Effect of thoracic kyphosis and lumbar lordosis by Korean Medical treatment involving The Jeongham Therapy: 8 cases (정함요법을 포함한 한의학적 치료방법이 흉추후만 요추 전만에 미치는 영향: 환자 8례)

  • Objectives : To introduce the Jeongham Therapy which means a mandibul-rectifying treatment. And to show meaningful changes of thoracic kypkosis and lumbar lordosis by the Jeongham Therapy. Methods : We reviewed 8 patients having abnormal thoracic kyphosis and lumbar lordosis who were treated at the JeongHam Korean Medical Clinic. We measured thoracic kyphosis and lumbar lordosis by 3D Body Analyzer 'IBS 2000'. Results : Between before and after the Jeongham Therapy, statically significant differences were found in the thoracic kyphosis and lumbar lordosis. And in these values of measurements, the thoracic kyphosis and lumbar lordosis diminish after the Jeongham Therapy. Conclusion : These results suggest that the Jeongham Therapy is a meaningful therapy to diminish the thoracic kyphosis and lumbar lordosis.

  • Hemifacial spasm is a distressing condition characterized clinically by paroxysmal and an involuntary movement in muscles innervated by the facial nerve on one side of the face. Blockade of the facial nerve can be performed percutaneously, without any serious complications. There are certain clinical problems associated with the conventional procedure, such as severe pain and technical difficulties to find facial nerve. This report describes a fluoroscope guided facial nerve block. This new technique reduced the difficulties in identifying the facial nerve and decreased the suffering associated with the conventional way of facial nerve block.

  • The pain clinic at the Chonbuk National University Hospital opened in 1983. We analyzed 3,212 patients who visited the clinic during the 10 years from 1983 to 1993. The patients visited total of 11,628 times. The most frequent age group was in the fifties. About an half of the patients complained of low back pain, and the epidural block was most frequently given as a treatment. We need more personnels involving in pain management and more equipments in diagnosing and treating the patients with chronic pain. Understanding and cooperation among departments under the basis of closer interdepartmental relationship would be essential for the further development of the pain clinc.

  • Continuous epidural infusion, a combination of local anesthetic and opioid, have been widely administered for treatment of chronic cancer pain. A serious complications of epidural block is paraplegia which can also be caused by : direct spinal cord injury, epidural hematoma, epidural abscess, ischemic change, neurotoxicity, preexisting disease. Continuous epidural block for pain control of patient with cervical cancer was performed at $T_{12}/L_1$ interspace. A 4 cm catheter was inserted cephalad into the epidural space. After four months, back pain and motor weariless of lower extremities progressively developed. Spine CT showed bony destruction and soft mass-like lesion at $T_9$ & $T_{12}$ spine. We propose paraplegia was caused by spinal cord compression which resulted from vertebral metastasis of cervical cancer.

  • Hemifacial spasm is a distressing condition characterized by involuntary, intermittent, unilateral twitching of, all or parts, of the muscles innervated by facial nerve. This ailment is most common among middle-aged women. Neither causative agent nor reliable treatment has been established because etiology of idiopathic hemifacial spasm has remained undefined to date. This report describes two cases of hemifacial spasm treatment by injection of Botulinum toxin. An injection of Botulinum toxin provided relief of hemi facial spasm. Botulinum toxin therapy is an effective and convenient treatment of hemifacialspasm.

A Study on Types of Family System, Family Stress, Family Resources and Copying Strategies: Clinic-Normal Family Comparision (임상-정상가족의 가족체계 유형 및 가족스트레스, 가족자원과 대응책략에 관한 연구)

  • The purpose of this study is to measure family stress, family resources, copying strategies and the types of family system. For this purpose, the data were collected from healthy families(231) and clinic families(103). The main results are as follows: 1. Partially family stress, family resources, copying strategies was different by the demographic chrateristics(age, yeares of marriage, job, education, family life cycle family income religion and types of family system). 2. The type of family system was meaning factor for the family stress study. 3. In the case of healthy family, family stress was negative related with family resources, but healthy families used copying strategies variously. 4. In the case of clinic family, family stress was related with family resources and copying strateges strongly. 5. Classifying the types of family system, clinic families were classified extreme family(20.5%), midrange family(39.7%), balanced family(39.7%) and healthy family were classified extreme(13.1%), midrange family(25.8%) balanced family(61.1%).

  • Objectives : The purpose of this study is to report the effectiveness of Gagamhwajung-hwan in five severe alopecia areata patients. Methods : This case study was conducted for five severe alopecia areata patients who visited Korean Medicine Clinic. Herbal medicine(Gagamhwajung-hwan) was prescribed to all five patients. Improvements of patients were evaluated by photographs and SALT(Severity of alopecia tool) scale. Results : According to photographs, hair loss of each patient was completely improved. SALT scale was also decreased in all five patients. Conclusions : This study shows the possibility of using Gagamhwajung-hwan to treat severe alopecia areata.

  • Objectives : Therapeutic effect of Balance Appliance of functional cerebrospinal therapy (FCST) for meridian and neurologic yinyang balance was observed in a refractory torticollis case. Methods : A post-traumatic severe torticollis case with 18 years of duration was managed by the Balance Appliance on temporomandibular joint (TMJ), combined with acupuncture and manual medicine. Results : Assessment was made by self assessment of subjective symptoms and clinical observacon. The patient reported over-90% remission and returned to ordinary daily life after 20months of therapy, which effect was reported to maintain for 9 months. Conclusions : An impressive effect was observed and further clinical and biological research on FCST is expected.

  • We experienced two case of persistent Horner's syndrome which might be side effect of stellate ganglin block(SGB). Case one, a 35 year old male patient with severe pain and hyperesthesia of the right thumb. We performed repeated SGB with 1% mepivacaine 5 ml on the right side daily. After 9 times of SGB, he had continued ptosis, photophobia for one month and miosis for 6 months. Second case, the patients was a 21 year old male. He visited at our pain clinic to be cured of both sudden deafness which had begun two week ago. We performed SGB on both sides alternatively twice a day. About a week later, hearing ability of the left ear was recovered. After then we performed SGB only the right side. 34 SGB was performed in two months after that he got persistent Horner's syndrome.
