Development and Validation of an Integrated Healthy Workplace Management Model in Taiwan

  • Background: Impacts of exposure are generally monitored and recorded after injuries or illness occur. Yet, absence of conventional after-the-effect impacts (i.e., lagging indicators), tend to focus on physical health and injuries, and fail to inform if workers are not exposed to safety and health hazards. In contrast to lagging indicators, leading indicators are proactive, preventive, and predictive indexes that offer insights how effective safety and health. The present study is to validate an extended Voluntary Protection Programs (VPP) that consists of six leading indicators. Methods: Questionnaires were distributed to 13 organizations (response rate = 93.1%, 1,439 responses) in Taiwan. Cronbach α, multiple linear regression and canonical correlation were used to test the reliability of the extended Voluntary Protection Programs (VPP) which consists of six leading indicators (safe climate, transformational leadership, organizational justice, organizational support, hazard prevention and control, and training). Criteria-related validation strategy was applied to examine relationships of six leading indicators with six criteria (perceived health, burnout, depression, job satisfaction, job performance, and life satisfaction). Results: The results showed that the Cronbach's α of six leading indicators ranged from 0.87 to 0.92. The canonical correlation analysis indicated a positive correlation between the six leading indicators and criteria (1st canonical function: correlation = 0.647, square correlation = 0.419, p < 0.001). Conclusions: The present study validates the extended VPP framework that focuses on promoting safety and physical and mental health. Results further provides applications of the extended VPP framework to promote workers' safety and health.

대학병원 간호사의 교대근무 형태에 따른 사회심리적 스트레스 (The Effect of the Shift Work by Types on Sociopsychological Stress in the University Hospital Nurses)

  • Objectives: To determine the effect of the various type and number of shift work on sociopsychological stress in the university hospital nurses in Seoul, South Korea. Methods: Data was collected through a self-administered questionnaire that included general characteristics, the type and number of shift work in the past 4 weeks and the an sociopsychological stress based on PWI-SF. Logistic regression analysis was conducted to analyze the effect of shift work by types on sociopsychological stress of university hospital nurses using SAS 9.4 and it adjusted general characteristics by the additional models. Results: When the number of night shift work 3 or 4 compared to 0, sociopsychological stress was significantly high(OR=2.16, 95% CI=1.29-3.65). When 5 or more, it was higher(OR=2.39, 95% CI=1.55-3.72). Two continuous night shift work compared to 0 increased sociopsychological stress(OR=2.37, 95% CI=1.49-3.81) and 3 or more even more(OR=2.70, 95% CI=1.72-4.27). If the shift work type changes 14 or 15 times compare to below 11, sociopsychological stress is increased and even higher when 15 or more times but it's not significant after adjusting for covariates. Conclusions: It suggests that workplace with nurse expose to shift work should be on lookout for more than three night shift work, more than two continuous night shift work and more than 14 shift work change which can lead to burnout, absenteeism, depression and decreased productivity.

코로나19 대유행 시 지역사회 요양병원 종사자의 근무경험: 혼합연구방법 (Working Experience of the Community-based Long-term Care Hospital Workers during the COVID-19 Pandemic: Mixed Methods Research)

  • Purpose: This study is a mixed methods research that was conducted to verify factors affecting the working experience of community-based long-term care hospital workers during the COVID-19 pandemic. Methods: The study was carried out from July 19 to November 3, 2021 for 340 nurses who worked at 10 long-term care hospitals located in G city. Results: As the study results, factors that affected job stress of the workers working at community-based long-term care hospitals included job satisfaction (β=-.27, p<.001), work demand (β=-.25, p<.001), fatigue (β=.19, p=.001), and cooperation and leadership (β=-.12, p=.049). It was found that the participants were struggling with physical and mental stress caused by the increased workload due to the preventative measures taken to stop the infection and spread of COVID-19. Despite this, they accepted their situation as necessary to overcome the pandemic and shared the quarantine guidelines of the government and community health centers while actively responding to prevent the spread of COVID-19 under the leadership of their supervisors. However, they were experiencing psychological and emotional burnout in the prolonged pandemic situation. Conclusion: It is considered necessary to help relieve their stress and provide psychological and mental support by adopting a policy to develop and apply comprehensive programs.

