Secular change in waist circumference and waist-height ratio and optimal cutoff of waist-height ratio for abdominal obesity among Korean children and adolescents over 10 years

  • Purpose: This study aimed to evaluate the time trends of waist circumference (WC) and waist-height ratio (WHR), and to present WC and WHR distributions with optimal WHR cutoff for abdominal obesity in Korean children and adolescents. Methods: We performed a retrospective cross-sectional analysis of data from 13,257 children and adolescents (6,987 boys and 6,270 girls) aged 6-18 years who were included in the third to sixth Korea National Health and Nutrition Examination Survey (KNHANES, 2005-2015). Linear regression analyses were used to identify secular changes in WC and WHR by age, sex, and KNHANES waves. A receiver operating characteristic curve analysis was used to determine the optimal WHR cutoff values for abdominal obesity and cardiometabolic risk factors. Results: The mean WC and WHR distributions from 2005 to 2015 showed no significant secular changes between the KNHANES 4 waves (P for trend ${\geq}0.05$ in all ages and both sexes). The mean WCs in the present study were lower than those in the 2007 Korean National Growth Charts. The mean WHR at ages <13 years was statistically higher in the boys than in the girls, but did not significantly differ between the sexes among those aged 13 to 18 years. The optimal WHR cutoff for abdominal obesity was 0.48 (area under the curve, 0.985; 95% confidence interval, 0.985-0.985) in the 13- to 18-year-old adolescents. Conclusion: WC and WHR showed no secular changes over 10 years. The optimal WHR cutoff for abdominal obesity of 0.48 is useful for diagnosing and managing obesity and thus preventing obesity-related cardiometabolic complications in 13- to 18-year-old Korean adolescents.

소아청소년 비만의 임상적 이해 (Clinical Perspectives on Obesity in Children and Adolescents)

  • The prevalence of obesity in children and adolescents is increasing worldwide. Obesity in children and adolescents not only increases the risk of transitioning to obesity in adulthood but also increases the risk of cardiometabolic diseases such as high blood glucose, high blood pressure, dyslipidemia, fatty liver, and hyperinsulinemia during childhood. The goal of treating obesity in children and adolescents is not to focus on weight loss but to help children reach a healthy weight while maintaining normal growth appropriate for their age and sex. To achieve this goal, regular physical activity and exercise, dietary modification, improvement of obesity-prone environmental factors, and behavioral changes are required for a healthy lifestyle. If appropriate weight control is not achieved through lifestyle modifications, pharmacotherapy may be considered for adolescents with severe obesity aged 12 and above. Recent clinical trials have reported the efficacy and safety of pharmacotherapy in severely obese adolescents. Currently, two medications can be prescribed in Korea for patients with obesity aged 12 and above: Orlistat and Liraglutide. However, despite effective weight control through drug treatment, body weight may increase again after treatment discontinuation. Therefore, it is crucial to evaluate adherence to health behaviors during visits and continue to educate on lifestyle modifications, even during pharmacotherapy, to minimize weight regain.

우리나라 성인의 사상체질과 생활습관 상태에 따른 대사증후군 발생 위험 상관성 (A Relationship of Constitution Type, Lifestyle Status and Metabolic Syndrome Incidence in Korean Adults)

  • Objectives We aimed to identify the incidence of metabolic syndrome (MetS) and its clustering components according to constitution type and lifestyle risk factors in Korean adults. Methods This study included 1,978 adults aged 30-55 years from the Korean Medicine Daejeon Citizen Cohort (KDCC) study. We defined lifestyle factors including smoking, alcohol consumption, physical activity, sleep, dietary quality, and weight status. Total lifestyle scores were created based on the six lifestyle factors (ranging from 0 to 5 factors) and classified into two groups: unhealthy (0-2 factors), or healthy (3-5 factors). Cox proportional hazard regression was used to estimate the hazard ratio (HR) and 95% confidence intervals (CIs) of primary endpoints: MetS events and their clustering components. Results During a median follow-up of 2.2 years, we documented 125 new onsets of MetS. Compared with participants with healthy, the HR of unhealthy participants was 2.401 (95% CI: 1.497-3.851) for MetS incidence. After adjusting for covariates, TE type with unhealthy was higher HR values of abdominal obesity (HRs: 1.499, 95%CI: 1.061-2.117) and hypertension (HRs: 1.840, 95%CI: 1.032-3.277), respectively. Conclusion Unfavorable lifestyle factors were highly associated with the prevalence of MetS and its clustering such as abdominal obesity and hypertension in Korean adults with TE. Tailored health management is needed to consider individual traits and healthy lifestyles to prevent cardiometabolic diseases.

