Service Quality beyond Access: A Multilevel Analysis of Neonatal, Infant, and Under-Five Child Mortality Using the Indian Demographic and Health Survey 2015~2016

  • Purpose: The purpose of this study was to derive contextual indicators of medical provider quality and assess their relative importance along with the individual utilization of antenatal care (ANC) and institutional births with a skilled birth attendant (SBA) in India using a multilevel framework. Methods: The 2015~2016 Demographic and Health Survey (DHS) from India was used to assess the outcomes of neonatal, infant, and under-five child mortality. The final analytic sample included 182,980 children across 28,283 communities, 640 districts, and 36 states and union territories. The contextual indicators of medical provider quality for districts and states were derived from the individual-level number of ANC visits (<4 or ${\geq}4$) and institutional delivery with SBA. A series of random effects logistic regression models were estimated with a stepwise addition of predictor variables. Results: About half of the mothers (47.3%) had attended ${\geq}4$ ANC visits and 75.8% delivered in institutional settings with SBAs. Based on ANC visits, 276~281 districts (43.1~43.9%) and 13~16 states (36.5~44.4%) were classified as "low" quality areas, whereas 268~285 districts (41.9~44.5%) and 8~9 states (22.2~25.0%) were classified as "low" quality areas based on institutional delivery with SBAs. Conditional on a comprehensive set of covariates, the individual use of both ANC and SBA were significantly associated with all mortality outcomes (OR: 1.17, 95% CI: 1.08, 1.26, and OR: 1.10, 95% CI: 1.02, 1.19, respectively, for under-five child mortality) and remained robust even after adjusting for contextual indicators of medical provider quality. Districts and states with low quality were associated with 57~61% and 27~43% higher odds of under-five child mortality, respectively. Conclusion: When simultaneously considered, district- and state-level provider quality mattered more than individual access to care for all mortality outcomes in India. Further investigations are needed to assess the importance of improving the quality of health service delivery at higher levels to prevent unnecessary child deaths in developing countries.

The influence of attitudes toward culture and experiences of cultural exchanges on the multi-cultural awareness in health science college students (보건계열 대학생의 문화 태도, 문화 교류가 다문화 인식에 미치는 영향)

  • The purpose of current study was to examine how the attitudes toward culture and the experiences of cultural exchanges could influence on the multi-cultural awareness in health science college students in South Korea. Convenience sampling was conducted for data collection; thus, total 407 health science college students of ${\bigcirc}{\bigcirc}$ University in K province were recruited and asked to complete the survey for this study. As a result, those who had the experiences of contacting immigrants, had the experiences of contacting immigrants via the school activities, and took the classes related with multi-culturalism showed high multi-cultural awareness. In addition, attitudes toward culture and experiences of cultural exchanges showed positive relationship with a multi-cultural awareness. At last, based on the regression analysis, attitudes toward culture and experiences of cultural exchanges showed significant influences on the multi-cultural awareness in health science college students in South Korea. Health science students could play important role in immigrants' physical and psychological health care in the future. Based on the findings of this study, we strongly suggest to provide proper education and various activities of multi-culturalism to those students. It would lead to a high quality of care and firm health care system for the immigrant population residing South Korea.

  • As modern people's needs to be healthy and beautiful are increasing, medical skin care industry is growing both in quantity and quality. Therefore, this study investigated job demand and the moderating effect of job resource in employees who provided medical skin care services and the effects of job demand and job specialty on job satisfaction. The subjects of the study were medical estheticians in Gwangju City and Jeonnam Province and were interviewed with the use of a questionnaire. The data was analysed using SPSS 21.0 and a frequency analysis, a factor analysis, a reliability analysis, a correlation analysis were conducted and the results are presented as follows: The results are presented as follows: First, as for job demand, only cooperation had the moderating effect of job resource. Second, when the effect of job resources on medical skin care specialty was inspected, it was discovered that cooperation and equipment application had a positive effect on the specialty. Third, when the effect of job demand and specialty on job satisfaction was inspected, it was discovered that job tension had a negative effect on satisfaction and workload and specialty had a positive effect on satisfaction.

