개의 비루에서 분리한 원인균의 항생제 내성 (Antibiotic Resistance of Bacterial Isolates from Nasal Discharges of Dogs with Respiratory Diseases)

  • Bacterial pathogens were isolated from 36 dogs with respiratory signs, that were submitted to Veterinary Clinics in Jeju, including Veterinary Medical Teaching Hospital in Cheju National University. Of 36 isolates, 16 (44.4%) bacterial pathogens were Gram-positive and 20 (55.6%) were Gram-negative. Gram-positive bacteria identified with API Staph were 12 S. intermedius (33.3%), 2 S. aureus (5.6%), 1 S. haemolyticum (2.8%), and 1 S. xylosus (2.8%). Gram-negative organisms identified with API 20E or API NE included 8 Bordetella bronchiseptica (22.2%), 6 Escherichia coli (16.7%), 4 Pasteurella spp. (11.1%), 1 Enterobacter intermedius (2.8%), and 1 Oligella ureolytica (2.8%). Both Staphylococcus spp. isolates and Gram-negative pathogens were resistant to one or more antibiotics, including ampicillin (AM), amoxicillin/clavulanic acid (AMC), chloramphenicol (C), cefazolin (CZ), erythromycin (E), gentamicin (GM), kanamycin (K), lincomycin (L), oxacillin (OX), trimethoprim/sulfamethoxazole (SXT), and tetracycline (TE). All Staphylococcus spp. were susceptible to AMC, OX and VA, while many isolates were highly resistant to L (87.5%), E (68.8%), P (62.5%), and AM (56.3%). Antibiotic-resistant patterns of staphylococcal isolates were shown ranges from single to 9-resistant patterns. Resistant rates to antibiotics of Gram-negative bacteria were usually higher than those of Staphylococcus spp. in this study. Most Gram-negative bacteria were highly resistant to L (90.0%), AM (85.0%), E (85.0%), P (85.0%), OX (80.0%), and CZ (75.0%). B. bronchiseptica isolates showed 5 to 8 antibiotics-resistant patterns and Pasteurella spp., 2 to 8-resistant patterns. In particular, all 6 E. coli isolates were resistant to more than 9 different kinds of antibiotics, including one strain resistant to all antibiotics tested.

  • In this study, a total of 2,119 samples was taken from bovine feces and carcass from March 2002 to December 2003. And those were examined for the presence of enterohemorrhagic E coli O157:H7. The properties of the isolates were characterized for biochemical features, serotypes, virulence genes and antimicrobial susceptibility. Forty five strains($3.7\%$) of E coli O157:H7 were isolated from 1,208 fecal samples and were not detected in carcass using immunomagnetic separation technique and selective media. In multiplex PCR using stx1, stx2, eaeA and hlyA primers, the amplified bands at 180 bp, 255bp, 384bp and 534bp were observed, respectively. In antimicrobial susceptibility test, all isolates were susceptible to amoxicillin/clavulanic acid and cefazolin. The isolates were most resistant to sulfisoxazole($24.4\%$), followed by streptomycin($22.2\%$), tetracycline($20.0\%$). Eight strains($17.8\%$) of 45 isolates showed the multi-resistant patterns with over 3 drugs.

Characterization of Extended Spectrum $\beta$-Lactamase Genotype TEM, SHV, and CTX-M Producing Klebsiella pneumoniae Isolated from Clinical Specimens in Korea

