  • Since it has generally been considered that high-hat diets promote carcinogenesis, fat intake of less than 30% of total calories has been recommended to reduce the risk of cancer. Specific dietary guidelines for fat intake to reduce the risk of colon cancer have not yet been established. In order to determine the level of dietary fat needed the risk of colon cancer, rats were fed one of four experimental fat diets, very low(7% of total calories from corn oil, VLC), low(15% LC), medium (30%, MC), and high fat(45%, HC). Cell proliferation as an intermediate biomarker of color carcinogenesis was measured by the in vivo incorporation of bromodeoxyuridine into DNA.   • 현재까지 다양한 암의 발병 원인이 밝혀졌는데, 그 중 하나로써 DNA에 돌연변이가 축적되어 유전체가 불안정 해짐에 따라 암이 발생될 수 있는 기작들이 주목받고 있다. 생물정보학과 유전체학의 발달에 따라 질병 연구에 있어서 보다 더 정확하고 신뢰성 있는 바이오마커를 찾는 것이 가능해졌다. 따라서, 생물정보학과 유전체학의 연구 기반을 바탕으로 한 암의 바이오마커는 암의 조기진단뿐만 아니라, 더 나아가 암 발생 예측과 암환자의 예후 진단에 적용될 수 있다. 최근 들어 인간 유전체에서 약 45%를 차지하는 이동성 유전인자(transposable elements, TEs)가 유전자의 발현 조절과 DNA의 돌연변이를 유도함으로써 다양한 질병에 영향을 미친다는 사실이 밝혀짐에 따라, 이러한 이동성 유전인자들이 암의 발생과 어떤 연관이 있는지에 대한 연구 또한 활발히 진행되고 있다. 따라서 우리는 이동성 유전인자가 대장암과 어떤 연관성이 있는지에 대해 조사를 하였으며, 이를 어떻게 바이오마커로 활용할 수 있는지 알아보았다. 우선, 이동성 유전인자 중 인간 유전체에 많이 존재하면서 유전체에 많은 영향을 미치는 LINE-1 (long interspersed nuclear element-1, L1)과 Alu, LTR (long terminal repeat) 위주로 확인하였다. 흥미롭게도, 대장암 세포에서 LINE-1의 저메틸화, APC 유전자 내에 LINE-1 삽입, Alu의 저메틸화와 과메틸화, LTR 삽입에 따른 isoform 발생 등이 특징적으로 나타나는 것을 알 수 있었다. 또한 원발암유전자에서의 L1 저메틸화가 대장암 전이의 바이오마커로 쓰일 수 있다는 것과 Alu에 의한 MLH1 돌연변이가 가족성 및 유전성 대장암에서 흔히 발견된다는 것을 알 수 있었다. 이 때 이동성 유전인자에 의하여 영향 받는 유전자들의 발현을 in silico 발현 분석을 통하여 분석하였으며, 조직별, 성별 특이적 발현 양상을 제시하였다. 이들을 토대로 대장암 바이오마커를 개발하여 유전성 대장암의 예측 및 대장암 진단 또는 대장암 예후 예측을 통한 개인 맞춤형 치료에 활용할 수 있을 것으로 예상된다.

Is Early Detection of Colon Cancer Possible with Red Blood Cell Distribution Width?

  • Background: Red cell distribution width (RDW) is one of the standard parameters with blood cell counts. Much previous research has indicated that it increases in cases of systemic inflammation or cardiametabolic incident. However, information on the relation of RDW with solid tumors causing systemic inflammation is limited. In the present research, we examined the relation of RDW with malignant and benign lesions of the colon. Materials and Methods: 115 patients with colon polyps (group 1), and 30 with colon cancer (group 2) who were diagnosed histopathologically in our clinic between January 2010-January 2013 were scanned retrospectively. Patients with anemia, hematologic diseases and active inflammation were excluded. RDW, mean corpuscular volume (MCV), hemoglobin (Hgb) and platelet (Plt) measurements were recorded and their relations with the malignant and benign lesions of the colon were examined. Results: Both groups were similar in age and gender distribution. RDW values of patients with colon cancer were significantly higher than the patients with colon polyp (p=0,01). No significant differences were detected between the two groups in terms of MCV and Plt values (p>0,05). Conclusions: RDW can be used as an early warning biomarker for solid colon tumors. Further prospective research is required on the relations of cheap and easily measured RDW parameters with colon malignancies.

