Regenerative effect of recombinant human bone morphogenetic protein-2/absorbable collagen sponge (rhBMP-2/ACS) after sequestrectomy of medication-related osteonecrosis of the jaw (MRONJ)

  • Objectives: Beyond the original application approved by the U.S. Food and Drug Administration, recombinant human bone morphogenetic protein-2 (rhBMP-2) is used for medication-related osteonecrosis of the jaw (MRONJ) treatment because of its bone remodeling enhancement properties. The purpose of the study was to investigate the bone formation effect of rhBMP-2/absorbable collagen sponge (ACS) in patients with MRONJ. Materials and Methods: In this retrospective cohort study, 26 female patients diagnosed with MRONJ and who underwent mandibular sequestrectomy at Ajou University Dental Hospital from 2010 to 2018 were included. The experimental group was composed of 18 patients who received rhBMP-2/ACS after sequestrectomy, while the control group was composed of 8 patients who did not receive rhBMP-2/ACS after sequestrectomy. A total dose of 0.5 mg of rhBMP-2 was used in the experimental group at a concentration of 0.5 mg/mL. Follow-up panoramic X-rays were taken immediately after the surgery and more than 6 months after the surgery. Using those X-rays, a radiographic index of bone defect area was calculated using the modified Ihan Hren method, which measures radiographic density of the normal bone and the defect site. Results: This study suggests that rhBMP-2 contributes to new bone formation. The mean radiographic index immediately after surgery and more than 6 months after the surgery for the experimental group was 68.4% and 79.8%, respectively. The mean radiographic index immediately after surgery and more than 6 months after the surgery for the control group was 73.4% and 76.7%, respectively (Wilcoxon signed rank test, P>0.05). The mean radiographic index increased 11.4% in the experimental group and 3.27% in the control group (Mann-Whitney U-test, P<0.05). Conclusion: Based on the results, use of rhBMP-2/ACS on bone defect sites after sequestrectomy could be a successful strategy for treatment of MRONJ patients.

  • Purpose: Recombinant human bone morphogenetic protein-2(rhBMP-2) has been evaluated as potential candidates for periodontal and bone regenerative therapy. In spite of good prospects in BMP applications, there is economically unavailable for clinical use in dental area. The purpose of this study was to evaluate the osteogenic effect of rhBMP-2 produced by E.coli expression system. Materials and methods: Eight-mm critical-size calvarial defects were created in 48 male Sprague-Dawley rats. The animals were divided into 6 groups of 8 animals each. Each group received one of the following: Negative control(sham-surgery control), positive control(absorbable collagen sponge(ACS) alone) and experimental(ACS loaded with rhBMP-2). Defects were evaluated by histologic and histometric parameters following 2- and 8-week healing intervals. Results: The experimental group showed significant defect closure at 2 and 8weeks than the sham surgery and positive control groups. Moreover, the experimental group showed significantly greater new bone and augmented area than the other groups at both 2 and 8weeks. Conclusion: rhBMP-2 produced by E.coli expression system may be effective for bone regeneration.

  • 피부재생을 위한 생체재료는 염증반응이 최소화되는 안정한 소재로 빠른 피부재생을 돕기 위해 우수한 생체활성과 생체친화성을 가져야 하며, 세포의 부착과 성장을 돕는 미세구조와 다공성이 있어야 한다. 본 연구에서는 새로운 콜라겐 원으로서의 축산부산물인 오리발을 사용하여 콜라겐을 추출하였고 이를 탈미네랄화된 골분demineralized bone powder, DBP), 돼지 소장점막하 조직(small intestinal submucosa, SIS)과 비교하기 위해 스펀지 형태로 제작하였다. 지지체의 물리, 화학적 특징은 SEM, FTIR을 통해 확인하였다. 세포를 파종하여 MTT를 통해 세포의 부착 및 증식률을 측정하였고, 전염증성 사이토카인의 발현도를 보기 위해 RT-PCR을 실시하였다. 또한 항산화 활성능력을 보기 위해 1,1-diphenyl-2-picrylhydrazyl(DPPH)를 측정하였다. 그 결과 오리발 콜라겐 지지체가 물리적 특성이 우수하고 생체적합성이며, 상처 치유제로서의 가능성을 보여주었다.