  • To investigate the antibiotic-resistant patterns and the gene types of extended-spectrum $\beta$-lactamase (ESBL)-producing Klebsiella pneumoniae, we collected 226 Klebsiella pneumoniae strains from three general hospitals with more than 500 beds in Busan, Korea from September 2004 to October 2005, The minimum inhibitory concentration (MIC) of antibiotics was measured using the Gram-negative susceptibility (GNS) cards of Vitek (Vitek system, Hazelwood Inc., MO, U.S.A.). Of the 226 K, pneumoniae isolates, 65 ESBL-producing K. pneumoniae strains were detected by the Vitek system and confirmed by the double-disk synergy test. TEM (Temoniera) type, SHV (sulfhydryl variable) type, and CTX-M (cefotaxime) type genes were detected by polymerase chain reaction. All 65 K. pneumoniae strains were resistant to ampicillin, cefazolin, cefepime, ceftriaxone, and aztreonam, and 83.0% of the organisms were resistant to ampicillin/sulbactam, 66.1% to tobramycin, 67.6% to piperacillin/tazobactam, 61.5% to ciprofloxacin, and 47.6% to trimethoprim/sulfamethoxazole, and 43.0% to gentamicin. TEM-type ESBLs (TEM-1 type, -52 type) were found in 64.6% (42 of 65) of the isolates, SHV-type ESBLs (SHV-2a type, -12 type, -28 type) in 70.7% (46 of 65) of isolates, and CTX-M-type ESBLS (CTX-M-15 type) in 45% (29 of 65) of isolates. Of the 65 ESBL-producing K. pneumoniae strains, two strains were found to harbor blaSHV-28, which were detected in Korea for the first time. Therefore, more investigation and research on SHV-28 are needed in order to prevent the ESBL type-producing K. pneumoniae from spreading resistance to oxyimino cephalosporin antibiotics.

Prevalence and Molecular Characterization of ESBL Producing Enterobacteriaceae from Highly Polluted Stretch of River Yamuna, India

  • The rapid increase in number and diversity of Extended Spectrum ${\beta}$-Lactamases (ESBLs) producing Enterobacteriaceae in natural aquatic environment is a major health concern worldwide. This study investigates abundance and distribution of ESBL producing multidrug resistant Enterobacteriaceae and molecular characterization of ESBL genes among isolates from highly polluted stretch of river Yamuna, India. Water samples were collected from ten different sites distributed across Delhi stretch of river Yamuna, during 2014-15. A total of 506 non duplicate Enterobacteriaceae isolates were obtained. Phenotypic detection of ESBL production and antibiotic sensitivity for 15 different antibiotics were performed according to CLSI guidelines (Clinical and Laboratory Standard Institute, 2015). A subset of ESBL positive Enterobacteriaceae isolates were identified by 16S rRNA gene and screened for ESBL genes, such as $bla_{CTX-M}$, $bla_{TEM}$ and $bla_{OXA}$. Out of 506 non-duplicate bacterial isolates obtained, 175 (34.58%) were positive for ESBL production. Susceptibility pattern for fifteen antibiotics used in this study revealed higher resistance to cefazolin, rifampicin and ampicillin. A high proportion (76.57%) of ESBL positive isolates showed multidrug resistance phenotype, with MAR index of 0.39 at Buddha Vihar and Old Delhi Railway bridge sampling site. Identification and PCR based characterization of ESBL genes revealed the prevalence of $bla_{CTX-M}$ and $bla_{TEM}$ genes to be 88.33% and 61.66%, respectively. Co-occurrence of $bla_{CTX-M}$ and $bla_{TEM}$ genes was detected in 58.33% of the resistant bacteria. The $bla_{OXA}$ gene was not detected in any isolates. This study highlights deteriorating condition of urban aquatic environment due to rising level of ESBL producing Enterobacteriaceae with multidrug resistance phenotype.

경남 남부지방에서 임상형 유방염의 원인균 분리 및 약제 감수성 시험 (Isolation and Antimicrobial Drug Susceptibility of Mastitic Pathogens from Dairy Cattles with Clinical Mastitis in Gyeongnam South Area)

  • The present study was conducted to investigate isolation and antimicrobial drug susceptibility of clinical mastitic milking total 610 (897 quaters) dairy cattles from 36 dairy farms in Gyeongnam south area (Cosung, Masan) during the period from March 1999 to August 2002. The results obtained were summerized as follows . 1. Incidence of bacterial infection in four quaters was showed that right anterior quarter was 178(19.8%), right posterior quarter was 292(32.6%), left anterior quarter was 148 (16.2%), and left posterior quarter was 279 (31.1%), respectively. Isolation rate of posterior two quarters was higher 2 times than anterior two quarters. 2. Incidence of double infections of 897 clinical mastitic milk were showed that single infection was 549 (61.2%), double infection was 167(18.6%), triple infection was 9(1%) and no isolation was 172(19.2%). 3. Isolation of infected etiologic bacteria was showed that Streptococci spp., was 267(31%), Staphylococci spp., was 267(41%), S aureus, was 48(5.6%), G(-) bacillus was 126(5.6%), and Corynebactrium spy. was 52(6%), respectively, from total 861 samples. 4. The results of antimicrobial drug susceptibility of all isolates were showed that Streptococci spp., Staphylococci spp., S aureus, (G)(-) bacillus, and Corynebactrium spp. were susceptible to cefuroxime, cefoperazone, amoxicillin, cefazolin, ampicilin, penicillin, gentamicin, erythromycin, cloxacilin, ciprofloxacin, sulfamethoxasole/trimetoprim, teteracyclin, and norfloxacin (> 70%), but some of them were resistant to neomycin, streptomycin colistin, and cephalothin(> 60). 5. The results of drug susceptibility obtained from each farms had different susceptibility, even though, etiological microorganisms were same in each farms.

