Accuracy of periodontal probe visibility in the assessment of gingival thickness

  • Purpose: The present study was undertaken to examine whether periodontal probe visibility (PV) accurately reflects gingival thickness (GT) and to identify factors affecting PV using cluster and multivariate analyses. Methods: The clinical characteristics of the maxillary central incisors (n=90 subjects) were examined. Clinical photographs, sex, PV, probing depth, gingival width, papilla height, GT as measured with an ultrasonic device, and the ratio of crown width to crown length were recorded. Multivariate analysis, using multinomial baseline-category logistic regression, was used to identify factors predictive of PV. Cluster analysis was used to identify gingival biotypes. Results: In the multivariate analysis, sex was the only significant predictor of PV (odds ratio, 6.48). Two clusters of subjects were created based on morphometric parameters. The mean GT among cluster A subjects was significantly lower than that among cluster B subjects (P=0.015). No significant difference was found between cluster A and B subjects in terms of PV score (P=0.583). Conclusions: Periodontal PV was not associated with GT as measured directly using an ultrasonic device. Sex was a highly significant predictor of periodontal PV. GT was found to be correlated with morphological characteristics of the periodontium.

Early fragment removal on in vitro fertilization day 2 significantly improves the subsequent development and clinical outcomes of fragmented human embryos

  • Objective: To determine whether fragment removal on in vitro fertilization (IVF) day 2 improved the subsequent development and pregnancy outcomes of fragmented embryos compared to similar-grade embryos without fragment removal. Methods: This study was a retrospective analysis involving 191 IVF cycles in which all embryos had over 10% fragmentation (grade 3 or 4) on day 2 of the IVF-embryo transfer cycle from March 2015 to December 2017. IVF cycles were divided into the fragment removal group (n = 87) and the no fragment removal group (n = 104) as a control cohort. Before fragment removal, embryos with fragmentation on day 2 were incubated in $Ca^{2+}$- and $Mg^{2+}$-free biopsy medium under paraffin oil for 30 minutes. Microsurgical fragment removal was performed with later-assisted hatching and a handmade suction micropipette that had an outer diameter of $30{\mu}m$. Results: There were no significant differences in the characteristics of the patients between the control and the fragment removal groups. After fragment removal and subsequent in vitro culture for 24 hours, the number of blastomeres ($7.1{\pm}1.7$ vs. $6.9{\pm}1.6$) was comparable between the transferred embryos in the two groups, but the morphological grade of the embryos in the fragment removal group ($1.9{\pm}0.7$) was significantly higher than that of the control group ($3.1{\pm}0.5$, p< 0.01). The clinical pregnancy (43.7%) and implantation rates (25.8%) in the fragment removal group were significantly higher than those in the control group (28.8% and 14.0%, respectively; p< 0.05). Conclusion: Early fragment removal on day 2 significantly improved the subsequent development and pregnancy outcomes of fragmented embryos.

  • Pluripotent stem cells have been generated from two embryonic sources. ES cells are generated from ICM of blastocyst stage embryos, and embryonic germ (EG) cells are generated from primordial germ cells (PGCs). Both ES and EG cells are pluripotent and present important characteristics such as high levels of alkaline phosphatase (AP) activity, multi-cellular colony formation, normal and stable karyotypes, continuously passaging ability, and the capability of differentiation into all three embryonic germ layers. Images of Benign Vocal Fold Lesions Using Videokymogram

  • Examination of the vibrations of the vocal fold is very important in patients with voice changes. The newly developed videokymography (VKG) takes images in real time and records irregular vibrations of the vocal fold. However, there are few data on VKG findings. We studied VKG to evaluate the vibratory characteristics of the vocal fold in benign vocal fold lesions. Unique vibratory patterns, blurred demarcation of the mucosal propagation, decreased margin amplitudes, asymmetry in phase or amplitude, and a level difference of the contact site were observed in each benign vocal-fold lesion. We conclude that each type of laryngeal lesion has specific characteristic which are revealed by VKG examination, these parameters can be quantified and used to objectively evaluate VKG findings. Based on these results, VKG can be used as a supplementary diagnostic tool.

  • Our retrospective study reports the clinical findings and population characteristics of 81 surgically treated dogs for spinal cord compression. We compared the outcome of dogs with different grades of neurological dysfunction due to spinal cord compression and focused on the long-term outcome of surgical treatment, for which there are only a few recent records. We recorded a 13.6% recurrence, regardless of the degree of dysfunction. However, the degree of dysfunction negatively affected the recovery length.

  • Background: Malignant melanoma is a cancer that demonstrates rapid progression and atypical clinically features with a poor prognosis. Aim: This study was performed to determine the clinical characteristics and treatment outcomes of patients with malignant melanoma in Turkey. Methods: The medical records of 98 patients between 2007-2012 at our centers were retrieved from the patient registry. Overall survival (OS) was calculated using the Kaplan-Meier method. Results: In our study, with the median follow-up of all patients with cutaneous MM of 46.3 months, the median OS rate of all cases was 43.6 months and 5-year OS was 48.6%. However, five-year OS rates of patients with localized disease (stage I-II) and node involvement (stage III) were 60.3% and 39.6%, respectively. The median OS of stage IV patients was 8.7 months and 1-year OS rate was 26.2%. We showed that advanced stage, male gender, and advanced age in all patients with MM were significant prognostic factors of OS. Conclusions: Compared with the results of current studies from Western countries, we found similar findings concerning demographical features, histological variables and survival analyses for our patients with cutaneous MM in Turkey.

