Inflammatory Breast Cancer: a Single Centre Analysis

  • Background: Inflammatory breast cancer (IBC) is an aggressive form of locally advanced breast cancer characterized by rapidly progressive breast erythema, pain and tenderness, oedema and paeu d'orange appearance. It accounts for 1-3% of all newly diagnosed cases of breast cancer in the west. Data on IBC from India are lacking. The aim of our study was to assess the clinical-pathological parameters and outcome of IBC at, All India Institute of Medical Sciences, a large tertiary care centre. Materials and Methods: We screened 3,650 breast cancer cases registered from January 2004 to December 2012 and found 41 cases of IBC. Data included demographics as well as clinical, radiological and histopathological characteristics, and were collected from clinical case records using the International Classification of Diseases code (C-50). Patients who presented with IBC as a recurrence, or who had a neglected and advanced breast cancer that simulated an IBC were excluded from this study. Results: The median age was 45 years (range 23-66). The median duration of symptoms was 5 months. The American Joint Committee on Cancer stage (AJCC) distribution was Stage III - 26 and IV - 15 patients. Estrogen receptor (ER), progesterone receptor (PR) positivity and human epidermal growth factor receptor 2 (HER2/neu) positivity were 50%, 46% and 60%, respectively. Triple negativity was found in 15% of the cases. All the non metastatic IBC patients received anthracycline and/ or taxane based chemotherapy followed by modified radical mastectomy, radiotherapy and hormonal therapy as indicated. Pathological complete remission rate was 15%. At a median follow-up of 30 months, the 3 year relapse free survival and overall survival were 30% and 40%respectively. Conclusion: IBC constituted 1.1% of all breast cancer patients at our centre. One third of these had metastatic disease at presentation. Hormone positivity and Her2 neu positivity were found in 50% and 60% of the cases, respectively.

Roles of CYP1A1 and CYP2E1 Gene Polymorphisms in Oral Submucous Fibrosis

  • Background: Oral submucous fibrosis (OSF) is a precancerous condition with a 4 to13% malignant transformation rate. Related to the habit of areca nut chewing it is mainly prevalent in South-east Asian countries where the habit of betel quid chewing is frequently practised. On chewing, alkaloids and polyphenols are released which undergo nitrosation and give rise to N-nitrosamines which are cytotoxic agents. CYP450 is a microsomal enzyme group which metabolizes various endogenous and exogenous chemicals including those released by areca nut chewing. CYP1A1 plays a central role in metabolic activation of these xenobiotics, whereas CYP2E1 metabolizes nitrosamines and tannins. Polymorphisms in genes that code for these enzymes may alter their expression or function and may therefore affect an individuals susceptibility regarding OSF and oral cancer. The present study was therefore undertaken to investigate the association of polymorphisms in CYP1A1 m2 and CYP2E1 (RsaI/PstI) sites with risk of OSF among areca nut chewers in the Northern India population. A total of 95 histopathologically confirmed cases of OSF with history of areca nut chewing not less than 1 year and 80, age and sex matched controls without any clinical signs and symptoms of OSF with areca nut chewing habit not less than 1 year were enrolled. DNA was extracted from peripheral blood samples and polymorphisms were analyzed by PCR-RFLP method. Gene polymorphism of CYP1A1 at NcoI site was observed to be significantly higher (p = 0.016) in cases of OSF when compared to controls. Association of CYP1A1 gene polymorphism at NcoI site and the risk of OSF (Odd's Ratio = 2.275) was also observed to be significant. However, no such association was observed for the CYP2E1 gene polymorphism (Odd's Ratio = 0.815). Our results suggest that the CYP1A1 gene polymorphism at the NcoI site confers an increased risk for OSF.

A Study on the High Pressure Pump Simulation Model of a Diesel Injection System (디젤 분사시스템의 고압펌프 시뮬레이션 모델에 대한 연구)

  • The high pressure pump of a diesel injection system compresses the fuel supplied at low pressure into high pressure fuel and maintains the fuel of the common rail at the required pressure level according to the engine operating conditions. The high pressure pump is required to operate normally in order to compress the fuel to a high pressure of 2000 bar during the entire lifetime of the vehicle. Consequently, a suitable design technique, material durability and high precision machining are required. In this study, the high pressure pump simulation model of a 1-plunger radial piston pump is modelled by using the AMESim code. The main simulation parameters are the displacement, flow rate and pressure characteristics of the inlet and outlet valves, cam torque characteristics, and operating characteristics of the fuel metering valve and overflow valve. In addition, the operating characteristics of the pump are simulated according to the parameter changes of the hole diameter and the spring initial force of the inlet valve. The simulation results show that the operation of the developed pump model is logically valid. This paper also proposes a simulation model that can be used for current pump design changes and new pump designs.