1세대 세파계 항생제인 Cefazedone의 MIC Test를 통한 비교 연구 (Comparative Study Using MIC Test for Cefazedone and other First Generation Cephalosporins)

  • The Minimum Inhibitory Concentration (MIC) of a first-generation cephalosporin derivative, Cefazedone (CZD; $PAZERON^R$ inj.) was determined by the two-fold serial agar dilution method. The in-vitro antibacterial activity of CZD against a wide variety of clinical isolates was compared with those of other first generation cephalosporins such as Methylol Cephalexin (CEX), Cefazolin (CEZ), Cefadroxil (CDX), Cephradine (CED), Ceftezol (CTZ) and one of second generation cephalsporin antibiotics, Cefotaxime (CTX). CZD had the most potent inhibitory effect against Gram-positive strains, when compared to the first-generation cephalosporin antibiotics tested in this study and CTX. The geometric MIC mean of CZD for Gram-positive strains was calculated as 0.386 kg/m{\ell}$, and those of CEX, CEZ, CDX, CTZ, CED, and CTX were 6.073, 0.894, 3.399, 0.748, 7.884 and 1.502 $kg/m{\ell}$, respectively. In addition, the geometric mean of CZD for staphylococclJs aureus strains was obtained as 0.340 $kg/m{\ell}$ and those of CEX, CEZ, CDX, CTZ, CED, and CTX 6.145, 0.534, 4.126, 0.442, 10.51, and 2.500 $kg/m{\ell}$, respectively. Against Gram-negative strains, CZD showed better antibacterial activity than CEZ, CDX, CTZ, and CED.

서해안 패류양식장에서 분리된 대장균의 항균제 내성 (Antimicrobial Resistance in Escherichia coli Isolated from Shellfish Farms on the West Coast of Korea)

  • A total of 423 shellfish (Oysters, Crassostrea gigas; short-necked clams, Ruditapes philippinarum and corb shells, Cyclina sinensis) were collected from the west coast of Korea, from which 277 Escherichia coli strains were isolated. The antimicrobial susceptibility patterns of isolated strains were analyzed for 22 antimicrobial agents used in Korea for clinical or veterinary therapy. The resistance of E. coli isolates to ampicillin (37.2%) was highest, followed by cephalothin (21.7%), cefazolin (19.9%), trimethoprim (15.2%). Antimicrobial resistance was present in 56.7% of E. coli isolates against at least one antimicrobial agent. Of 277 isolates, 44 (15.9%) were resistant to multiple antimicrobial agents.

Random Amplified Polymorphic DNA (RAPD) Analysis for Extended Spectrum ${\beta}-Lactamase$ Producing Klebsiella pneumonia Isolated from Clinical Specimens in Korea