Rotator cuff retear after repair surgery: comparison between experienced and inexperienced surgeons

  • Background: We hypothesized in this study that the characteristics of retear cases vary according to surgeon volume and that surgical outcomes differ between primary and revision arthroscopic rotator cuff repair (revisional ARCR). Methods: Surgeons performing more than 12 rotator cuff repairs (RCRs) per year were defined as high-volume surgeons, and those performing fewer than 12 RCRs were considered low-volume surgeons. Of the 47 patients who underwent revisional ARCR at our clinic enrolled in this study, 21 cases were treated by high-volume surgeons and 26 cases by low-volume surgeons. In all cases, the interval between primary surgery and revisional ARCR, degree of "acromial scuffing," number of anchors, RCR technique, retear pattern, fatty infiltration, retear size, operating time, and clinical outcome were recorded. Results: During primary surgery, significantly more lateral anchors (p=0.004) were used, and the rate of use of the double-row repair technique was significantly higher (p<0.001) in the high- versus low-volume surgeon group. Moreover, the "cut-through pattern" was observed significantly more frequently among the cases treated by high- versus low-volume surgeons (p=0.008). The clinical outcomes after revisional ARCR were not different between the two groups. Conclusions: Double-row repair during primary surgery and the cut-through pattern during revisional ARCR were more frequent in the high- versus low-volume surgeon groups. However, no differences in retear site or size, fatty infiltration grade, or outcomes were observed between the groups.

Comparison of Lactation Problems, Knowledge, and Adaptation on Breastfeeding between Users and Non-Users of Lactation Clinic (모유수유클리닉 이용군과 비이용군의 모유수유 문제, 지식 및 적응 비교)

  • Purpose: The purpose of this study was to compare the differences in characteristics, lactation problems, knowledge, and adaptation of breastfeeding between users and non-users of a lactation clinic. Methods: The descriptive study design was utilized using self-report questionnaires. 446 breastfeeding mothers participated in this study. Data were collected from lactation clinics, postpartum care centers, mother's culture centers, public health centers, pediatrics and obstetrics & gynaecology outpatient clinics in Seoul, Gyeonggi province, and Chungcheongnam province from August 20 to September 30, 2011. The questionnaires to measure lactation problems, knowledge and adaptation of breastfeeding were used. Data were analyzed using SPSS ver. 18.0 for Windows. Results: There was statistically significant differences in the breast feeding problems(t=5.71, p<.001) and breastfeeding knowledge(t=2.87, p=.004) between users and non-users of a lactation clinic. Conclusion: The results of this study may provide a foundation to develop nursing intervention for mothers in breastfeeding, and an evidence to expand the role of nurses as breastfeeding specialists.

Prevalence of herbal therapy usage in patients with psoriasis in Turkey

  • Many patients with psoriasis look for treatment options other than conventional treatment to control their disease with less side effects. We evaluated the prevalence and characteristics of herbal therapy usage among patients with psoriasis in Turkey. A self-prepared questionnaire about herbal therapies was applied to the psoriatic patients attended our dermatology clinic between October 2013 and December 2013. A total of 100 patients (55 male, 45 female; 7 - 80 years of age) were included in this study. Fortynine percent of patients used at least one form of herbal therapies. Juniperus oxycedrus and Aloe vera were found to be the most frequently used plants. 36.7% of the patients get benefit from herbal therapy. Only 12.2% of patients had informed their clinicians during or after herbal therapy usage. 22.4% of the patient had continued conventional treatment during herbal therapies. Side effects related with herbal therapy were developed in 26.5% of the patients. The risk of side effects was found to be 5.23 times more in patients using phytotherapies systemically compared with ones using topically. Our results show that herbal therapy usage is common among patients with psoriasis in Turkey. Since herbal therapies have the potential of interacting with the medical treatment modalities and causing life threatening allergic reactions, clinicians should be aware of plants used in psoriasis and possible adverse reactions related with them and also should not forget asking about herbal therapy history.

Clinical Analysis of 292 Cases of Tic Disorder in Oriental Medicine Clinic (한의원에 내원한 틱장애 환자 292례 증례분석)

  • Objectives : In this study, patients with tic disorders who visited an Oriental medicine clinic were examined for their demographic characteristics, characteristics of symptoms, relation to Attention-deficit Hyperactivity Disorder(ADHD) and peculiarity according to various variables such as motor and vocal tics. Methods : After surveying 292 patients who visited an Oriental medicine clinic with tic symptoms as main complaints for 17 months, SAS 9.1, a statistical program was used for statistical analysis. Results : 1. The BMI of male tic patients was significantly higher than female ones and it was similar to or higher than the normal group. 2. Patients who are eldest children were 1.7 times higher than those who are not eldest ones. 3. The most usual case of motor tics was the eye blink and the most one of vocal tics was a dry cough. 4. There was no significant difference between male and female patients for all symptoms of motor and vocal tics, but male patients had significantly more obsessions related to tics than female ones. 5. There was no significant difference in the age of initial occurrence of Transient tic disorder(TTD), Chronic tic disoder(CTD) and Tourette's disorder(TD). 6. For the general disorder of a tic and Conners' ADHD rating scale, there was no significance in TTD, CTD and TD. 7. 66% out of the total subjects of 197 cases were found to score more than 65 points in more than 1 items among 8 items such as the time, hearing, wrong alarm, mean response time and standard deviation in the response time, etc. of the ADHD diagnosis system(ADS). 8. The eye blink among motor tics was shown mainly by patients under 10 years old and the frown, movement of the head, shrug and movement of the arms were shown mainly by 11-19 years old patients. Conclusions : For the number, frequency, seriousness and inconvenience in life of tics, TD showed a significantly higher result than TTD and CTD.
