Dairy Dietary Calcium and Osteoporosis - An Overview

  • The osteoporosis is a disease characterized by lower bone mineral content, deterioration of bone tissue and a reduction in the protein and mineral matrix of the bone. The bone becomes more porous leading to increased bone fragility and risk of fracture, particularly of the hip, spine and wrist. Osteoporosis can result in disfigurement, lowered self·esteem, reduction or loss of mobility, and decreased independence. Adequate calcium intake through milk and milk products in childhood and adolescence is a decisive marker for obtaining a maximum bone mass (peak adult bone mass) and f3r the prevention of osteoporosis. Calcium is one of the most critical nutrients associated with the osteoporosis. Dietary calcium is of great significance for healthy skeletal growth and development. The bone mineral content and bone mineral density of young adults is directly related to the calcium intake through milk and dairy products. Milk and milk products are the important sources of calcium as the richness and bioavailability of this nutrient is very high as compared to other food products. If enough calcium is not supplemented through diet, calcium from the bone will be depleted to maintain the blood plasma calcium level. The article focuses on the various issues related to osteoporosis manifestation and the role of dietary calcium especially calcium derived from dairy products.

폐경기 골다공증 환자의 골다공증 지식, 건강신념, 자기효능감이 칼슘섭취행위 빈도에 미치는 영향 (Effects of the Knowledge, Health Belief, and Self-Efficacy about Osteoporosis on Calcium Intake Behavior for Postmenopausal Osteoporosis Patients)

  • Purposes: To identify the predictors of calcium intake behavior and examine the relationships among bone mineral density, osteoporosis knowledge, osteoporosis health belief, osteoporosis self efficacy and calcium intake behavior of postmenopausal osteoporosis patients. Methods: The subjects consisted of 94 patients. The measurement tools were osteoporosis knowledge test, osteoporosis health belief scale, osteoporosis self-efficacy and calcium intake frequency questionnaire. The data were analyzed using the SPSS WIN 11.0 program. Results: The bone mineral density of the lumbar were $0.75g/cm^2$, T-score -2.67 and the femur neck were $0.67g/cm^2$, T-score -2.30. There was statistically a significant correlation between calcium intake behavior and health motivation (r=0.449, p=0.000) among the osteoporosis health belief. In hierarchial multiple regression analysis, current spouse(12.8%) and health motivation(19.9%) of the osteoporosis health belief explained the 32.7% of variance in calcium intake behavior. Conclusion: Nursing intervention should be developed for increasing the calcium intake behavior through promoting health motivation for the postmenopausal osteoporosis women having no spouse currently.

서울.경기지역 성인여성의 골밀도와 혈압, 칼슘섭취의 상관성 연구 -2011년 국민건강영양조사를 중심으로- (Association of Bone Mineral Density and Blood Pressure, Calcium Intake among Adult Women in Seoul.Kyunggi Area - Based on 2011 KNHANES -)

  • This study was carried out to investigate bone mineral density (BMD), blood pressure, calcium, sodium intake and related biochemical variables, and to analyze the relationship between BMD, and blood pressure, physical characteristics, nutrient intakes of Korean adult women. The study subjects were 30~80years old, 513 women living in Seoul and Kyunggi area who participated in 2011 KNHANES. The study subjects were divided into three BMD groups; normal, n = 259 (50.4%), osteopenia, n = 169 (32.9%), and osteoporosis, n = 85 (16.7%). Average height and weight and fat free mass decreased with a decrease in BMD. Average BMD of normal, osteopenia, osteoporosis were 0.87, 0.79, $0.70g/cm^2$, and T-score were 0.42, -0.66, -1.62, respectively. Higher systolic blood pressure, alkaline phosphatase, and triglyceride were significantly associated with a decrease in the BMD. The rates of hypertension were significantly increased from normal to osteoporosis. i.e. 18.5% in normal, 34.3% in osteopenia and 63.5% in osteoporosis. Average nutrientintakes such as protein, fat, calcium, phosphorus, iron, sodium, potassium, vitamin A, vitamin B, vitamin B2, and vitamin C were significantly lower in subjects with lower BMD.n. Average calcium intake of normal, osteopernia and osteoporosis were 528.50, 416.96, 389.56 mg, respectively. There were significant negative correlations between calcium, phosphorus and age, systolic blood pressure, triglyceride. Also, there were negative correlation systolic blood pressure (incomplete sentence). In this study, we found low BMD according to low calcium intake increase the risk of osteoporosis and hypertension rather than sodium intake, also calcium intake decreased with increase age. Therefore, in order to prevent osteoporosis and hypertension, subjects need to be educated regarding the importance of calcium nutrients in diet.

