요양보호사의 노동인권에 관한 고찰 (Labor Human Rights for Care Workers)

  • 2007년 노인장기요양보험법이 제정되었고 이 법은 질병 및 장애가 있는 노인들에 대한 요양비용을 사회구성원들이 함께 부담하는 체계를 구축하여 노인 및 그 가족구성원의 삶의 질을 향상시키는데 그 목적이 있다. 노인장기요양보험제를 실현하기 위해 요양보호사제도가 마련되었다. 요양보호사란 거동이 불편한 노인을 시설에서 돌보거나 또는 노인이 거주하는 가정을 방문하여 보호활동을 벌이는 사람들을 말한다. 그런데 최근 발표된 국가인권위원회의 조사에 의하면 요양보호사들의 근무환경이 매우 열악한 것으로 드러났다. 저임금, 포괄임금의 남용, 장기간 근로, 인력배치기준 및 휴게시설 미비, 요양보호서비스 이외의 노무제공, 수급자에 의한 성희롱 등의 문제가 있는 것으로 나타난 것이다. 인구의 고령화가 빠른 속도로 진행되고 있고 노인들에 대한 요양보호의 중요성과 요양보호사들의 역할이 증대되고 있는 상황을 고려한다면 이들의 업무환경에 대한 개선이 절실히 필요하다. 이러한 견지에서 이 논문은 국가인권위원회가 발표한 자료를 바탕으로 노동인권의 측면에서 요양보호사들이 겪고 있는 문제점들을 살펴보고 효과적인 개선방안에 대해 논의 해본다. 결국 요양보호사들이 직면한 문제들을, 정부의 철저한 관리 감독과 요양보호사의 처우 개선 그리고 수급자의 인식 개선 등을 통해 바로잡는다면 더 나은 노동환경에서 요양보호사들이 질 높은 서비스를 제공할 수 있을 것으로 기대한다.

호스피스 자원봉사활동의 실태 및 지원에 관한 연구 - 대전ㆍ충청권을 중심으로

  • This study was conducted to examine the present state, types of services, difficulties, education programs, motivation, satisfaction and attitude of hospice volunteers and also to suggest supporting programs for the volunteer. For these purposes, a questionnaire was distributed among the 200 hospice volunteers in ten hospice organizations in Daejeon and Chungcheong province. In this research, 85.5% of the respondents were women and most of them were housewives in their forties and fifties. The majority of hospice volunteers had an education background of more than high school. Christianity (94%) was the dominant religion of volunteers. Among the service area of the hospice, emotional caring, such as listening closely and establishing empathy were the most frequently done by the respondents and the next one was spiritual care. Hospice volunteers who had started with altruistic motivation show more dynamic activities in emotional, spiritual, physical caring and so on, than those with non-altruistic motivation. Most of the respondents expressed that they felt a hospice volunteer was valuable. However they had a hard time to overcome the sorrow coming from sharing empathy with the dying patients. Those who had started with altruistic motivation usually took part in two times per a week and had more positive attitude. Volunteers who had been working more than five years showed better attitude than those who had been working less than three years. The longer the voluntary services, the more positive attitude experienced by volunteers. Hospice volunteers encountered the most considerable difficulty with lack of background knowledge and their actual capability. Insufficiency of time was the next reason for the considerable difficulty of the hospice volunteer. On the other hand, those who served more than twice per week replied that lack of background knowledge and their capability were the least considerable difficulty. Insufficiency of time was the reason of difficulty for male volunteers, as well as for females under thirty and those in their forties and fifties respectively. Generally most of the hospice volunteers (70%) were satisfied with their services and they usually satisfied with physical caring, spiritual caring, hospice's family caring. The less difficulty in voluntary services, more satisfaction for the volunteers in the end. There is a positive correlation between a attitude of the hospice volunteer and frequency of voluntary services while the volunteers' difficulty affect negatively their attitude as well as the frequency of voluntary services. Most of the hospice volunteers want to have more education about basic skills, volunteer's attitude and role, spirituality, the way of attending on the hospice and so on. Most of them considered discussion and sharing different cases as the most effective method. They also wish to had more supporting programs for the hospice volunteers(in the order of their needs) such as regular events, newsletters, personal concern, social meetings among the volunteers, insurance and minimum expenses. Based on the study results, more programs should be run in order to activate the voluntary services regardless of their gender and education background. A continuous practical supporting policy and education programs are required in order to provide special education and training courses considering every field of voluntary services. Expanding the role of the volunteers allows them more opportunities to take part in voluntary services and th activate participation. It is necessary to establish a new hospice system as a part public medical system, which can not only facilitate the voluntary services for a hospice but also enhance professional hospice volunteers. Finally, experts are needed to operate the voluntary services effectively.