Childcare Policies In Korea (우리나라의 보육정책)

  • As a childcare program of TaeHwa Christian Women's Institution in 1921, the childcare system in Korea was incepted. Since then, the political foothold of childcare system has steadily been advancing to provide high quality services to young children. In almost a hundred-year-history of public childcare in Korea, depending on the changes enforced on the related laws and regulations and varying perspectives over time, the administration office accountable for childcare policies has been authorized to the Ministries of Health, Social Affairs, Education, Labor, Home Affairs, Rural Development Administration, and/or others. But as of 1991, under the enactment of Infant and Child Care Act, it was changed to be administered by the unified authority of the Health and Social Welfare Ministry. Then, in 2004 and 2007, its statutory authority, respectively, transferred to the Ministry of Gender Equality and Family and back to the Ministry of Health and Social Welfare. Staring of the Infant and Child Care Act in 1991, Korean childcare policies have been managed by the dual systems of the Education Ministry and the Health and Social Welfare Ministry each holding jurisdiction over kindergartens and childcare centers, respectively. Faced with the recent marked decline of birth rate, diverse childcare policies are currently implemented in the pursuit of finding means to enhance the quality of childcare and to develop policies for the restoration of the low birth rate. This study presented distinct features of current childcare policies and discussed about future directions and challenges of these policies.

  • Personal Health Record(PHR) service can be helpful to patients with diseases requiring strict everyday care and medical treatment, such as diabetes or cancer. In this paper, we propose a PHR system specialized in collecting and analyzing health record data of cancer patients, and present the process of how the system can improve the efficiency of cancer treatment process. Through the smart device application, cancer PHR system obtains daily PHR data which is highly related and critical to cancer therapy. The analysis report is provided to the medical staff with an available format suited for Electronic Medical Record used at medical institution. With the final result of PHR analysis which is easily merged with medical chart, most efficient Chemotherapy treatment can be provided for the patients. Also it is possible for the patients to give the information of side-effect and other pain experience during therapy to their doctors without loss of information. The proposed PHR system has the effect of improving the quality of patient care by allowing the medical staff to acquire the main objective data necessary for drug prescription and medical care benefits.

  • It is the purpose of this study to address how to manage the urban vegitation using the concept of Urban Forestry which is relatively new to Korea. When we consider the Urban Forestry as a science, We should understand it interdisciplinary subject which includes Forestry, Horticulture, Urban Planning, Landscape Design, Landuse Planning, Business and Humanities. It may say that people and tree are the fundermental components of Urban Forestry. So there are two ways of Urban Forestry Applications-How people care for trees and How trees care for People-. For the application, this study places the focus on the monetary valuation, tree inventory system and traditional forestry application to urban forest management. Pubic Relation, Communication, Ordinances and Budget are also mentioned as a part of Urban Forestry Policy. Monetary valuation of trees and forests is very important for the proper cognition of their real value. So that, they may be equated and weighed against conflicting uses which would cause to be removed or severely mutilated. A tree inventory system which is the essential part of urban tree management can provide the pertinent information about the present condition of urban tree resource. It may aid in reducing the subjectivity of tree management decisions and stimulate them to be made rapidly and can help reduce potential municipal liability by identifying serious problems in time for corrective maintenance practices to be applied for the maximize community benefits and minimimize public nuisances or hazards. Managers can derive the information from the inventory and use it for the various management plan. When we see the structure of tree inventory system as one of the data base management system, Computer is the best equipment for the efficient management plan. Public relation and communication is also important factors to care the people for urban vegetation management. Volunteer management system is a good example for the public relation and communication. Those skills are need to develop for using the priceless, valuable human resources. Budget holds the key to the execution of Urban Forestry. Good inventory can provide for efficient budgeting stratiges through it's scientific analysis for the way of maximum benefits and minimum costs. Forest can be play a vital role for the aesthetic improvement and recreation in the city. This study suggests that the traditional sivicultural application not only improve the quality of scenic beauty and recreation but also the products of timber. So it is more needed to study for strong and scientific application to urban forest management.

  • The study has try to analyze firm-level marketing strategy for making inroads into United Arab Emirate(UAE) in the Middle East Rrgion. Korea's pharmaceutical medicine industry can overcome that growth limit by strategically advancing into the world market even the its market share is slight as of 2013. The results of Marketing Mix strategies to enter the UAE pharmaceutical medicine market are as follows: STP strategy and Marketing Mix strategy based on the findings of this study, the practical implications of the following. First of all, domestic pharmaceutical industries in Korea due to the domestic market, growth in the various institutional devices have limits on the expansion. On the other hand, supports the Government's active policy of UAE health care industry is booming. UAE Government medical facilities and health care in the health care industry in 2010 to improve the level of 80 billion dollars of investment. The UAE's medical sector is equipped with independent regulatory regime by the Emirates. The UAE is a foreign worker influx has been showing a high population growth rate, over the last 30 years, UAE resident population has increased about 7 times. The UAE Government to improve the quality of medical services, the private sector and the public to encourage the signing of partnership (PPP) can also be found in the regulation of foreign direct investment. The results of this study would play a role in analyzing a marketing strategy to make inroads into UAE pharmaceutical medicine market.