The Change of Bone Mineral Density by Bisphosphonates Therapy with Calcium-Antagonists in Osteoporosis

  • Imbalance in calcium and phosphorous metabolism due to aging or menopause leads to osteoporosis. In contrast to patients with normal blood pressure, hypertensive patients have a higher loss of calcium in the urine with its attendant risk of osteoporosis. The high blood pressure is associated with the risk of bone loss and abnormalities in calcium metabolism leading to calcium loss. So we retrospectively investigated the changes of bone mineral density (BMD) which drugs can have clinical influences over osteoporosis treatments of patients with calcium-antagonists as common antihypertensive drugs and with bisphosphonates which causes a most effective inhibition of osteoclasts resorption. As a result over 70 years of age group and within bisphosphonates group, alendronate 70 mg once-weekly group showed significant increase of BMD in lumbar area. Combination group of cilnidipine and $maxmarvil^{(R)}$ showed very significant decrease of BMD. In conclusion, it is desirable that combination therapy with calcium-antagonists is used carefully in the treatment of osteoporosis with high blood pressure.

  • In the light of the increasing concern over the prevalence of osteoporosis, hypertension, diabetes mellitus, we performed this study to review the correlation between the dietary calcium and vitamin D intake and these diseases. To this end, we investigated the effects of dietary calcium and vitamin D on these diseases. We observed that the intake of dietary calcium and vitamin D had a negative correlation with the incidences of osteoporosis, hypertension, and diabetes mellitus. Further, the intake of these two nutrients is expected to improve related mechanisms such as the renin-angiotensin system. Therefore, we suggest that dietary calcium and vitamin D have a beneficial effect on these diseases.

일개 도서지역 여성의 골다공증 지식, 자기효능감과 건강신념간의 관계 연구 (A Study on the Relationships between Osteoporosis Knowledge, Self-efficacy and Health Belief of Women in an Island)

  • This study was to examine the relationships between knowledge, health belief and self-efficacy of osteoporosis with the women residents in an island. Method: The subjects were 64 women who lived in an island located in Incheon metropolitan city. Data collection was performed by using questionnaire that included Osteoporosis Knowledge Test, Osteoporosis Self-Efficacy Scale and Osteoporosis Health Belief Scale by Kim, Horan & Gendler (1991). The Data were analyzed using SAS computer program that included descriptive statistics, t-test, ANOVA, Pearson correlation coefficient. Result: 1) The mean of osteoporosis knowledge was 10.03 in the range of 0 to 24, shows the relatively lower score than mean score. The mean of osteoporosis health belief variables were susceptibility 18.95, seriousness 19.05, benefits of exercise 22.35, benefits of calcium 21.81, barriers to exercise 16.95, barriers to calcium 13.13, and health motivation 19.75 in every range of 6 to 30. The mean of osteoporosis self-efficacy was 37.95 in the range of 12 to 60, shows a relatively higher score than mean score. 2) There were statistically significant differences in the degree of osteoporosis knowledge according to age, education. But There was no significant difference in the degree of osteoporosis and self-efficacy according to general characteristics. There were statistically significant differences in the degree of susceptibility according to religion, family income. There were statistically significant differences in the degree of seriousness, health motivation according to family income. There was statistically significant difference in the degree of barriers to exercise according to education. 3) There were statistically significant positive correlations between osteoporosis knowledge and benefits to exercise, benefits of calcium intake. There was statistically no significant correlation between osteoporosis knowledge and osteoporosis self- efficacy. There was statistically a significant positive correlation between osteoporosis self-efficacy and barriers to exercise. There was statistically a significant negative correlation between osteoporosis self-efficacy and health motivation. Conclusion: According to the result, osteoporosis education program including exercise, calcium intake should be operated to increase benefits to exercise and calcium intake for osteoporosis prevention. In addition, the program of improving self-efficacy should be designed and operated to decrase the perception of barriers to exercise and to increase the perception of health motivation of women in island.

골다공증 실혐모델 흰쥐의 칼슘대사에 대한 소뼈회분과 인산칼슘의 섭취 효과 (Effects of Bovine Ash and Calcium Phosphate on Calcium Metabolism in Postmenopausal Osteoporosis Model Rats)

  • The model rats with postmenopausal osteoporosis were comparatively observed with regard to the effects of bovine ash and calcium phosphate on calcium metabolism. The modelling design involved the five week-old week-old female SD-strain rats ovariectomized and fed a low-Ca diet(20% casein, 0.06% Ca and 0.38% P) for three weeks. The rats were divided into five groups, one of which was fed the low-Ca diet(basal), and the rest of which were divided into five groups, one of which was fed the low-Ca diet(basal), and the rest of which were fed four kinds of Ca-supplemental diets(20% protein, 1.06% Ca and 0.8% P) for three weeks. The Ca-suplements diets contained two kinds of Ca sources, bovine bone ash(BBA) or calcium phosphate, tribasic [Ca3(PO4)2] and two kinds of protein sources, casein or isolated soy protein(ISP). The model rats of postmenopausal osteoporosis fed basal diet showed a significant decrease in Ca utilization in reference to serum Ca concentration, breaking force of bone, Ca and P contents of bone, and Ca absorption and retention. However, the supply of Ca for three weeks demonstrated the improved utilization of Ca. One step further, BBA was more effective than calcium phosphate in improving Ca utilization in ISP-fed groups. On the other hand, no significant difference was seen in casein-fed groups. It is to conclude that BBA could be more effective in accelerating Ca utilization under vulnerable dietary or physiological conditions such as vegetable protein intake and osteoprosis.