울산시 1인가구의 가족가치관 및 돌봄노동에 대한 사례연구 (Family Values and Caring Work of Single-Person Households in Ulsan: A Case Study)

  • 본 연구의 목적은 1인가구의 변화하는 가족가치관과 매일 일상 속에서 이뤄지는 돌봄노동에 주목하여, 가족의 변화나 역동성을 파악하고 이에 대응하기 위한 제도 및 정책적 과제를 제안하는 것이다. 연령·혼인상태를 고려하여 울산시에 거주하는 1인가구 11명을 선정하였으며, 1인가구의 전반적인 일상생활, 가족가치관, 가사노동 및 돌봄 특성을 분석하였다. 이에 따른 울산시 1인가구의 특성은 자신의 삶을 유지하고 살아감에 있어 '단순하지만 불안정함'이 공존하고 있었으며, 가족가치관은 다양성에 대한 인식과 실제의 격차를 보이는 양면적 특성을 보이고 있었다. 또한 가족생활을 함에 있어 '원가족 돌봄 및 관계', '경제적 요인' 등에 대한 어려움을 호소하고 있었으며, '이중가치체계'가 두드러지게 나타났다. 마지막으로 1인가구는 안정된 삶을 위해 전통적인 가족형성의 방법에만 국한하는 것이 아닌, 고립·고독감 해소 및 관계 회복을 위해 사회관계망을 형성해나가고 있는 것으로 나타났다. 이에 따라 '가족인식', '가족생활', '가족 및 사회적 관계'의 각 영역에서 변화가 도모되었을 때 가족변화에 대응할 수 있을 것으로 보이며, 연구 결과에 기반한 정책과제를 도출하였다.

지역아동센터 종사자의 업무 역량이 아동서비스의 질에 미치는 영향: 자기효능감의 매개효과 (Effects of Working Competency of Local Child Centers on the Quality of Service for Children: The Mediation Effect of Self-Efficiency)

  • 본 연구는 지역아동센터의 돌봄 서비스가 공공성을 강화하고 좋은 품질의 아동서비스를 제공하는 기관으로 자리매김하는 것이 중요하다는 인식에서 시작되었다. 특히 종사자의 업무역량은 중요한 요인이며, 아동서비스의 질은 지역아동센터의 중요한 평가지표로 작용하고 있다. 본 연구의 목적은 지역아동센터 종사자의 업무역량이 아동서비스의 질에 미치는 영향을 확인하고 자기효능감이 업무역량에 매개하여 아동서비스의 질에 미치는 영향을 검증하데 있다. 자료의 분석방법은 위계적 회귀분석을 실시하였으며, 자기효능감의 매개효과를 알아보기 위해 Barron & Kenny 방법을 사용하여 분석하였다. 또한 자기효능감의 매개효과 유의성을 검증하기 위하여 sobel test를 실시하였다. 본 연구의 결과 첫째, 지역아동센터 종사자의 업무역량 수준에 따라 아동서비스의 질이 달라질 수 있는 것으로 나타났다. 둘째, 지역아동센터 종사자의 업무역량이 아동서비스의 질에 미치는 영향에서 자기효능감의 매개효과가 부분매개하는 것으로 나타났다. 연구결과는 종사자의 업무역량 강화를 통해 아동서비스의 질을 개선할 수 있으며, 종사자의 자기효능감을 높임으로서 아동서비스의 질이 향상됨을 알 수 있다. 지역아동센터 종사자의 업무역량과 자기효능감을 강화하여 좋은 품질의 서비스를 제공하려는 노력이 필요하며, 이를 통해 지역아동센터가 돌봄 서비스의 중추적인 역할을 할 수 있을 것을 기대된다.

  • 본 연구는 최근 우리사회에서 중요한 화두가 되고 있는 저출산 고령화 대책, 사회적 일자리 창출 등의 영향으로 사회적 주목의 대상이 되고 있는 사회서비스 정책의 동향을 살펴보고, 거시적인 관점에서 대안을 제시하는데 목적을 두고 있다. 현재의 사회서비스 정책은 중앙과 지방의 단절, 최소생활보장 장치의 미흡, 서비스 일자리와 취약한 서비스 대상자에 대한 균형의 결여 등이 문제로 제기되었다. 이에 대하여 이용자의 선택과 참여를 증진하는 수단으로서의 시장기제의 도입과 시장의 부정적인 영향을 최소화할 수 있도록 하는 견제장치로서의 반시장기제의 통합을 대안으로 제시하였다.

  • Respect for human life and respect for human rights are basic values which the organized nursing profession has urged its members to adhere to in their service to human beings. This study was designed and carried out to identify ways to reconcile often conflicting basic values in practice. This study focused on ethical dilema experienced by nurses who were caring for political offenders in prison. Concrete case study was presected to show solutions to the problems.

