Seminoma in a Mixed Dog (개에서 발생한 정세포종 발생 증례)

  • A 10-year-old mixed male dog, weighed 4.2 kg, was referred to an animal hospital in Pusan. Clinical signs were generalized alopecia, cough, cardiac murmur, dehydration, and right side cryptorchidism. Testis was surgically removed, fixed in formalin and submitted to Diagnostic Laboratory, Chonbuk National University. Grossly, right testis was enlarged. A bulging tumor mass of right testis was homogeneous and grayish white in cut surface, but left testis was normal. Microscopically, no border lines of tubules were formed and the cells grew diffusely, forming sheets with scant supporting stroma. Tumor cells were flirty uniform in size and round or polyhedral, and had discrete cellular lines. The nuclei were large and of variable size, and ovoid, round, vesicular but the cell cytoplasm was scanty. Mitotic figures were common. And giant cells and vacuolated histiocytes were scattered, which is called starry sky appearance. This is the case of seminoma with diffuse type in a mixed dog.

Early Surgical Intervention of Active Infective Endocarditis (심내막염 환자의 수술적 치료)

  • During one year period from Sep. 1986 to Sep. 1987, we have experienced 6 cases of infective endocarditis requiring surgical interventions. All 6 patients had class IV or V cardiac disability at the time of surgery. The indication for surgery was rapidly progressive congestive heart failure in all cases. Four patients underwent aortic valve replacement including one double valve replacement. Two other patients required other surgical procedures, removal of large left atrial vegetation mass in one patient and excision of destroyed pulmonary valve and aortic vegetation in the other patient. Two patients died; one of mitral annulus rupture after release of aortic clamp and the other of mediastinal bleeding 3 months after replacement of aortic valve. Three out of 4 survivors are in NYHA Class I and the remaining patient is in Class II. We emphasize that early operative intervention is life-saving in patients with persistent or progressive congestive heart failure, irrespective of the activity of the infective process or the duration of antibiotic therapy.

Cardiac Metastasis of Malignant Melanoma - A case report - (심장으로 전이된 악성 흑색종 - 1례 보고 -)

  • We report a case of a resection of very large intracavitary metastatic malanoma causing obstruction of the right ventricular inflow and outflow tract of the heart. A 49-year-old woman with dyspnea and generalized edema was seen. Echocardiography reveal an intra cavitary mass occupying the entire right ventricle and pericardial effusion. The lesion was palliatively resected using a cardiopulmonary bypass and was confirmed as a malignant melanoma. The patient is alive and improved symptomatically 30days after the operation.

OMICS approaches in cardiovascular diseases: a mini review

  • Ranked in the topmost position among the deadliest diseases in the world, cardiovascular diseases (CVDs) are a global burden with alterations in heart and blood vessels. Early diagnostics and prognostics could be the best possible solution in CVD management. OMICS (genomics, proteomics, transcriptomics, and metabolomics) approaches could be able to tackle the challenges against CVDs. Genome-wide association studies along with next-generation sequencing with various computational biology tools could lead a new sight in early detection and possible therapeutics of CVDs. Human cardiac proteins are also characterized by mass spectrophotometry which could open the scope of proteomics approaches in CVD. Besides this, regulation of gene expression by transcriptomics approaches exhibits a new insight while metabolomics is the endpoint on the downstream of multi-omics approaches to confront CVDs from the early onset. Although a lot of challenges needed to overcome in CVD management, OMICS approaches are certainly a new prospect.

The relationships of body mass index, waist-to-height ratio, and body fat percentage with blood pressure and its hemodynamic determinants in Korean adolescents: a school-based study

  • Purpose: Obesity is an important risk factor for hypertension in adolescents. We investigated the relationship of obesity-related indices (body mass index [BMI], waist-to-height ratio [WHR], and body fat percentage [%BF]) with blood pressure and the hemodynamic determinants of blood pressure in Korean adolescents. Methods: In 2008, 565 adolescents, aged 12-16 years, were examined. The %BF of the participants was measured by bioelectrical impedance analysis. Echocardiography and brachial artery pulse tracing were used to estimate the stroke volume (SV), cardiac output (CO), total vascular resistance (TVR), and total arterial compliance (TAC). Results: We noted that BMI, WHR, and %BF were positively correlated with systolic blood pressure (SBP) and diastolic blood pressure (DBP). The positive correlation between BMI and blood pressure (SBP and DBP) persisted after adjustment for WHR and %BF. However, after adjustment for BMI, the positive associations between blood pressure (SBP and DBP) and WHR as well as %BF, were not noted. With regard to the hemodynamic factors, BMI, but not WHR and %BF, was an independent positive factor correlated with SV and CO. TVR had an independent negative association with BMI; however, it was not associated with WHR or %BF. Moreover, we noted that BMI, WHR, and %BF did not affect TAC. Conclusion: In Korean adolescents, BMI had an independent positive correlation with SBP and DBP, possibly because of its effects on SV, CO, and TVR. WHR and %BF are believed to indirectly affect SBP and DBP through changes in BMI.

