개별적 상호작용을 통한 대학생 흡연자의 금연목표 달성률 및 금연특성 조사연구 (A Study through Individual Interaction on the Achievement Rate of Smoking Cessation Goal and Characteristics Related to Smoking Cessation in College Smokers)

  • Purpose: This study was to examine the achievement rate of smoking cessation, to identify obstacles to smoking cessation, and to find means to achieve the goal of smoking cessation in college smokers. Method: This study was conducted from April 26th to September 13th 2004 and used a one-shot design. The subjects selected by convenient sampling were 29 college smokers who smoked over one cigarette a day, had a positive level of urine cotinine, participated in smoking cessation education 3 times. Thereafter, individual interaction was processed between the researcher and the subject using an interaction instrument. Data were analyzed based on frequencies.,percentages and means using SPSS/Win 10.0. Results: The achievement rate of smoking cessation was 20.7% (6 students). The biggest obstacles smoking cessation were smoking stimuli (29 students) and lack of control (25 students). Among detailed obstacles, the biggest one was smoking at regular times, which was followed by withdrawal symptoms, smoking on drinking, and company with other smokers. The most effective means of smoking cessation mentioned by the subjects were in order of avoiding drinking situations, taking deep breaths, and exercising. Conclusion: The results of this study, using King's theory, showed that individual interaction is effective in achieving smoking cessation. Therefore, it is suggested to make further study and broaden smoking cessation education for college smokers.

Public Health Challenges of Electronic Cigarettes in South Korea

  • Electronic cigarettes (e-cigarrettes) were recently introduced and advertised as a smoking cession device in South Korea. As the social norm to quit smoking has gained hold in the country, the number of e-cigarette users is growing rapidly. This phenomenon should be urgently considered, because of the lack of research that has been conducted to examine the safety of e-cigarettes and its efficacy as a smoking cessation aid. This paper raises several public health concerns on e-cigarettes in South Korea. Uncertain regulations of the government on e-cigarettes are contributing to an increase of e-cigarette users and allowing the e-cigarette industry to circumvent existing regulations. The aggressive marketing activity of this industry is also a core factor that is responsible for the rapid increase of e-cigarette use, in particular among the youth. Following the enforcement of tobacco control, some cigarette smokers may be encouraged to purchase e-cigarettes in order to circumvent the regulations, even though the dual use of e-cigarette and cigarette may be more harmful. Until there is clear evidence of the e-cigarette’s safety, it is recommended that the industry’s marketing and promotional activities be banned and closely monitored, and public campaigns be initiated to educate the public regarding e-cigarettes.

담뱃갑 경고그림 및 메시지 효과 분석 - 흡연자, 비흡연자와 금연자를 중심으로 (Analysis for Effectiveness of Cigarette Warning Picture and Message - Focusing on Smoker, Non-smoker, Ex-smoker)

  • 본 연구는 흡연 여부(흡연집단, 비흡연집단, 금연집단)에 따른 담뱃갑 경고그림 및 메시지 효과를 분석하기 위한 서술적 조사이다. 연구대상자는 G도에 거주하는 성인 대상으로 총 219명이 참여하였다. 자료수집은 2017년 6월 4일부터 6월 14일까지 실시하였다. 연구진행 방법은 보건복지부의 경고그림위원회에서 선정한 2016년 담뱃갑 경고그림과 경고메세지 10종(심장질환, 뇌졸중, 폐암, 후두암, 구강암 등)을 활용하여 설문조사를 실시하였다. 연구결과는 담뱃갑 경고그림과 경고메시지의 노출 후 흡연여부에 따른 금연의도는 흡연자, 비흡연자와 금연자 그룹 간에 통계적으로 유의한 차이를 보였다(F=36.08, p<.001). 따라서 국민건강증진법에 따라 흡연자의 금연 효과를 향상시키기 위하여 TV 금연공익광고를 포함한 담뱃갑 경고그림 및 경고메시지를 주기적으로 교체할 것을 제언한다.

