Cochlear Implant Failure in the Pediatric Population

  • Background and Objectives: In cochlear implant (CI) surgery, the results and causes of revision and reimplantation may guide surgeons in establishing surgical protocols for revision surgery with safe audiological outcomes. The aim of this study was to review our experience in terms of etiology, surgical strategy, and hearing outcomes in pediatric patients who underwent CI removal and reimplantation. Subjects and Methods: All patients received implants of the same brand. Pre and postoperative Categories of Auditory Performance score and aided free-field pure tone audiometry thresholds were noted. In vivo integrity tests were performed for each patient and the results of ex vivo tests of each implant were obtained from manufacturer. Results: A total of 149 CIs were placed in 121 patients aged <18 years. The revision rate in children was 6.7% (10/121 children). Six patients had a history of head injury leading to a hard failure. The causes of reimplantation in others were soft failure (n=1), electrode migration (n=1), infection (n=1), and other (n=1). All patients showed better or similar postreimplantation audiological performance compared with pre-reimplantation results. Conclusions: It is very important to provide a safe school and home environment and educate the family for reducing reimplantation due to trauma. Especially for active children, psychiatric consultation should be continued postoperatively.

  • This research paper is to analyze the specific situation for the 157 failure cases of Seoul Metro power cable since 1990 and to consider the countermeasures. This is, also, to prevent accidents by analyzing the current state of each Line, the failure of causes, railroad tracks, installation year, opening year, and monthly failure conditions and by presenting the countermeasures. In addition, this research is to discuss the present installation situation of Seoul Metro power cable, the types of power cable, symptoms, causes and the countermeasures.

인공호흡기 치료를 받은 흉부외상 환자의 임상적 고찰 (Clinical experience of ventilator therapy in chest trauma)

  • A clinical evaluation was performed with a population of 49 patients of chest trauma, who were diagnosed to undergo ventilator therapy, and had gone through ventilator therapy at the Department of Thoracic and Cardiovascular Surgery, Kyungpook University Hospital. One of most common causes of chest trauma was vehicle accidents [77.5%] with the prevalent age group being their forties. The common findings were multiple rib fractures [89.8%], hemopneumothrax [81.6%], lung contusion [61.2%] and flail chest [44.9%]. Their common combined injuries were the orthopedics and neurosugical injuries [86.7%]. Complications caused by chest trauma were pneumonia, respiratory failure, atelectasis, barotrauma and empyema. Pulmonary infections were commonly associated with mechanical ventilation in the long term group and were best prevented by using bronchial hygiene therapy.The mortality rate was 5.8% of the total patients and that was 38.8% of the patients, who needed ventilator therapy. The causes of death were pneumonia, respiratory failure, acute renal failure and hypovolemic shock. Mechanical ventilation has an important place in the treatment of patients with severe chest trauma.

  • To guarantee 20-25 years to the lifetime of the PV modules without failure, reliability test of the module is very important. Field-aged test of the outdoor environment is required. However, due to time constraints, accelerated testing is required to predict the lifetime of PV modules and find causes of failure. Failure is caused by many complex phenomena. In this study, we experimented two accelerated tests about corrosion and fatigue, respectively. First, temperature cycling test for fatigue were tested and Coffin-Manson equation was analyzed. Second, damp heat test for corrosion were tested and Eyring equation were analyzed. Finally, using two-mode failure model, we suggest a new lifetime model that analyze the phenomenon by combining two kinds of data.

FMEA에서 고장 심각도의 탐지시간에 따른 위험성 평가 (Risk Evaluation in FMEA when the Failure Severity Depends on the Detection Time)

  • The FMEA is a widely used technique to pre-evaluate and avoid risks due to potential failures for developing an improved design. The conventional FMEA does not consider the possible time gap between occurrence and detection of failure cause. When a failure cause is detected and corrected before the failure itself occurs, there will be no other effect except the correction cost. But, if its cause is detected after the failure actually occurs, its effects will become more severe depending on the duration of the uncorrected failure. Taking this situation into account, a risk metric is developed as an alternative to the RPN of the conventional FMEA. The severity of a failure effect is first modeled as linear and quadratic severity functions of undetected failure time duration. Assuming exponential probability distribution for occurrence and detection time of failures and causes, the expected severity is derived for each failure cause. A new risk metric REM is defined as the product of a failure cause occurrence rate and the expected severity of its corresponding failure. A numerical example and some discussions are provided for illustration.

  • The service life of tools in metal forming technology is to a large extent limited by wear and fatigue fracture of the active elements. This presents a basic request for tool cost minimization and reduction of extensive machine down time, caused by premature tool failure. Currents developments are dominated by steps to reduce the causes of tool failure. A main problem of forming technology remains the insufficient reliability of tools due to a large and incalculable life time fluctuation. Only a systematic investigation of the failure mechanisms and operational loading of tools can lead to future improvements in tool layout, that is optimization of tool usage.

  • FMEA(Failure Mode and Effect Analysis) is a failure analysis method for the system to identify the potential failure modes, and their effects and causes to reduce or mitigate the critical effects of the system. FMEA for railway was introduced with reliability of railway system, and this was used for identifying and analysing the possible hazards qualitatively to meet the requirements in early stage of business. In general, the overall failure data of system could be managed from design stage by FMEA, and also the countermeasures to prevent many predicted failures could be established by identification of failure modes and assement of failure effects by FMEA. Using these advantages of FMEA, the effectiveness of reliability improvement could be expected if FMEA is applied continuously in operation stage. It is essential that railway vehicles are maintained with high level of safety and reliability not to happen any failures in operation. This paper is proposed the proper FMEA for maintenance of railway vehicles compared with existing FMEA.

  • Nuclear and thermal power plants must provide the turbines with an appropriate degree of high temperature and high pressure steam, to produce the optimum electricity. Additionally, in the event of system and power system failure during electrical production, the steam is immediately disabled, to protect the turbines and generators rotating at high speed. The plant thus uses a special steam control valve system for turbine control, which is opened by force of the hydraulic servo actuator and closed by a large steel spring force. In this study, the causes of failure of the turbine control valve system, a key device of the power plants, were analyzed, and the causes of failure were improved relative to reliability of the equipment.
