Optimal orthodontic treatment could be possible when a orthodontist can predict and control tooth movement by applying a planned force system to the dentition. The moment to force(M/F) ratio at the bracket, has been shown to be a primary determinate of the pattern of tooth movement. As various n/F ratios are applied to the bracket on the tooth crown, strain distribution in periodontium can be changed, and the center of rotation in tooth movement can be determined. It is, therefore, so important in clinicalorthodontics to know the strain distribution in a force system of a M/F ratio. The purpose of this study was to analyze the strain distribution in orthodontic force system by strain gauge attached to tooth root, and to evaluate the usage of the method. For this study, an experimental upper anterior arch model was constructed, where upper central incisors, on the root surface of which, 8 strain gauges were attached, were implanted In the photoelastic resin, as in the case of 4mm midline diastema. Three types of closing of upper midline diastema closure were compared : 1. with elastomeric chain(100g force) in no arch wire, 2. elastomeric chain in .016“ round steel wire, 3. elastomeric chain in .016”x.022“ rectangular steel wire. The results were as follows. 1. Strain distributions on labial, lingual, mesial and distal root surface of tooth were able to be evaluated with the strain gauge method, and the patterns of tooth rotation were understood by presuming the location of moment arm. 2. Extrusion and tipping movement of tooth was seen in closing in no arch wire, and intrusion and bodily movement was seen with steel arch wire inserted.
Background: The benefit of superior vena cava (SVC) resection in thoracic malignancies remains controversial. We analyzed the results of extended resection in patients with thoracic malignancy involving the SVC. Material and Method: From March 2000 to March 2009, we performed surgical resection and reconstruction in 18 thoracic malignancies involving the SVC. Ten male and 8 female enrolled and their mean age was 56 years. Result: SVC reconstruction was performed in 9 patients with polytetrafluoroethylene (PTFE) graft. Primary closure was possible in 6 patients by partially clamping the SVC. Patch angioplasty was performed in 3 patients with PTFE or autologous pericardial patch. Three-year survival was 58.0% and median survival time was 24.5 months. Disease specific survival and recurrence free survival were not significantly different between lung cancer and mediastinal malignancy. Obstruction of graft was detected in 4 patients during follow-up; SVC graft obstruction in 1 patient, and accessory graft between the innominate vein and right atrium in 3 patients. Conclusion: Extended resection of thoracic malignancies involving the SVC was a feasible method in selected patients. Although the morbidity rate was relatively high, mid-term survival was acceptable when complete resection was possible.
Background: This study is to suggest the optimal method as a treatment for the patent ductus arteriosus in the premature infants. Material and Method : Between April 1994 and April 1997, 45 premature infants with evidence of a hemodynamically significant patent ductus arteriosus associated with cardiopulmonary compromise underwent indomethacin therapy, surgical treatment, or both. Thirty-nine infants received indomethacin and twelve infants among them were surgically ligated because of indomethacin failure(5) or complications(7). Six infants, who weighed less than 1,500 gm at birth, were referred for primary surgical ligation because of contraindication to indomethacin therapy. Result: The failure rate of indomethacin therapy was 43%(17/39) and the complications(13/39, 33%) to the indomethacin were associated with a high morbidity and mortality. Among the infants who underwent ligation, there were no failures and complications related to the operation. This data suggests that in the premature neonate with a hemodynamically significant PDA, (1) indomethacin therapy is associated with a high failure rate and significant complications, (2) surgical duct closure is associated with minimal morbidity. Conclusion: Although the results of this study cannot suggest the optimal management for PDA in premature infants, primary surgical ligation may be considered. However, long-term studies will be needed to confirm this later.

In the present work, we investigate the hydrodynamic behavior of a turbulent bore, such as tsunami bore and tidal bore, generated by the removal of a gate with water impounded on one side. The bore generation system is similar to that used in a general dam-break problem. In order to the numerical simulation of the formation and propagation of a bore, we consider the incompressible flows of two immiscible fluids, liquid and gas, governed by the Navier-Stokes equations. The interface tracking between two fluids is achieved by the volume-of-fluid (VOF) technique and the M-type cubic interpolated propagation (MCIP) scheme is used to solve the Navier-Stokes equations. The MCIP method is a low diffusive and stable scheme and is generally extended the original one-dimensional CIP to higher dimensions, using a fractional step technique. Further, large eddy simulation (LES) closure scheme, a cost-effective approach to turbulence simulation, is used to predict the evolution of quantities associated with turbulence. In order to verify the applicability of the developed numerical model to the bore simulation, laboratory experiments are performed in a wave tank. Comparisons are made between the numerical results by the present model and the experimental data and good agreement is achieved.
