A Case Report of Precocious Puberty in a Female Patient : Significant Improvement in Controlling the Sex Hormone Levels (성조숙증으로 진단된 여자 환아에서 호르몬 수치가 개선된 치험 1례)

  • Objectives The purpose of this study is to find out how taking oriental herbal medicine may affect the sex hormone levels in a patient who was diagnosed with prematurity. Methods We prescribed 120 cc of oriental herbal medicine twice daily for a month to a patient suspected of the precocious puberty due to 11 pg/ml of the estradiol level. Upon finishing the course of oriental herbal medicine, the patient was retested for the follicular stimulating hormone, luteinizng hormone and estradiol levels. Results After administration, all levels of luteininzing hormone, follicular stimulating hormone and estradiol were decreased. There was no diagnosable evidence for the idiopathic central precocious puberty in the gonadotropin releasing hormone stimulation. Conclusions Oriental herbal medicine is a good alternative treatment of choice for the precocious puberty. However, more in-depth studies are to be followed.

Outcomes of preimplantation genetic diagnosis using either zona drilling with acidified Tyrode's solution or partial zona dissection

  • Objective: To review the outcomes of preimplantation genetic diagnosis (PGD) using zona drilling with acid Tyrode's solution (chemical zona pellucida drilling, chemical ZD) and those of partial zona dissection (PZD). Methods: Clinical outcomes of seventy-one couples undergoing 85 PGD cycles from January 2005 to December 2010 were included. Blastocyst formation and the hatching rate, clinical pregnancy rate, ongoing pregnancy rate, implantation rate, and fetal gender ratio of the PZD and chemical ZD groups were compared. Results: Application of PZD resulted in a significantly higher rate of clinical pregnancy (40.7% vs. 15.4%, p=0.022), ongoing pregnancy (35.6% vs. 11.5%, p=0.023), and implantation (18.1% vs. 5.7%, p=0.007) compared with chemical ZD. Among non-transferred embryos, the rate of blastocyst formation on day 5 (49.1% vs. 39.5%, p=0.016) and hatching on day 6 (47.2% vs. 26.5%, p<0.001) were also significantly higher in the PZD group. Conclusion: The mechanical zona dissection (ZD) method showed better outcomes than chemical ZD in terms of the blastocyst development and pregnancy rate. In this study, the fact that chemical ZD was conducted in different period from mechanical method should be considered in interpreting the result.

Metabolic Syndrome Prevalence in Renal Cell Cancer Patients

  • Purpose: Renal cell carcinoma (RCC) is increasingly being recognized as a metabolic disease in recent studies. The aim of the present study was to identify the prevalence of metabolic syndrome (MetS) and its association with RCC among urologic patients. Materials and Methods: The study included a total of 355 participants (117 adult RCC patients and 238 age matched controls) divided into groups, with and without MetS diagnosed using the criteria of the American Heart Association/The National Heart Lung and Blood Institute. Groups were compared statistically and logistic regression analysis was performed to investigate the impact of MetS criteria on RCC risk. Results: Of the 117 RCC patients, 52 (44.4%) and of the 238 controls, 37 (15.5%) had MetS. A significant association (p<0.001) was found between the presence of MetS and RCC (OR: 4.35; 95% CI=2.62-7.21). As the number of MetS components accumulated from 3 to 5, RCC risk increased likewise from 4 to 6 times. Conclusions: MetS is more prevalent in RCC patients in Turkey compared to controls. Risk increases with the number of coexisting MetS components.

A Comparative Study on the Intake of Health Foods by Outpatients of Hospitals of Oriental Medicine and Oriental Medicine Clinic (한방의료기관별 내원 환자의 건강식품 사용 현황 조사)

  • Purpose: A survey was conducted by questionnaire to investigate outpatients' intake status of health food. Methods: The survey of visiting oriental OB&GY clinic in 4 hospitals of oriental medicine and 1 oriental medical clinic has found the status of health food intake. Results: 1. Total respondents were 339(Male: 54, Female: 264) and their average age was $41.4{\pm}11.8$. Outpatients of oriental medicine clinic were 113, outpatients of hospitals of oriental medicine were 226. 2. Among the respondents, 156(40%) persons were taking health food as of today and the proportion of health food intake by oriental medicine clinic's outpatients is higher than the proportion of outpatients of hospitals of oriental medicine. 3. There is no difference in educational background and income among the outpatients who visited the oriental medicine clinic or hospitals of oriental medicine. And the middle income group's health food intake ratio was the highest. 4. The survey showed that heath food intake ratio got higher as persons became old. 5. Among the health foods, the multi-vitamin was most frequently taken. And omega 3, Ginseng steamed red, Vitamin C, Glucosamine, Calcium m were also frequently taken. Conclusion: A study for health food intake status by clinic is indispensable to establish the standard for efficient intake of health food.