Effects of a 12-week Combined Exercise Training Program on the Body Composition, Physical Fitness Levels, and Metabolic Syndrome Profiles of Obese Women (12주간의 복합운동이 비만여성의 신체조성, 체력 및 대사증후군에 미치는 영향)

  • Purpose: The purpose of this study was to examine the effect of a 12-week combined exercise training program on the body composition, physical fitness levels, and metabolic syndrome profiles of obese women. Methods: Twelve obese women were assigned to the combined exercise training program group. The women underwent training for 70-90 min/d, three times per week for a period of 12 weeks. Paired samples t-tests were performed using SPSS ver. 17.0 for analysis of the results. Results: The results of this study showed that body-composition parameters such as weight, fat-free mass, body fat mass, body-mass index, body fat, waist-hip ratio, basal metabolic rate, and intra-abdominal fat, physical fitness parameters such as muscle strength, muscle endurance, flexibility, and cardiac endurance, and metabolic syndrome biomarkers such as triglyceride levels, high-density lipoprotein cholesterol levels, glucose levels, systolic blood pressure, and waist circumference before participation the training program differed significantly from those after participation in the training program (p<0.05). However, diastolic blood pressure before participation in the training program did not differ significantly from that after participation in the training program (p>0.05). Conclusion: We concluded that a 12-week combined exercise training program could be a good exercise program for improvement of the body composition, physical fitness levels, and metabolic syndrome profiles of obese women.

Assessment of Left Ventricular Function with Single Breath-Hold Magnetic Resonance Cine Imaging in Patients with Arrhythmia

  • Purpose: To evaluate quantification results of single breath-hold (SBH) magnetic resonance (MR) cine imaging compared to results of conventional multiple breath-hold (MBH) technique for left ventricular (LV) function in patients with cardiac arrhythmia. Materials and Methods: MR images of patients with arrhythmia who underwent MBH and SBH cine imaging at the same time on a 1.5T MR scanner were retrospectively reviewed. Both SBH and MBH cine imaging were performed with balanced steady state free precession. SBH scans were acquired using temporal parallel acquisition technique (TPAT). Fifty patients ($65.4{\pm}12.3years$, 72% men) were included. End-diastolic volume (EDV), end-systolic volume (ESV), stroke volume (SV), ejection fraction (EF), myocardial mass, and LV regional wall motion were evaluated. Results: EF, myocardial mass, and regional wall motion were not significantly different between SBH and MBH acquisition techniques (all P-values > 0.05). EDV, ESV, and SV were significant difference between the two techniques. These parameters for SBH cine imaging with TPAT tended to lower than those in MBH. EF and myocardial mass of SBH cine imaging with TPAT showed good correlation with values of MBH cine imaging in Passing-Bablok regression charts and Bland-Altman plots. However, SBH imaging required significantly shorter acquisition time than MBH cine imaging ($15{\pm}7sec$ vs. $293{\pm}104sec$, P < 0.001). Conclusion: SBH cine imaging with TPAT permits shorter acquisition time with assessment results of global and regional LV function comparable to those with MBH cine imaging in patients with arrhythmia.

Left Ventricular Outflow Obstruction due to a Left Ventricular Myxoma (좌심실 점액종에 의해 발생한 좌심실 유출로 협착증)

  • Once it is diagnosed, immediate surgical extirpation is desirable for treating left ventricle myxoma that's accompanied with stenosis of the left ventricle outflow tract. This is because this condition may potentially induce fatal complications such as cerebral infarction or myocardial infarction that's triggered by myxoma embolus, or even sudden death due to coronary malperfusion. An 18-year-old male with the chief complaint of NYHA class II exertional dyspnea was found to have a $4{\times}3\;cm^2$ sized mass on transthoracic ultrasonography, which was shown to move down the left ventricle outflow tract on the systolic phase. The mass was immediately extirpated by incision of the left ventricle; the mass was finally diagnosed as a myxoma. The patient was discharged on at the 10th day postoperatively without any complications. On the 22-month follow-up observation made at the out-patient clinic after discharge, there have been no noticeable, significant changes seen on physical examination or the cardiac ultrasonography.

  • A ten-year-old Yorkshire Terrier developed serious abdominal distension and respiratory distress. Radiography and ultrasonography revealed a hyperechoic mass around the aorta that was contiguous with the right atrium and main pulmonary artery. It was resulted in failure of the right side of the heart including tricuspid regurgitation, hepatomegaly with dilation of the hepatic vein and severe ascites due to a large, expansile mass. Computed tomography(CT) identified a large mass originating at the cardiac hilar region and spanning from the cranial vena cava to the caudal vena cava. The tumor had invaded the cranial vena cava, caudal vena cava, heart and pleural wall. A tentative diagnosis of chemodectoma was assigned to the tumor through a fine needle aspiration. This report focuses on the typical features of imaging diagnosis of heartbase tumors by radiography, ultrasonography and CT.

Primary Malignant Peripheral Nerve Sheath Tumor of the Pulmonary Vein: A Case Report (폐정맥의 원발성 악성 말초신경초종: 증례 보고)

  • Primary masses rarely originate from the heart and great vessels, and a malignant peripheral nerve sheath tumor (MPNST) is extremely rare. A 76-year-old male with pleural effusion underwent contrast-enhanced computed tomography, which revealed a hypoattenuating mass involving the right pulmonary vein and left atrium. Ultrasonography showed that the mass originated from the right pulmonary vein. Surgical resection confirmed an MPNST that originated from the pulmonary vein. We report the first Korean case of a primary MPNST originating from the pulmonary vein. We have also described the radiologic findings suggestive of a pulmonary vein mass.
