Relationship between Health Concern, Knowledge of Cardio-cerebrovascular Disease, and Health-Promoting Behaviors among the Elderly (노인의 건강관심도와 심뇌혈관질환에 대한 지식 및 건강증진행위와의 관계)

  • Purpose: The purpose of this study was to find out the relationship between health concern, knowledge of cardio-cerebrovascular disease, and health-promoting behaviors among the elderly in the community. Method: The subjects of the study were 142 senior citizens aged 65 years or older in Cheongju-si, Chungcheong-do. Data were collected using structured questionnaires that included questions on the general characteristics, health concerns, knowledge of cardio-cerebrovascular disease, and health-promoting behaviors. The collected data were analyzed by the statistical program SPSS 20.0. The frequencies, percentages, averages, and standard deviations of the collected data were analyzed using a t-test, ANOVA, Mann Whitney U test, and Pearson's correlation coefficient, basing on the purpose of the study. The Scheffe test was carried out as a posteriori test. Result: The elderly were found to have a high health concern, but their knowledge of cardio-cerebrovascular disease was low, and their health-promoting behaviors were above the intermediate level. There was a significant positive correlation between each variable, which means that the higher the health concern was, the more knowledge an individual had of cardio-cerebrovascular disease, and the more health-promoting behaviors one had. Conclusion: This suggests that the elderly in the community should be encouraged to engage in the actions that promote their health by raising their level of health concern and awareness of diseases through health education, basing on their individual characteristics.

Numerical Model for Cerebrovascular Hemodynamics with Indocyanine Green Fluorescence Videoangiography

  • Objective : The use of indocyanine green videoangiography (ICG-VA) to assess blood flow in the brain during cerebrovascular surgery has been increasing. Clinical studies on ICG-VA have predominantly focused on qualitative analysis. However, quantitative analysis numerical modelling for time profiling enables a more accurate evaluation of blood flow kinetics. In this study, we established a multiple exponential modified Gaussian (multi-EMG) model for quantitative ICG-VA to understand accurately the status of cerebral hemodynamics. Methods : We obtained clinical data of cerebral blood flow acquired the quantitative analysis ICG-VA during cerebrovascular surgery. Varied asymmetric peak functions were compared to find the most matching function form with clinical data by using a nonlinear regression algorithm. To verify the result of the nonlinear regression, the mode function was applied to various types of data. Results : The proposed multi-EMG model is well fitted to the clinical data. Because the primary parameters-growth and decay rates, and peak center and heights-of the model are characteristics of model function, they provide accurate reference values for assessing cerebral hemodynamics in various conditions. In addition, the primary parameters can be estimated on the curves with partially missed data. The accuracy of the model estimation was verified by a repeated curve fitting method using manipulation of missing data. Conclusion : The multi-EMG model can possibly serve as a universal model for cerebral hemodynamics in a comparison with other asymmetric peak functions. According to the results, the model can be helpful for clinical research assessment of cerebrovascular hemodynamics in a clinical setting.

A Families's Burden on a Cerebrovascular Accident Patient and the Demand for Nursing Home (뇌혈관질환자 가족의 부담감과 가정간호 요구도)

  • Purpose: This is to find out the correlation between a families burden on a cerebrovascular accident patient and the demand for nursing home. Method: The data was established by using structured questionnaires for 102 nursing families of cerebrovascular accident patient at three university hospitals. Result: 1) The total burden was showed to be at 3.21, which was some what higher than average. Total average of demand for nursing home was at 3.30, which was higher than average. 2) This showed static correlations between the economic, social, physical dependent and psychological burden, and demand for nursing home. The result represents that the higher burden showed a significantly positive correlation to the higher demand for nursing home. Conclusion: If a systemic support of nursing home is prepared the differentiated nursing home matched perfectly on each burden, the burden on a family of the patient can be decreased effectively.

  • A clinical study was carried out 1 case of ischemic cerebrovascular accident which occurred at puerperium in a 27 year-old woman followed by vaginal delivery The results were as follows: 1. In the clinical view, it was thought that the cause of this case is intracranial venous thrombus, because the symptom-headache, foaming at the mouse, acromyotonia, hemiparesis- occurred in the postpartum and functional outcome is good. 2. In this case, the patient's functional system of spleen-stomach is usually weak, and due to delivery the functional system of spleen-stomach is more insufficient. so it can not control damp-phlegm(濕痰). In addition, vital energy of liver stagnate.

