A drug use evaluation (DUE) program has been defined as an "authorized a quality assurance process designed to ensure that drugs are used appropriately, safely, and effectively". A retrospective DUE study on ceftazidime has been conducted by reviewing patients charts following criteria for drug use evaluation established by Am.S.Hosp.Pharm. Total 60 charts of patient treated with ceftazidime in Seoul National University Hospital from Jan. 1.1993 to July.31.1993 were retrospectively reviewed. As a result, 44 cases(73%) were met with the criteria for the justification of use. In analysing process indicators including culture and sensitivity test, CBC, LFT, history of anaphylaxis monitoring before initial dose, vital sign monitoring were relatively well documented showing the accepted level above 80%, while appropriated dose were infrequently documented with low accepted level. In outcome analysis, blood culture after discontinuing drug were rarely documented, with accepted level of 46%. For using effectively and safely, ceftazidime is recommended to be administrated to optimal indication, and monitored actively for preventing adverse effect.
Binding of four cephalosporins(cefaclor, cefpiramide, ceftazidime, ceforanide) to bovine serum albumin was studied. Difference spectrophotometry was employed to evaluate the nature and the degree of association of cephalosporin-albumin complex. 2-(4'-hydroxybenzen azo) benzoic acid was used as the uv spectrophotometric probe for measuring the binding of cephalosporins to bovine serum albumin. Competitive bindings between cephalosporins and probe were observed. For the binding of cephalosporins to bovine serum albumin, three binding sites were identified. The binding constants of cefaclor, ceforanide, ceftazidime and cefpiramide were $12.57\;{\times}\;10^{-2}M^{-1}$, $6.49\;{\times}\;10^{-2}M^{-1}$, $4.70\;{\times}\;10^{-2}M^{-1}$ and $6.20\;{\times}\;10^{-2}M^{-1}$ respectively.
To investigate the antibiotic resistant patterns of the bacteria producing ESBL, we isolated one organism of Klebsiella pneumoniae from a clinical laboratory in Busan. The organism that produces ESBL gene was detected by double disk synergy test and the presence of three ESBL genes (TEM-1, SHV-12, CTX-M-15) was confirmed by polymerase chain reaction and DNA sequencing analysis. To analyse the characteristics of three ESBL genes, we performed transconjugation, transformation and cloning experiment with the organism. The MIC of Klebsiella pneumoniae was revealed that ceftazidime, cefotaxime and ceftriaxone were $256\;{\mu}g/ml,\;128\;{\mu}g/ml\;and\;128\;{\mu}g/ml$ respectively. The MIC of conjugant (E. coli $RG176^{Na(r)}$) af was revealed that ceftazidime, cefotaxime and ceftriaxone were $256\;{\mu}g/ml,\;64\;{\mu}g/ml\;and\;128\;{\mu}g/ml$ respectively. The MIC of transformant (E. cofi $DH5{\alpha}$) was revealed that ceftazidime, cefotaxime and ceftriaxone were $128\;{\mu}g/ml,\;32\;{\mu}g/ml,\;and\;32\;{\mu}g/ml$ respectively, The MIC of cloned organism of SHV-12 gene (E. coli $DH5{\alpha}$) was revealed that ceftazidime, cefotaxime and ceftriaxone were $128\;{\mu}g/ml,\;8\;{\mu}g/ml,\;and\;32\;{\mu}g/ml$ respectively. The results indicated that MIC of conjugant was higher than MIC of transformant and also SHV-12 gene were not resistant against cefotaxime antibiotic.

DWC-751은 그람양성 및 음성균주에 대하여 광범위한 항균스펙트럼을 가지는 것으로 나타났다. 그람양성균 S. aureus에 대하여는 cefpirome과 동등하며, cefotaxime 보다 4배, ceftazidime보다 16배 우수하였고 그람음성균에 대하여 DWC-751의 항균력은 cefpirome, cefotaxime보다 2배, ceftazidime보다 4-8배 우수하였다. Ps. aeruginosa에 대한 DWC-751의 항균력은 ceftazidime과 거의 동등한 항균력을 나타내었고, cefpirome보다 2배, cefotaxime보다 4-8배 우수한 항균력을 나타내었다. 임상분리균주 및 ofloxacin 내성균주에 대한 DWC-751의 항균력은 표준균주에 대한 결과와 같이 대조약물보다 우수하였다. 전신감염치료효과에 있어서 Streptococcus pyogenes, Serratia marcescens, Acinetobacter calcoaceticus, Morganella morganii, Proteus mirabilis에 대한 동물실험 결과, ED$_{50}$치에 의한 효능은 cefotaxime 보다 우수하였으며,Enterobacter cloacae, Pseudomonas aeruginosa에 대하여는 ceftazidime과 거의 동등하였다.