CFD Analysis on the Hydro Turbine by the Existence of Blade Holes (블레이드 타공에 따른 수차의 유동해석)

  • Considering that most sewage treatment facilities have a water head of less than 2.0 m and a constant flow rate, the development of a small hydro power generation device capable of maintaining stable power generation and efficiency is urgently needed. In this study, a numerical analysis using the CFD code was carried out to develop a drag force type vertical axis hydro turbine for the improvement of the production efficiency of small-scale hydro energy underlow flow velocity conditions. The blade pressure changes and internal flows were analyzed in the presence or absence of hydro turbine blade holes at a flow velocity of less than 2.0 m/s. The pressure distribution of the hydro turbine blades with holes was found to be about 5.1 % lower than that of the hydro turbine blades without holes. The analysis of the internal flow around the water tank and hydro turbine blade revealed that the flow velocity varied with the vector distribution and that the flow velocity of the hydro turbine blades with holes was 5.6 % less than that of the hydro turbine blades without holes. It is believed that forming a hole in the blade may be helpful for its structural safety.

Association between Single Nucleotide Polymorphisms in the Dgat2 Gene and Beef Carcass and Quality Traits in Commercial Feedlot Steers

  • Diacylglycerol acyltransferase (DGAT) is a key enzyme that catalyzes the final and rate-limiting step of triglyceride synthesis. Both DGAT1 and DGAT2 genes code proteins with DGAT activity. Studies have shown DGAT1 polymorphisms associate with intramuscular fat deposition in beef cattle, but fewer associations between DGAT2 and beef cattle economic traits have been reported. The objective of this study was to investigate single nucleotide polymorphism (SNP) in intron3 of bovine DGAT2 and evaluate the associations of that with carcass, meat quality, and fat yield traits. Test animals were 157 commercial feedlot steers belonging to 3 Chinese native breeds (22 for Luxi, 24 for Jinnan, and 23 for Qinchuan), 3 cross populations (20 for Charolais${\times}$Fuzhou, 18 for Limousin ${\times}$Luxi, and 17 for Simmental${\times}$Jinan) and 1 Taurus pure breed population (16 Angus steers). In the current study, 15 SNP were discovered in intron3 and exon4 of DGAT2 at positions 65, 128, 178, 210, 241, 255, 270, 312, 328, 334, 365, 366, 371, 415, and 437 (named as their positions in PCR amplified fragments). Only 7 of them (128, 178, 241, 270, 312, 328, and 371) were analyzed, because SNP in three groups (65-128-255, 178-210-365 and 241-334-366) were in complete linkage disequilibrium within the group, and SNP 415 was a deletion and 437 was a null mutation. Frequencies for rare alleles in the 3 native breed populations were higher than in the 3 cross populations for 178 (p = 0.04), 270 (p = 0.001), 312 (p = 0.03) and 371 (p = 0.002). A general linear model was used to evaluate the associations between either SNP genotypes or allele substitutions and the measured traits. Results showed that SNP 270 had a significant association with the fat yield associated with kidney, pelvic cavity, heart, intestine, and stomach (KPHISY). Animals with genotype CC and CT for 270 had less (CC: -7.71${\pm}$3.3 kg and CT: -5.34${\pm}$2.5 kg) KPHISY than animals with genotype TT (p = 0.02). Allele C for 270 was associated with an increase of -4.26${\pm}$1.52 kg KPHISY (p = 0.006) and $-0.92{\pm}0.45%$ of retail cuts weight percentage (NMP, Retail cuts weight/slaughter body weight) (p = 0.045); allele G for 312 was associated with an increase of -5.45${\pm}$2.41 kg KPHISY (p = 0.026). An initial conclusion was that associations do exist between DGAT2 gene and carcass fat traits. Because of the small sample size of this study, it is proposed that further effort is required to validate these findings in larger populations.

A Web-based Survey for Assessment of Korean Medical Treatment Clinical Practice Patterns for Neck Pain and Cervical Intervertebral Disc Displacement (경항통 및 경추 추간판 탈출증의 한의진료 임상현황조사를 위한 웹기반 설문조사)

  • Objectives : While neck pain is highly prevalent in Korea, studies examining its Korean medical treatment are currently lacking. The aim of this study was to assess current Korean medical treatment practice patterns for neck pain. Methods : A preliminary questionnaire was developed to investigate current practice patterns of neck pain and cervical intervertebral disc displacement (IDD) treatment, and underwent further revision through external review. The final questionnaire was distributed as a web-based survey to 18,289 potential respondents by email. Results : The response rate was 3.34 %. Most participants replied that they received multiple Korean medicine interventions for neck pain treatment consisting of such methods as acupuncture, wet cupping, interferential current therapy and electroacupuncture. A total of 378 respondents acknowledged that diagnostic testing was needed for neck pain treatment. The most commonly used Korean Standard Classification of Diseases (KCD) principal diagnosis code for neck pain was M542, and for cervical IDD was M501. Conclusion : This survey study helps determine current practice patterns of neck pain, and recognizes the need for use of diagnostic devices in neck pain treatment. These results are further anticipated to provide basic data for clinical practice guidelines (CPGs) and future studies using Korean National Health Insurance and Health Insurance Review & Assessment Service data.