  • Klebsiella pneumoniae is the leading cause of nasocomial infection and the most commonly isolated from clinical specimens. $Extended-spectrum-{\beta}-lactamase$ (ESBL) producing K. pneumoniae infection was associated with a significantly longer duration of hospital stay and greater hospital charges. The purpose of this study is to investigate the antibiotic resistant patterns and the DNA fingerprint types of extended-spectrum ${\beta}-lactamase$ (ESBL) producing K. pneumoniae. 223K. pneumoniae strains were collected from three general hospitals with more than 500 beds in Busan, Korea from September 2004 to October 2005. The minimum inhibitory concentration (MIC) of antibiotics was measured using the Gram negative susceptibility (GNS) cards of VITEK (Vitek system, Hazelwood Inc., MO). Random amplified polymorphic DNA method was used to detect DNA fingerprint of the organisms. Of the 226 K. pneumoniae isolates 65 ESBL-producing K. pneumoniae strains were detected by the Vitek system and confirmed by the double-disk synergy test. All the 65K. pneumoniae strains were resistant cefazolin, cefepime, ceftriaxone and aztreonam, and 83.0% of the organisms were resistant to ampicillin/sulbactam, 66.1% to tobramycin, 67.6% to piperacillin/tazobactam, 61.5% to ciprofloxacin, and 47.6% to trimethoprim/sulfamethoxazole and 43.0% to gentamicin. The RAPD patterns were distincted as 10 types by three random 10-mer primers (208, 272, 277). Among ten type patterns, three types (Ic, IIb, IIIe) were remarkably represented at patient of internal department, nerve surgery department, general surgery department, and neonatal room. These results indicate that RAPD can be useful for DNA of strains typing in the epidemiological investigations. Therefore more investigation are needed in order to prevent the ESBL type-producing K. pneumoniae from spreading resistance to oxyimino cephalosphorin antibiotics.

Microbiological Contamination and Sanitary Management of Aquarium Water in the Busan Area

  • This research has been performed by our own investigation, also cooperated with Health and Sanification Division and each of district offices in Busan metropolitan city. After choosing, we collected water samples five times for microbiological examination. As a result of investigating in 160 water samples from urban areas, we could detect 88 cases of Vibrio spp. Furthermore, there were four cases exceeding the acceptable limit of aquarium water (100,00/mL) and another four cases exceeding the limit of Coliform group (1,000 below/100 mL). As a result of investigating that we performed for 271 cases of water samples from coastal areas from April to November, we could detect 130 cases of vibrio species and 10 cases of Coliform group. After performing 17 kinds of antibiotic susceptibility test for 41 cases of isolated Vibrio parahaemolyticus, 27 cases showed tolerance to Amplicllin (AM), all of 31 cases showed intermediate resistance only to Cefazolin (CF) but had sensitivity to the rest of them. As a result of performing antibiotic susceptibility test, serum test and PFGE gene analysis on each 10 pair of Vibrio parahaemolyticus detected concurrently from intake-pipe water and, aquarium water, we couldn't get data showing that they are clearly same species in three kinds of test. In addition, UV sterilization, Ozonization and so on. Based on our research, intake pipe didn't have a problem with microbiological contamination so we are sure that the germ came from supplied fish had caused that kind of contamination. For effective management, UV sterilization or Ozonization which can be handled consistently should be adopted in aquarium.

Characterization of Extended-Spectrum-$\beta$-Lactamase Genotype TEM, SHV and CTX-M from Clinical Isolates of Klebsiella pneumoniae and Comparison with Antibiotic Susceptibility Test

  • Resent studies have reported increased isolation of extended-spectrum $\beta-lactamase$ (ESBL) producing strains at several hospital in Korea. We studied to investigate the isolation rates of ESBL strains from clinical isolates of Klebsiella pneumoniae and to characterize differences in types using analyses of genotyping and antibiotic susceptibility test. Antibiotic susceptibility test with confirmation of ESBL by double disk synergy test was performed on the 54 ESBL strains of Klebsiella pneumoniae from a hospital in Busan. Transfer of resistant gene in ESBL strains resistant to 3rd generated antibiotics was confirmed by transconjugation test using E. coli $RG176^{nal(r)}$. blaTEM, blaSHV, blaCTX-M genes were detected by PCR. ESBL producing strains had 100% of resistant rate to ampicillin, azteronam, cefazolin, cefepime and ceftriaxone ($\beta-lactam$ antibiotics). Forty strains of bla TEM$(74\%)$, 41 strains of bla SHV $(76\%)$, 23 strains of bla CTX-M $(43\%)$ were found, respectively. The strains had one or more genes. They had high resistant rates to $\beta-lactam$ antibiotics including cephalosporin. The resistant rates of strains with multiple resistant genes were higher than those of strains with single resistant gene.