Choice motivation of dental clinics by patients (환자의 치과병·의원 선택동기)

  • Objectives: The purpose of the study was to investigate the choice motivation of dental clinics by patients. Methods: A self-reported questionnaire was completed by 577 patients visiting the dental clinics from October 6 to 25, 2014 in G city after explaining the purpose of the study and receiving the informed consent. The questionnaire consisted of sociodemographic characteristics and choice motivation of dental clinics using Likert 5 point scale. Results: 1. The traffic convenience was the highest in those who were the 30s (3.37) and unmarried person (3.30). Those having moderate oral health condition showed 3.23. The regular checkup group had 3.33. The physical environment was the highest in the 20s (3.37) and the unmarried persons (3.33). Those having master's degree showed 3.86. The reputation of the dental clinic image was the highest in those having master's degree (3.63). Those having moderate oral health condition had 3.28. The main reason for dental clinic visit was tooth extraction (3.79, p<0.05). 2. The top priority choice for the dental clinic was the access to the dental clinic and the next reason was reputation for the dentist. The other choice factors included trust for the dentist, recommendation, and night treatment. Conclusions: The important choice for the dental clinic included the access to the dental clinic and reputation for the dentist.

A Tie-Over Dressing Using a Silicone Tube to Graft Deep Wounds

  • Background The most common cause of skin graft failure is the collection of blood or serous fluid underneath the graft. In our study, we describe the use of silicone tube for tie-over dressing to secure the skin graft margins with the aim of decreasing loss of the skin graft, particularly in grafting of deep wounds. Methods Between March 2008 and July 2011, we used this technique in 17 patients with skin defects with depths ranging from 3.5 to 8 mm (mean, 5.5 mm). First, the skin graft was sutured with 3/0 silk suture material from its corners. Then, a silicone round drain tube was sutured with 3/0 absorbable polyglactin 910 over the margins of the graft. Finally, long silk threads were tied over the bolus dressing, and the tie-over dressing was completed in the usual fashion. Results The mean follow-up was 7 months (range, 2-10 months) in the outpatient clinic. Graft loss on the graft margins due to hematoma or seroma was not developed. The results of adhesion between the graft and wound bed peripherally was excellent. Conclusions In our study, we suggest that use of a silicone tube for additional pressure on the edges of skin grafts in case of reconstruction of deep skin defects.

A Study on Applying the Human Scale for Physical Therapy Room Plan in Mid-sized Medical Clinic (인체척도 적용에 의한 중소규모 의원의 물리치료실 계획에 관한 연구)

  • Efficient space plans are built upon relations of both human and physical compositions, and the standard of the interaction is basically the human scale. Especially in medical areas, the character of the users within human scale design environment is quite significant. Unlike large hospitals with great plans and scalability performed by experts, mid-sized clinics tend to have limited space, designed by non-specialists who highlight aesthetics over functionality in floor plans which leads to poor quality in medical service. For this reason, this study will include the application of human scale based therapy rooms within the clinic from a physical standpoint and will further analyze and pinpoint any room for improvement to further enable more efficient ergonomic space planning. In this study, theoretically will contemplate over human scale, mid-sized clinic, therapies, and therapists, while the case study will include status of the recent human scale based mid-sized clinic from a physical viewpoint, classification and analysis amongst human and physical compositions chosen by discretion from either a newly opened or a newly re-modeled clinic within the last 2 years, which has 15 beds or more, minimum 3 physical therapists, with more than 5 different therapy facilities in the metropolitan area in order to measure the human scale of therapy rooms in a mid-sized clinic. As a result, the area where improvement can practically be applied is the physical composition, which does not include human scale like human composition factors as therapists and patients, and hence, I suggest the human scale applied efficient medical space plan become the base of this study and look forward to subsequent improvement in the quality of medical services.