Study on Methanol Conversion Efficiency of Steam-Methanol Reforming on Pipe Shape and Flow Rate Variation in Curved Channel (수증기-메탄올 개질기의 곡유로 채널형 관 형태 변화에 따른 메탄올 전환율 및 유동 특성에 관한 수치해석적 연구)

  • This is a numerical study on the curved channel type of hydrogen reformer using the commercial code of fluid dynamics. We numerically compared the numerical model in a previous study model and the modelling of a tube type curved channel. In the result of numerical analysis on 4 types of curved channel reformers, the methanol conversion efficiency of type 1~4 were 45.0%, 45.3%, 45.6%, 45.6% respectively, and there was hardly any difference by ${\pm}0.6%$. In light of flow characteristics, the rectangle type tube and the type 2 with $45^{\circ}$ turn showed most uniform flow characteristics and concentration distribution of methanol, and the circular type tube and the type 3 with $90^{\circ}$ turn had most un-uniform flow characteristics and concentration distribution of methanol. We concluded that the design for curved channel reformer has to have rectangle type tube with curve of almost $45^{\circ}$ as in the type of curved pipe with $45^{\circ}$ turn.

Implementation and Performance Evaluation of the Wireless Transaction Protocol Using UML/SDL (UML과 SDL을 이용한 무선 트랜잭션 프로토콜의 구현과 성능 평가)

  • In this paper, we design and implement the Wireless Transaction Protocol (WTP) proposed by the Wireless Application Protocol (WAP) forum using a protocol development tool, SDL Development Tool (SDT). And we conduct a comparative performance evaluation of the WTP implementation with other three implementations that are based on different implementation models respectively: the server model, the coroutine model, and the activity-thread model. To implement WTP, we first use Unified Modeling Language (UML) for analyzing the protocol requirement and defining the protocol engine architecture. Next, we use Software Development Language (SDL) to design the protocol engine in details and then generate the WTP implementation automatically with the aid of SDT The code size of the WTP implementation generated by SDT is 62% larger than the other three implementations. However, its throughput and system response time for transaction processing is almost equal to the other three implementations when the number of concurrent clients is less than 3,000. If more than 5,000 concurrent clients tries, the transaction success rate abruptly decreases to 10% and system response time increases to 1,500㎳, due to the increased protocol processing time. But, it comes from the fact that the load overwhelms the capacity of the PC resource used in this experimentation.

An Experimental Study on the Clogging of Sand Filter in a Model Filtration-Pond (천변여과지 모형에서 여재모래의 폐색현상 실험연구)

  • A pilot-scale sand-box experiment was performed in order to investigate the effect of cross-flow velocity on the clogging of the filter sand in a model filtration pond. The clogging phenomenon was observed during the operation with the cross-flow varied in stages in a range of 0~40 cm/sec, and the experimental result was analyzed using a numerical code. Results showed that the cross-flow velocity in this range had no influence on the development of clogging and that clogging occurred mostly on the filter-surface. It was found that while the production rate decreased from $5m^3/m^2-day$ to $3m^3/m^2-day$ the clogging coefficient of the top 50 cm layer increased up to about 30,000 sec, which corresponded to 87% of the clogging coefficient of the total 2.4 m layer. Of the clogging coefficient of the top 50 cm layer, surface clogging constituted 90% while the other 10% was intermediate clogging. It was also found that the surface clogging increased while the intermediate clogging remained constant as the operation continued, and that filtrate turbidity along the filtration depth remained constant in spite of the increase in clogging.

A Numerical Study on the Geometry Optimization of Internal Flow Passage in the Common-rail Diesel Injector for Improving Injection Performance (커먼레일 디젤인젝터의 분사성능 개선을 위한 내부유로형상 최적화에 관한 수치적 연구)

  • The common-rail injectors are the most critical component of the CRDI diesel engines that dominantly affect engine performances through high pressure injection with exact control. Thus, from now on the advanced combustion technologies for common-rail diesel injection engine require high performance fuel injectors. Accordingly, the previous studies on the numerical and experimental analysis of the diesel injector have focused on a optimum geometry to induce proper injection rate. In this study, computational predictions of performance of the diesel injector have been performed to evaluate internal flow characteristics for various needle lift and the spray pattern at the nozzle exit. To our knowledge, three-dimensional computational fluid dynamics (CFD) model of the internal flow passage of an entire injector duct including injection and return routes has never been studied. In this study, major design parameters concerning internal routes in the injector are optimized by using a CFD analysis and Response Surface Method (RSM). The computational prediction of the internal flow characteristics of the common-rail diesel injector was carried out by using STAR-CCM+7.06 code. In this work, computations were carried out under the assumption that the internal flow passage is a steady-state condition at the maximum needle lift. The design parameters are optimized by using the L16 orthogonal array and polynomial regression, local-approximation characteristics of RSM. Meanwhile, the optimum values are confirmed to be valid in 95% confidence and 5% significance level through analysis of variance (ANOVA). In addition, optimal design and prototype design were confirmed by calculating the injection quantities, resulting in the improvement of the injection performance by more than 54%.