Association between Cigarette Smoking History and Mortality in 36,446 Health Examinees in Korea

  • Background: It is well known that smoking is a preventable factor for all-cause mortality; however, it is still questionable how many years after smoking cessation that people will have reduced risk for mortality, in particular in those with a high interest in their own health. We aimed to examine the association between time since quitting smoking and total mortality among past-smokers relative to current smokers. Materials and Methods: We enrolled 36,446 health examinees that voluntarily taken with diverse health check-up packages of high cost burden in 1995-2003 and followed them till death by 2004. The history of cigarette smoking consumption was collected using a self-administrative questionnaire at the first visit time. Mortality risk by smoking cessation years was analyzed using Cox's proportional hazard model. Results: Compared to non-smokers, male smokers over 15 pack-years had higher risk for total mortality (HR=1.60, 95%CI 1.23-2.14). The mortality risk in female smokers with same pack-years was more pronounced than that in male smokers (HR=2.83, 95%CI 1.17-7.04) despite a small number of cases. Compared to current smokers, a decrease of total mortality was observed among those who ceased smoking, and inverse dose-response was found with years after cessation: RR 0.98 (95%CI, 0.64-1.41) (<2 yrs), 0.60 (95%CI, 0.43-0.83) (3-9 yrs), and 0.58 (95%CI, 0.43-0.79) (${\geq}10$ yrs). Conclusions: A reduced risk of total mortality was observed after 3 years of smoking cessation. Our findings suggest that at least 3 years of smoking cessation may contribute to reduce premature mortality among Asian men.

흡연자의 폐활량에 관한 조사 (Studies on vital capacity in a smoker)

  • It is well known that there is an adverse effect of long-term cigarette smoking on pulmonary function. Therefor we attempted to consider the vital capacity for position changes in a smokers and non-smokers. The pulmonary functions on sitting and supine positions were measured in 28 young healthy students fer the change of vital capacity. Forced expiratory flow-volume curve were performed sitting position and supine position and smoking. The results were summarized as follows; 1) The spirometric values(VC, FVC, FEV$_{1}$) were progressively decreased from sitting position to 30minutes after supine position in a non-smoking group(p<.05). 2) The VC, FVC. FEV$_{1}$, FEF25 ${\sim}$ 75% were decreased from sitting position to 30minutes after supine position in a smoking group(p< .01). The PEF and FEF25% were decreased from supine position to after smoking(p< .05). 3) non-smoking group and smoking group not showed significant change(p> .05). But the spirometric values were more decreased nonsmokers than smokers.

건강한 중년 남성에서 커피 음용 습관이 혈중 총 콜레스테롤 값에 미치는 영향 (The Effect of Coffee Consumption on Serum Total Cholesterol Level in Healthy Middle-Aged Men)

