter's multiple tic disorder. Also, the therapist's intervention techniques included exploring experiences with the family of origin, shedding light on the multigenerational transmission process, exploring dysfunctional attempts at solutions, the therapist's self-disclosure, providing similar cases, dealing with resistance, and suggesting a new communication method. To solve the problem, the therapist helped the family separate the daughter spatially from her parents. Therefore, the therapist's intervention helped reduce the daughter's tics and improve relationships among the family.

  • 본 논문에서는 최근 많은 관심을 받고 있는 안드로이드를 활용한 경증 자폐성장애인을 위한 보완대체의사소통 지원프로그램을 구현하였다. 보완대체의사소통이란 구어 및 비구어적 의사표현하기 어려운 사람들을 위해 사용하는 의사소통체계로서, 본 프로그램에서는 자폐장애인의 의사소통과 의사소통 언어의 선택적 빈도를 측정하는 방법과 자폐 아등의 지적 장애인의 언어에 대한 기본적인 훈련을 하는 방법을 소개한다. 본 논문에서는 보완대체 의사소통에서의 언어표상기법을 활용하여 여러 의사소통의 자유가 없는 사용자들이 효과적인 의사소통 및 학습을 할 수 있도록 개발하였으며, TTS(Text to Speech)를 사용하여 사용자의 의사를 육성으로 전달할 수 있도록 하였다. 그림판기능을 제공하여 사용자의 의사전달의 폭을 넓히고 언어빈도 측정을 통한 사용자의 언어사용빈도 그리고 자폐아의 경우 의식적 무의식 의사전달에 따른 백분율 수치를 두어 도움을 주도록 구현하였다.

HTML5기반 보완·대체 의사소통 판 제작 및 활용 시스템 (An HTML5 based AAC Board Making System)

  • 보완대체의사소통(AAC)은 의사소통의 어려움을 해결하는 광범위한 지원책으로 구어를 이용한 의사소통을 보완하거나 대신하기 위하여 사용되는 여러 가지 형태의 비구어적 의사소통 방법이다. 우리나라의 잠재적인 AAC 수요자가 늘어가면서 AAC에 대한 필요성이 증가하고 있으나 실질적인 활용이 활발하게 이루어지지 않고 있는 실정이다. 이에 본 논문은 웹을 기반으로 하여 제작자 그룹 간 의사소통 판을 제작하고 공유하며 판 사용자가 의사소통 판을 효율적으로 활용할 수 있도록 하는 의사소통 판 시스템을 구현하였다. 본 논문에서 구현한 시스템을 통하여 병원, 학교, 직장 등 여러 환경에서의 의사소통 판의 활용에 기여하고자 한다.

한국판 아스퍼거 증후군 진단척도(The Korean version of Asperger Syndrome Diagnostic Scale : K-ASDS) 의 신뢰도 및 타당도 연구 (A STUDY OF RELIABILITY & VALIDITY FOR THE KOREAN VERSION OF ASPERGER SYNDROME DIAGNOSTIC SCALE)

  • 연구 목적 : 국내에서는 아스퍼거 장애를 평가하고 진단하는 구조화된 검사 도구의 개발이 거의 이루어지지 않은 상태로, 아스퍼거 장애에 대한 진단적 타당도의 검증 및 향후 연구의 활성화를 위해 본 연구에서는 한국판 아스퍼거 증후군 진단 척도(the Korean version of Asperger's Syndrome Diagnostic Scale)의 신뢰도와 타당도를 검증하는 연구를 수행하였다. 방법 : 소아정신과 외래를 방문한 아동들 중 DSM-IV 진단기준과 소아정신과 의사에 의해 아스퍼거 장애, 전반적 발달 장애(PDD)와 비전형 전반적 발달 장애(PDD NOS), 의사소통 장애로 진단된 아동을 대상으로 그들의 부모들에게 한국어로 번역한 ASDS를 실시하였다. 총 55명이 연구에 포함되었으며(아스퍼거 장애 15명, PDD와 PDD NOS 20명, 의사소통 장애 20명), 연령의 범위는 5세 1개월부터 15세 6개월까지였다. 공존 타당도 및 변별 타당도를 알아보기 위하여 또 다른 아스퍼거 장애 질문지와 자폐 증상을 평가하는 K-CARS를 함께 실시하였다. 결과 : 자료 분석 결과, K-ASDS의 내적 합치도는 Chronbach's $\alpha=0.88$로 높게 나타났으며, 각 하위척도별 내적 일치도 또한 높았다. K-ASDS와 아스퍼거 장애 질문지 간의 상관도 통계적으로 유의미하였고, 하위 척도 상에서 Asperger disorder, PDD, Communication disorder 집단 간의 차이도 유의미하였다. 판별 분석 결과, K-ASDS가 자폐 장애와 의사소통 장애로부터 아스퍼거 장애를 유의미하게 구분해주는 판별 함수를 얻었으며, 이들 장애를 구분해주는 전체 정확 판별율은 $71\%$였다. 결론 : 본 연구 결과, 한국판 ASDS의 신뢰도와 타당도가 입증되었으며, 아스퍼거 장애 아동들을 평가하고 진단하는데 유용하게 사용될 수 있을 것이다.

언어장애인을 위한 안드로이드 기반 의사소통보조 어플리케이션 (An Android Application for Speech Communication of People with Speech Disorders)

  • Voice is the most common means for communication, but some people have difficulties in generating voice due to their congenital or acquired disorders. Individuals with speech disorders might lose their speaking ability due to hearing impairment, encephalopathy or cerebral palsy accompanied by motor skill impairments, or autism caused by mental problems. However, they have needs for communication, so some of them use various types of AAC (Augmentative & Alternative Communication) devices in order to meet their communication needs. In this paper, a mobile application for literate people having speech disorder was designed and implemented by developing accurate and fast sentence-completion functions for efficient user interaction. From a user study and the previous study on Korean text-based communication for adults having difficulty in speech communication, we identified functionality and usability requirements. Specifically, the user interface with scanning features was designed by considering the users' motor skills in using the touch-screen of a mobile device. Finally, we conducted the usability test for the application. The results of the usability test show that the application is easy to learn and efficient to use in communication with people with speech disorders.