Knowledge and Attitudes about HIV/AIDS among Health Care Officers in S. Korea (보건복지 공무원의 에이즈에 대한 지식과 태도에 대한 연구)

  • Objective: This study explores the level of knowledge and attitude towards HIV/AIDS and the relationship between knowledge and attitude among social workers and nurses working in the public sector in Gwangju, S. Korea. Methods: The sample was composed of 121 nurse and 124 social workers. For the purpose of this research participants completed a questionnaire designed to assess their knowledge and attitudes about HIV/AIDS. The data was collected between December 2008 and January 2009. All data was analyzed using SPSS WIN version 12.0 for technical statistics. t-Test, ANOVA and multiple regression analysis was employed. Results: First, the result of this research showed that the level of participants knowledge on HIV/AIDS was 85.9%. The nurses score was significantly higher than social workers on knowledge. Second, the outcome also showed that nurses generally had a more positive attitude about HIV/AIDS patients than social workers but it was not significant. Third, participant's attitudes towards HIV/AIDS was significantly influenced by the level of knowledge and previous education about HIV/AIDS. Fourth, the results also showed that knowledge on HIV/AIDS is the most important influencing factor towards the participants attitude. Conclusion: This result has important implications for future education programs designed for health care professionals including nurses and social workers. Given the importances of the role of public officers as practitioners and policy makers on HIV/AIDS, the education program should not only focus on lectures but also include a sharing of practical experience and knowledge.

A Study on the Application of Welfare Service Type on the Basis of the Tenants Needs and Locational Characteristics in Each Housing (단지별 거주자의 요구 및 입지특성에 따른 복지 서비스 유형 적용에 관한 연구)

  • Within twenty years, Permanent Affordable Housing provided for people's security of housing in late 1980's yields the principles of equity with tenant's in new one supplied, thus being confronted by the requirements that existing welfare service for tenants should be improved. Reflecting on these, this study means to prepare the basic data for the welfare policy as applying welfare service types on basis of housing locational characteristics, tenants tier compared with welfare service needs of permanent affordable housing tenants. For achieving this aim, the range and mixed use mechanism of welfare service are investigated as reviewing the related research and findings. Based on this investigation, 7 types of welfare service(Independence, Half-support, Child care, Juvenile, Self-support, Social-mix, and Single household) are established. On the analysis of survey, the welfare service needs are investigated as grouping with locational characteristics and survey target. In addition, the welfare service types are reorganized by analysis of tenant's demographic and locational characteristic on each housing and are compared with tenants needs. Although, the demographic of permanent affordable housing generally shows that the tenants are constituted with aged people, the housing with high ratio of children and juvenile can be applied with Child Care type and Juvenile type. As a result of comparison, the ratio of independence type, juvenile type, and single household type are more on the large housing and housing on metropolitan area, and the ratio of half-support type and child care type are more on the small housing and housing on small city area.

Labor Transition and Exclusion of Unprivileged Female Breadwinners (저소득 여성가구주의 노동이행과 배제)

  • The aim of this study is to examine how labour transition of female householders is proceeded, and to identify mechanisms that cause them to be excluded from labor market. For this, thirteen lone mothers who have had labor experience in the market, while raising children were interviewed. This qualitative case study was proceeded by way of participants' interviews and questionnaires. Finding shows, impoverished lone mothers were horizontally moving incessantly on second labor market. As poverty worsens by years, poor lone mothers tend to be more dependant on the government's support and to transit downward to work-poor labor market. The unprivileged women were excluded from multi-dimentions: market structure, social relations and gender norms. The sex-segmental and human capital-oriented market has excluded women who gave up their career for caring. Female breadwinners were frequently excluded from social relations and opportunities for labor due to their stigma. sometimes they were self-excluded from the market for they had to care for their children. In conclusion, suggestions are given to enhance qualities of lone mother's lives and help them exit from poverty. It is necessary to implement positive labor policy with labor flexibility and stability and to practice socialization of care as care support system is very important for maintaining their jobs. Above all, social awareness of gender role must be changed.