  • In present study, the authors investigated the possible effect of coffee consumption on serum cholesterol level in 1017 men between the ages of 40 and 59 years, who were randomly selected from the members of Seoul Cohort Study. Serum total cholesterol data was collected with other serologic indices (e.g. systolic blood pressure, diastolic blood pressure, hight, weight, etc.) through the program of biennial health check-up offered by Korean Medical Insurance Corporation (KMIC). The amount of coffee consumption was assessed by a self-administered questionnaire through mailing. Other confounding factors, such as age, body mass index, cigarette smoking, alcohol consumption, physical activity, and other dietary intake pattern were also determined by the questionnaire. The differences in means of serum total cholesterol in compared to non consumers were $-0.4{\pm}3.56mg/dl$ for those drinking less than 1 cup a day, $-0.6{\pm}3.60mg/dl$ for those drinking 1 cup a day, and $7.1{\pm}3.41mg/dl$ for those drinking more than 2 cups a day. Since smoking interacted the relationship between coffee consumption and serum total choleaterol, we re-analyzed those relationship in smokers and non-smokers separately Other atherogenic behaviors were well correlated with total cholesterol, so we adjusted the mean values of serum total cholesterol through multivariate model selection with age(r=0.12), total cigarette index (cigarette-years; r=0.10), Quetelet's index ($Kg/m^2$, r=0.16), daily calory expenditure (kcal/day, r=0.06), weekly meat and poultry consumption(g/week, r=0.05), weekly fish consumption (g/week, r=0.08), other caffeinated beverage intake (cups/week), and the amount of sugar and prim added to the coffee. Among those variables only age, Quetelet's index, fish consumption, and total cigarette index (in smokers) were remained in the models. After adjustment, the corresponing differences of total cholesterol in smokers were changed to $0.4{\pm}5.24mg/dl,\;-0.5{\pm}4.97mg/dl,\;and\;8.9{\pm}4.78mg/dl$, which were significantly different among themselves (P=0.011). In non-smokers, however, the differences were not statistically significant (P=0.76). Adjusted mean values of systolic blood pressure and diastolic blood pressure were also determined to evaluate the direct effect of coffee to cardiovascular system, but their means were not significantly different by coffee consumption(p=0.18 for SBP, p=0.48 for DBP). Asuming instant coffee in the most popular type of coffee in Korea, the association observed in our study between coffee and serum total cholesterol, especially in smokers, is very interesting finding for the connection between coffee and serum total cholesterol, because only 'boiled coffee' tend to show significant lipid raising effect rather than to other types of coffee, like filtered or espresso, in most of the western countries. We concluded that people who drink coffee more than 2 cups a day have significantly higher serum total cholesterol level than those who never drink coffee, especially in smokers.

인슐린비의존형 노인 당뇨병환자의 엽산섭취와 혈장 호모시스테인 수준 (Folate Intakes and Plasma Homocysteine Levels in the Elderly Patients with NIDDM)

  • Hyperhomocysteinemia has been shown to be associated with increased risks for macrovascular angiopathy. The present study was conducted to document the relationship between plasma homocysteine levels and the presence of macrovascular angiopath in patients with Type II diabetes mellitus. Plasma total homocysteine was determined by a HPLC-fluorescence detection method in a total of 238 subjects, 127 diabetic patients and 111 control. Dietary information including folate intake was assessed by a 24-hour recall in a total 113 subjects, 70 diabetic patients and 43 control. Folate intake was significantly lower inn patients with diabetes mellitus than that in the control. The lowered folate intake in the diabetic patients was mainly due to reduced intakes of fruits and vegetables. The average plasma homocysteine level of patients with MA was 14.2$\mu$㏖/L, which was significantly higher than that of either the patients without MA(11.4$\mu$㏖/L) or the control(11.5$\mu$㏖/L). Twenty five percent of the diabetic patients were positively correlated with age (r=0.20), body weight (r=0.28), body mass index (r=0.28), body mass index (r=0.18), diastolic blood pressure (r=0.20), and total cholesterol (r=0.14). cigarette smokers had significantly higher levels of plasma homocysteine than the non-smokers. Further prospective studies are needed to investigate whether folate or other B vitamin supplementation could be beneficial for the prevention of hyperhomocysteinemia or macrovascular angiopathy in the diabetic patients.

Nutritional Status Associated with Smoking and Other Factors in Korean Adults Women

  • An increasing number of women are becoming victims of their smoking habits. Cigarette smoking is implicated as a major cause or several chronic diseases. This study was performed to investigate the nutritional status associated with smoking and other factors like drinking or fruit and vegetable consumption frequency in Korean adult women. The 2001 Korean National Health and Nutrition Survey provided the current adult women sample (n = 2900), who were categorized as smoking (n= 141) and nonsmoking group (n = 2759). The smoking women declared significantly lower intakes of carbohydrate, calcium and vitamin C and higher alcohol than nonsmokers, but with nonsignificant difference of BMI. The smoking and drinking group showed the lowest intakes in most of the nutrients, the lowest BMI and significantly elevated blood pressure. The smoking group with fruit and vegetable consumption frequency less than 3 times a day also showed lower intakes of most of the nutrients and significantly higher systolic blood pressure. Less proportion of smokers perceived their health status very good or good and more smokers felt depressed often. In conclusion it seems cigarette smoking is associated with the decreased nutrient intakes in adults women. When smoking is connected with other unhealthy dietary habits like drinking or lower fruit and vegetable consumption, it may exacerbate the nutritional status and cause elevated blood pressure.

