Experience of Job Satisfaction in Clinical Nurses : Application of Focus Group Methodology (임상간호사의 직무만족 경험: 초점집단 연구방법 적용)

  • Purpose: The purpose of this study was to describe the experiences of job satisfaction in clinical nurses. Methods: Data was collected from three focus groups composed of 17 hospital nurses. Each focus group had an interview for an average of two and a half hours with the guidance of researchers. The main question was 'how do you describe your lived experience of job satisfaction as a clinical nurse?' Qualitative data from the field notes and transcribed notes were analyzed using a grounded theory methodology developed by Strauss and Corbin. Results: The core category of experience of job satisfaction in clinical nurses was identified as 'Finding success.' Supportive interpersonal relationships and environment affected this category. In the process of attaining job satisfaction through finding success, the participants were using four interactional strategies such as giving meaning, finding self-esteem, extending the horizon of life, and strengthening self-capability. The dimensions of job satisfaction in clinical nurses were the sense of achievement, stability, and pride. Conclusion: The results of this study yields very useful information for nursing mangers to design a program which enhances job satisfaction of clinical nurses based on interactional strategies.

  • Early diagnosis, accurate assessment, and localization of peritoneal metastasis (PM) are essential for the selection of appropriate treatments and surgical guidance. However, available imaging modalities (computed tomography [CT], conventional magnetic resonance imaging [MRI], and 18fluorodeoxyglucose positron emission tomography [PET]/CT) have limitations. The advent of new imaging techniques and novel molecular imaging agents have revealed molecular processes in the tumor microenvironment as an application for the early diagnosis and assessment of PM as well as real-time guided surgical resection, which has changed clinical management. In contrast to clinical imaging, which is purely qualitative and subjective for interpreting macroscopic structures, radiomics and artificial intelligence (AI) capitalize on high-dimensional numerical data from images that may reflect tumor pathophysiology. A predictive model can be used to predict the occurrence, recurrence, and prognosis of PM, thereby avoiding unnecessary exploratory surgeries. This review summarizes the role and status of different imaging techniques, especially new imaging strategies such as spectral photon-counting CT, fibroblast activation protein inhibitor (FAPI) PET/CT, near-infrared fluorescence imaging, and PET/MRI, for early diagnosis, assessment of surgical indications, and recurrence monitoring in patients with PM. The clinical applications, limitations, and solutions for fluorescence imaging, radiomics, and AI are also discussed.

Experiences of Nursing Students in Caring for Pediatric Cancer Patients

  • Background: This study was performed to determine the experiences of nursing students in caring for paediatric cancer patients and their families. Materials and Methods: This qualitative survey was carried out with 14 students studying in the nursing department of the Faculty of Health Sciences of a university in Edirne, Turkey. Data were obtained through focus group interviews and evaluated based on a qualitative content analysis. Results: It was determined that students, for the most part, experienced problems related to communication, sadness, helplessness, fear, anxiety, resentment and anger. In addition, the students were affected most often by effects of the disease and invasive procedures on paediatric cancer patients and their families during the process of caring for them in the oncology clinic. Conclusions: It would be useful to inform nursing students, prior to clinical practice, about the special needs of paediatric cancer patients and families who stay in oncology clinics, and to follow up with appropriate guidance during the clinical practices.

A Survey of students' satisfaction on Oriental Dermatology Clerkship in oriental medical clinic (일반 한의원을 통한 한방 피부외과 임상실습 만족도 조사)

  • Objectives : The purpose of this study is to evaluate of Oriental Dermatology Clerkship in oriental medical clinic by using students' satisfaction questionnaire. Methods : In 2011, the evaluation of students' satisfaction was conducted by the Department of oriental dermatology, Dong-guk University College of Oriental Medicine. The questionnaire filled-up by 89 six-year students who completed the clerkship schedule of first semester. The questionnaire were consisted of 17 items. Results : Most of the students gave a positive evaluation to the overall clerkship. Especially the satisfaction level of professional guidance and practice environment was so high. But The problem was the lack of opportunity for direct participation. The most useful educational activities was performing clinical skills under faculty supervision. The most constraining activities was difficulty interacting with patients as a student. Conclusions : It is necessary to develop continuous program that provide for better quality clinical clerkship with oriental medical clinic.