가족가치관과 돌봄노동지원정책 욕구의 관련성 연구 (A Relation between Family Values and Needs for Care-Support Family Policy)

  • Traditional familism and family value is known as the value that most Koreans share with. Strong family solidarity and family-centered perception among Koreans influences other social values and ideology. Under the family value, caring for family members is family responsibility instead of government responsibility. Previous studies argued that the family value played a role to impede the development of family policy in Korea. The aim of this study was to explore a relation between the family value and the needs for care-support family policy. This study investigated how the family value were related to the specific needs for care-support family policy. The data were drawn from the Seoul Families Survey conducted on 2006 by Seoul Women and Family Foundation. The survey data consisted of 2,500 married males and females living in Seoul. The statistical techniques used for analysis were frequencies, means, t-test, ANOVA, crosstabs, multiple regression models, and multinomial logit models. The major findings of this study were as followings. First, while the traditional familism appeared to be held at a certain level, the general attitudes towards cohabitation, divorce, and single-parent family seemed to be less traditional. Second, the familism was found to be partly associated with the needs for the care-support family policy. The respondents who had less traditional value on arriage and child-rearing showed the higher level of needs for daycare center. This finding implied that nontraditional attitudes were related to the needs for an alternative care service such as caring through facilities rather than to the needs for supportive or complementary services. Lastly, the respondents who had higher level of traditional familism showed a higher preference for direct economic service (supportive service) than for other types of service in child care. And the less traditional their attitudes towards marriage and child-rearing, the more likely they are to prefer flexible child care services and programs to other types of child care services. These results implied that the family value was partly influential to family policy. However, it is worthy to note that the family value was related to family policy preference rather than to family policy needs. In other words, traditional family value appeared to influence the types of family policy rather than the level of needs for family policy.

우리나라 요양시설 입소노인 주 돌봄자의 우울에 대한 융복합 연구 (A Convergence Study on Depression of Main Caregiver for Elderly in Korean Nursing Home)

  • 본 연구의 목적은 우리나라 요양시설 입소노인의 주 돌봄 제공자의 우울 정도를 조사하고 이에 영향을 미치는 관련요인을 파악하기 위해 시도되었다. 연구방법은 서술적 조사연구로, G도에 위치한 5개의 요양시설 입소노인의 주 돌봄자를 대상으로 시행되었으며 수집된 자료는 SPSS/WIN 25.0 progiam을 이용하여 208명의 설문지를 분석하였다. 연구결과 요양시설 입소노인 주 돌봄자의 우울에 차이를 보인 특성은 확대가족인 경우, 요양시설 입소 기간이 5년 미만이며 입소 결정자가 가족이며 입소비용이 많고, 요양시설까지의 거리가 1시간 이내, 요양시설 서비스에 만족하지 못할 때 우울 정도에 유의한 차이를 보였다. 따라서 향후 요양시설 입소노인 주 돌봄자의 우울 경감을 위한 간호중재 프로그램의 개발과 적용에 유용한 기초자료가 되리라 본다.

노인성 치매환자를 간호하는 가족의 부담감에 대한 연구 (A Study on the Care burden of Family Caregivers for Senile Dementia)

  • The objective of study is to investigate the degree of burden of family who care for the elderly with dementia and to find out the factors related to the family burden as a pilot study. The number of subjects was total 137 and they were the families who were caring for elderly with dementia. The data was analyzed by SPSS/PC, t-test ANOVA. The results were as follows: 1. The score of family burden who were caring for elderly with dementia was 2.90 and the score of burden was high in case of the limitation of indivisual social activities. which was 3.09 and the lowest score was in case of the health of physical and limited which was 2.58 2. There was the correlation the level of dementia and the degree of burden of family who care for the elderly with dementia in character of dementia (p<0.05). 3. There was the correlation between the abnormal activities of the elderly with dementia and the degree of burdens of family (r=0.330). 4. There was the correlation between ADL of the elderly with dementia and the degree of burdens of family (r=-0.330). 5. The service duration had statistically significant correlation with burden In the character of family (P<0.05). 6. The using facility had statistically significant correlations with burden in the character of socity (P<0.05).

아동간호의 본질적 토대와 사명에 관한 논고(論考) (A Critical Discussion on the Academic Fundamentals and the Missions of Child Health Nursing)

  • Purpose: To reilluminate academic fundamentals and missions of child health nursing (CHN). Methods: Critical review of literature. Results & Conclusion: The academic fundamentals of CHN were analyzed for three different basis; philosophical, theoretical, and legal & ethical basis. The philosophical basis of CHN was summarized as six beliefs; A child is an important human resource and a valuable asset for future society; A child should be respected as a unique and dignified human being; A child has his/her own unique developmental needs; A child is a vulnerable client and should be advocated for; Atraumatic care should be provided to each child; Child health care should be family-centered. The essence of the theoretical basis were reilluminated into caring theory and client advocacy theory. The legal basis of CHN was stated as pertaining to the various child-related laws and international conventions, such as UN Convention on the Rights of the Child. The ethical basis were stated as 4 principles of biomedical ethics and The UNESCO Universal Declaration on Bioethics and Human Rights. The mission of the CHN was stated and the role of CHN was described as one who is a child rights advocator, professional caring service provider, policy maker, health educator, researcher.