Diffusion is a powerful tool to understand geological processes recorded in terrestrial rocks as well as extraterrestrial materials. Since the diffusive exchange of elements or isotopes may have occurred differently in the solar nebula (high temperature and rapid cooling) and on the parent bodies (fluid-assisted thermal metamorphism at relatively low temperature), it is particularly important to model elemental or isotopic diffusion profiles within the mineral grains to better understand the evolution of the early solar system. A numerical model with the finite difference method for the fast grain boundary diffusion was established for the exchange of elements or isotopes between constituent minerals in a closed system. The fast grain boundary diffusion numerical model was applied to 1) 26Mg variation in plagioclase of an amoeboid olivine aggregate (AOA) from a CH chondrite and 2) Fe-Mg interdiffusion between chondrules, AOA, and matrix minerals in a CO chondrite. Equilibrium isotopic fractionation and equilibrium partitioning were also included in the numerical model, based on the assumption that equilibrium can be reached at the interfaces of mineral crystals. The numerical model showed that diffusion profiles observed in chondrite samples likely resulted from the diffusive exchange of elements or isotopes between the constituent minerals. This study also showed that the closure temperature is determined not only by the mineral with the slowest diffusivity in the system, but also strongly depends on the constituent mineral abundances.

Background: Some controversy still exists concerning the operative indications of coronary fistulas. Nevertheless, a short-term and long-term outcomes are excellent with surgical interventions. In this study, we assessed our surgical results on this disease entity during the last 20 years. Anatomic diversity was described as well. Material and Method: From April 1986 to March 2005, 20 patients with coronary fistulas underwent surgical correction in Seoul National University Children's Hospital. Their medical records were reviewed retrospectively. Result: Twelve patients ($60\%$) were asymptomatic prior to surgery. All had electrocardiogram and echocardiogram and all but 3 had coro-nary angiogram preoperatively. Anatomically, none of them had two or more coronary fistulas. The sites of origin were left coronary system in 11 patients and right in 9. The draining sites were right ventricle in 11, right atrium in 3, left ventricle in 3, main pulmonary artery in 2, and superior vena cavae in 1. All of the involved, the coro-nary arteries were dilated or aneurismal. In 1 case, there was atherosclerotic change but no ischemic evidence in preoperative electrocardiogram. Operative techniques included external obliteration (13), internal obliteration (5), and both (2). External obliteration was done by ligation of the fistulous tract only in T patients, by fstula ligation plus plication in 3 and by plication or patch closure via fistulotomy in 3. There was no operative mortality. All of postoperative morbidities including transient sinus arrhythmia (2), complete atrioventricular block (1), decreased left ventricular function (2), ventricular tachycardia (1), pericarditis (1), and seizure (1) improved on discharge. The mean follow-up was 55.1$\pm$50.2 months (4.0 months${\~}$18.0 years) and there were no recurrences of fistula. There was 1 second operation for aortic root aneurysm, which developed after external patch closure of right coronary fistula. Conclusion: We demonstrated here that coronary fistulas can be cured with excellent clinical outcome and low operative risk under precise diagnosis. Understanding the anatomic diversity will help to construct surgical plans.
Background: Minimally invasive surgery is currently popular, but this has been applied very sparingly to cardiac surgery because of some limitations. Our study evaluated the safety and efficacy of atrial septal defect (ASD) closure through a video-assisted mini-thoracotomy. Material and Method: Fifteen patients were analyzed. Their mean age was $31{\pm}6$ years. The mean ASD size was $24{\pm}5mm$ and there were 3 cases of significant tricuspid regurgitation. The working window was made through the right 4th intercostal space via a $4{\sim}5cm$ inframammary skin incision, CPB was conducted with performing peripheral cannulation. After cardioplegic arrest, the ASDs were closed with a patch (n=11) or direct sutures (n=4), and the procedures were assisted by using a thoracoscope. There were 3 cases of tricuspid repair and 1 case of mitral valve repair. The mean CPB time and aortic occlusion time were $160{\pm}47\;and\;70{\pm}26 $minutes, respectively. Result: There was no mortality, but there were 3 minor complications (one pneumothorax, one wound dehiscence and one arrhythmia). The mean hospital stay was $5.9{\pm}1.8$ days. The mean follow-up duration was $10.7{\pm}6.4$ months. The follow-up echocardiogram noted no residual ASD or significant tricuspid regurgitation. Three patients suffered from pain or numbness. Conclusion: This study showed satisfactory clinical and cosmetic results. Although the operative time is still too long, more experience and specialized equipment would make this technique a good option for treating ASD.