Major pathogenic Gram-negative organisms such as P. aeruginosa, Serratia species, E. coli, Enterobacter species which are isolated from the specimens in large medical centers are greatly resistant to the commonly used antibiotics. Gram-negative bacilli, which had been isolated in Yeungnam University Hospital during the period from December 1992 to April 1993 and turned out to be resistant to the primary antibiotics susceptibility test for chloramphenicol, ampicillin, cephalothin, gentamicin, tetracyclin, amikin and tobramycin, were subjected to the secondary antibiotics susceptibility test for aztreonam, ceftazidime, ciprofioxacine, cefotaxime, cefamandole, piperacillin, ticarcillin and sulfamethoxazole trimethopime. Out of 315 tested organisms, 167 organisms(53%) were resistant to all secondary antibiotics in vitro. Antimicrobial activity of ceftazidime(37.1%), aztreonam(11.%), ciprofioxacine(7.9%) against Gram negative bacilli were slightly more active than other antibiotics tested, while cefamandole was not active to all the Gram-negative bacilli tested. According to the specimens, E. coli was the most frequently resistant organisms to the primary antibiotics from urine, A. baumanii, from respiratory system and wounds, and P. aeruginosa from various specimens. In summary, Gram negative bacilli resistant to the primarily applied antibiotics also were resistant to the secondary antibiotics. Rearrangement of the antibiotics disks for the antibiotic susceptibility test should be considered.

Bacteria that are resistant to several different groups of antibiotics have increased during the past few years. The importance of surveillance of antimicrobial resistance is now widely recognized. Unfortunately, this development has not been documented continuously in veterinary medicine in Korea. Therefore, the clinical prevalence and trend of antimicrobial susceptibility of aerobic isolates were investigated in this study. Total 121 isolates of aerobic bacteria were isolated from clinical specimens of dogs and cats at Veterinary Medical Teaching Hospital of Seoul National University from May 2001 to October 2002. Among them, the most common isolated species was Staphylococcus spp. (48 isolates), followed by E.coli (26 isolates), Enterococcus spp. (21 isolates), Klebsiella pneumoniae (9 isolates), Streptococcus spp. (6 isolates), Enterobacter cloacae (3 isolates), Pseudomonas aeruginosa (3 isolates), Corynebacterium xerosis (2 isolates), Chryseomonas spp. (2 isolates), and Providencia stuartii (1 isolate). The susceptibility of isolates to antibiotics was determined by the disk diffusion method. Gram-positive bacterial isolates were showed high susceptibilities to amikacin, amoxacillin/clavulanate, ceftazidime, and oxacillin, while Gram-negative bacterial isolates were showed high susceptibilities to amikacin and ceftazidime. Staphylococcus spp. were showed high susceptibilities to amikacin, amoxicillin/clavulanate, ceftazidime, cephalothin, and oxacillin. Streptococcus spp. and E.coli were showed high susceptibilities to amikacin and ceftazidime. Of the 48 staphylococci, seven Methicillin Resistant staphylococci were observed (14.6%), distributed among S. auricularis (1), S. hemolyticus (2), S. sciuri (1), S. saprophyticus (1), S. warneri (2) isolates. One strain of E.coli and one strain of Corynebacterium xerosis were resistant to all antibiotics tested. And, resistance trends between the you 2000 (from July 1999 to September 2000) and 2002 (from May 2001 to October 2002) were compared. Resistance to antibiotics was increased in both Gram-positive and Gram-negative bacterial isolates (p< 0.05). The resistance rates of Staphylococcus spp., E.coli and Klebsiella pneumoniae to all antibiotics tested were also increased (p<0.05). This study investigated increasing resistance between the year 2000 and 2002 in Veterinary Medical Teaching Hospital of Seoul National University. Surveillance resistance is helpful to alert to veterinarian and select of appropriate therapy. Antimicrobial susceptibility surveillance of isolates should urgently be continued in veterinary medicine.