Study on Perception About Traditional Korean Medicine (TKM) Hospital Treatment among the General Population and Patients Visiting a TKM Hospital (일반인 및 내원환자의 한방병원 진료에 대한 인식도 연구)

  • Objectives : This study was carried out to measure perceptions about Traditional Korean Medicine hospitals and provide data for improvement of TKM hospital management. Methods : We surveyed the general population from 1st to 17th December, 2008, and patients visiting a TKM hospital from 1st June to 9th December, 2008. Results : Result as follows. 1. The response ratio for the generally preferred medical institution was in the order of western medical clinic, pharmacy, general hospital, TKM clinic, TKM hospital. 2. The response ratio for the preferred medical institution for herbal medicine treatment was in the order of TKM clinic, herbal medicine store, TKM hospital, pharmacy. The preferred medical institution for acupuncture treatment was in the order of TKM clinic, TKM hospital, acupuncture clinic, public health center. 3. The most common response to reason for receiving traditional Korean medical care was "traditional Korean medical care is more effective than western medical care." 4. The proportion of first treatment and second treatment of patients visiting at TKM hospital was about 1:1. 5. 72.3% of patients had been to another medical institution. 6. The response ratio for the first visited medical institution for the same disease was in the order of general hospital, western medical hospital, western medical clinic, TKM clinic. 7. The response ratio for the concern about TKM treatment was in the order of expensive charge for TKM treatment, uncertainty of TKM treatment effect. Conclusions : Through this research, we can understand the perception about TKM hospital treatment of the general population and patients visiting TKM hospitals.

Effect of Melissa officinalis L. leaf extract on lipid accumulation by modulating specific adipogenic gene transcription factors in 3T3-L1 adipocytes

  • The objective of this study was to investigate the effects of a hypodermic injectable solution comprised of an LPM LB meso solution containing Melissa officinalis L. leaf extract (LPM) on the lipogenesis in the 3T3-L1 cells line. The lipid accumulation measured by oil red o staining in the 3T3-L1 adipocytes treated with LPM, which was reduced in a dose dependent manner and showed 91.7 to 62.9% compared to control group. Its effectiveness with a 50% solution was significantly higher than the hydroxycitric acid (positive control) treatment without showing cell cytotoxicity. In a quantitative real-time PCR, it was demonstrated that the LPM treatment appeared to upregulate the mRNA expression of the adipogenesis-related genes, which included the peroxisome proliferator-activated receptor gamma (50% concentration) while down-regulating the CCAAT-enhancer binding protein alpha (50% concentration) and the sterol regulatory element-binding protein 1c (10, 25, and 50% concentrations). The results from the current study suggest that the LPM could be useful biomaterials that can inhibit obesity in the 3T3-L1 cells, which could possibly be by regulating the specific adipogenic gene transcription factors.

Knowledge, attitudes and practices toward antimicrobial usage: a cross-sectional study of layer and pig farm owners/managers in Chiang Mai, Lamphun, and Chonburi provinces, Thailand, May 2014 to February 2016

  • This study aimed at determining the current knowledge, attitude, and practices (KAP) of layer and pig farmers to antimicrobial usage and its consequences. Description of such KAP could provide insights useful for promoting the rational use of antimicrobials in livestock. From May 2014 to February 2016, a survey involving 251 respondents in Chiang Mai, Lamphun, and Chonburi provinces, Thailand, was conducted by using a validated questionnaire. More than half (51.0%) of the respondents incorrectly believed that antimicrobial drug efficacy could not be reduced by using sub-recommended dosages, 61.2% had misconceptions about non-therapeutic antimicrobial use, and 66.9% inaccurately felt that antimicrobials were also effective for virus and fungi. Over half (50.6-55.2%) did not see the need to follow instructions or advice of veterinarians. Moreover, only 10.4% regularly relied on responses to drugs sensitivity tests when evaluating the effectiveness of antimicrobials. Overall, assessment of KAP regarding antimicrobial usage indicated that the majority of respondents had low levels of knowledge of antimicrobials, neutral rather than positive attitudes, and employed poor practices in the use of antimicrobials. The results indicate improvements in KAP could be helpful in developing more effective interventions by farmers, reduce antibiotic usage, and slow the growth of antimicrobial resistance.