경상남도 간호사의 직장선택, 직무수행의 어려움, 이직의도에 영향을 미치는 요인 (A Study on Factors Affecting the Workplace Selection, Job Performance Difficulties, and Turnover Intention of Nurses in Gyeongsangnam-do)

  • Purpose: This study aims to provide primary data for policy alternatives by identifying the problem of the nursing workforce shortage. Methods: For quantitative data, 446 questionnaires were analyzed. The mean and standard deviation were used for content description. ANOVA analysis and Scheffe? test were used to compare the differences according to the hospital level. For qualitative data, 1:1 in-depth and group interviews were conducted for six participants. Results: The factors nurses prioritized when choosing a workplace were salary, commuting distance, and work-life balance. Clinical nurses cited low wages, heavy workloads, and burnout as the most considerable difficulties in performing their duties. Factors influencing nurse's turnover intention were low wages, unmanageable workload, and rotation to unwanted departments in that order. New nurses tend not to apply to small-medium-sized hospitals, experienced nurses in their 30s-40s leave hospitals due to childcare and shift work difficulties, and nurses in their 50s and older tend to move to nursing homes rather than acute hospital settings. Conclusion: In this study, wage and workload were mentioned as the most critical factors in nurses' workplace selection, job performance difficulties, and turnover intention, so it is necessary to pay attention to this aspect when improving treatment for nurses.

민원담당 공무원의 감정노동이 심리적 소진에 미치는 영향 - 사회적 지지와 지각된 스트레스의 매개효과를 중심으로 - (The Effect of Emotional Labor on Psychological Exhaustion of Civil Affairs Officials - Focused on the Mediation Effect of Family and Social Support and Perceived Stress -)

  • 본 연구는 민원담당 공무원의 감정노동이 사회적 지지와 지각된 스트레스를 매개로 심리적 소진에 어떠한 영향을 미치는지 구조적 관계를 살펴보고, 매개효과를 검증하는 데 그 목적이 있다. 그 결과, 첫째, 민원담당 공무원의 감정노동은 사회적 지지, 지각된 스트레스, 심리적 소진에 정적(+) 영향을 미치고, 사회적 지지는 심리적 소진에 유의한 부적(-) 영향을 미치고 있었다. 지각된 스트레스는 심리적 소진에 정적(+) 영향이 있는 것으로 나타났다. 둘째, 민원담당 공무원의 감정노동과 심리적 소진과의 관계에서 사회적 지지와 지각된 스트레스의 매개효과를 검증결과 부분매개효과가 확인되었다. 개입전략을 효과적으로 수립하기 위해서는 먼저, 매개변인으로 확인된 사회적 지지 강화와 지각된 스트레스의 감소 촉구에 우선적으로 개입하고 적극적으로 개선할 것을 제언한다.

부양자의 죄책감이 수발도움 요청행위에 미치는 영향 (The Effect of Primary Caregivers' Guilt Feelings on their Request Behaviors for Help with Caring)

  • 죄책감은 요보호노인의 주부양자가 가지기 쉬운 역기능적인 감정으로 수발부담을 이중으로 가중시키고 있음에도 불구하고 죄책감이 수발행위에 미치는 영향에 대해 다룬 실증연구는 거의 미비하다. 이에 본 연구는 노인부양에 대한 유교적인 가치가 아직 남아 있는 우리사회에서 부양자가 가지는 죄책감에 착안하여 죄책감이 주부양자의 부양부담과 수발도움요청행위에 미치는 영향에 대해 알아보는 것을 목적으로 하고 있다. 조사는 60세 이상의 요보호노인을 수발하고 있는 주수발자 220명을 대상으로 방문 조사하였다. 죄책감 측정도구는 수발자용으로 자체 개발된 척도를 사용하였고(${\alpha}=.949$), 죄책감은 4가지 요인으로 분류되어 자기통제결핍형, 자원결핍형, 소진형, 규범형으로 명명하였다. 연구결과는 다음과 같다. 첫째, 가족수발자의 죄책감은 부양부담감과 양의 상관관계를 이루며 부양부담감은 죄책감의 4가지요인에 고루 영향을 미치는 것으로 나타났다. 둘째, 주부양자가 수발도움을 요청할 때, 동거가족과 이웃에 대해 죄책감을 느끼며, 죄책감이 적을 경우에 주간보호서비스를 이용하고 있었다. 특히 주부양자에게 영향을 미치는 죄책감요인은 동거가족에게는 규범적인요인, 이웃에게는 자원결핍요인, 그리고 주간보호서비스이용에서는 소진요인으로 나타났다. 이러한 결과는 수발도움 요청에 있어 발생하는 부양자의 역기능적인 감정인 죄책감이 수발부담을 더욱 가중시키고 있음은 물론 부양자 역할을 분담하는데 있어 어려움을 초래할 수 있다는 것을 알 수 있다. 이에 부양자의 정신건강을 위해서는 부양자가 가지는 죄책감에 대해 이해하고 대응하는 프로그램개발이 필요할 것이다.