Effects of Clinical Practicum Reinforcement Program on Nursing Student's Clinical Competency and Satisfaction in the ICU (중환자실의 현장실무 강화교육 프로그램이 간호학생의 임상수행능력과 임상실습교육 만족도에 미치는 효과)

  • Purpose: To identify the effects of clinical practicum reinforcement program on nursing student's clinical competency and satisfaction in the ICU. Methods: The study utilized a quasi-experimental pretest-posttest research design. The participant was 76 senior nursing students, who were engaged in two week-long clinical practicum in ICU at the C university hospital in a G city, Korea. The clinical practicum reinforcement program was applied to the experimental group (n=39), while the control group (n=37) was involved in the conventional practicum program. The collected data were analyzed using descriptive analysis, $x^2$/Fisher's exact test, and t-test, ANCOVA with SPSS/WIN 18.0 program. Results: After 2 weeks' clinical practicum, the experimental group showed a statistically significant increase in the clinical performance abilities by self evaluation, when compared to the control group. Clinical instructor's evaluation on the student's clinical performance revealed that the experimental group showed higher level in nursing activity than that of the control group (p<.001). However, there was no significant difference between the two groups in terms of the satisfaction on the clinical practicum program except the learning guidance and the evaluation. Conclusion: The clinical practicum reinforcement program is an effective one for improving the student's clinical competency.

Phenotypic and Genotypic Detection of Metallo-β-Lactamase Producing Pseudomonas aeruginosa

  • This study was undertaken to evaluate phenotypic and genotypic methods for detection of Metallo-Beta-Lactamases (MBLs) among nosocomial Pseudomonas aeruginosa. Of the 50 P. aeruginosa isolates from clinical specimens, 20 were evaluated for carbapenem resistance and screened for MBL by double-disk synergy test and combined-disk test. Nineteen strains (95%) were found to be MBL producers among the 20 P. aeruginosa. MBL positives were further confirmed by Polymerase Chain Reaction (PCR). For the IMP and VIM types of MBLs, PCR analysis was performed on 19 of the 20, and 10 were positive for VIM MBL type. This study reports the validation of a simple and accurate MBL detection method that can be easily incorporated into the daily routine of a clinical laboratory. Early detection of MBL-carrying organisms, including those with susceptibility to carbapenems, is of paramount clinical importance, as it allows rapid initiation of strict infection control practices as well as therapeutic guidance for confirmed infection.Key Words : Hepatitis A virus (HAV), Anti-HAV, Hospital workers, Prevalence, Vaccination

Clinical Characteristics of Patients after Aryloxyphenoxy Propionate Herbicide Ingestion (Aryloxyphenoxy propionate 계열 제초제 중독환자의 임상 양상)

  • Purpose: No studies have been conducted to investigate the acute toxicity of aryloxyphenoxypropionate herbicides in humans following ingestion. Therefore, this study was conducted to investigate the clinical characteristics of aryloxyphenoxypropionate herbicide poisoning and provide guidance for physicians treating patients who have ingested these types of herbicides. Methods: A retrospective observational case series was conducted using ten patients with history of aryloxyphenoxy propionate herbicide. Data were collected for clinical manifestation, management and final outcome. Results: The most common symptoms were gastrointestinal irritation and an altered mental state (Glasgow Coma Scale<15). An elevated lactate level was a common laboratory abnormality, and prolonged QTc interval was commonly observed. These clinical features normalized within one day of supportive treatment. Conclusion: The acute toxicity of aryloxyphenoxypropionate herbicides in humans is manageable with supportive treatment. However, physicians should take into account depressed consciousness, the possibility of arrhythmia, and an elevated lactate level when planning their treatment strategy.