Effects of Carbonated Water Intake on Constipation in Elderly Patients Following a Cerebrovascular Accident (노인 뇌졸중환자의 변비완화를 위한 탄산수 음용의 효과)

  • Purpose: This study was done to identify effects of carbonated water intake on constipation in elders who have experienced a cerebrovascular accident (CVA) and are bed-ridden. Methods: Forty elderly patients with CVA were randomly assigned to one of two groups in a double-blind study. Patients in the experimental group drank carbonated water and those in the control group drank tap water for two weeks. Six patients dropped out during the study period. Data were analyzed by repeated measured ANCOVA and the covariance was the dose of laxatives used for the two weeks. Results: Frequency of defecation increased significantly and symptoms of constipation decreased significantly for patients in the experimental group. Conclusion: The study results suggest that the intake of carbonated water is an effective method for the intervention of constipation in elderly patients with CVA.

  • Purpose: The purpose of this study was to examine the differences in requirement and activity related to workers' health promotion according to business type and enterprise size. Methods: This study is a secondary analysis of data extracted from a survey on the Enterprise Health Promotion Index. For the present analysis, 7,784 companies were selected. Results: Requirements for lifestyle management and activities to prevent musculoskeletal disorders were the highest. The requirement and activity of musculoskeletal disorders and cerebrovascular disease were high in the manufacturing industry. Meanwhile, in the non-manufacturing industry, the requirement and activity of job stress management were high. Small-sized companies had a high requirement to prevent musculoskeletal disorders and cerebrovascular disease. Conclusion: Government support should be strengthened in workers' lifestyle management. Additionally, workers' health promotion in small-sized companies needs to focus on preventing musculoskeletal disorders and cerebrovascular disease.

Blood-neural barrier: its diversity and coordinated cell-to-cell communication

  • The cerebral microvessels possess barrier characteristics which are tightly sealed excluding many toxic substances and protecting neural tissues. The specialized blood-neural barriers as well as the cerebral microvascular barrier are recognized in the retina, inner ear, spinal cord, and cerebrospinal fluid. Microvascular endothelial cells in the brain closely interact with other components such as astrocytes, pericytes, perivascular microglia and neurons to form functional 'neurovascular unit'. Communication between endothelial cells and other surrounding cells enhances the barrier functions, consequently resulting in maintenance and elaboration of proper brain homeostasis. Furthermore, the disruption of the neurovascular unit is closely involved in cerebrovascular disorders. In this review, we focus on the location and function of these various blood-neural barriers, and the importance of the cell-to-cell communication for development and maintenance of the barrier integrity at the neurovascular unit. We also demonstrate the close relation between the alteration of the blood-neural barriers and cerebrovascular disorders.

  • Recently, accumulated clinical experience and advanced neuroradiological techniques have led to a better understanding of pediatric cerebrovascular disease (CVD), which was once considered rare. Approximately 10% of pediatric neurosurgical patients have CVD; therefore, it is no longer uncommon to pediatricians and pediatric neurosurgeons. Furthermore, children with CVD tend to recover better than adults after stroke because the immature brain is more plastic than the mature one. Therefore, active treatment often proves more rewarding in pediatric medicine than in adult medicine.

  • Cerebrovascular and cardiovascular diseases (CVDs) are a major cause of deaths among workers as well as in general population in Korea. Occupational stress like long working hours or overwork can cause sudden unexpected death in a worker who had been suffering from an aggravated preexisting condition such as hypertensive or arteriosclerotic diseases, The Korean government has compensated the cases of cerebrovascular disease and cardiovascular disease due to overwork, but neither a resonable definition of overwork nor detailed evaluation guideline has been officially provided to verify overwork, so that the prevention of, as well as compensation for workers' occupational stress-related cardiovascular attacks, cannot be carried out efficiently. A thorough understanding of the etiopathology of WR-CVDs can be very helpful in developing a prevention strategy.

Cost-Effectiveness Analysis of Home Health Care Program for Cerebrovascular Accident Patients (뇌졸중환자 가정간호 및 방문간호서비스의 비용효과 비교)

  • Purpose of this study is to compare the cost effectiveness of home care services for the cerebrovascular accident patients by the type of institution. The method is the secondary analysis using the patients' charts. 107 subjects and 1.417 visits were sampled from each type of home care institution such as one hospital based home care center. one KNA home care center, one urban health center, one rural health center and one health care post. Result: There were differences in the functional status of patients and the service contents and frequencies provided by the type of home care institution, The cost per visit for one unit of ADL by the hospital based home care was higher than by the community-based home care. Conclusion: It was suggested that the referral system among the home care institutions would be developed to improve the cost-effectiveness.
