Congenital Bronchoesophageal Fistula Causing Only Chronic Cough : One case (만성 기침을 주증상으로 한 선천성 기관지-식도루 1예)

  • There were so many causes of chronic coughing including postnasal drip, pneumonia, nasal polyp, asthma, interstinal lung disease etc. Congenital bronchoesophageal fistula was not usually thought as cause of chronic coughing. A 46-year-old female patient suffered from chronic coughing without usual causes. Her chest X-ray viewed normally. She coughed especially after swallowing foods. So we recommended her esophagogram and it revealed broncho-esphageal fistula. She underwent surgical resection of broncho-esophageal fistula. She was well without cough after the surgery. We reported a case of congenital broncho-esphageal fistula that had caused chronic coughing without any evidence of pneumonia, malignancy, tuberculosis, bronchiectasis, inflammation, asthma, nasal polyp, etc. So we should suspect the bronchoesophageal fistula when patients cough chronically with eating, and recommend the esophagogram.

  • A cat who is a 15-year-old and spayed female visited an animal clinic with severe coughing symptoms. Since the cat's coughing symptoms had worsened from the age of 10 and X-rays showed a bronchial pattern in the lungs, it was diagnosed as Chronic Obstructive Pulmonary Disease (COPD). She received three injections of stem cells isolated from the amniotic membrane on days 0, 7, and 23. Although there was no improvement in the clinical findings on the x-ray, the number of coughing was significantly reduced. In addition, even after long-term follow-up post treatment for a month, she was stable with almost no coughing.

  • Objectives: We intented to find out the effect of Gami-Eumcheonyijintang to patients who complain with coughing and to make it useful for treating the patients with the symptom. Materials and Methods: We examined the medical of 33 patients who visited with the symptoms of coughing, sputum in Kyungsan University, Hospital of Oriental Medicine from April, 1996 to February, 1999. and we collected the result of the administration with Gami-Eumcheonyijintong. We aimed to find any change in the symptoms, reappearance after recovery, and any special facts with the medicine surveying by telephoning to the patients. Results : the results are as follows, 1. It showed that patients who were under thirties showed higher rate of recovery(75.00%) than who were over thirties(52.94% ). 2, The acute-stage patients who were recovered in less than four weeks showed higher rate of recovery than those chronic-stage patients who were recovered in more than eight weeks. 3. The patients who didn't have the symptoms of sputum or had less serious problems of it with severe coughing showed relatively the highest rate of recovery(73.91% ). 4. The patients who showed the coughing symptom right after URI without any past constitutional history showed the highest rate of recovery(73.68% ). Conclusion : From the above result, we found that Gami-Eumcheonyijintang is more effective in early coughing symptom after URI than chronic stage coughing symptom. It is thought that more studies about medicine composition are needed in the further.

  • The Atrophy of lung among chronic asthenia disease is a kind of tuberculosis. It is divided in two types. One is the Atrophy of lung with consumptive fever, the other is the Atrophy of lung with consumptive cold. Four prescriptions can be used treat this disease. Pulmonary abscess(肺癰) is infected by poisonous fatcors of wind and heat. It's symptoms like coughing, chest pain, fever and phlegm with pus after coughing are appeared generally, in case of serious condition, pyemia after coughing can be appeared. This disease is applicable to lung abscess, bronchiectasis, lung gangrene. Six prescriptions can be used to treat this disease. Coughing and congestion of the head(咳嗽上氣) is called Lung-distension(肺腸). Its symptom is dyspnea caused by more expiration rather than inspiration. This disease is divided into two types like chronic asthenia type and inflammatory type. Seven prescriptions can be used to treat this disease.

  • Severe chronic coughing infrequently causes rib fractures and these fractures belong to Stress fracture and these rib fractures are given the term of "cough fractur". Cough fracture usually occurs in one rib and in the middle of the rib between the costochondral junction and the costal angle. The case is a report about a young woman with multiple rib fractures which were caused by severe coughing. The Chest PA and Rib series test were normal but Rib fractures were founded by Bone scan test. Treatment was Herb-medication, rest and restriction of work and symptoms were reduced.

  • Coughing and sputum are symptoms of sequela of Pneumonia. Oriental medicine call it Hae suo Hae su is common sign of lung disease. Hominis Placenta is the dried placenta of a healthy women. The nature of Hominis Placenta is warm and its taste sweet-salty. It has correspondence to the meridians of lung and kindey. Hominis Placenta acupuncture therapy has effect on invigoration of vital energy nourishing blood and tonifying the essence. It can be applied to the disease as Asthma, pulmonary tuberculosis, chronic hepatitis, liver cirrhosis, degenerative change and cerebrovascular disease.   • Asthma is recognized today as an inflammatory disease of the lung characterized by acute non-specific airway hypersensitiveness in association with chronic pulmonary inflammation. Gamijihwang-tang(GJT), a fortified prescription of YMJHT, is applied for the treatments of chronic coughing and asthma, and post-delivery coughing and asthma in the gynecology. Also in the clinical practice, GJT is known to be very effective for controlling coughing and asthma as a cold sequoia. In this study, we investigated the effects of GJT on the lipopolysaccharide (LPS)-induced nitric oxide (NO) and prostaglandin $E_2$ ($PGE_2$) production, and on the level of inducible nitric oxide synthase (iNOS) and Cyclooxygenase-2 expression in murine macrophage RAW 264.7 cells. We found that GJT inhibited LPS-induced NO and $PGE_2$ production in a dose dependent manner. Furthermore, GJT inhibited the expression of LPS-induced iNOS and COX-2 protein and mRNA expression in RAW 264.7 macrophages. Treatment with GJT of RAW 264.7 cells transfected with a reporter construct indicated a reduced level of LPS-induced nuclear factor-KB (NF-kB) activity and effectively lowered NF-kB binding as measured by transient transfection assay. These results suggest that the main inhibitory mechanism of the GJT may be the reduction of iNOS and COX-2 gene expression through blocking of NF-kB activation.

  • Cough, the most common symptom, encountered in the outpatient clinic can be caused by various underlying diseases. It defines as chronic cough that the duration of cough is more than 8 weeks with a normal chest X-ray findings. The cause of cough can be found out for more than 90% through the appropriate diagnostic approach and Upper airway cough syndrome, Asthma and Gastroesophageal reflex disease are the most common causes of disease to non-smokers. Chronic cough can be due to not one reason but various reasons and achieve good results by a systematic approach to diagnosis and a concrete treatment on the basis of the sufficient understanding of the underlying disease.

Cervical lung lobe herniation in a Pekingese dog

  • An 8-year-old intact male Pekingese (weighing 13 kg) was presented for evaluation of chronic coughing. Thoracic radiography found a redundant tissue swelling (protruded on expiration and collapsed on inspiration) on the ventral neck just cranial to the thoracic inlet. Fluoroscopy also identified that the cranial portion of the right/left cranial lung lobe was seen to protrude through the thoracic inlet into the ventral neck on expiration. On the echocardiogram, there were no abnormal jets in all cardiac valves and no dilation in all cardiac chambers. Based on diagnostic imaging studies, the case was diagnosed as cervical lung lobe herniation.

  • Congenital bronchoesophageal fistula is a rare anomaly that can appear in adult life uncommonly. There was a largest review that include 100 cases in the literature. In our case, he was 48 years old male patient who admitted for chronic coughing and recurrent lobar pneumonia on the right lower lobe since 10 years old. We could confirmed the fistular preoperatively by barium swallow examination and performed division of the fistula. The patient’s postoperative course was unremarkable. According Braimbridge’s classification, it was belonged to the type II.