Relationship Between Non-alcoholic Fatty Liver Disease and Decreased Bone Mineral Density: A Retrospective Cohort Study in Korea

  • Objectives: The aim of this retrospective cohort study was to investigate whether non-alcoholic fatty liver disease (NAFLD) was associated with incident bone mineral density (BMD) decrease. Methods: This study included 4536 subjects with normal BMD at baseline. NAFLD was defined as the presence of fatty liver on abdominal ultrasonography without significant alcohol consumption or other causes. Decreased BMD was defined as a diagnosis of osteopenia, osteoporosis, or BMD below the expected range for the patient's age based on dual-energy X-ray absorptiometry. Cox proportional hazards models were used to estimate the hazard ratio of incident BMD decrease in subjects with or without NAFLD. Subgroup analyses were conducted according to the relevant factors. Results: Across 13 354 person-years of total follow-up, decreased BMD was observed in 606 subjects, corresponding to an incidence of 45.4 cases per 1000 person-years (median follow-up duration, 2.1 years). In the model adjusted for age and sex, the hazard ratio was 0.65 (95% confidence interval, 0.51 to 0.82), and statistical significance disappeared after adjustment for body mass index (BMI) and cardiometabolic factors. In the subgroup analyses, NAFLD was associated with a lower risk of incident BMD decrease in females even after adjustment for confounders. The direction of the effect of NAFLD on the risk of BMD decrease changed depending on BMI category and body fat percentage, although the impact was statistically insignificant. Conclusions: NAFLD had a significant protective effect on BMD in females. However, the effects may vary depending on BMI category or body fat percentage.

The Association of Body Fat and Arterial Stiffness Using the Brachial-Ankle Pulse Wave Velocity

  • Background: BMI alone may not serve as an index of obesity because it does not reflect body composition. The present study aimed to compare arterial stiffness as assessed by the brachial-ankle pulse wave velocity (ba-PWV) among groups defined by body fat percentage (pBF) and BMI. Methods: This cross-sectional study was based on 1,700 participants (1,044 men and 656 women) who completed a health screening examination at a national hospital between January 2011 and February 2016. Participants were divided into four groups according to BMI and pBF: normal fat and normal weight (NFNW); excessive fat and normal weight (EFNW); normal fat and obese (NFO); and excessive fat and obese (EFO). The ba-PWV and other cardiometabolic factors were compared among the four groups in men and women separately. Results: For both sexes, the NFNW group had a lower metabolic risk compared to that in the other groups (EFNW, NFO, and EFO). After adjusting for multiple variables, the NFO males had a significantly lower ba-PWV compared to those in the other groups, including NFNW males. The NFO group had significantly more skeletal muscle mass and muscle mass compared the other groups (P<0.05). Among women, the NFNW group had a significantly lower ba-PWV compared the other groups, even after adjusting for multiple variables. Conclusion: Lower pBF in obese men may be associated with improved cardiovascular risk.

A Healthy Dietary Pattern May Have a Protective Effect Against Cardiovascular Disease Through Its Interaction With the MC4R Gene Polymorphism

  • Polymorphisms in the melanocortin 4 receptor (MC4R) gene with occurrence and progression of chronic diseases such as obesity and cardiovascular disease (CVD) have long been addressed but there is a lack of evidence for complex interrelationships, including direct and indirect effects of these variables. This review specifically focuses on studying the effects of healthy diet interaction and MC4R polymorphisms on the development of CVD. The quantity and quality of carbohydrates and proteins consumed are related to obesity susceptibility and cardiometabolic risk factors. A healthy dietary pattern such as a Mediterranean dietary can modulate the association between MC4R polymorphisms (rs17782313) and the risk of CVDs. Also, the Nordic diet can reduce lipid profiles such as low-density lipoprotein cholesterol (LDL-C) and total cholesterol levels. On the other hand, MC4R interaction with the dietary inflammatory index decreases high-density lipoprotein cholesterol levels and increases LDL-C and triglyceride (TG) levels. Additionally, the DASH diet decreases TG, atherogenic index of plasma, systolic blood pressure, diastolic blood pressure, and serum glucose. The interaction between MC4R genes and diets plays an important role in the development of CVD. Adherence to healthy diets such as the Mediterranean, Nordic, Anti-inflammatory, and Dash diets might be an efficient strategy to prevent CVD. The potential for personalized diets to be developed for the treatment and prevention of CVD and its related comorbidities is expected to expand as this field develops.

