  • According to the literature surrey, we summarized the treatment and prescription for Su-Cheon(水喘), Hwa-Cheon(火喘) and Cardiac Asthma as follows. 1. The aim of treatment for Su-cheam(水喘) is I-Su-Hwa-Daw(利水火痰), Pynng-Cheon-Ick-Qi(平喘益氣) and Mokbangki-tang(木防己湯), Jungryuk daejosapye-tang(亭曆大棗瀉肺湯) Socheong ryong-tang(小靑龍湯), Gagamshinki-hwan(加減腎氣丸) and Shinbi-tang(神秘湯) are often used. 2. The sim of treatment for Hwa-Chean(火喘) is Cheong-Kuem-Kwang-Hwa(淸金降火祛痰) and Backho-tang(白虎湯) and Dodamtang-gamibang(導痰湯加味方) are useful. 3. In Oriental medicine, Sangmak-Sun(生脈散) and Sambutang-gamibang(參附湯加味方) for Ink-Qi-Ryum-Eum(益氣斂陰) and On-Yang-I-Su-Qi(溫湯利水化氣) are often used for cardiac asthma. 4. As the treatment for Su-Cheon(水喘), Hwa-Cheon(火喘), and cardiac asthma are Similar to, it is supposed to be need of careful differential diagnosis and treatment.

  • In order to investigate experimentally the clinical effects of Chungsimjeongchun-Tang that have been widely used for cardiac asthma, dyspnea and coughs secondary to the appoplexy and cardiac diseases, the water extract of Chungsimjeongchun-Tang was conducted to the effects on the anti-histamine action and respiratory system in mice, guinea-pigs, rabbits and cats. The following results of Chungsimieongchun-Tang were obtained. Spontaneous motility of the isolated ileum of mice and rabbits was strongly suppressed and contractions of the isolated ileum of mice and guinea-pigs induced by acetylcholine chloride, barium chloride and histamine were remarkably inhibited. Anti-histamine action was noted. Antitussive action on the mechanically irritative cough in cats was recognized. Expectorant action on the leakage of dye in the respiratory tract of rabbits was also shown.

  • Congestive heart failure is a cardiac dysfunction that can't supply sufficient amount of blood. The disorder usually causes hypertension, ischemic cardiac disease and give rise to pulmonary congestion and pulmonary epileptic edema. The symptoms of the disease are dyspnea, palpitation, edema, etc. We treated a 82 year-old female patient who had severe dyspnea, nausea, and dizziness. At the point of differentiation of syndromes(辨證), the case was diagnosed as Su-Cheon(水喘) induced by Yang-deficiency of Both Spleen and Kidney(脾腎陽虛) and was administered Yerunpang(胃?湯). After two weeks of the treatment, we witnessed improvement in the symptoms of CHF and general depressed condition. This report describes the process and contents about the way the patient was treated.

  • Plastic bronchitis is a rare disease characterized by marked airway obstruction, via the formation of large gelatinous or rigid airway cast. In Korea, there were a few case reports with plastic bronchitis not in adults, but in children. So we report a case of an adult who was diagnosed as plastic bronchitis with eosinophilic casts, with no history of atopic and cardiac disease.

  • Oriental pear was used as treatment of asthma, control of blood pressure tonic medicine vasoaction, diabetes in oriental medicine. In this study, it was investigated that pear phenolic compound effects on cardiovascular system as blood pressure and renin and atrial natriuretic peptide(ANP) in plasma, Cardiac hypertrophy. The experiments were performed on Sprague Dawley rats, 2K1C hypertension model was prepared by constricting the left renal artery with a sliver clip. Animals were then divided into four groups, 5mg/Kg(PPC-A) 10mg/Kg(PPC-B) 15mg/Kg(PPC-c) and control group. Pear phenolic compound solution were supplied with them for 3weeks, a day's interval. The results are that The blood pressure was significantly decreased at 15days in PPC-A group than control group. The plasma ANP was increased in PPC-A and PPC-C group, and cardiac hypertrophy was significantly decrease in PPC-C group compared with control group.

The blood pressure was more decreased in pear pectin 10 mg/kg than in apple pectin. The plasma ANP was decreased in pear pectin 10 mg/kg, and renin was increased in same concentration of drug. Cardiac hypertrophy had a tendency to decrease in pear pectin 15 mg/kg, but was not statistically significant compared to control group.

  • It has been reported that 50~70% of child asthma, bronchial asthma in adult, and allergic rhinitis are caused by house dust mite. The antigen extracted from house dust mite has been used for effective treatment against allergic diseases and for clinical test. This house dust mite antigen has been entirely imported from abroad. However, the composition and content of all the antigen imported vary from a brand to other brand. Thus, we need to standardize the composition and content of the antigen by developing it domestically. We proceeded pre-clinically general pharmacological test and toxicological test that are required for the eventual human use by utilizing the house dust mite cultured in Korea. In order to obtain information on general pharmacological tests such as its toxic signs in tissues or organs which are mainly affected, we examined the effect of house dust mite on the tensions of the isolated tissues and heart rates of cardiac muscle by recording with force displacement transducer of polygragh (Glass Model 7). We determined lethality of antigen extracted from house dust mite in mice and guinea pigs. We examined acute and subacute toxicity by administrating house dust mite extract of 500, 100, 20 times of the expected clinical dose. In male and female mice and guinea pigs, given a sigle intraperitoneal dose of antigen, $LD_{50}$ values were over 5.0 $\textrm{m}{\ell}$/kg, respectively. In animals administrated with house dust mite, there were no significant change of clinical symptom, body weight, food consumption, water consumption, eye examinations, urinalysis, blood biochemistry, and histopathological examinations in any animals tested. We found no toxic effect of this house dust mite. These results show that the house dust mite cultured by us could be used in the development of medicine against allergic diseases caused by the antigen of house dust mite.

