The purpose of this study is to determine the working conditions and demands of the assistant workforce at childcare centers, and to provide suggestions on policies for childcare teachers. To achieve the purpose of the study, a total of 190 assistant teachers, nursing helpers, alternative teachers and others were surveyed online during the month of August 2017. The survey examined the employment route and working motives, working conditions and environment, job and educational difficulties for analyzing the work conditions and demands of ancillary staff. For the analysis method, SPSS 12.0 was used to analyze frequency and difference. The main findings are as follows. First, childcare center assistants were able to work at their respective times, making it mandatory for them to enter the workforce, and making job openings for ancillary teachers. Second, 73.7% said the daily working hours were '4 hours to 8 hours' under the conditions and 57.9% said 'More than 510,000 won to less than 1 million won.' Average amount of rest time per day was about 30 minutes, and 17% had no rest periods. Third, based on the job and education status, ancillary personnel performed "infant and child guidance and interaction", "Cleaning and cleanliness related tasks" most actively, and "care for cleaning and cleanliness" and "care for morning and night care". Fourth, in the job trouble and difficulty, ancillary staff found it difficult to satisfy with wage related complaints and job insecurity, and the reason for agreeing to the non-regular workforce becoming a regular worker was found to be due to the stability of employment. Lastly, the most urgent tasks to solve the problem of non-regular workers were to fill the wage gap between regular and irregular workers, shorten the working hours and improve the working conditions. The policy implications are presented based on the above findings.
In radiology department, where patients with a variety of diseases receive their tests, there is a large possibility of cross contamination of nosocomial infection. Magnetic resonance imaging (MRI) tests take particularly more time than other tests do, which increases the possibility of being exposed to cross contamination. Therefore, this research examines the status of MRI equipment sterilization and investigates the bacterial distribution on head coils, which have the most frequent contact with patients, patient fixation blocks, and bores, which are confined spaces. The status of MRI equipment disinfection was examined by a survey targeting 150 employees, and the distribution of bacteria was measured in ten medical facilities. The result of bacterial distribution tests on MRI equipment showed various bacteria, including Staphylococcus, Acinetobacter, Sphingomona, Pantoea agglomeranss, Micrococcus, Bacillus, Saprophyticus, Brevundimona, and Myroidesspecies. The result of examining the stat us of MRI room disinfection showed that the disinfections of the head coil, block, and bore were implemented well, and the largest proportion was carried out once a day in the morning. The time and implementation of disinfection by the disinfection manager showed that they were implemented well when the manager was the MRI room examiner. The disinfection after examining a patient using an appropriate disinfectant is mandatory to prevent cross contamination.
Due to a change of food culture, many patients suffering from various diseases such as hypertension, heart disease, stroke, cancer. However, it takes a long time in the hospital for many patients due to the administration before the patient care process. In this paper, we propose a m-Healthcare service model that patients can receive medical services without the inconvenience offers by reducing the administrative hospital treatment that can automatically recognize through the hospital installed RFID readers when the patient patients with various diseases are foreign to the hospital. In particular, the proposed model improves the operational efficiency of the existing healthcare system by shortening the treatment time for medical personnel to help patients in emergency situations can determine automatically the patient's status does not give the disease type and condition of the patient to health care personnel. Objectives : The most frequently cited health related slogan in North Korea is that socialistic medicine is preventive medicine. It implies that North Korea puts preventive medicine at the operational center of its national health care system. This study aimed at examining and comparing preventive medicine practices in North Korea with those of South Korea. Methods : Efforts have been made to obtain a textbook for analysis its contents. Many people have iassisted in the study by joining the interview. Some of these people are as follows : a former professor of PyongYang Medical School, NK physicians living in South Korea, WHO staffs, diplomatic officials, etc. The major items of analysis consisted of industrial medicine and hygiene, nutrition, school health, epidemiology, health statistics and heath policy & management. Results : Public health philosophy is finely noted and well integrated in the operation of the North Korean national health care system, particularly in the area of industrial medicine and hygiene. Preventive medicine with a strong health surveillance system spanning a number of broad social organizations is a major tool to improve the health of the people in North Korea. The emphasis on preventive medicine has a close relationship with the 'Juche Philosophy' and the shortage of pharmaceuticals and medical equipment. To cope with the shortage problem, North Korean health workers are encouraged to grow medicinal herbs. We have found that they put little effort into teaching newly emerging diseases, such as AIDS, VDT syndrome, hazards of EMF, and agricultural chemical poisonings. Of the subjects of the preventive medicine text, 78.9% coincide with those of South Korean industrial health manuals and 34.2% with South Korean epidemiology texts. However, an absolute difference was found to exist between the health policies and management systems. Conclusion : In North Korea, the concept of preventive medicine functions as the basic philosophic strategy of the national health care system. It differs greatly from the South Korean system in both practice and educational content. Its contribution to society is simply incomparable to that of South Korea. More communication and further study is called for in order to improve the preventive medicine practices in the future.
