1. Outline of the Study A boom of baby hotels is taking place in Japan. Baby hotel means day care centers that renders services day and night for babies of working mothers. These baby hotels are thriving because many mothers welcome the system because of their needs. However, accidents occurred in a row recently and therefore baby hotel has suddenly become a social issue. According to the ministry of health welfare, there are 587 baby hotels in Japan accommodating about 10,000 children and they are mostly located around amusement centers. The reasons that these baby hotels thrive in spite of the fact that there are 22,000 day care centers in Japan are : (1) shortage of day care centers compared to the needs (2) most centers do not take in babies below 1 year. This study aims to describe the present status of the baby hotels and the background of the phenomena that made it possible for them to thrive with no permission from the government. This study mostly deals with the phenomena in Tokyo. 2. Day Care Policy in Japan and the Problems of Baby Hotels The present review deals with the problems from the following 6 dementions. (1) the shortage of day care centers (2) inadequate system for the care of babies (3) inadequate daily hours of day care centers (4) Unflexable enrollment system of day care centers (5) imperfect enfollment process & guidelines (6) lack of alternative institutions 3. Result The Ministry of Health & Welfare affered the following ways to cape with the problems of baby hotels : (1) restraint of the baby hotels (2) utilization of existing day care centers for the care of babies. In my opinion, these suggestions seem too superficial in solving the problems. I would rather suggest that the Japanese government review the whole system of child welfare administration. The Child welfare Law of Japan made a clear statement in its article 24 that, "if day care centers are not available, alternative institution for proper care of children must be sought for." My suggestions for the alternative care are : (1) public nursing rooms (2) family day care (3) extention of kindergarten hours (4) foster family care.

The purpose of this study was to analyze the work of visiting health care providers in public health centers. A descriptive analysis of self-records for work data from 875 visiting health care providers working in 242 public health centers in South Korea was done. The results of the study are as follows: 1. The number of households for each visiting health care provider ranged from under 100 households to over 500 households. 2. Low performance was found for several items on the work list for visiting health care providers. 3. There were significant differences in the work performance of visiting health care providers between nurses and nursing assistants. 4. There were significant differences in the work performance of visiting health care providers according to region. In conclusion. work performance of visiting health care providers was low and differed according to type of occupation and region. This study suggest the need for further studies which analyse the quality of visiting health care providers and services, and the visiting health care system.

The purpose of this study is to derive policy implications in allocating day care centers for the elderly with more efficiency and/or equity by calculating and analysing the accessibility scores of individual dongs to day care centers for the elderly in Seoul using GIS spatial analysis techniques. The study finds that the spatial distribution of the centers does not respond to the distribution of the potential users very well and that dongs in Gangseo-gu and Songpa-gu at the outskirt of Seoul has the lowest level of accessibility. The findings of the study has policy implications in the management of day care centers for the elderly. First, in order to improve the accessibility of the elderly to the centers, an increase in the number and the capacity of the centers needs to be made especially in areas with lower level of services provided. Second, if policy decision is made in the way to increase the capacity of the existing centers rather than to increase the number of centers due to, for example, the budget limit, capacity expansion needs to be made in the centers with higher proximity in order for more elderly people to use the centers more frequently with easy access. Finally, this type of accessibility analysis techniques needs to be used to allocate, expand, and evaluate other types of care facilities for the elderly and social welfare facilities in order to preserve the welfare right of the users of the facility who usually have a lower mobility and to assure the necessity of the resource investment.