한국인의 시리얼 섭취실태와 영양 및 건강상태와의 관련성 연구 - 2012년도 국민건강영양조사 자료를 이용하여 - (The relationship of ready-to-eat cereal consumption with nutrition and health status in the Korean population based on the Korea National Health and Nutrition Examination Survey 2012)

  • 본 연구는 2012년 한국국민건강영양조사 자료를 이용하여 한국인의 성별, 연령별 시리얼 섭취실태를 알아보았다. 연령을 1~18세, 19~75세로 구분하여 시리얼 섭취여부에 따른 영양소 섭취량과 영양상태를 살펴보았고, 19~75세 성인을 대상으로 체위 및 혈액의 생화학적 지표 분석과 질병 유병률 등 건강상태를 비교하였다. 그 결과를 요약하면 다음과 같다. 한국인 중 시리얼을 먹은 사람의 비율은 3.8%이었고 시리얼 섭취량은 연령별로 볼 때 12~18세에서 가장 많았으며 남자가 여자보다 많았다. 시리얼 섭취여부와 사회경제적인 지표와의 관계를 chi-square test한 결과는 19~75세에서 연령군, 교육, 거주지, 직업군과 유의적인 관계가 나타났다. 영양소 섭취량은 시리얼 섭취군이 비섭취군 보다 모든 연령층에서 칼슘, 티아민, 리보플라빈, 나이아신, 비타민 C의 섭취량이 유의적으로 높게 나타났으며 19~75세는 에너지와 지방의 섭취도 유의적으로 높게 나타났다. 시리얼 섭취군이 비섭취군보다 영양소의 평균필요량보다 적게 섭취하는 사람들의 백분율은 칼슘, 비타민 A, 티아민, 리보플라빈, 나이아신, 비타민 C 에서 유의적으로 낮았고, 상한섭취량보다 많이 섭취하는 사람들의 비율은 19~75세에서 칼슘, 인, 철, 비타민 A에서 유의적으로 낮게 나타났다. 즉, 시리얼 섭취군이 비섭취군보다 대부분의 영양소 섭취량은 높았으며, 평균필요량보다 적게 섭취하는 사람들의 백분율과 상한섭취량보다 많이 섭취하는 사람들의 백분율은 낮은 것으로 나타나 영양상태가 좋은 것으로 나타났다. 시리얼 섭취여부에 따른 조사대상자의 체위 및 혈액 생화학적 지표 분석 결과 시리얼 섭취군이 비섭취군에 비해 수축기 혈압, 이완기 혈압, 혈청의 중성지방, 총콜레스테롤의 수치가 유의적으로 낮게 나타났다. 질병유병률은 시리얼 섭취군이 비섭취군에 비해 고혈압과 고콜레스테롤혈증에서 유의적으로 낮게 나타났으며 고혈압의 경우 오즈비가 0.19로 시리얼 섭취군이 비섭취군에 비해 고혈압의 발생이 유의적으로 81% 감소하는 것으로 나타났다 그 외 질병 (비만, 고중성지방혈증, 저HDL-콜레스테롤혈증, 당뇨병, 빈혈, 대사증후군)의 경우 오즈비가 모두 1 미만으로 시리얼 섭취군은 비섭취군에 비해 질병 발생 확률이 감소하는 경향으로 나타났다. 본 연구에서는 한국인을 대상으로 시리얼을 섭취하는 사람들과 섭취하지 않는 사람들의 영양섭취상태와 질병유병률을 비교한 결과 시리얼을 섭취하는 사람들이 섭취하지 않는 사람들에 비해 영양상태는 좋고 질병유병률은 낮은 것으로 나타났으므로 시리얼 섭취가 영양과 건강에 매우 긍정적인 효과를 나타내는 좋은 식품인 것으로 사료된다. 외국의 경우 아침에 시리얼과 우유를 먹는 것이 아침식사의 중요한 식사패턴의 하나로 자리매김하고 있는데 반해, 현재 한국인은 시리얼을 섭취하는 비율이 매우 적고 또한 시리얼을 식사대용으로 먹는 사람들이 많지 않은 실정이다. 그러므로 시리얼 섭취에 따른 영양상태의 우수성과 질병예방을 고려하여 한국인의 식습관이나 식문화와 조화를 이루면서 시리얼이 한국인의 식사에 많이 이용되도록 다양한 연구와 프로그램이 수행되면 좋을 것으로 사료된다. 또한 본 연구에서 사용한 data는 횡단적인 자료이므로 많은 제한점이 있는 것으로 생각되며 앞으로는 꾸준히 종단적인 연구도 필요할 것으로 사료되며, 시리얼, 당, 기타 가공식품 등의 과잉섭취에 대한 연구도 함께 수반되어야 할 것으로 사료된다.