The optimal dosage of hyaluronic acid for bone regeneration in rat calvarial defects

  • Purpose: Hyaluronic acid (HA) affects angiogenesis and promotes the migration and differentiation of mesenchymal cells, thereby activating the osteogenic ability of osteoblasts. Although studies on the action of HA during bone regeneration are being actively conducted, the optimal dose of HA required for bone regeneration remains unclear. Therefore, the purpose of this study was to elucidate the most effective HA dose for bone formation using a rat critical-size defect model. Methods: Thirty rats were randomly divided into 5 groups, with 6 rats in each group. An absorbable collagen sponge soaked with HA or saline was used to fill an 8-mm defect, which was then covered with a collagen membrane. Different treatments were performed for each group as follows: (1) saline control, (2) 1 mg/mL HA, (3) 25 mg/mL HA, (4) 50 mg/mL HA, or (5) 75 mg/mL HA. After a healing period of 4 weeks, micro-computed tomography and histological analysis were performed. The obtained values were analyzed using analysis of variance and the Tukey test (P<0.05). Results: At week 4, the 75 mg/mL HA group had the highest bone volume/total volume ratio, new bone, and bone fill among the 5 groups, and these values were significantly different from those observed in the control group (P<0.01) and 1 mg/mL HA group (P<0.001). More active bone formation was observed in the higher-dose HA groups (25 mg/mL, 50 mg/mL, and 75 mg/mL HA), which included a large amount of woven bone. Conclusions: The 75 mg/mL HA group showed better bone formation than the other groups (1, 25, and 50 mg/mL HA and control).

  • 알지네이트와 탈미네랄골분(DBP) 하이브리드 스펀지는 각각 DBP가 0, 10, 20, 40 및 80% 첨가된 스펀지로, 연골 재건을 위해 제작되었다. 세포의 증식은 MTT를 통해 관찰했으며, 형태학적, 조직학적, 생물학적 관찰 그리고 RT-PCR을 1, 2, 및 3 주에 실시하였다. 세포생존율에 있어서 20% DBP/알지네이트 스펀지에서 다른 스펀지보다도 가장 좋은 결과를 보였으며, 광학전자현미경 관찰에서 또한 20% 스펀지에서 가장 좋은 세포부착률과 증식률을 확인하였다. 마지막으로, 조직학적 평가에서 20% 스펀지에서 연골세포의 표현형이 가장 잘 유지됨과 동시에 산성뮤코 다당류와 제2형 콜라겐이 잘 형성된 것을 확인하였다. 이로써 DBP/알지네이트 하이브리드 스펀지는 연골 조직공학을 위한 지지체로써 활용될 수 있을 것이다.

SIS 스폰지와 골수유래줄기세포를 이용한 조직공학적 골분화 유도 (Effects of SIS Sponge and Bone Marrow-Derived Stem Cells on the Osteogenic Differentiation for Tissue Engineered Bone)

  • 소장 점막하 조직(SIS)은 면역반응이 없어 생체재료로 널리 사용되고 있다. 본 연구에서는 SIS를 스폰지 형태로 제조하여 1-ethyl-3-(dimethylaminopropyl) carbodiimide hydrochloride(EDC)를 이용하여 경화시켰으며, SIS 스폰지의 구성 윈소를 알아보기 위해 원소분석(EA)과 에너지 분산 X선 분광계(EDS)를 사용해 분석하였다. 또한 SIS 함량과 EDC의 농도에 따른 섬유아세포의 부착도 및 성장도를 알아보기 위해 methylthiazoletetrazolium,(MTT)을 실시하였다. 이 스폰지에 골수 간엽 줄기세포(BMSCs)를 파종해 4주 동안 조직공학적 골분화를 유도하였다. 골분화 유도를 위해 골분화 배지를 사용했으며, 배지에 따른 BMSCs의 세포 성장도와 alkaline phosphatase(ALP) 활성을 측정해 보았다. 또한 역전사 중합연쇄반응을 통해 골분화 여부를 관찰하였다. SEM 관찰 결과 모든 스폰지에서 균일한 형태의 열린 다공이 형성되었음을 확인할 수 있었다. RT-PCR 결과 4주 동안 골분화 배지를 주었을 때 제 I형 교원질이 발현됨을 확인할 수 있었으며, ALP 결과에서도 골분화 배지에서 ALP활성이 높게 나타남을 볼 수 있었다. 결론적으로 제조한 SIS 스폰지는 조직공학적 담체로써 우수한 특성을 보이고 있었으며, 또한 BMSCs의 골분화 유도에도 좋은 결과를 보임으로써 조직공학적 골 재생에 잠재적인 가능성을 가지고 있음을 확인할 수 있었다.