중년여성의 골다공증에 대한 지식과 인지요인과의 관계 (A Study on the Relationships between Knowledge about Osteoporosis and Cognitive Factors in Middle-aged Women)

  • Purpose: This study was to examine the relationships between knowledge about osteoporosis and cognitive factors in middle-aged women. Method: The subjects were 293 middle-aged women. Data collection was performed by using a questionnaire that included an Osteoporosis Knowledge Test, Osteoporosis Self-Efficacy Scale and Osteoporosis Health Belief Scale developed by Kim, Horan & Gendler(1991). Data was analyzed using the SPSS Win 10.0 program for descriptive statistics, and the Pearson correlation coefficient. Result: The mean osteoporosis knowledge was 14.0. The subscale means of osteoporosis health belief variables were: susceptibility 15.9, seriousness 17.2, benefits of exercise 22.9, benefits of calcium 21.4, barriers to exercise 20.9, barriers to calcium 22.5, and health motivation 18.6. The mean osteoporosis self-efficacy was 39.3 with a moderate score. There were significantly positive correlations among knowledge about osteoporosis, health belief, and self-efficacy about osteoporosis. Conclusion: Osteoporosis knowledge, health belief, and osteoporosis self-efficacy are related. Therefore, knowledge through education is an important factor in behavioral changes and it contributes to increase the health belief and self-efficacy of osteoporosis.

골다공증 모델 흰쥐에서 고수준의 단백질의 칼슘 섭취가 칼슘대사 및 신장기능에 미치는 영향 (Effects of High Protein and Calcium Intakes on Calcium Metabolism and Renal Function in Ovariectomized Osteporosis Rat Model)

  • This study was to investigate interaction between dietary protein and Ca levels in Ca metabolism and renal function in osteporosis rats. Five week-old female rats were fed a low Ca diet for 4 weeks after ovariectomy operation to establish rat models of osteoporosis. The ovariectomized osteoporosis rats were divided into six groups and were fed experimental diets which contained two levels of protein, normal (20%) and high(40%) , and three levels of Ca, low (0.06%), normal (0.47%) and high(0.94%) for 4 weeks , respectively. The ovaricetmized rat model of osteoporosis showed a remarkable decrease in serum Ca concentration, fresh weight and breaking force of femur, Ca and P contents of femur, and apparent absorption and retention of Ca. The supplementations of Ca and P contents of femur, and apparent absorption and retention of Ca. The supplementations of Ca at the dietary levels of normal and high levels significantly enhanced Ca bioavailability shown in the above experimental rat models of osteoporosis, regardless of dietary protein levels ; whereas the rats which were fed the low Ca diet demonstrated rather a decrease in its bioavailability. Irrespectively of the dietary Ca levels, the rats which were fed high protein diet exhibited an increase in kidney weight, urinary Ca, volume and hydroxyproline, and glomerular filtration ratio(GFR). The results show that dietary protein and calcium levels affect the renal function and Ca metabolism independently, while the interaction between protein and calcium have not been shown.

미혼 여성의 골다공증 인지정도와 자기효능감에 관한 연구 (A Study on the Level of Awareness and Self-Efficacy of Osteoporosis in Young Women)

  • Purpose: This study was done to determine the level of awareness and self-efficacy and their relationships to osteoporosis among young women. Methods: The participants were 309 young women living in Seoul. The data were collected using questionnaires from July to September 2009. The Osteoporosis Awareness Scale consists of five areas with a total of 31 questions and the Self-efficacy Scale consists of 18 questions. Data were analyzed using descriptive analysis, t-test, ANOVA, Scheffe' test and Pearson's correlation coefficients. Results: The average level for osteoporosis awareness was 2.22, of a possible 4.00 and significant difference were related to participants' previous encounter with any information about osteoporosis, behavior to prevent osteoporosis, experience of weight control, and frequency of vegetable and calcium-rich foods intake. The average level of self-efficacy for osteoporosis was 2.81, of a possible 4.00 and significant difference related to participants' educational level, occupation, behavior to prevent osteoporosis, smoking, method of weight control, regular exercise, and frequency of vegetable and calcium-rich foods intake. There were significant positive correlations among awareness and self-efficacy about osteoporosis. Conclusion: This study suggests that health care professionals need to provide effective interventions for young women to enhance their osteoporosis awareness and self-efficacy for preventing osteoporosis.