우리나라 성인의 사상체질에 따른 식생활 상태와 신체구성의 상관성에 관한 단면분석연구 (A relationship of healthy eating status and body composition in Korean adults by their constitution type: A cross-sectional study)

  • Background Adherence to healthy diet acts as a key role to modify sedentary lifestyle in real life setting. Constitution type of traditional Korean medicine has been prediagnosed to risk factors of cardiometabolic diseases. This study aims to evaluate the associations between body composition and healthy eating status in Korean adults by their constitution type. Methods Of 4046 participants from Korean Medicine Daejeon Citizen Cohort study, Korean Medicine Daejeon Citizen Cohort (KDCC) study were included for analysis. Demographic, health-related behaviors and Korean Medicine (KM) type were surveyed based on a general health-related questionnaire. Anthropometric measurements and dietary factors by using Korean Healthy Eating Index (KHEI) were assessed only in the half of the original participants. Results 50.8% of Taeeum was observed from eligible 1967 participants (66.7%, women). The highest KHEI score was observed in soyang group (52.0±0.3, p=0.006) compared to other two groups. In taeeum group, lower appendicular skeletal muscle mass (ASM)(%)(Taeeum: 38.7±0.1 vs. Soeum/Soyang: 39.2±0.1, p < 0.05), and higher percent of body fat (PBF)(%) (Taeeum: 32.9±0.2 vs. Soeum/Soyang: 29.0±0.2, p < 0.05) by the lowest tertile (T1) of the KHEI score, respectively. When the KHEI score increased by 1 point in taeeum group, a positive relationship was observed, which increased by 0.015% of ASM, however, a negatively related to in which body fat mass (BFM) decreased by 0.022 kg and PBF decreased by 0.024%. Conclusion Customized nutritional management is required that could help maintaining physical health and diet by their constitution type.

Executive Summary of the 2021 International Conference of Korean Dementia Association: A Report From the Academic Committee of the Korean Dementia Association

  • Recently, aducanumab, a beta amyloid targeted immunotherapy, has been approved by the US Food and Drug Administration for the treatment of Alzheimer's dementia (AD). Although many questions need to be answered, this approval provides a promising hope for the development of AD drugs that could be supported by new biomarkers such as blood-based ones and composite neuropsychological tests that can confirm pathologic changes in early stages of AD. It is important to elucidate the complexity of AD which is known to be associated with other factors such as vascular etiologies and neuro-inflammation. Through the second international conference of the Korean Dementia Association (KDA), researchers from all over the world have participated in the exchange of opinions with KDA members on the most up-to-date topics. The Academic Committee of the KDA summarizes lectures to provide the depth of the conference as well as discussions. This will be an important milestone to widen the latest knowledge in the research of AD's diagnosis, therapeutics, pathogenesis that can lead to the establishment of future directions.

Association Between Meat Consumption and Carotid Intima-Media Thickness in Korean Adults With Metabolic Syndrome

  • Objectives: The effect of meat consumption on cardiometabolic risk has been continuously studied, but their associations are not conclusive. The aim of this study is to examine the association between the consumption of meat or red meat and carotid intima-media thickness (IMT) in healthy Korean adults. Methods: This study evaluated 2374 community-dwelling adults (933 men and 1441 women) who were free of cardiovascular disease or cancer, living in a rural area in Korea. Total meat and red meat intakes were assessed with a validated 103 item-food frequency questionnaire. Carotid IMT was evaluated ultrasonographically, IMTmax was defined as the highest value among IMT of bilateral common carotid arteries. Results: After adjustment for potential confounding factors, the mean IMTmax tended to increase in higher meat consumption groups in both men and women with metabolic syndrome (p for trend= 0.027 and 0.049, respectively), but not in participants without metabolic syndrome. Frequent meat consumption (${\geq}5$ servings/week) was significantly associated with higher IMTmax in men with metabolic syndrome (by 0.08 mm, p=0.015). Whereas, the association was not significant in women (by 0.05 mm, p=0.115). Similar but attenuated findings were shown with red meat intake. Conclusions: Our findings suggest that a higher meat consumption may be associated with a higher carotid IMT in Korean adults with metabolic syndrome. The frequent meat consumption (${\geq}5$ servings/week), compared with the others, was associated with a higher carotid IMTmax only in men with metabolic syndrome. Further research is required to explore optimal meat consumption in people with specific medical conditions.