Expression of Smoothened Protein in Colon Cancer and its Prognostic Value for Postoperative Liver Metastasis

  • Backgrouds: The hedgehog (Hh) signaling pathway is composed of patched (PTCH) and smoothened (SMO), two transmembrane proteins, and downstream glioma-associated oncogene homolog (Gli) transcription factors. Hh signaling plays a pathological role in the occurrence and development of various cancers. Methods: To investigate the expression of SMO protein in colon cancer and its association with clinicopathological parameters and postoperative liver metastasis, immunohistochemistry was performed with paraffin-embedded specimens of 96 cases. Relationships between SMO protein expression and clinicopathological parameters, postoperative liver metastasis were analyzed. Results: IHC examination showed that SMO protein expression was significantly increased in colon cancer tissues compared to normal colon tissues (P = 0.042), positively related to lymph node metastases (P = 0.018) and higher T stages (P = 0.026). Postoperative live metastasis-free survival was significantly longer in the low SMO expression group than in those with high SMO expression ($48.7{\pm}8.02$ months vs $28.0{\pm}6.86$ months, P=0.036). Multivariate analysis showed SMO expression level to be an independent prognostic factor for postoperative live metastasis-free survival (95% confidence interval [CI] =1.46-2.82, P = 0.008). Conclusions: Our results suggest that in patients with colon cancer, the SMO expression level is an independent biomarker for postoperative liver metastasis, and SMO might play an important role in colon cancer progression.

Disease Prediction Using Ranks of Gene Expressions

  • A large number of studies have been performed to identify biomarkers that will allow efficient detection and determination of the precise status of a patient’s disease. The use of microarrays to assess biomarker status is expected to improve prediction accuracies, because a whole-genome approach is used. Despite their potential, however, patient samples can differ with respect to biomarker status when analyzed on different platforms, making it more difficult to make accurate predictions, because bias may exist between any two different experimental conditions. Because of this difficulty in experimental standardization of microarray data, it is currently difficult to utilize microarray-based gene sets in the clinic. To address this problem, we propose a method that predicts disease status using gene expression data that are transformed by their ranks, a concept that is easily applied to two datasets that are obtained using different experimental platforms. NCI and colon cancer datasets, which were assessed using both Affymetrix and cDNA microarray platforms, were used for method validation. Our results demonstrate that the proposed method is able to achieve good predictive performance for datasets that are obtained under different experimental conditions.

Functional Analysis of B7-H3 in Colonic Carcinoma Cells

  • B7-H3 is a newly discovered member of the B7/CD28 superfamily which functions as an important T-cell immune molecule. It has been reported recently that B7-H3 is highly expressed in many cancer cells, the data indicating that it may be a regulation factor contributing to tumor-resistance. In our study, we used bioinformatics to identify differentially expressed genes between colonic cancer cells and normal colonic cells, aiming to analyze mechanisms and identify sub-pathways closely related to progression, with the final aim of finding small molecule drugs which might interfere this progression. We found that ajmaline is one related factor which may enhance self-immunity in colon carcinoma therapy and B7-H3 plays important roles with regard to immunoreactions of colonic cancer cells. All the results indicate that H7-B3 is a favorable prognostic biomarker for colon carcinomas, providing novel information regarding likely targets for intervention.

