The Relationship between Clinical Grading of Carpal TunnelSyndrome and Sensory nerve Conduction Velocity (수근관 증후군의 임상증상정도와 감각신경전도검사와의 관계)

  • Background: Carpal tunnel syndrome (CTS) is a common condition characterized by entrapment neuropathy of the median nerves. Clinical manifestations are the most important findings for diagnosis and assessment of therapeutic effects. But, objective indicators, such as electrophysiological findings, are also valuable supplementary tools. This study investigated the relationship between clinical grading and sensory nerve conduction velocity (SNCV) of median proper palmar digital nerve (MPPDN) in CTS patients. Method: This study was done on 90 upper limbs of 53 patients with CTS (men: 6, women: 47, age: 26~69 years, mean age; 52 years). Each SNCV of MPPDN was recorded with bar electrode using antidromic method. Each SNCV was compared with clinical grading of CTS. The clinical grades of CTS were designated as follows; group 1 is mild symptoms, 2 is moderate symptoms, and 3 is severe and longstanding symptoms. Result: In thumb, the SNCV of MPPDN was not different significantly between 3 groups (p=0.817). In the index finger, the SNCV was the fastest in the group 1, but faster in group 3 than in group 2 (p=0.001). In the middle and ring fingers, SNCV was decreased in higher clinical grading groups (middle finger: p=0.015, ring finger: p=0.044). Conclusion: SNCV of MPPDN of middle and ring finger correlated with the clinical grading of CTS. SNCV of index finger was the fastest in group 1. But SNCV of thumb did not correlate with the clinical grading of CTS.

Correlation between Arterial Stiffness and Physiological Parameters (동맥경화도와 생리학적 변수들 간의 상관관계)

  • Arterial stiffness(AS) is an important pathologic state of vascular injury. This study was carried out to elucidate the effect of physiological variables on brachial-ankle pulse wave velocity(BAPWV), index of AS. Four hundred adults(volunteers) participated in this study. Body indices, biochemical, cardiac and inflammatory markers, and right(Rt)- and left(Lt)-BAPWV were measured. Body mass index(BMI), Rt- and Lt-BAPWV, glucose, triglyceride, alkaline phosphatase(ALP), gamma-glutamyl transferase(GGT), creatinine, uric acid, troponin-I(TNI), NT-proBNP and high sensitivity C-reactive protein(hs-CRP) levels were higher than the reference value of each variable. Rt- and Lt-BAPWV were directly correlated with age, body weight, BMI, glucose, ketone, aspartate aminotransferase, alanine aminotransferase, ALP, GGT, total cholesterol, low density lipoprotein, lipoprotein(a), apolipoprotein-B, blood urea nitrogen, heart rate, TNI, creatine kinase, CK-MB, lactic dehydrogenase, myoglobin, hs-CRP, lipase, reumatoid factor, fibrinogen and D-dimer (P<0.05, P<0.01, P<0.001 or P<0.000, respectively), but inversely associated with total bilirubin, uric acid, apolipoprotein-A1 and GFR (P<0.05). These observations suggest that a variety of physiological variables may influence BAPWV, resulting in increased risk or prevention of cardiovascular and/or cerebrovascular attacks. Therefore, physiological variables affecting BAPWV should be regularly controlled.

Study on Sasang Constitutional Characteristics using Sasang Personality Questionnaire and Sasang Digestive Function Inventory in Clinical Level (의원급 임상에서 SPQ와 SDFI를 이용한 사상체질별 특징 연구)

  • Sasang Digestive Function Inventory (SDFI) and Sasang Personality Questionnaire (SPQ) were developed on the basis of clinical experiences of Korean medicine hospital and previous studies have used these for analyzing Sasang constitutional characteristics mainly in hospital level. The purpose of this study is to analyze Sasang constitutional characteristics using SDFI and SPQ in clinical level. The SPQ, SDFI, height and weight were measured with 79 patients over 18-years-old from Jan. 2015 to Apr. 2016 in the clinic, Daegu, Korea that specializes in tinnitus and performs Sasang constitution diagnosis. We used ANOVA in analyzing the differences of the scores of SPQ, SDFI, BMI and PI in each Sasang types (Tae-Yang, So-Yang, Tae-Eum), and Bonferroni test or Dunnett's T3 for post verification. The SDFI-T(F=3.215) and SDFI-D(F=4.347) scores were significantly different(p<0.05) and SPQ score was not different in each Sasang types(p>0.05). And Weight(F=4.294), BMI(F=12.386) and PI(F=14.490) were significantly different in each Sasang types(p<0.001). We found that SDFI(SDFI-D) and BMI PI can explain the Sasang constitutional characteristics, that is equivalent to previous studies and might be used for clinical index in making objective questionnaire for Sasnag constitutional diagnosis in clinical level.