  • Purpose: Bone morphogenetic protein (BMP) is a potent differentiating agent for cells of the osteoblastic lineage. It has been used in the oral cavity under a variety of indications and with different carriers. However, the optimal carrier for each indication is not known. This study evaluated the bone regenerative effect of rhBMP-2 delivered with different carrier systems. Materials and Methods: 8 mm critical-sized rat calvarial defects were used in 60 male Sprague-Dawley rats. The animals were divided into 6 groups containing 10 animals each. Two groups were controls that had no treatment and absorbable collagen membrane only. 4 groups were experimentals that contained rhBMP-2 only and applied with absorbable collagen sponge($Collatape^{(R)}$), $MBCP^{(R)}$, Bio-$Oss^{(R)}$ each. The histological and histometric parameters were used to evaluate the defects after 2- or 8-week healing period. The shape and total augmented area were stable in all groups over the healing time. Results: New bone formation was significantly greater in the rhBMP-2 with carrier group than control group. rhBMP-2/ACS was the highest in bone density but gained less new bone area than rhBMP-2/$MBCP^{(R)}$ and rhBMP-2/Bio-$Oss^{(R)}$. The bone density after 8 weeks was greater than that after 2 weeks in all groups. However, rhBMP-2 alone failed to show the statistically significant difference in new bone area and bone density compared to control group. Also $MBCP^{(R)}$ and Bio-$Oss^{(R)}$ particles remained after 8 weeks healing period. Conclusion: These results suggest that rhBMP-2 with carrier system is an excellent inductive agent for bone formation and we can use it as the predictable bone tissue engieering technique. Future study will likely focus on the kinetics of BMP release and development of carriers that is ideal for it.

Bone regenerative effect of recombinant human bone morphogenetic protein-2 after cyst enucleation

  • Background: The aim of this study is to quantitatively evaluate the effect of rhBMP-2 for repair of bone defects after cyst enucleation using the osteogenesis index (OI). Methods: Under general anesthesia, 10 patients (12 lesions) underwent oral or maxillofacial surgery for cyst enucleation. Postoperatively, 12 lesions were divided into two groups: group A (six lesions) was treated with absorbable collagen sponge (ACS) in combination with rhBMP-2, and group B (six lesions) was treated with ACS alone. After 3 months, cone-beam computed tomographic scans were obtained to measure changes in the volume of the lesions. We then calculated the OI of each group at two different Hounsfield units to determine any statistically significant difference between these two groups (Mann-Whitney U test). Results: As tested at the level of new bone, the mean OI was 72.37 % in group A and 55.08 % in group B -a statistically significant difference (p = 0.041). As tested at the level of mature bone, the mean OI was 27.47 % in group A and 18.88 % in group B, but the difference was not statistically significant (p = 0.394). Conclusions: The application of rhBMP-2 after maxillofacial cyst enucleation accelerated new bone formation in the bone defects. Thus, the use of rhBMP-2 in combination with ACS may be considered an alternative to conventional bone grafting in some patients with postoperative bone defects.

  • The purpose of this study is to report on five cases treated with bone graft using recombinant human bone morphogenetic protein-2 (rhBMP-2) on jaw defects after cyst enucleation. We performed bone graft with BMP (rhBMP-2+${\beta}$-tricalcium phosphate/hydroxyapatite) wrapped with a collagen sponge on jaw defects after cyst enucleation. Postoperative panoramic radiographs were taken periodically. After 1 to 12 months, bone remodeling was observed in the jaw defect area. In our cases, there were few signs or symptoms of recurrence of the cyst during the follow-up period.

Stepwise verification of bone regeneration using recombinant human bone morphogenetic protein-2 in rat fibula model

  • Objectives: The purpose of this study was to introduce our three experiments on bone morphogenetic protein (BMP) and its carriers performed using the critical sized segmental defect (CSD) model in rat fibula and to investigate development of animal models and carriers for more effective bone regeneration. Materials and Methods: For the experiments, 14, 16, and 24 rats with CSDs on both fibulae were used in Experiments 1, 2, and 3, respectively. BMP-2 with absorbable collagen sponge (ACS) (Experiments 1 and 2), autoclaved autogenous bone (AAB) and fibrin glue (FG) (Experiment 3), and xenogenic bone (Experiment 2) were used in the experimental groups. Radiographic and histomorphological evaluations were performed during the follow-up period of each experiment. Results: Significant new bone formation was commonly observed in all experimental groups using BMP-2 compared to control and xenograft (porcine bone) groups. Although there was some difference based on BMP carrier, regenerated bone volume was typically reduced by remodeling after initially forming excessive bone. Conclusion: BMP-2 demonstrates excellent ability for bone regeneration because of its osteoinductivity, but efficacy can be significantly different depending on its delivery system. ACS and FG showed relatively good bone regeneration capacity, satisfying the essential conditions of localization and release-control when used as BMP carriers. AAB could not provide release-control as a BMP carrier, but its space-maintenance role was remarkable. Carriers and scaffolds that can provide sufficient support to the BMP/carrier complex are necessary for large bone defects, and AAB is thought to be able to act as an effective scaffold. The CSD model of rat fibula is simple and useful for initial estimate of bone regeneration by agents including BMPs.