The prevention of life-style related diseases is an increasingly important issue in Japan, because not only have the number of patients with life-style related diseases increased but also medical care costs. This paper gives recent strategies for the prevention of cardiovascular diseases through life-style modification. Health objectives for the year 2010, called "Healthy Japan 21", were established in 2000 by the Ministry of Health, Labour and Welfare and the Health Promotion Act was enacted in 2002 to promote this health policy. However, the prevention efforts for life-style related diseases have not been effective in regard to the evaluation of the strategy objectives. The reform of the medical care system which included a new nationwide prevention strategy for life-style related diseases was presented in 2006. The new strategy starting from April 2008 included a "specific health checkup" and "specific health education" for those with metabolic syndrome. The specific health checkup is used to screen people according to criteria of the metabolic syndrome and divide them into 3 groups. These groups will receive specific health education. The purpose of this strategy is the early detection of those who have cardiovascular risk factors, and the early management of the clustering of cardiovascular risk factors of obese people aged 40-74 years old. It is mandatory for every insurer to conduct a specific health checkup and specific health education under the new Act. The implementation rate of the specific health checkup and the specific health education, and a reduction rate of individuals with metabolic syndrome among insured people will be evaluated every year. The national objective is to increase the rate of those undergoing the specific health checkup to 80% and the rate of those receiving the specific health education to 60% by the year 2015. The national objective also targeted a reduction rate of 25% for those with metabolic syndrome. This new strategy will be the biggest intervention trial in the world, and it will produce a big health care market in Japan. Not only public administrative institutions but also private institutions are now preparing to take part in this new strategy. However, various tasks remain, such as training more professionals in health education, developing more evidence based practices, and encouraging cooperation with various sectors, to enforce this new strategy.
This study is to examine health care utilization and morbidity of disabled and non-disabled children in Korea to evaluate the health disparities. We used medical claim data of 2010 from the National Health Insurance Service-National Sample Cohort data. As a result of the analysis, the disabled children are not in good health condition because they have more frequency of medical service use, hospitalization rate, and more number of diseases and spent more on medical expenses than non-disabled children. Patterns of the most frequent disease differ from significantly between disable and non-disabled. Disabled children had a higher prevalence of selected birth defects and selected health conditions associated with physical disability and a lower prevalence of selected infection disease than children without disability. In conclusion, Health policy changes that would extend the access to health service for children with disabilities characterized by frequent medical care, hospitalization, excessive medical expenditure and complex diseases.
The purpose of this study was to identify the characteristics and side effects relevance of physically restrained elderly patients with dementia in the nursing hospitals. The data were collected from nursing care providers working at the 5 nursing hospitals with survey and analyzed 190 data using SPSS 21.0 program. The finding showed that age of '75 to 84' years occupied 58.3%, '17-24' hours of the total restrained time of the day 22.6%, night time 57.4% and no fall down history 54.2%. Age showed relevance with local edema and problem of joint construction(p<.05), specifically '85 and over' years having 25% occurrence in the problem of joint construction. Walking ability showed relevance with pressure ulcer, but in the patients who could walk it showed more frequency than in the patients who could not walk. The total restrained time of the day showed relevance with skin redness, local edema, problem of joint construction(p<.05). Based on this finding, it would be necessary to apply differential care policies according to the characteristics of patients when we care the restrained patients with dementia.
The purpose of this study is to examine whether the effect of service satisfaction regarding the accessibility of child care services varies between dual-earner and single-earner families. In particular, we conducted the analysis by dividing the service accessibility into sub-areas. The study sample consisted of 267 parents living in Seoul and Gyeonggi region. As a result of the analysis, accommodation in use time and affordability influenced satisfaction in the single-earner families, and information acquisition and acceptability in the dual-earner families. For single-earner families, the more convenient the use time of the childcare institution and the less the service cost burden, the higher the satisfaction. The dual-earner families had higher satisfaction levels if it was easier to obtain information about the childcare centers and weaker the religious characteristics of the childcare centers. Based on the research results, it is necessary to differentiate the burden of cost and provide more flexible operation of childcare service operating hours. In addition, it is necessary to devise a policy method to increase the transparency of information to childcare institutions.
The primary goal of this study was to investigate factors affecting job satisfaction of injured workers who returned to work. There are two types of injured workers' returning to work returning to pre-injury job and getting a new job. After separating the two types of workers we conducted the multiple regression analysis on the data from the Workers'Compensation Insurance Panel of the Korea Workers' Compensation and Welfare Service Corporation(2014). The sample consisted of 1,333 occupationally injured workers who were finished with medical treatment(693 workers returned to previous work place, and 640 workers found new work places). Main results are as follows. First, injured workers who got new jobs were vulnerable to sustainability to job, work status, average monthly wage as well as job satisfaction compared with injured workers who returned to pre-injury job. Second, gender, educational level, skill fitness, average monthly wage, welfare benefits, family income/leisure life/residential environment/social relation satisfaction, and maintenance of relation with business owner during medical care were related to job satisfaction of injured workers who returned to pre-injury job. Third, gender, work status, skill fitness, continuous work possibility, average monthly wage, family income/leisure life/social relation satisfaction, substitute worker for assigned task during medical care and maintenance of relation with business owner during medical care affected on job satisfaction of injured workers who got new jobs. Based on these findings implications for policy and interventions were discussed in regards to job satisfaction of injured workers.