This study is conducted to compare how the Western.Oriental medical practitioners and health professionals perceive reciprocal development strategies respectively of Western.Oriental Medicine in Korea. A total of 3,273 persons were questioned by direct distribution, e-mail, and mail with a self-developed questionnaire. Of those questioned, 362 responded (the response rate of 11.1%), and of them n persons (Western medical practitioners 206, Oriental medical practitioners 90, and health professionals 64) were analyzed with SPSS for Windows. The results were as follows: The need for legislation on the mutual development system for Western.Oriental Medicine was recognized by 66.9% (218 persons) of respondents. Among them, largest group who agreed with this need was the health professionals. Western medical practitioners chose the "the difference of approach methods on the diseases between Western.Oriental Medicine" as their response, and Oriental medical practitioners & health professionals selected "the indifference or bias of Western medical practitioners"as the reasons for the inactivity in developing a mutual system of Western.Oriental Medicine. Therefore, Western medical practitioners and health professionals selected the category of "the reformation of educational system", while Oriental medical practitioners selected the category of "the activation of joint research on Western.Oriental medical care" as the most important condition for setting a precedent. Also, Western medical practitioners preferred "cooperative health care system for Western medical care supported by Oriental medical care", but Oriental medical practitioners and health professionals preferred “cooperative health care system of Western.Oriental medicine on equal terms" In conclusion, Western '||'&'||' Oriental medical practitioners have to make every effort to close the gap between differing views through mutual understanding and respect if joint research of Western.Oriental medical care is to become a reality. Background: This study aimed to analyze the impact of community health care resources on the place of death of older adults with dementia compared to those with cancer in South Korea, using public administrative big data. Methods: Based on a literature review, we selected person- and community-level variables that can affect older people's decisions about where to die. Data on place-of-death and person-level attributes were obtained from the 2013 death certification micro data from Statistics Korea. Data on the population and economic and health care resources in the community where the older deceased resided were obtained from various open public administrative big data including databases on the local tax and resident population statistics, health care resources and infrastructure statistics, and long-term care (LTC) insurance statistics. Community-level data were linked to the death certificate micro data through the town (si-gun-gu) code of the residence of the deceased. Multi-level logistic regression models were used to simultaneously estimate the impacts of community as well as individual-level factors on the place of death. Results: In both the dementia (76.1%) and cancer (87.1%) decedent groups, most older people died in the hospital. Among the older deceased with dementia, hospital death was less likely to occur when the older person resided in a community with a higher supply of LTC facility beds, but hospital death was more likely to occur in communities with a higher supply of LTC hospital beds. Similarly, among the cancer group, the likelihood of a hospital death was significantly lower in communities with a higher supply of LTC facility beds, but was higher in communities with a higher supply of acute care hospital beds. As for individual-level factors, being female and having no spouse were associated with the likelihood of hospital death among older people with dementia. Conclusion: More than three in four older people with dementia die in the hospital, while home is reported to be the place of death preferred by Koreans. To decrease this gap, an increase in the supply of end-of-life (EOL) care at home and in community-based service settings is necessary. EOL care should also be incorporated as an essential part of LTC. Changes in the perception of EOL care by older people and their families are also critical in their decisions about the place of death, and should be supported by public education and other related non-medical, social approaches.
This study aimed to analyze the trends of studies on estimation of cost for early childhood education and child-care services provided by both the kindergarten and child-care centers. A total of 21 papers including academic journals and policy research reports were collected. The papers were analyzed with the annual trends, research contents, and estimation methods of cost for early childhood education and child-care services. The main results of this study were as follows. Firstly, the studies on estimation of cost for early childhood education were performed earlier than the ones on estimation of cost for child-care services. The research contents were composed of analyzing operating expenses and estimating cost for early childhood education and child-care services. Secondly, there were similarities in the estimating processes of cost for early childhood education and child-care services, but the definition of operating expenses used for the estimation of cost differed. Thirdly, there were differences in the items of operating expenses inputted to estimate cost and in the calculation procedures of items in studies. Finally, based on the results, this study suggested what future studies and policy should consider in the estimation of cost for early childhood education and child-care services.
Although providing universal coverage for health care through the National Health Insurance(NHI) is a remarkable achievement, the issue of limited benefit coverage of the NHI has been at the core of national debate over how to improve its coverage. This study aims to evaluate benefit extension strategies and implemented policies with regard to the NHI since 1989 using 'policy window theory' proposed by John W. Kingdon. Understanding problem stream, policy stream, political stream, and coupling streams regarding the NHI, in particular benefit extension, would contribute to broaden policy debates and to develop more effective strategies for the future. Historically, political stream had opened policy window in the past two decades and policy streams can be characterized by three waves. Three streams have been coupled since 2003 and the government had a strong will to fulfill better performance of NHI coverage. Study findings indicate that identification of problem structure regarding NHI benefit was not connected with policy stream tightly. In addition, there has been limited discussion on policy goal and principles for extension coverage of the NHI. Policy strategies to improve coverage of the NHI should be linked to characteristics of problem and sought solutions under the principle which is expected to be sustainable through consensus in the society.

Purpose: The purpose of this study was to identify manpower acquisition strategies of nurse and nurse aide in long-term care Method: The study was proceed using following three stages: to explore workforce status and labor environment with database analysis, literature reviews, to explore the best practices, practical experiences with expert interviews, with focus group interview, to identify problems and present acquisition strategies Results: Acquisition strategies were increasing of pay, improvement of working condition, development of practical educational program, diversification of entry paths, establishing clear roles for nursing staffs, diversification of training program. Conclusion: We found that the acquisition strategies of nurse and nurse aide can be as an useful methods to increase manpower in long-term care for higher quality of services and care.

Korean health insurance has adopted preliminary DRG payment system through 8 DRGs from 1997. But present DRG payment system gives economic incentives for hospitals to hire less nurse. This study was attempted to develope DRG adjust index to differentiate DRG price by nurse staffing level for nursing care quality. Method: We analyzed inpatient care cost by medical institute and developed DRG adjust index to differentiate DRG price by nurse staffing level. Results: Among same medical institute, inpatient care cost are very different according to hospital's nurse staffing level. In the case of casarean section, inpatient care cost of the 1st grade general hospital are more expensive 85,732won than the 6th grade hospital. The cost difference are 8.24% of total casarean section DRG price and 16.48% of DTG variable price. We developed DRG adjust index-a to apply DRG variable price and index-b to apply DRG total price for compensation cost difference of hospitals. Conclusions: DRG price adjust index will give economic incentive for hospitals to hire more nurse and improve nursing care quality.
Purpose: This study was conducted to explore the functional status and long-term care services for the community-dwelling low-income elderly. Method: A descriptive research design was used in this study. The functional status of the participants was obtained using Minimum Data Set-Home Care Version 2.0 and the long-term care services were identified via Michigan's choice. Total of 154 persons aged 65 years or older completed Korean Minimum Data Set-Home Care Version 2.0 on the community dwelling low-income elderly. Results: The average of Activities of Daily Living was 4.19, and the range was 0-55, while the average of Instrument of Activities of Daily Living was 4.85 and the range was 0-56. Among the subjects, 46.1% belonged to the Information and Referral group and 1.3% to the Nursing Home group. Severe daily pain was reported by 14.9%, and 76.6% of the participants had impaired vision. The Activities of Daily Living was difference according to living with, education, vision, and depression. The long-term care services differed according to gender, pain, vision, hearing, and depression. Conclusion: The support policy for the elderly needed to focus on impaired visual and depression to enhance the activities of daily living. Moreover, there is a need for the Information and Referral group to arrange and develop nursing intervention resources.