During 2006, the antibiotic resistance rate were investigated in gram negative bacteria. Resistance to piperacillin were detected at 60% in Escherichia coli, and 37% in Klebsiella pneumoniae. Ceftriaxone were detected 58% in Enterobacter cloacae, 52% in Acinetobacter baumannii, 43% in Enterobacter aerogenes and 76% were detected in Serratia marcescens. Between 1998 and 2007 antibiotic resistance rate were decreased in seven types antibiotic drugs. but, ceftazidime were increased from 12 to 20% during this times. In addition, E. coli, E. cloacae, A. baumannii and E. aerogenes were more isolated from May to June and K. pneumoniae and S. marcescens were more isolated from July to September. We should monitor and control antibiotic use and regularly survey antibiotic resistance patterns among pathogens in the hospital.
Effect of antibiotics belonging to three different groups, including third generation cephalosporins, aminoglycosides, and quinolones, on the outer membrane protein (OMP) profile of Klebsiella pneumoniae was examined. It was found that a new OMP (porins) of 40 kDa molecular mass was expressed in Klebsiella pneumoniae, when grown in the presence of ceftazidime, whereas new proteins with 30 kDa and 22 kDa masses were detected in the presence of ofloxacin. The immunoblot analysis showed that the new proteins of 40 kDa and 30 kDa molecular masses were expressed on the outer envelope, when being exposed to antibiotics ceftazidime and ofloxacin, respectively. This finding is important, as the outer surface comes in contact with the immune system, and therefore may have a bearing on the outcome of the disease.
The in vitro antibacterial activities of LB10522, a new catechol-substituted cephalosporin, were compared with those of cefpirome, ceftazidime, ceftriaxone, and cefoperaz one against clinical isolates and laboratory standard anaerobes. LB10522 had broad spectrum antibacterial activities against both gram-positive and gram-negative microorganisms. It was most active against gram-positve bacteria among the reference cephalosporins tested. Against gram-negative strains such as the family Enterobacteriaceae, LB10522 showed an activity comparable to that of cefpirome. But LB10522 was more potent than ceftazidime, ceftriaxone and cefoperazone. In particular, Pseudomonas aeruginosa was highly susceptible to LB10522, which was 32-fold and 64-fold more active than ceftazidime and cefpirome, respectively. Against anaerobic strains, the activity of LB10522 was similar to those of reference compounds. LB10522 exhibited potent therapeutic activities against experimental local infections in mice. The therapeutic effect of LB10522 against urinary tract infection (UTI) caused by P. aeruginosa 1912E in mice was superior to that of cefpirome. Against experimental respiratory tract infection (RTI) caused by K. pneumoniae DT-S in mice, LB10522 was as effective as cefpirome. The in vivo efficacy of LB10522 was correlated well with its in vitro activity.

The study aims primarily to evaluate the resistance of antibiotics and the prevalence of these enzymes among Escherichia coli the most frequent isolate of Enterobacteriaceae producing Extended-Spectrum ${\beta}$-Lactamase(ESBLs), to differentiate the types of enzymes in these isolates. Total 74(26.2%) Strains of producing ESBLs among the 282 E. coli isolates were isolated from hospitals of Chungcheong area (Daejeon, Chungnam, and Chungbuk) during a 6 month-period from February to July, 2013. 282 E. coli isolates including ESBL shown resistance rates of aztreonam 30.8%, Cefotaxim 30.9%, and Ceftazidime 32.2%, 74 isolates producing ESBLs in E. coli were resistant rates to Aztreonam 58.1%, Cefotaxim 100%, and Ceftazidime 63.5% of ${\beta}$-lactam antibiotics. CTX-M-2 (48 isolates) was the most prevalent type of ESBLs identified. Followed the order of frequency by PER-1 (28 isolates), VEB-1 (26 isolates) and CTX-M-8 (20 isolates), of the 74 isolates, 2 isolates only showed GES-1 in Chungnam province. Accurate identification type of ESBLs would aid in hospital infection control. This would give aid to the physician to prescribe more appropriate antibiotics.