Prevalence and Impact of Comorbidities in Individuals with Chronic Obstructive Pulmonary Disease: A Systematic Review

  • This study aimed to describe the prevalence of comorbidities associated with chronic obstructive pulmonary disease (COPD) and their relation with relevant outcomes. A systematic review based on the PRISMA methodology was performed from January 2020 until July 2021. The MEDLINE, Lilacs, and Scielo databases were searched to identify studies related to COPD and its comorbidities. Observational studies on the prevalence of comorbidities in COPD patients and costs with health estimates, reduced quality of life, and mortality were included. Studies that were restricted to one or more COPD pain assessments and only specific comorbidities such as osteoporosis, bronchitis, and asthma were excluded. The initial search identified 1,409 studies and after applying the inclusion and exclusion criteria, 20 studies were finally selected for analysis (comprising data from 447,459 COPD subjects). The most frequent COPD comorbidities were: hypertension (range, 17%-64.7%), coronary artery disease (19.9%-47.8%), diabetes (10.2%-45%), osteoarthritis (18%-43.8%), psychiatric conditions (12.1%-33%), and asthma (14.7%-32.5%). Several comorbidities had an impact on the frequency and severity of COPD exacerbations, quality of life, and mortality risk, in particular malignancies, coronary artery disease, chronic heart failure, and cardiac arrhythmias. Comorbidities, especially cardiovascular diseases and diabetes, are frequent in COPD patients, and some of them are associated with higher mortality.

Clinical Evaluation of Pneumomediastinum in Adult (성인에서 기종격증의 임상적 고찰)

  • Pneumomediastinum[Mediastinal emphysema is characterized by the presence of air in the mediastinum, and classified as spontaneous[occurring without obvious cause or secondary[caused by a trauma or artificial ventilation . To study the clinical evaluation of pneumomediastinum, data were obtained from 20 patients. The incidences of spontaneous pneumomedisastinum were 5 and those of secondary pneumomedistinum were 15 cases. The mean age was 21.2$\pm$3.4 years[$\pm$SD in spontaneous pneumomediastinum and 44.1$\pm$20.0 years[$\pm$SD in secondary pneumomediastinum. There were 16 male and 4 female patients. The common presenting compliants were retrosternal pain in 19 patients[95% , dyspnea in 12[60% , and hoarsness in 2[10% . The predisposing factors were asthma,excessive exercise and vomiting in spontaneous pneumomediastinum;trauma, artificial ventilation, tracheostomy, the rupture of trachea or esophagus in secondary pneumomediastinum. The physical findings were subcutaneous emphysema in 17 patients[85% , Hamman`s sign in 11 patients[55% and decreased cardiac dullness in 2 patients[10% . Spontaneous pneumomediastinums were managed conservatively, however, surigical procedures were needed in secondary pneumomediastinums. There was no recurrence, but one patinet died of tension pneumomedistinum. We concluded that spontaneous pneumomediastinum is uncommon, usually benign, and self-limited and secondary pneumomedistinum due to trauma or artificial ventilation is more increasing, and necessitates the early, aggressive intervention.

Perceived Stress, Mood State, and Sympotms of Stress of the Patient with Chronic Illness (만성질환자의 스트레스 지각, 기분상태, 스트레스 증상에 관한 연구)

  • Purpose: The purpose of this study was to identify the factors influencing Symptoms of Stress in patients with chronic illness. Method: Data were collected by questionnaires from 1,748 patients with chronic disease in General Hospital in Seoul. Chronic diseases of were cardiac disease including hypertension, peptic ulcer, pulmonary disease included COPD and asthma, DM, and chronic kidney disease. The data were analyzed using descriptive statistics, pearson correlation coefficients, and stepwise multiple regression. Result: 1. The level of symptoms of stress was moderate(M=2.17). 2. The score of symptoms of stress showed significantly positive correlation with the score of mood state(r=.58, p=.00), perceived stress(r=.57, p=.00), and ways of coping(r=.33, p=.00). The symptoms of stress showed significantly negative correlation with the score of social support(r=-.37, p=.00) and self-esteem(r=-.19, p=.00). 3. The most powerful predictor of symptoms of stress was mood state and the variance explained was 34%. A combination of mood state, ways of coping, perceived stress, social support, and duration of illness account for 45% of the variance in symptoms of stress of the patients with chronic illness. Conclusion: This study suggest that mood state, ways of coping, perceived stress, and social support are significantly influencing factors on symptoms of stress of the patients with chronic illness.