Relationship between Colonic Polyp Type and the Neutrophil/Lymphocyte Ratio as a Biomarker

  • Aim: We designed this study to investigate the neutrophil lymphocyte ratio as a biomarker in distinguishing colonic polyps which are neoplastic or non-neoplastic. Materials and Methods: One hundred and twenty-five patients with colonic polyps were enrolled into the study. The following data were obtained from a computerized patient registry database: mean platelet volume (MPV), uric acid (UA), platelet count (PC), alkaline phosphatase (ALP), gamma-glutamyl transpeptidase (GGT) and the neutrophil to lymphocyte ratio (NLR). Exclusion criteria were active infectious disease, hematological disorders, and malignancies. Colonic polyps divided into two groups as neoplastic polyps (tubular adenoma, villous adenoma, tubulovillous adenoma) and non-neoplastic polyps (hyperplastic polyps, inflammatory pseudopolyps etc). The relationship between colonic polyp type and NLR was evaluated with statistical analysis. Results: There were 67 patients (53.6%) with neoplastic and 58 (46.4%) patients with non-neoplastic polyps. Mean NLRs of neoplastic and non-neoplastic groups were respectively $3.32{\pm}2.54$ and $2.98{\pm}3.16$ (P<0.05). Conclusion: Although sensitivity and specificity are not high, NLR may be used as a biomarker of neoplastic condition of colonic polyps.

Expression of anoctamin 7 (ANO7) is associated with poor prognosis and mucin 2 (MUC2) in colon adenocarcinoma: a study based on TCGA data

  • Colon adenocarcinoma (COAD) is the predominant type of colorectal cancer. Early diagnosis and treatment can significantly improve the prognosis of COAD patients. Anoctamin 7 (ANO7), an anion channel protein, has been implicated in prostate cancer and other types of cancer. In this study, we analyzed the expression of ANO7 and its correlation with clinicopathological characteristics among COAD patients using the Gene Expression Profiling Interactive Analysis 2 (GEPIA2) and the University of Alabama at Birmingham CANcer (UALCAN) databases. The GEPIA2, Kaplan-Meier plotter, and the Survival Genie platform were employed for survival analysis. The co-expression network and potential function of ANO7 in COAD were analyzed using GeneFriends, the Database for Annotation, Visualization and Integrated Discovery (DAVID), GeneMANIA, and Pathway Studio. Our data analysis revealed a significant reduction in ANO7 expression levels within COAD tissues compared to normal tissues. Additionally, ANO7 expression was found to be associated with race and histological subtype. The COAD patients exhibiting low ANO7 expression had lower survival rates compared to those with high ANO7 expression. The genes correlated with ANO7 were significantly enriched in proteolysis and mucin type O-glycan biosynthesis pathway. Furthermore, ANO7 demonstrated a direct interaction and a positive co-expression correlation with mucin 2 (MUC2). In conclusion, our findings suggest that ANO7 might serve as a potential prognostic biomarker and potentially plays a role in proteolysis and mucin biosynthesis in the context of COAD.

라디오믹스 기반 직장암 수술 위험도 예측을 위한 MRI 반자동 선택 바이오마커 검증 연구 (A Study on MRI Semi-Automatically Selected Biomarkers for Predicting Risk of Rectal Cancer Surgery Based on Radiomics)

  • Currently, studies to predict the risk of rectal cancer surgery select MRI image slices based on the clinical experience of surgeons. The purpose of this study is to semi-automatically select and classify 2D MRI image slides to predict the risk of rectal cancer surgery using biomarkers. The data used were retrospectively collected MRI imaging data of 50 patients who underwent laparoscopic surgery for rectal cancer at Gachon University Gil Medical Center. Expert-selected MRI image slices and non-selected slices were screened and radiomics was used to extract a total of 102 features. A total of 16 approaches were used, combining 4 classifiers and 4 feature selection methods. The combination of Random Forest and Ridge performed with a sensitivity of 0.83, a specificity of 0.88, an accuracy of 0.85, and an AUC of 0.89±0.09. Differences between expert-selected MRI image slices and non-selected slices were analyzed by extracting the top five significant features. Selected quantitative features help expedite decision making and improve efficiency in studies to predict risk of rectal cancer surgery.