궐련 흡연 청소년과 전자담배를 중복 사용하는 흡연 청소년의 금연시도 영향요인 비교 (Comparison of the Factors for Attempts to Quit Smoking by Adolescent using Tobacco Cigarettes only and those Adolescents using Tobacco Cigarettes Together with Electronic Cigarettes)

  • Purpose: This study was conducted to compare and analyze the smoking-related social-ecological factors affecting attempts to quit smoking by adolescents using tobacco cigarettes only and also those adolescents using tobacco cigarettes along with electronic cigarettes. Methods: This study, as secondary analysis research, used the raw data from the 14th Korea youth risk behavior survey 2018. The data was analyzed by frequency analysis, the Rao-scott χ2-test and logistic regression analysis when considering the complex sample's analysis. Results: On logistic regression analysis, during their first smoking period, intense physical activities and having friends who smoked were associated with significantly more attempts to quit smoking by cigarettes smokers, and their first smoking period and experiences of undergoing smoking cessation education were associated with significantly more attempts to quit smoking for dual smokers of both tobacco cigarettes and electronic ones. Conclusion: As a result of this study, we identified the need to differentiate different types of smoking cessation counseling and education according to the types of smoking in adolescents.

일 지역 고등학생의 흡연상태에 따른 흡연지식과 흡연태도 (High School Students' Knowledge and Attitude about Smoking according to Their Smoking Status)

  • Purpose: This study was to provide basic information for preventing high school students from smoking by examining their actual smoking condition and their knowledge and attitude about smoking. Method: The subjects were 515 students selected from six(6), high schools in Jecheon. Data were collected using a questionnaire from the 1st to the 17th of December 2002. The author prepared the tool by modifying the scale of knowledge and attitude about smoking developed by Moon(2001). Data were analyzed with SPSS/WIN program using frequencies, percentages, $x^2$-test, t-test, ANOVA. Duncan test and Pearson's correlation coefficient. Result: 1. The percentage of ex-smoking students was 29.9% of male students and 18.6% of female ones. The percentage of currently smoking students was 23.9% of male students and 3.0% of female ones. The percentage of non-smoking students was 46.2% of male students and 78.4% of female ones, 2. There were not significant differences in smoking knowledge according to smoking status in male and female students. 3. There were significant differences in smoking attitude according to smoking status in male(F=32.70, p=.000) and female(F=11.15, p=.000) students. 4. In smoking status according to general characteristics, there were significant differences according to the type of school($x^2$=32.93, p=.000), grade($x^2$=11.91, p=.018), educational level of the father($x^2$=30.52, p=.000) and the mother($x^2$=14.23, p=.027), monthly allowance($x^2$=17.19, p=.028), harmony of family members($x^2$=15.23, p=.019), school life($x^2$=26.96, p=.00l) and school record($x^2$=30.24, p=.000). 5. In smoking status according to smoking-related characteristics, there were significant differences according to experience in cigarette purchase($x^2$=150.04. p=.000), experience in errands for cigarette($x^2$=20.00, p=.000), knowledge of cigarette price($x^2$=72.52, p=.000), close friends' smoking($x^2$=107.41. p=.000) and smoking of brothers and sisters($x^2$=16.97, p=.002). 6. There were significant correlation between knowledge and attitude about smoking in the whole students(r=.337. p=.000), ex-smokers(r=.324. p=.000), non-smokers(r=.369. p=.000) but there was not a significant correlation between knowledge and attitude about smoking in current smokers(r=.128. p=.30l). Conclusion: Smoking prevention education should considered students' smoking status. Current smokers need smoking prevention education aiming to change their attitude toward smoking in order for them to reduce smoking and to practice smoking cessation.