소방공무원의 직무스트레스와 사회심리적 스트레스와의 연관성에 관한 연구 (A Study on the Relationship between Job Stress and Socio-Psychological stress of firefighter)

  • Purpose: Firefighters were subjected to be exposed to 24-hour shiftwork and high level of job stress, but there is little study of fire fighters about between Job Stress and Psychological stress in Korea. This study was performed to evaluate job stress of fire fighters grouped by different job types and analyze the health effect of job stress by using a PWI-SF(Psycho-social Well-being Index, Short Form). Methods : This study was to measure the degree of job stress and socio-psychological stress of firefighters working at 119 safety center and analyze the relationship between job stress and mental health, through self-administered questionnaire survey on 918 fire fighters working at fire department located in Korea, during October in 2008. Question items of the questionnaire consist of general characteristics, job stress, and socio-psychological stress. Job Stress Measurement Scale for Koreans was used to measure job stress, while PWI-SF used for socio-psychological stress. Collected data was analyzed through frequency analysis, ${\chi}^2$ test, one-way ANOVA, correlation analysis, multi-regression analysis using a statistic program, SPSS/10.0 Ver for Windows, and statistical significance level was 0.05. Result : Accordingly, this paper is written for the purpose of scrutinizing the reason of 119 rescuers' job stress and Socio-Psychological stress. This thesis tries to analyze a problem and provide contents through many variables concerned and theoretical investigation with the actual research of questionnaire. The results of the study are summarized as follows : Fire fighters feel relatively high stressful in their performance. Job stress, organizational commitment and burnout did not have been influence upon by individual cases such as economic condition, regular exercise etc. Finally, as a result of step by step regression analysis, the variable that can explain Socio-Psychological stress the best was negative emotion scale(NE). Conclusions : It is necessary to conduct repeated studies in the future, since the study implied that there was a contrary relationship between job stress and socio-psychological stress. In order to reduce job stress and maintain better mental health, it is more important than anything else to help firefighters have willingness and efforts to sustain a healthy life, so the development of various programs and education to raise their awareness should be implemented.

가정간호 교육요구도 조사 연구 (Education Needs for Home Care Nurse)