Incivility experienced by nursing students in relations with nurses during clinical practice (현장실습 중 간호사와의 관계에서 간호대학생이 경험한 무례함)

  • Purpose: The purpose of this study was to describe the meaning of incivility experienced by nursing students in relations with nurses during clinical practice. Methods: This is a descriptive qualitative study that included thirty participants using convenience sampling in universities in S, J, Ch, and C provinces. Data saturation was achieved and methodological rigor was established. Qualitative content analysis was used to inductively determine categories. Results: The four categories derived from analysis were 'left alone at clinical field', 'treated as an assistant', 'not respected as a person' and 'ignored future dreams'. Conclusions: Nursing students continue to carry out repetitive, low-risk tasks, for which they are not properly trained due to the lack of practical guidance in clinical practice. During clinical practice, they are not respected as persons by nurses and are negatively influenced by nurse professionals. A solution to this requires organic cooperation and policy preparation at universities and hospital sites to improve practical training of nursing students.

Developing Activity Based Clinical Pathway for Patients with Total Hip Replacement (활동기준관리를 적용한 인공고관절 전치환술 Clinical Pathway 개발)

  • Purpose : The purpose of the present study is to develop a clinical pathway applied with activity-based management methodology for efficient clinical management to cope with rapid changing medical environments. Method : After making a preliminary pathway based on which a conceptual frame of reference was established to develop a clinical pathway, the final one was confirmed by verifying experts validity and clinical validity. The ultimate activity-based clinical pathway was restructured after clarifying, schematizing and analyzing the whole activities of clinical pathway in accordance with the conducting process of activity-based management. Result : A clinical pathway for total hip replacement was developed, in which the vertical axis consisted of assessment, examination, consultation, medication, treatment, diet, activity, and education, and the horizontal axis was composed of six days of hospitalization. Then, on the basis of the development, the clinical pathway including the improved contents and information was restructured after making the reform measure by analyzing each of activities in the pathway. And the list of contents related to the clinical management activity was made, which was described its main contents and the pre-activities that ought to be completed before conducting each of activities in the pathway. Conclusion : The clinical pathway applied with activity-based management may be used as a standard guidance for providing continuous and consistent patient care. It will provide the information for nursing activities to nurses and the management information about hospital and nursing activities to the hospital administrators. It will also be used as a tool for communication between medical staff. Besides, it will contribute to creating profits for the hospital by shortening the length of stay in patients.

The effect of ego-resilience, family support and teaching effectiveness on clinical practice satisfaction of dental hygiene students (치위생과 학생의 자아탄력성, 가족지지, 교수효율성이 임상실습 만족도에 영향을 미치는 요인)

  • Objectives: The aim of this study was to examine factors influencing satisfaction with clinical practice in dental hygiene students. Through this study, we suggested efficient guidance to increase satisfaction levels with clinical practice. Methods: A self-reported questionnaire was filled out by 235 students of clinical dental hygiene in Seoul Gyeonggi Chungcheong Kyongsang and Jeolla province from June 9 to 30, 2017. The questionnaire consisted of questions on general characteristics (6 items), clinical practice characteristics (7 items), ego-resilience (14 items), family support (24 items), teaching effectiveness(35 items), and clinical practice satisfaction (30 items). Data were analyzed using SPSS 19.0. One way ANOVA, the Scheffe Post-hoc test, and the Pearson correlation coefficients were reviewed, and a multiple regression analysis was conducted. Cronbach's alpha of ego-resilience, family support, teaching effectiveness, clinical practice satisfaction were 0.784, 0.892, 0.954 and 0.935, respectively. Results: ego-resilience was 3.24 points, family support was 3.24 points, teaching effectiveness was 2.93 points, clinical practice satisfaction 3.44 points. The meaningful variables which influenced clinical practice satisfaction were the ego-resilience, family support and teaching effectiveness. These factors explained 40.6% of the variance in clinical practice satisfaction. Conclusions: One of the most significant predictors of clinical practice satisfaction in dental hygiene students was teaching effectiveness. Therefore, a teaching program to improve eaching effectiveness should be developed and applied.