The Effect of Ozonized Water Irrigation in the Circuits of Professional Ultrasonic Scalers for Causal Therapy of Stage I Periodontitis: A Randomized Clinical Study

  • Background: Periodontitis is a chronic inflammatory condition associated with dysbiosis of the oral microbiota. The aim of the present clinical study was to explore the adjunctive effect of ozonized water irrigation in the circuits of ultrasonic scalers for the full-mouth decontamination of patients with periodontitis Stage I or II. Methods: The study was a randomized, single-blinded, parallel-group clinical trial. The test group (n=25) was treated with ultrasonic scalers irrigated with ozonized water, whereas the control group (n=25) received normal tap water irrigation within the ultrasonic scalers used during the professional mechanical debridement. Full mouth plaque score, bleeding score, probing pocket depth, and the gingival index were evaluated at baseline, two, and 4 weeks after treatment. The pain perceived and dental anxiety were also assessed after treatment by means of the visual analog scale (VAS). Results: All periodontal parameters resulted in significant improvement for both study groups. The effect of the treatment group on the gingival index was significant, in particular, patients in the test group experienced a greater reduction in this score. No significant differences could be observed with regards to the average probing depth, full mouth plaque index and bleeding score. Patients treated with ozonized water running in the circuits of ultrasonic scalers displayed also lower scores for pain and dental anxiety. Conclusion: The present study showed a significant clinical effect on gingival inflammation attributable to adjunctive ozone irrigation during nonsurgical periodontal therapy. Further studies, including patients with severe periodontitis and greater sample sizes, are recommended to test the clinical effect of ozonized water in the circuits of ultrasonic scalers.

Acute toxicity, Dermal and Ocular Irritation Studies of Taglisodog-eum ointment (탁리소독음 피부외용제형의 급성경피독성시험, 피부자극시험 및 안점막자극시험)

  • Objectives : Taglisodog-eum(TSE), a poly herbal formula, has been widely used to improve carbuncles by removing inflammation of the lymphatic channels in Traditional Korean Medicine. We previousely reported the action mechanism of TSE on experimental atopic dermatitis and the establishment of formulation for TSE ointment. In this study, we examined the toxicity test on skin and eye irritation by TSE ointment to prove the safety of Taglisodog-eum ointment in clinical use. Methods : Acute skin toxicity of the TSE ointment was evaluated in Sprague-Dawley(SD) rats. After dermal administration of TSE ointment(2,000mg/kg), body weight, mortality, and clinical signs of the rats were observed for 14days. Primary skin irritation and ocular irritation tests for TSE ointment were performed in male New Zealand White Rabbits. In dermal and ocular irritation test, body weight, mortality, clinical signs, Primary Irritation Index(P.I.I.), and The Index of Acute Ocular Irritaion(I.A.O.I.) of rabbit were observed after applying at abraded skin and eye balls with Taglisodog-eum ointment. Results : In the results of acute skin toxicity, no significant differences were found in body weight, the clinical sign and the mortality between control and TSE ointment treated group. In primary dermal irritation test, body weight, the clinical sign and the mortality were not significantly changed and Primary Irritation Index(P.I.I.) was 0.8, indicating TSE ointment as weak irritant material. In ocular irritation test, The Index of Acute Ocular Irritaion was 0.0, indicating TSE ointment as non-irritating to the eye of the rabbits. To evaluate toxicity of the TSE ointment in animal test, body weight, the clinical signs, the skin and eye irritation check were conducted; TSE ointment was considered to be weak dermal irritant in test animals. The no response of eye irritation test was observed in this experimental condition. Conclusions : According to the above toxicity test, We consider that this results is helpful for saying about the safety of TSE ointment in clinical use.

Machine Learning-Based Prediction of COVID-19 Severity and Progression to Critical Illness Using CT Imaging and Clinical Data