Background: This study was undertaken in infant patients with isolated ventricular septal defect(VSD) to determine the effect of surgical closure on ventricular systolic time interval, as a parameter for ventricular performance, by echocardiography. Material and Method: Thirty patients were enrolled. Mean age of patients at operation was 6.5$\pm$3.2 months and all patients had non-restrictive VSD. We checked the left atrium/aorta(LA/Ao) ratio, left ventricle ejection fraction(EF), left ventricular systolic time interval(LVSTI), and right ventricular systolic time interval(RVSTI). Echocardiographic studies were done before surgical correction and postoperative periods(postopl: within 2 weeks, postop2: between 4 and 6 months, postop3: between 1 and 2 years). Result: LA/Ao ratio decreased significantly at immediate postoperative period compared to preoperative period and sustained during further follow-up period(from 1.74$\pm$0.37 to 1.36$\pm$0.24*, 1.32$\pm$0.22*, and 1.27$\pm$0.19*, p<0.01). LV EF had not changed during follow-up periods(from 65.1$\pm$7.0 to 62.3$\pm$9.5, 62.8$\pm$5.7, and 64.1$\pm$6.9). LVSTI decreased significantly at postop2 and sustained during further follow-up period (from 0.46$\pm$0.13 to 0.46$\pm$0.11, 0.37$\pm$0.08*, and 0.34$\pm$0.07*, p<0.01). RVSTI decreased significantly at postop3(0.33$\pm$0.08 to 0.32$\pm$0.08, 0.31$\pm$0.07, and 0.27$\pm$0.05*, p<0.01). Conclusion: We found that right and left ventricular systolic time intervals had decreased over the period of 1 year after surgical correction of VSD. Therefore, it is necessary to observe the change of ventricular function during that period.
Frequent pattern mining, which is one of the major areas actively studied in data mining, is a method for extracting useful pattern information hidden from large data sets or databases. Moreover, frequent pattern mining approaches have been actively employed in a variety of application fields because the results obtained from them can allow us to analyze various, important characteristics within databases more easily and automatically. However, traditional frequent pattern mining methods, which simply extract all of the possible frequent patterns such that each of their support values is not smaller than a user-given minimum support threshold, have the following problems. First, traditional approaches have to generate a numerous number of patterns according to the features of a given database and the degree of threshold settings, and the number can also increase in geometrical progression. In addition, such works also cause waste of runtime and memory resources. Furthermore, the pattern results excessively generated from the methods also lead to troubles of pattern analysis for the mining results. In order to solve such issues of previous traditional frequent pattern mining approaches, the concept of representative pattern mining and its various related works have been proposed. In contrast to the traditional ones that find all the possible frequent patterns from databases, representative pattern mining approaches selectively extract a smaller number of patterns that represent general frequent patterns. In this paper, we describe details and characteristics of pattern condensing techniques that consider the maximality or closure property of generated frequent patterns, and conduct comparison and analysis for the techniques. Given a frequent pattern, satisfying the maximality for the pattern signifies that all of the possible super sets of the pattern must have smaller support values than a user-specific minimum support threshold; meanwhile, satisfying the closure property for the pattern means that there is no superset of which the support is equal to that of the pattern with respect to all the possible super sets. By mining maximal frequent patterns or closed frequent ones, we can achieve effective pattern compression and also perform mining operations with much smaller time and space resources. In addition, compressed patterns can be converted into the original frequent pattern forms again if necessary; especially, the closed frequent pattern notation has the ability to convert representative patterns into the original ones again without any information loss. That is, we can obtain a complete set of original frequent patterns from closed frequent ones. Although the maximal frequent pattern notation does not guarantee a complete recovery rate in the process of pattern conversion, it has an advantage that can extract a smaller number of representative patterns more quickly compared to the closed frequent pattern notation. In this paper, we show the performance results and characteristics of the aforementioned techniques in terms of pattern generation, runtime, and memory usage by conducting performance evaluation with respect to various real data sets collected from the real world. For more exact comparison, we also employ the algorithms implementing these techniques on the same platform and Implementation level.
Purpose: The latissimus dorsi muscle flap is a versatile flap used in a variety of reconstructive procedures. The most common complication of LD muscle flap is donor site seroma, reported to occur in 20 to 79 percent of cases. The formation of dead space under the flap is intimately associated with seroma formation. The authors think that the use of progressive tension suture at closing donor site can decrease the formation of dead space and ultimately reduce the incidence of donor site seroma. Methods: A retrospective review was performed with 38 patients who underwent latissimus dorsi muscle harvest for breast reconstruction from March 2003 to September 2004. Progressive tension sutures were used during donor site closure in 22 patients. This group was compared with controls group(16 patients) who underwent latissimus dorsi muscle harvest without using this technique. Operation time, length of hospital stay, period of drainage, complication, and satisfaction about postoperative scar of donor site were examined. Results: The average length of hospital stay was 10.2 days and 12.7 days, and the mean duration of drainage were 7.3 days and 11.7 days in each progressive suture group and control group. These results were statistically significant (p<0.05). In the 22 patients who underwent progressive tension suture, none had seroma, hematoma or skin necrosis. In control group(16 patients), there happened one seroma formation and one partial skin necrosis. These complications were healed by aspiration of seroma and wound revision. The patients' satisfaction was not statistically significant, but the higher points were given by the patients who underwent progressive tension suture. Conclusion: This technique, progressive tension suture, is an effective method to reduce or eliminate donor site seroma, which is the most common complication associated with latissimus dorsi muscle harvest.