  • In 1990 Home Care Education Programs started when legislation established certification for Home Care Nurses. The Ministry of Health and Welfare proposed a home care education curriculum which has 352 class hours and 248 hours of 'family nursing and practice'. Though Home Care Education Programs have been offered in 11 home care educational institutes, there has been no formal revision for the home care education programs. Also a first and second home care demonstration projects have been carried out, but there has been no research on outcomes for home care education as applied in home care practice. The purposes of this study were to identify the important content areas for home care nursing as perceived by home care nurses, and to identify their clinical competence in each of these areas, and from these to identify the education needs. The sample was 107 home care nurses who were working in home care demonstration hospitals and community-based institutions which have been offering home care services. Responses were received from 88 nurses, comprising a 82.2% return rate, and 86 were included in the final analysis. The instrument used was a modification of the instrument developed by Caie-Lawrence et(1995) and Moon's(1991) instrument on home care knowledge. The instrument's Cronbach's coefficient was 0.982. Among the respondents, 64% were working at home care demonstration hospitals and 36% were working at community-based institutions. Their home care experiences were from one month to six years, with a mean of 20.6 months. The importance rating for home care education content was 3.42 0.325, which means importance was rated relatively high. Technical aspects of home care were identified the most important. Five items 'education skill', 'counseling skill', 'interview skill', 'wound care skill', 'bed sore care skill' received 100% importance ratings. The competency rating was 2.87 0.367 and 'technical aspects of home care' was the highest, and 'application to home care skill' was the lowest. Home care nurses' education needs were identified and compared to the importance ratings and competency ratings. Eleven items were identified as the highest in the importance areas and eleven items were in the lowest competency areas. High importance ratings matched with low competency ratings determined training needs, but there was no matching items in this study. In the lowest competency areas four items were excluded, because of not being applicable in current home care practice. Therefore total eighteen items were identified as home care education needs. These items are 'bed sore care skill', 'malpractice', 'wound care skill', 'general infection control', 'change and management of tracheostomy tubes', 'CVA patient care', 'Hospice care', 'pain management', 'urinary catheterization and management', 'L-tube insertion and managements', 'Respirator use and management skill', 'infant care', 'prevention to burnout', 'child assessment', 'CAPD', 'infant assessment', 'computer literacy', and 'psychiatry patient care'.

치과 CAD/CAM 시스템으로 제작한 코발트-크롬 소결 금속 코핑의 변연 및 내면 적합도 평가 (Evaluation of Marginal and Internal Gap of Cobalt-Chromium Sintering Metal Coping Fabricated by Dental CAD/CAM System)

  • 본 연구에서는 CAD/CAM 시스템을 이용하여 제작한 코발트-크롬 주조 금속 코핑과 코발트-크롬 소결 금속 코핑의 변연 및 내면 적합도를 비교 평가하여 다음과 같은 결론을 도출하였다. 코발트-크롬 금속 코핑의 변연 및 내면 적합도를 비교 평가는 기존의 주조방식인 CCM 그룹보다 소결과정만으로 제작한 CSM 그룹이 전반적으로 모두 우수하였다. 또한 CSM 그룹의 변연 적합도는 임상적으로 허용할 수 범위인 $120{\mu}m$ 이내 존재하였다는 결과를 통해서 그동안 매몰, 소환, 주조인 복잡한 과정을 거치면서 보철물의 오차가 발생 하였으나 소결과정만으로 공정 과정의 오차를 줄임으로써 완성도 높은 코발트-크롬 금속 코핑 제작이 가능하다고 생각된다.

콜 센터에서 이직요인이 직무만족에 미치는 영향에 있어 스마트워크와 상사지원 융복합 서비스의 조절효과에 관한 연구 (The moderating effects of converging smart work and supervisor's support in the study of turnover on job satisfaction in call centers)

  • 본 논문의 연구목적은 콜센터 상담사의 이직요인을 학문적 실무적 관점에서 고찰하고 이에 대한 완화방안으로 콜센터 서비스에 스마트워크 기술과 상사지원 서비스의 융복합이 상담사의 직무만족에 정의 효과를 나타내는지 여부를 검증하는데 있다. 본 연구는 상담사의 이직요인에 대한 선행 연구의 이론적 배경을 토대로 스마트워크 효과와 상사지원효과를 조절변수로 하는 연구모델과 가설을 설정하고, 금융분야의 콜센터 상담사를 대상으로 설문 조사를 실시하였다. 가설검증을 위하여 자료 분석은 SPSS 20.0을 사용하였으며, 영향관계 검증을 위해서는 다중회귀분석, 조절효과 검증을 위해서는 위계적 회귀분석을 사용하였다. 실증분석 결과 상담사의 개인적 이직요인에 대한 스마트워크 조절효과와 업무적 이직요인에 있어 업무 과부하는 상사지원이 상담사의 직무만족에 유의한 정(+)의 조절효과가 있는 것으로 나타났지만, 직무탈진감에 대한 상사지원 조절효과는 부(-)의 효과가 나타났다. 이는 상담사의 속사정을 모르는 지나친 상사의 지원은 오히려 상담사 개인의 자존심에 역효과를 주어 이직율 완화에 부정적인 영항을 줄 수 있다는 점을 의미할 수 있다.