  • Objective: To develop a machine learning (ML) pipeline based on radiomics to predict Coronavirus Disease 2019 (COVID-19) severity and the future deterioration to critical illness using CT and clinical variables. Materials and Methods: Clinical data were collected from 981 patients from a multi-institutional international cohort with real-time polymerase chain reaction-confirmed COVID-19. Radiomics features were extracted from chest CT of the patients. The data of the cohort were randomly divided into training, validation, and test sets using a 7:1:2 ratio. A ML pipeline consisting of a model to predict severity and time-to-event model to predict progression to critical illness were trained on radiomics features and clinical variables. The receiver operating characteristic area under the curve (ROC-AUC), concordance index (C-index), and time-dependent ROC-AUC were calculated to determine model performance, which was compared with consensus CT severity scores obtained by visual interpretation by radiologists. Results: Among 981 patients with confirmed COVID-19, 274 patients developed critical illness. Radiomics features and clinical variables resulted in the best performance for the prediction of disease severity with a highest test ROC-AUC of 0.76 compared with 0.70 (0.76 vs. 0.70, p = 0.023) for visual CT severity score and clinical variables. The progression prediction model achieved a test C-index of 0.868 when it was based on the combination of CT radiomics and clinical variables compared with 0.767 when based on CT radiomics features alone (p < 0.001), 0.847 when based on clinical variables alone (p = 0.110), and 0.860 when based on the combination of visual CT severity scores and clinical variables (p = 0.549). Furthermore, the model based on the combination of CT radiomics and clinical variables achieved time-dependent ROC-AUCs of 0.897, 0.933, and 0.927 for the prediction of progression risks at 3, 5 and 7 days, respectively. Conclusion: CT radiomics features combined with clinical variables were predictive of COVID-19 severity and progression to critical illness with fairly high accuracy.

The Clinical Report on the Case of Cauda Equina Syndrome Using Multiple Korean Medicine Therapies (복합적 한방 치료를 시행한 마미증후군 수술 실패 환자 1례에 대한 증례 보고)

  • Objectives : The purpose of this study is to evaluate clinical effect of Korean medical treatments for the patient who were diagnosed as the Cauda Equina Syndrome. Methods : The patient was treated by Korean medical treatments including acupuncture, pharmacopuncture, herbal medication. The Improvement of clinical symptoms was assessed by Visual Analog Scale(VAS), Oswestry Disability Index(ODI) and Muscle Manual Test(MMT). Results : After treatments, VAS and ODI were generally decreased, MMT was improved in case. Conclusions : The Oriental medical treatments might be effective methods to treat the patient who were diagnosed as the Cauda Equina Syndrome.

A STUDY ON PERIODONTAL DISEASE SEVERITY AND MYELOPEROXIDASE IN GINGIVAL CREVICULAR FLUID (치주질환 심도와 치은열구액내 Myeloperoxidase에 관한 연구)

  • This investigation was undertaken to determine the relationship between the amount of polymorphonuclear leukocyte(PMN) enzyme myeloperoxidase(MPO) in gingival crevicular fluid(GCF) collected from active or control site and gingival disease status described by clinical indices(gingival index, papillary bleeding index, pocket depth, periotron unit). The results were as follows : 1. MPO activity/site was greater at active sites than at control sites. 2. According to increasing the clinical parameters, MPO/sites was higher statistically (P<0. 01, P<0.05). 3. High MPO(unit/site) groups was higher statistically than low MPO(unit/site) groups in various clinical parameters. 4. Correlation coefficients between MPO(unit/site) and GI, MPO($unit/{\mu}l$ GCF) and periotron unit were 0.4782, -0.5901, respectively.

Correlation of Hemoglobin and RBC count with Adiponectin Level

  • Adiponectin is secreted specifically by adipose tissue and regulates lipid and glucose metabolism. In addition, adiponectin has been found to inhibit inflammatory process and possibly atherogenesis. This study was done to compare hematological parameters, serum lipids with serum adiponectin level according to body mass index (BMI) and gender in eighty eight Korean adults aged 40 to 68. In this study, the association between serum adiponectin and other variables including RBC counts, hemoglobin, hematocrit and albumin were investigated. RBC, hemoglobin status, was inversely associated with serum adiponectin levels in normal adult women. Adiponectin levels were significantly lower in men than women (p<0.01). In men, serum adiponectin levels were negatively correlated with body mass index (BMI) (p<0.05). However, no correlations were found in women. These results might imply that the regulation of key adipokines such as adiponectin might be a strategy for the prevention or treatment of obesity-associated diseases.

A clinical survey of distally extending cantilever bridges (후방연장 계속가공의치의 임상적 조사)

  • This study was to compare the patients wearing comfortable distally extending cantilever bridges (DECBs) with those having discomfortable ones, in the aspect of the periodontal condition & prosthodontic status. The subjects in the present study were 164 patients appointed to Seoul National University Dental Hospital. One group was consisted of 101 patients wearing comfortable DECBs and the other group were made of 64 patients who had felt discomfort. On clinical parameters, there were no significant difference between 2 groups in plaque index, tooth mobility & probing depth, but gingival index was higher in group wearing discomfortable DECBs. In prosthodontic status, the ratio of pontic oversize was higher in the group of discomfortable DECBs, but there were no significant difference in the view of crown overcontour, overhanging margin & interproximal space closure between 2groups. This study failed to clarify causal factors of discomfortable DECBs.
